Incidence and Outcomes of Surgical Site Infections after Adult Cardiac Surgery: A Single-Center Experience

Background: Surgical site infections (SSIs) remain a challenging medical problem, especially in cardiac surgery patients. There is a lack of studies evaluating the rate of and outcomes of SSIs following cardiac surgeries in Saudi Arabia. Aims: This study aimed to determine the incidence of SSIs after adult cardiac surgeries that were done in Madinah Cardiac Center, Saudi Arabia. Further, to identify the outcomes and risk factors contributing to death among surgical site infection patients. Methods: This was a 6-year, single-center, retrospective cohort study that included 93 consecutive patients who underwent adult cardiac surgery between August 2016 and August 2022. All adult patients aged 18 years or older who had clinical evidence of postoperative surgical wound infection were included. Preoperative, operative, and postoperative data (early and late) were collected from medical records. Microbiological culture reports and clinical outcomes were also recorded. Results: The study revealed a 9.1% (93/1021) incidence rate of SSIs. Of the 93 patients with SSIs, 60 had superficial incisional infections and 33 had deep infections with incidence rates of 5.9% and 3.2%, respectively. In-hospital mortality due to SSI was recorded in 4 out of 93 patients with an incidence rate of 4.3%. There was a significant association between the in-hospital mortality and the type of SSIs (p = 0.014). All non-survivors had deep SSIs. The type of cardiac surgery also showed a significant association with the in-hospital mortality (p = 0.017). Furthermore, the median duration of antibiotic administration was significantly longer in the non-survivors than in the survivors (72.5 vs 17, respectively, p Conclusions: In conclusion, the incidence of surgical wound infections following cardiac surgery is not low (9.1%);of which 3.2% were deep infections. Our previous in vitro and ex vivo experiments show that selected probiotic lactic acid bacteria can bind aflatoxin B (AFB). We propose that these bacteria, given to subjects exposed to AFs via diet, will reduce the absorption of dietary AFs from the intestine. Organic Rankine Cycle Based Geothermal Energy for Power Generation in Egypt

Low temperature geothermal resources are located in many areas and represent a high potential energy resource. One of the most common technologies, efficient and to exploit this type of resource is the binary cycle technology. Organic Rankine Cycle (ORC) is one of the main types of binary cycles. electricity generation from low enthalpy geothermal energy using ORC is a talented technology. This paper addresses the design of binary cycle power plant utilizing one of the low temperature geothermal resource of temperature 92°C using four alternative working fluids: Butane, Isobutane, Pentane and 1,1,1,3,3-Pentafluoropropan (R245fa). Bir Nabi is the well under consideration which located in the Eastern desert, Egypt. Three operation parameters: geothermal temperature, reinjection temperature and geothermal flow rate are taken into consideration to analyze the performance of the power plant for different fluids. A performance analysis is conducted on ORC binary power plant using MATLAB programming to study the variation of output power and efficiency with the operation parameters. Also, the effect of these parameters on the area of ORC binary cycle power plant components;preheater, evaporator and condenser is presented. The geothermal resources temperatures are in the range of 90°C to130°C, the mass flow rate of the geothermal fluid ranges between 10 kg/s and 50 kg/s and reinjection temperature ranges from 30°C to 70°C. The results indicate that, the highest output power and plant efficiency are obtained with Pentane.

In this study,CdS/ZnO(2:3 mol%)thin films are successfully deposited on quartz substrates by using the sputtering *** images on the structural and optical characteristic features of CdS/ZnO thin films before and after annealing are *** CdS/ZnO thin films are annealed respectively at temperatures of 373 K,473 K,and 573 K and the structural features are examined by XRD,ATR-FTIR,and *** optical properties of CdS/ZnO thin films such as refractive indices,absorption coefficients,optical band gap energy values,Urbach energy values,lattice dielectric constants,and high frequency dielectric constants are determined from spectrophotometer data recorded over the spectral range of 300 nm-2500 *** parameters are investigated by using a single-oscillator *** spectra of CdS/ZnO thin films show an overall decrease in their intensity peaks after *** third-order nonlinear optical parameter,and nonlinear refractive index are also estimated.

The quality of five commercial naphthalene sulfonated superplasticizers are assessed on the basis of DTA, EDAX, XRD, FTIR and HPLC methods of analyses, as well as their chemical and physical properties. The slump behavior of Portland cements with varying C3 A/C4 AF ratio is determined using the five admixtures. For the superplasticized concrete, the slump loss, initial setting time and up to 90-day compressive strength values are reported. It is found that, the polymer characteristics of the superplasticizers affect greatly the workability of the cement pastes and concrete. Also increasing the ferrite content in cement leads to the elimination of the differences of the polymer characteristics and causes a good fluidity of the pastes irrespective of the quality of the superplasticizer.

