Background:Fever of unknown origin(FUO)in developing countries is an important dilemma and further research is needed to elucidate the infectious causes of ***:A multi-center study for infectious causes of FUO in lower middle-income countries(LMIC)and lowincome countries(LIC)was conducted between January 1,2018 and January 1,*** total,15 participating centers from seven different countries provided the data,which were collected through the Infectious DiseasesInternational Research Initiative *** adult patients with confirmed infection as the cause of FUO were included in the *** severity parameters were quick Sequential Organ Failure Assessment(qSOFA)≥2,intensive care unit(ICU)admission,vasopressor use,and invasive mechanical ventilation(IMV).Results:A total of 160 patients with infectious FUO were included in the ***,148(92.5%)patients had community-acquired infections and 12(7.5%)had hospital-acquired *** most common infectious syndromes were tuberculosis(TB)(n=27,16.9%),infective endocarditis(n=25,15.6%),malaria(n=21,13.1%),brucellosis(n=15,9.4%),and typhoid fever(n=9,5.6%).Plasmodium falciparum,Mycobacterium tuberculosis,Brucellae,Staphylococcus aureus,Salmonella typhi,and Rickettsiae were the leading infectious agents in this study.A total of 56(35.0%)cases had invasive procedures for *** mean qSOFA score was 0.76±0.94{median(interquartile range[IQR]):0(0–1)}.ICU admission(n=26,16.2%),vasopressor use(n=14,8.8%),and IMV(n=10,6.3%)were not ***,38(23.8%)patients had at least one of the severity *** mortality rate was 15(9.4%),and the mortality was attributable to the infection causing FUO in 12(7.5%)***:In LMIC and LIC,tuberculosis and cardiac infections were the most severe and the leading infections causing FUO.

Does Love Forgive Everything？ Sexual Assault as an Attack Which Is normalized in Turkish Television Series

Encroachment, which can generally be defined as appropriation of the other＇s right, comes first as one of the most traumatic incidents among sexual crimes. When the statistical data about rape are examined in Turkey, it is seen that the ratios of rape incidents are at an extent that cannot be underestimated. The problem of rape is covered in some series which are broadcast on TV and popular among wide range of audience in Turkey. Fatmagiil ＇iinSu^u ne？, Oyle Bir Geger Zaman Ki and ]fret, which are among these series, are chosen as the sample in this study. Since it is certainly one of the important functions of media to raise awareness of the public, dealing with rape in series and films as a social problem is usual and even crucial. However, the basic issue here is how this incident is handled and what kind of message is given to the audience, who takes the main characters as model. The aim of this study is to explore how the rape incidents are covered and how the solution is reached in these aforementioned series. In this study, these three television series are examined through purposeful sampling method and also, the answers to the questions mentioned as the aims of this study are analyzed through a feminist perspective. As a result of this analysis, it has been found out that even though rape incident is one of the worst events a woman can experience in these series, it is turned into a crime that can be forgiven and the reason of forgiving is love.

Comparison of aerobic conjunctival bacterial flora in pregnant, reproductive-aged and postmenopausal women

AIM: To evaluate the effect of hormonal status on aerobic conjunctival flora in women. METHODS: One hundred fifty-eight women [reproductive-aged (n=55), pregnant (n=51), and postmenopausal (n=52)] who admitted to outpatient clinic of Obstetrics and Gynecology Department of Denizli State Hospital were enrolled. Age, body-mass index (BMI), obstetric history, cigarette smoking, drug usage, presence of systemic disease, and intraocular pressure (IOP) were recorded for each patient. The samples were taken from the lower fornix with two culture swabs and directly incubated in culture containing 5% sheep blood, eosin-methylene blue and chocolate agar. The other swab specimen was Gram stained. All growths and microscopic results were analyzed. RESULTS: The coagulase-negative Staphylococcus was the predominant organism isolated in the conjunctival samples in both three groups. The aerobic microorganism growth rate for all isolated aerobic organisms revealed no significant change in the three groups (P>0.05). The conjunctival culture positivity rates were similar in the three groups (49% in reproductive-aged, 57% in pregnant and 58% in postmenopausal women) (P>0.05). Age, IOP, BMI, gravidity, parity, cigarette smoking, drug usage, and presence of systemic diseases did not have an effect on culture positivity in three groups. COnCLUSIOn: Results of this study showed that conjunctival aerobic flora and bacterial colonization did not differ between reproductive-aged, pregnant and postmenopausal women.

