Association of the glycemic background patterns and the diabetes management efficacy in poorly controlled type 2 diabetes

BACKGROUnD Inadequate glycemic control in patients with type 2 diabetes(T2DM)is a major public health problem and a significant risk factor for the progression of diabetic *** To evaluate the effects of intensive and supportive glycemic management strategies over a 12-month period in individuals with T2DM with glycated hemoglobin(HbA1c)≥10%and varying backgrounds of glycemic *** This prospective observational study investigated glycemic control in patients with poorly controlled T2DM over 12 *** were categorized into four groups based on prior glycemic history:newly diagnosed,previously well controlled with recent worsening,previously off-target but now worsening,and HbA1c consistently above 10%.HbA1c levels were monitored quarterly,and patients received medical,educational,and dietary support as *** analysis focused on the success rates of good glycemic control and the associated factors within each *** The study showed significant improvements in HbA1c levels in all *** most significant improvement was observed in individuals newly diagnosed with diabetes:65%achieved an HbA1c target of≤7%.The results varied between participants with different glycemic control histories,followed by decreasing success rates:39%in participants with previously good glycemic control,21%in participants whose glycemic control had deteriorated compared to before,and only 10%in participants with persistently poor control,with mean HbA1c levels of 6.3%,7.7%,8.2%,and 9.7%,*** one year,65.2%of the“newly diagnosed patients”,39.3%in the“previously controlled group”,21.9%in the“previously off-target but now worsened'”group and 10%in the“poorly controlled from the start”group had achieved HbA1c levels of 7 and *** In poorly controlled diabetes,the rate at which treatment goals are achieved is associated with the glycemic background characteristics,emphasizing the need for tailored ***

Autoimmune enteropathy(AIe)is a rare immune mediated disorder primarily affecting children,characterized by chronic diarrhea,malabsorption,vomiting,weight loss and villous *** has also been observed in adults presenting diagnostic and treatment challenges due to its overlap with other gastrointestinal disorders such as celiac *** diagnostic criteria for AIe include small bowel villous atrophy,lack of response to dietary restrictions,presence of anti-enterocyte antibodies,and predisposition to autoimmunity without severe *** criteria emphasize characteristic histological findings and exclusion of other causes of villous *** is associated with various autoimmune disorders and can present with overlapping features with Celiac disease,including villous atrophy but without significant intraepithelial *** primarily involves immunosuppression using corticosteroids,calcineurin inhibitors,and anti-tumor necrosis factor therapy,alongside nutri-tional *** the challenges,understanding AIe’s diverse manifest-ations and improving diagnostic criteria are essential for effective management and improved patient *** research is needed to elucidate the pathogenesis,disease progression and long-term outcomes of AIe.

Simplified seismic scenario analysis of existing masonry buildings accounting for local site effects

This study discusses the effects of local sites and hazard amplification on the seismic vulnerability assessment of existing masonry *** this context,a rapid seismic evaluation procedure was implemented on an old masonry building stock in the historical center Galata,located inİstanbul,to determine the seismic risk priority of the built *** scenarios were generated for all soil classes,different moment magnitudes,and source-to-site distances to obtain more accurate results for the seismic vulnerability assessment of the studied building ***,damage distributions estimated under nine different scenarios with/without site effects were compared and illustrated in maps to discuss changes in vulnerability owing to amplification *** this study,by re-examining the rapid seismic evaluation procedure by including geo-hazard-based assessment,the importance of site effects on the vulnerability and risk assessment of built heritage was *** proposed framework integrating field data and local site effects is believed to advance the current applications for vulnerability assessment of masonry buildings and provide an improvement in the application of rapid seismic assessment procedures with more reliable results.

