Clinicopathological features and surgical outcome of patients with fibrolamellar hepatocellular carcinoma(experience with 22 patients over a 15-year period)

AIM To evaluate the clinicopathological features and the surgical outcomes of patients with fibrolamellar hepato-cellular carcinoma(FL-hCC)over a 15-year period. METhODS This is a retrospective study including 22 patients with a pathologic diagnosis of FL-hCC who underwent hepatectomy over a 15-year period. Tumor characteristics,survival and recurrence were evaluated. RESULTS There were 11 male and 11 female with a median age of 29 years(range from 21 to 58 years). Two(9%)patients had hepatitis C viral infection and only 2(9%)patients had alpha-fetoprotein level > 200 ng/m L. The median size of the tumors was 12 cm(range from 5-20 cm). Vascular invasion was detected in 5(23%)patients. Four(18%)patients had lymph node metastases. The median follow up period was 42 mo and the 5-year survival was 65%. Five(23%)patients had a recurrent disease,4 of them had a second surgery with 36 mo median time interval. Vascular invasion is the only significant negative prognostic factor CONCLUSION FL-hCC has a favorable prognosis than common hCC and should be suspected in young patients with non cirrhotic liver. Aggressive surgical resection should be done for all patients. Repeated hepatectomy should be considered for these patients as it has a relatively indolent course.

In this research work,sol-gel technique was employed to prepare the strontium based spinel ferrite nanoparticles（SrFe2O4） with different ratios of terbium（Tb）.Different characterization techniques were used to investigate the structural,morphological,dielectric and magnetic properties of the prepared samples.X-ray diffraction（XRD） result suggests that face-centered cube spinel nanocrystalline structure is *** size of the SrFe2O4decreases with rising of Tb *** morphology,shape and size of the SrFe2O4were examined by scanning electron microscopy（SEM） analysis and results reveal inhomogeneous distributions of the nanostructures with high *** electrical resistivity of the SrFe2O4increases with rising of Tb ratio,which is confirmed from the cyclic *** is observed that dielectric constant of all the samples decreases with the increasing frequency *** is determined that the dielectric constants of the spinel ferrites are frequency dependent and decrease with increasing the frequency of applied electric *** magnetic behavior of SrFe2O4with different ratios of Tb was studied and it is found that the saturation magnetization values of samples decrease with increase in the substitution of Tb3+at octahedral sites for Fe3+.This decrease in the values of Msis also attributed to spin at surface of nanoparticles.

With the increase of energy consumption worldwide in several domains such as industry,education,and transportation,several technologies played an inﬂuential role in energy conservation such as the Internet of Things(IoT).In this article,we describe the design and implementation of an IoT-based energy conser-vation smart classroom system that contributes to energy conservation in the edu-cation *** proposed system not only allows the user to access and control IoT devices(e.g.,lights,projectors,and air conditions)in real-time,it also has the capability to aggregate the estimated energy consumption of an IoT device,the smart classroom,and the building based on the energy consumption and cost model that we ***,the proposed model aggregates the estimated energy cost according to the Saudi electricity Company(SEC)***,the model aggregates in real-time the estimated energy conservation percentage and estimated money-saving percentage compared to data collected when the system wasn't *** feasibility and beneﬁts of our system have been validated on a real-world scenario which is a classroom in the college of computer science and engineering,Taibah University,Yanbu *** results of the experimental studies are promising in energy conservation and cost-saving when using our proposed system.

Information security is one of the most important methods of protecting the confidentiality and privacy of internet *** greater the volume of data,the more the need to increase the security methods for protecting data from *** task can be challenging for researchers in terms of managing enormous data and maintaining their safety and *** of digital content is a major issue in maintaining the privacy and secrecy of *** this end,digital watermarking is based on the concept of information security through the insertion and detection of an embedded watermark in an efficient *** methodologies in the research on digital watermarking result in data distortion after embedding the *** scenario can reduce the efficiency of detecting watermarks as well as violate data *** study adapts a robust reversible histogram shifting(hS)technique for textual data in relational ***,the study presents a digital watermarking scheme intended for document copyright protection and proof of *** major advantage of the proposed watermarking scheme is that it can protect digital data and preserve data *** the best of our knowledge,this research methodology is promising for use in the hS expansion model for watermarking data on nonnumeric *** addition,the results showed that despite exposing the database to an insertion attacks at 50%and 75%of the watermark,the proposed algorithm can recovermore than half of the embedded watermark in all addition and alteration attacks *** a result,the watermark information can be retained and restored completely.

Background: The efficacy of octreotide to prevent postoperative pancreatic fistula(POPF) of pancreaticoduodenectomy(PD) is still controversial. This study aimed to evaluate the effect of postoperative use of octreotide on the outcomes after ***: This is a prospective randomized controlled trial for postoperative use of octreotide in patients undergoing PD. Patients with soft pancreas and pancreatic duct he primary end of the study is to compare the rate of ***: A total of 104 patients were included in the study and were divided into two randomized *** were no significant difference in overall complications and its severity. POPF occurred in 11 patients(21.2%) in group I and 10(19.2%) in group II, without statistical significance(P = 0.807). Also, there was no significant differences between both groups regarding the incidence of biliary leakage(P = 0.083), delayed gastric emptying(P = 0.472), and early postoperative mortality(P = 0.727).Conclusions: Octreotide did not reduce postoperative morbidities, reoperation and mortality rate. Also, it did not affect the incidence of POPF and its clinically relevant variants.
