Effect of mucin production on survival in colorectal cancer:A case-control study

AIM: To investigate the impact of mucin production on prognosis in colorectal cancer, in terms of overall survival (OS) and time to disease progression (TTP) in patients with mucinous compared to those with non-mucinous colorectal cancer (nMCRC), matched for age, gender, and tumor ***: Thirty five patients with mucinous colorectal cancer (MCRC) were matched for age, gender, and tumor stage with 35 controls having nMCRC. OS and TTP were compared among 4 groups divided according to mucin content: group A (50%-75% mucin), group B (75%-100% mucin), group C or controls (n). Group D consisted of all patients with tumors having n (controls and groups A together).RESULTS: Median survival in MCRC and nMCRC groups was 46.2 and 112.9 mo, respectively (P = 0.26). OS in groups A and B was 70.1 and 32.8 mo (P = 0.46), and in groups B and D was 32.8 and 70.1 mo, respectively (P = 0.143). TTP in MCRC and nMCRC was 50.17 and 44.77 mo, respectively (P = 0.795). TTP in groups A, B, and D was 70.1, 24.8, and 65.5 mo, respectively. Twenty-eight percent of patients with MCRC had poorly differentiated adenocarcinoma versus 8.6% in nMCRC patients (P = 0.028).COnCLUSIOn: MCRC is associated with a non-significant decrease in median survival and TTP, particularlywhen mucin content is > 75% of tumor volume. However, it tends to be more poorly differentiated. A larger study matching for stage and grade is needed.

Prognostic factors in patients with advanced cholangiocarcinoma: Role of surgery, chemotherapy and body mass index

AIM: To study the factors that may affect survival of cholangiocarcinoma in Lebanon. METHODS: A retrospective review of the medical records of 55 patients diagnosed with cholangio- carcinoma at the American University of Beirut between 1990 and 2005 was conducted. Univariate and multivariate analyses were performed to determine the impact of surgery, chemotherapy, body mass index, bilirubin level and other factors on survival. RESULTS: The median survival of all patients was 8.57 mo (0.03-105.2). Univariate analysis showed that low bilirubin level (nd chemotherapy administration were significantly associated with better survival (P = 0.012, 0.038 and 0.038, respectively). In subgroup analysis on patients who had no surgery, chemotherapy administration prolonged median survival significantly (17.0 mo vs 3.5 mo, P = 0.001). Multivariate analysis identifiedonly low bilirubin level nd chemotherapy administration as independent predictors associated with better survival (P nCLUSIOn: Our data show that palliative and postoperative chemotherapy as well as a bilirubin level ndependent predictors of a significant increase in survival in patients with cholangiocarcinoma.

Solving nonlinear Stochastic Diffusion Models with nonlinear Losses Using the Homotopy Analysis Method

This paper deals with the construction of approximate series solutions of diffusion models with stochastic excitation and nonlinear losses using the homotopy analysis method (HAM). The mean, variance and other statistical properties of the stochastic solution are computed. The solution technique was applied successfully to the 1D and 2D diffusion models. The scheme shows importance of choice of convergence-control parameter to guarantee the convergence of the solutions of nonlinear differential Equations. The results are compared with the Wiener-Hermite expansion with perturbation (WHEP) technique and good agreements are obtained.

Background: The response rates and duration of peginterferon alpha (PEG-IFn-α) and ribavirin combination therapy in chronic hepatitis C genotype 4, the prevalent genotype in the Middle East and Africa, are poorly documented. Aims: To compare the efficacy and safety of 24, 36, or 48 weeks of PEG-IFn-α-2b and ribavirin therapy in chronic hepatitis C genotype 4. Methods: In this prospective, randomised, double blind study, 287 patients with chronic hepatitis C genotype 4 were randomly assigned to PEG-IFn-α-2b (1.5 μg/kg) once weekly plus daily ribavirin (1000-1200 mg) for 24 weeks (group A, n = 95), 36 weeks (group B, n = 96), or 48 weeks (group C, n = 96) and followed for 48 weeks after completion of treatment. Early viral kinetics and histopathological evaluation of pre-and post treatment liver biopsies were performed. The primary end point was viral clearance 48 weeks after completion of treatment. Results: Sustained virological response was achieved in 29%, 66%, and 69%of patients treated with PEG-IFn-α-2b and ribavirin for 24, 36, and 48 weeks, respectively, by intention to treat analysis. no statistically significant difference in sustained virological response rates was detected between 36 and 48 weeks of therapy (p = 0.3). Subjects with sustained virological response showed greater antiviral ef-ficacy(e) and rapid viral load decline from baseline to treatment week 4 compared with non-responders and improvement in liver histology. The incidence of adverse events was higher in the group treated for 48 weeks. Conclusion: PEG-IFn-α-2b and ribavirin for 36 or 48 weeks was more effective in the treatment of chronic hepatitis C genotype 4 than treatment for 24 weeks. Thirty six week therapy was well tolerated and produced sustained virological and histological response rates similar to the 48 week regimen.

Objective: To evaluate clinical, genetic, and electrophysiologic features of patients with Andersen-tawil syndrome (ATS) in the United Kingdom. Methods: Clinical and neurophysiologic evaluation was conducted of 11 families suspected to have ATS. Molecular genetic analysis of each proband was performed by direct DnA sequencing of the entire coding region of KCnJ2. Control samples were screened by direct DnA sequencing. The electrophysiologic consequences of several new mutations were studied in an oocyte expression system. Results: All 11 ATS families harbored pathogenic mutations in KCnJ2 with six mutations not previously reporte d. Some unusual clinical features including renal tubular defect, CnS involvemen t, and dental and phonation abnormalities were observed. Five mutations (T75M, D 78G, R82Q, L217P, and G300D) were expressed, all of which resulted in nonfunctio nal channels when expressed alone, and co-expression with wild-type (WT) KCnJ2 demonstrated a dominant negative effect. Conclusion: Six new disease-causing m utations in KCnJ2 were identified, one of which was in a PIP2 binding site. Mole cular expression studies indicated that five of the mutations exerted a dominant negative effect on the wild-type allele. KCnJ2 mutations are an important caus e of ATS in the UK.