Traumatic brain injury is a global health crisis,causing significant death and disability *** that follows traumatic brain injury has serious consequences for neuronal survival and cognitive impairments,with astrocytes involved in this *** traumatic brain injury,astrocytes rapidly become reactive,and astrogliosis propagates from the injury core to distant brain *** astroglial proteins are downregulated near the traumatic brain injury core,while pro-inflammatory astroglial genes are *** altered gene expression is considered a pathological remodeling of astrocytes that produces serious consequences for neuronal survival and cognitive *** addition,glial scar formed by reactive astrocytes is initially necessary to limit immune cell infiltration,but in the long term impedes axonal reconnection and functional *** therapeutic strategies for traumatic brain injury are focused on preventing acute ***,cannabinoids,progesterone,beta-blockers,and cerebrolysin demonstrate neuroprotective benefits but most of them have not been studied in the context of *** this review,we discuss the cell signaling pathways activated in reactive astrocytes following traumatic brain injury and we discuss some of the potential new strategies aimed to modulate astroglial responses in traumatic brain injury,especially using cell-targeted strategies with miRnAs or lncRnA,viral vectors,and repurposed drugs.

Hematological profile of COVID-19 infected children before and after the spread of the Omicron variant in Istanbul

Objective:To examine the effect of the COVID-19 virus,especially the Omicron variant,on hematological parameters of hospitalized pediatric patients during the COVID-19 ***:Medical records of pediatric COVID-19 patients hospitalized at Kartal Dr.Lütfi K?rdar City Hospital in Istanbul,Turkey,between March 2020 and May 2022 were retrospectively reviewed to analyze data regarding demographics,SARS-Co V-2 infection polymerase chain reaction(PCR)test results,reversetranscriptase(RT)-PCR for other respiratory agents,duration of hospital stay,and hematological and biochemical laboratory ***:Out of 467 children with a confirmed diagnosis of SARSCo V-2 infection,94(20.1%)had Omicron infection and 373(79.9%)were infected with other variants;the Omicron group had younger patients than the remaining samples(Pe most frequent clinical symptoms in all children were cough(53.5%)and fever(32.3%),followed by vomiting(20.8%).Lung involvement in the Omicron group(10.6%)was significantly lower than in the remaining samples(29.8%)(Pemoglobin and lymphocyte levels were lower in the Omicron-infected group(both Pe prothrombin time,activated partial thromboplastin time,international normalized ratio,and D-dimer levels were significantly higher in this group(Pnd P=0.023,respectively).In terms of lung involvement,those with lung involvement were significantly older(Pnclusions:Although lung involvement was less common with Omicron infection,this group had greater hematological system involvement,such as anemia,lymphopenia,D-dimer elevation,and coagulation disorders.

F4-TCnQ concentration dependence of the current voltage characteristics in the Au/P3HT:PCBM:F4-TCnQ/n-Si(MPS) Schottky barrier diode

In this study, we investigate some main electrical parameters of the gold/poly（3-hexylthiophene）：[6,6]-phenyl C61 bu- tyric acid methyl ester：2,3,5,6-tetrafluoro-7,7,8,8-tetracyanoquinodimethane/n-type silicon （Au/P3HT：PCBM：F4-TCnQ/n- Si） metal-polymer-semiconductor （MPS） Schottky barrier diode （SBD） in terms of the effects of F4-TCnQ concentration （0%, 1%, and 2%）. F4-TCnQ-doped P3HT：PCBM is fabricated to figure out the p-type doping effect on the device per- formance. The main electrical parameters, such as ideality factor （n）, barrier height （ФB0）, series resistance （Rs）, shunt resistance （Rsh）, and density of interface states （nss） are determined from the forward and reverse bias current-voltage （l-V） characteristics in the dark and at room temperature. The values of n, Rs, ФB0, and nss are significantly reduced by using the 1% F4-TCnQ doping in P3HT：PCBM：F4-TCnQ organic blend layer, additionally, the carrier mobility and current are increased by the soft （1%） doping. The most ideal values of electrical parameters are obtained for 1% F4-TCnQ used diode. On the other hand, the carrier mobility and current for the hard doping （2%） become far away from the ideal diode values due to the unbalanced generation of holes/electrons and doping-induced disproportion when compared with 1% F4-TCnQ doping. These results show that the electrical properties of MPS SBDs strongly depend on the F4-TCnQ doping and doping concentration of interfacial P3HT：PCBM：F4-TCnQ organic layer. Moreover, the soft F4-TCnQ dop- ing concentration （1%） in P3HT：PCBM：F4-TCnQ organic layer significantly improves the electrical characteristics of the Au/P3HT：PCBM：F4-TCnQ/n-Si （MPS） SBDs which enables the fabricating of high-quality electronic and optoelectronic devices.