A new species of Zanclognatha Lederer,1857 of the family Erebidae,*** sp.n.,one species of Family Erebidea,Asota egens(Walker,1854)and two species of Family noctuidae:Meganephria retinea Gyulai and Ronkay,1999 and Amphipyra subrigua Bremer and Grey,1853,are reported for the first time from *** addition,larval feeding habits with their host plants for three species are newly recognized from *** of adults,larvae for Zanclognatha querciella and Meganephria retinea,and the genitalia of male or female for all species are provided.

Climate change adaptation, greenhouse gas mitigation and economic profitability of conservation agriculture: Some examples from cereal systems of Indo-Gangetic Plains

Achieving sustainability of the cereal system in the Indo-Gangetic Plains（IGP）of India under progressive climate change and variability necessitates adoption of practices and technologies that increase food production,adaptation and mitigation in a sustainable *** paper examines conservation agriculture（CA）from the perspective of：（i）increased yield and farm income,（ii）adaptation to heat and water stresses,and（iii）reduction in greenhouse gas（GHGs）*** analyses and conclusions are based on the literature and evidences from a large number of on-station as well as farmers’field trials on CA in the cereal systems of *** analyses show that CA-based system substantially reduces the production cost（up to 23%）but produces equal or even higher than conventional system;thereby increasing economic profitability of production ***-based production systems also moderated the effect of high temperature（reduced canopy temperature by 1–4°C）and increased irrigation water productivity by 66–100%compared to traditional production systems thus well adapting to water and heat stress situations of *** continuous monitoring of soil flux of CO2,n2O and CH4 revealed that CA-based rice-wheat systems emit 10–15%less GHGs than conventional *** is the first time that CA and its components are synthesized and analyzed from food security-climate change *** this holistic analysis,we suggest that wide-scale promotion of suitable CA practices by integrating into national agriculture development strategy is a way forward to address food security,climate change adaptation and mitigation challenges faced by present agriculture.

Crescentic glomerulonephritis in children:a single centre experience

Background:Crescentic glomerulonephritis(CsGn)is characterized by crescents in 50%or more of glomeruli and clinically by a sudden and progressive decline in renal ***:We evaluated the etiology,clinical features,prognostic factors and long-term outcome of *** January 2000 and December 2010,45 children(26 girls,19 boys)with biopsy-proven CsGn(>50%crescents)were investigated ***:The mean age of the patients was 130.86±33.77 *** mean duration of symptoms prior to diagnosis was 26±12 days(4-40 days).Most of the children had hypertension(62.2%),macroscopic hematuria(73.3%),oligoanuria(44.4%),edema(51.1%)and purpuric rash(40%)at *** final clinical status of the patients was complete remission(n=21),partial remission(n=5)or chronic kidney disease(n=19).Adverse outcomes were significantly associated with a long duration between the onset of symptoms and treatment(P=0.038),the presence of oligoanuria(P=0.006),a severe decreased glomerular filtration rate(GFRn/1.73m^(2))and the need for dialysis(P=0.003)on admission,the ratio of crescents(>75%)(P=0.03),and the ratio of fi brous crescents(P=0.015).Conclusion:The outcome of CsGn in children continues to be poor,and it should be treated as a renal emergency.