This paper deals with the Bayesian inferences of unknown parameters of the progressively Type II censored Weibull-geometric (WG) distribution. The Bayes estimators cannot be obtained in explicit forms of the unknown parameters under a squared error loss function. The approximate Bayes estimators will be computed using the idea of Markov Chain Monte Carlo (MCMC) method to generate from the posterior distributions. Also the point estimation and confidence intervals based on maximum likelihood and bootstrap technique are also proposed. The approximate Bayes estimators will be obtained under the assumptions of informative and non-informative priors are compared with the maximum likelihood estimators. A numerical example is provided to illustrate the proposed estimation methods here. Maximum likelihood, bootstrap and the different Bayes estimates are compared via a Monte Carlo Simulation

L-Arginine Supplementation Alters Maternal Blood Biochemical Attributes and Milk Composition Relative to Neonatal Traits of <i>Najdi</i>Ewes

In a 2 × 3 factorial arrangement, 3 levels of L-arginine (0, 75 and 150 mg/kg BW per day) were tested at two stages of pregnancy (Early 60 days vs. Late 60 days) on the maternal blood metabolites and postnatal milk composition relative to the newborn traits of Najdi ewes. Thirty adult pregnant Najdi ewes were randomly assigned into 6 treatments (n = 5). Control early (CE) ewes were orally administered 50 mL normal saline (0.9% NaCl) daily during the first 60 days of pregnancy;control late (CL) ewes were similar to CE except saline was given at the last 60 days of pregnancy;low dose of L-arginine at early pregnancy (LE);low dose of L-arginine at late pregnancy (LL);high dose of L-arginine at early (HE) and high dose of L-arginine at late pregnancy (HL). Blood and milk samples were collected on a weekly basis. Data of lambing and newborn traits were considered. There were non-significant differences in litter size due to L-arginine;however lamb birth and weaning weights increased by 35% in LE-ewes. Arginine did not change blood protein and albumin. Low arginine decreased lipid fractions, creatinine, urea, and milk fat;however, it increased AST, ALT, milk lactose and milk ash. Administration of L-arginine at 75 mg/Kg BW per day for 60 days at an early gestation in ewes improved the maternal body health and milk composition for the welfare of the neonates.

Natural Products Modulate the Multifactorial Multidrug Resistance of Cancer

Multidrug resistance (MDR) is a critical problem in cancer chemotherapy. Cancer cells can develop resistance not only to a single cytotoxic drug, but also to entire classes of structurally and functionally unrelated compounds. Several mechanisms can mediate the development of MDR, including increased drug efflux from the cells by ABC-transporters (ABCT), activation of metabolic enzymes, and defective pathways towards apoptosis. Many plant secondary metabolites (SMs) can potentially increase sensitivity of drug-resistant cancer cells to chemotherapeutical agents. The present thesis investigates the modulation of MDR by certain medicinal plants and their active compounds. The inhibition of ABCTs (P-gp/MDR1, MRP1, BCRP) and metabolic enzymes (GST and CYP3A4), and the induction of apoptosis are useful indicators of the efficacy of a potential medicinal drug. The focus of this study was the possible mechanisms of drug resistance including: expression of resistance proteins, activation of metabolic enzymes, and alteration of the apoptosis and how to overcome their resistance effect on cancer cells. The overall goal of this review was to evaluate how commonly used medicinal plants and their main active secondary metabolites modulate multidrug resistance in cancer cells in order to validate their uses as anticancer drugs, introduce new therapeutic options for resistant cancer, and facilitate the development of their anticancer strategies and/or combination therapies. In conclusion, SMs from medicinal plants exhibit multitarget activity against MDR-related proteins, metabolic enzymes, and apoptotic signaling, this may help to overcome resistance towards chemotherapeutic drugs.

Effect of prolonged intake of iron enriched diet on testicular functions of experimental rats

Iron deficiency anemia represents a common nutritional problem which affects many societies allover the world and iron fortified diet has been suggested as one of possible tools to combat and solve such problem. Present study was designed to illustrate the effect of dietary iron intake on certain biochemical markers dealing with oxidative stress, inflammatory response and cellular alterations of testicular tissues. Adult male rats which were fed on biscuits fortified with iron (0.3% ferrous sulfate) daily for 10 weeks (iron group) showed increased serum iron, ferritin, tumor necrosis factoralpha (TNF-α), nitric oxide (NO) and decreased Testosterone level (p elial cells. Apparent reduction in number of sperms and spermatogenic cells were also observed. These symptoms may demonstrate that prolonged intake of Biscuit fortified with iron causes certain testicular damage through certain mechanism.