AIM: To investigate variants of immunity-related GT-Pase family M (IRGM) and nKX2-3 genes and genotype-phenotype in Eastern European patients with inflammatory bowel disease (IBD).METHODS: We analyzed 1707 Hungarian and Czech subjects with Crohn’s disease (CD) (n = 810, age: 37.1 ± 12.6 years, duration: 10.7 ± 8.4 years) and ulcerative colitis (UC) (n = 428, age: 43.7 ± 15.0 years, duration: 12.6 ± 9.9 years), as well as 469 healthy controls. IRGM rs13361189, nKX2-3 rs10883365 and ECM1 rs13294 polymorphisms were tested by LightCy-cler allele discrimination. Detailed clinical phenotypes were determined by reviewing the medical charts. RESULTS: nKX2-3 rs10883365 variant allele was as-sociated with increased risk for CD (P = 0.009, OR = 1.24, 95% CI = 1.06-1.48) and UC (P = 0.001, OR = 1.36, 95% CI = 1.13-1.63), whereas variant IRGM allele increased risk for CD (P = 0.029, OR = 1.36, 95% CI = 1.03-1.79). In contrast, ECM1 rs13294 was not associat-ed with either CD or UC. In CD, the variant IRGM allele was associated with a colon-only location (P = 0.02, OR = 1.62, 95% CI = 1.07-2.44), whereas in UC, the ECM1 variant was associated with cutaneous manifestations (P = 0.002, OR = 3.36, 95% CI = 1.48-7.63). Variant alleles did not predict resistance to steroids or azathio-prine, efficacy of infliximab, or need for surgery. COnCLUSIOn: nKX2-3 and IRGM are susceptibility locifor IBD in Eastern European patients. Further studies are needed to confirm the reported phenotype-genotype associations.

Aim: To investigate the effect of melatonin on ischemia–reperfusion (I/R) injury in rat ovary. Material and Method: A total of 24 Wistar rats were divided into four groups. The rats were first numbered randomly and then randomly divided into four equal groups: sham, torsion, detorsion and melatonin groups. In group I (n = 6) sham, as well as in group II ovary torsion was not performed and no drug was administered. In group III, 1 hour of ischemia and 2 hours of reperfusion were performed and no drug was given. In group IV, melatonin was dissolved in 1% ethanol just before use. Melatonin was injected 1 hour before torsion to the torsion plus melatonin group. Right ovaries were surgically removed in all groups. Result: Malondialdehyde (MDA) levels were measured. We suggested that administration of melatonin would be in treating ovaries from torsion/detorsion induced damage in humans. The histopathological changes such as vascular congestion, edema, hemorrhage, and follicular degeneration were found to be decreased in the melatonin + ischemia-reperfusion group. Ischemia-reperfusion group: PECAM-I expression is positive in vessel of the stromal area. The follicles and interfolicular area infiltrated with CD68 positive cells were increased in a time of ischemia-reperfusion exposure dependent manner. Conclusions: Melatonin after ischemia-reperfusion CD31 and CD68 expressions is weak, due to the reduction of the inflammatory effects and hemorrhage is thought that melatonin is effective.

目的實行由護士主導的經腹股溝血管行心導管手術病人術後傷口院外跟進計劃,縮短病人因傷口問題延長住院時間。方法於2018年9月~2019年4月實行術後傷口院外跟進計劃(跟進組n=27),内容包括:出院前病人及家屬傷口護理指導、派發健教單張、安排兩次出院後回病區傷口跟進服務(常規出院後第3天及第7天)或兩次傷口外展服務(行動不便病人),對比2018年1月~2018年8月該類病人住院時間(對照組n=25),觀察計劃效果。結果兩組於年齡、性別、術式均沒有顯著性差異(P>0.05),術後平均住院時間:跟進組為4.4±1.8天、對照組為6.4±2.2天,具有顯著性差異(P<0.01),跟進組病人均未有發生手術部位感染,住院組發生1例手術部位感染。結論經護士主導的傷口院外跟進計劃能有效減少病人術後住院時間。
