Epidemiological, Clinical Aspects and Outcome of Measles in a Low-Income Country in 2023

Introduction: Measles remains a worrying health problem in sub-Saharan African countries. There have been measles outbreaks in Cameroon with the most recent occurring between October 2022 and September 2023. The Centre region of Cameroon was the most affected in the country and being at the frontline of the fight against this illness, we conducted this study in order to determine the epidemiological and clinical characteristics as well as the outcome of children who contracted the measles and were hospitalized at the Yaoundé Gynaeco-Obstetric and Pediatric Hospital. Methodology: We conducted a cross-sectional descriptive and prospective study for one year from October 2022 to November 2023 corresponding to the duration of the outbreak. We included all children admitted in the pediatric unit for measles and its complications during this period. Results: In total, 60 children were enrolled. Their caregivers were mostly their mothers who had a mean age of 34.71 ± 9.55 years living in significant precarious conditions for the majority. The median age of children was 16 months (09 - 30 months). Twenty six percent of children (16) were less than 9 months. Girls were predominant (55%). Most children were not up to date with their routine EPI vaccination (80%) and most did not receive the measles and Rubella vaccine (76.67%). Clinical manifestations, included fever, conjunctivitis, and cough. Skin rash and catarrh (98.33% and 86.67% respectively) are the case definition signs of measles. The outcome was unfavorable for 4 children. One had a neurological disorder: coma and three presented with pneumonia and severe respiratory distress (6.67%). Conclusion: Measles is still being a reality and claiming lives in our context, emphasis should be made on immunization coverage and if possible, advocacies should be formulated to decrease the age of measles vaccine administration.

Expression of matrix metalloproteinases and tissue inhibitor metalloproteinases increases in X-irradiated rat ileum despite the disappearance of CD8a T cells

AIM： To investigate their expression and activity in the rat ileum after exposure to ionizing radiation along with that of the cellular effectors and molecular mediators involved in the regulation of MMPs. METHODS： Rats were exposed to a single 10-Gy dose of X-rays delivered to the abdomen. A combination of methods, such as zymography, immunohistochemistry and real time reverse transcriptase-polymerase chain reaction, were used to localize and quantify MMPs and the molecules involved in MMP activating and inhibitory pathways （plasmin/ plasminogen, TIMPs）, CD^8＋, as well as inflammatory （interleukin （IL）-1β, IL-8, tumor necrosis factor-s, TNF-α） and fibrogenic mediators （transforming growth factor- β1-3） within ileal tissue at 1, 3, and 7 d after irradiation. RESULTS： A marked increase in MMP-2 and -14 mRNA and protein levels associated with an increased activity of MMP-2 was observed in irradiated ileal tissue. MMP-2 and -14 expression was mainly observed in inflammatory, epithelial, and mesenchymal cells, whereas a slight increase in MMP-3 expression was detected in the few infiltrating macrophages at d i after irradiation. Conversely, MMP-1, -7, and -9 mRNA levels were not found to be affected by abdominal irradiation. Irradiation was found to induce disappearance of CD^8＋ cells. Furthermore, we have observed that TNF-α and IL-1β protein levels increased 6 h after irradiation, whereas those of IL-8 only increased after 3 d and was concomitant with neutrophil infiltration. In addition, the expressions of molecules involved in MMP activating and inhibitory pathways （urokinase-type plasminogen activator andtissue-type plasminogen activator; TIMP-1, TIMP-2, and plasminogen activator-inhibitor-i） were found to be increased after abdominal irradiation. A time fractional model to represent rainfall process

This paper deals with a stochastic representation of the rainfall process. The analysis of a rainfall time series shows that cumulative representation of a rainfall time series can be modeled as a non-Gaussian random walk with a log-normal jump distribution and a time-waiting distribution following a tempered a-stable probability law. Based on the random walk model, a fractional Fokker-Planck equation （FFPE） with tempered a-stable waiting times was obtained. Through the comparison of observed data and simulated results from the random walk model and FFPE model with tempered a-stable waiting times, it can be concluded that the behavior of the rainfall process is globally reproduced, and the FFPE model with tempered a-stable waiting times is more efficient in reproducing the observed behavior.

Iterative Factors Favoring Collaboration for Interorganizational Resilience:The Case of the Greater Montréal Transportation Infrastructure

Between members of a network, interorganizational resilience is favored by effective collaboration and coordination during a crisis. The quality of that collaboration depends on various iterative factors present between these organizations before the occurrence of a *** find that these factors are iterative since collaboration factors follow a mutually reinforcing cycle: collaboration within a crisis management network is conditioned by a general agreement, which is in turn conditioned by the extent to which the institutions coordinate themselves prior to crisis. We evaluated the factors that promote collaboration between public and private organizations that manage the Greater Montréal transportation *** factors are based on adaptive management processes such as mutual agreements, common organizational culture, knowledge and financial resources, levers of power,regulations, and pressure. Crisis management coordination represents the ability to build and assess the effectiveness of common response plans to risks to which they are exposed. We show how these processes vary depending on the links between private and public organizations.

Fading indirect effects in a warming arctic tundra

Indirect interactions in food webs can strongly influence the net effect of global change on ecological communities yet they are rarely quantified and hence remain poorly understood. Using a 22-year time series, we investigated climate-induced and predator-mediated indirect effects on grazing intensity in the tundra food web of Bylot Island, which experienced a warming trend over the last two decades. We evaluated the relative effects of environmental parameters on the proportion of plant biomass grazed by geese in wetlands and examined the temporal changes in the strength of these cascading effects. Migrating geese are the dominant herbivores on Bylot Island and can consume up to 60% of the annual production of wetland graminoids. Spring North Atlantic Oscillation, mid-summer temperatures and summer abundance of lemmings （prey sharing predators with geese） best-explained annual variation in grazing intensity. Goose grazing impact increased in years with high temperatures and high lemming abundance. However, the strength of these indirect effects on plants changed over time. Grazing intensity was weakly explained by environmental factors in recent years, which were marked by a sharp increase in plant primary production and steady decrease in grazing pressure. Indirect effects do not seem to be reversing the direct positive effect of warming on wetland plants. We suggest that cascading effects on plants may lag considerably behind direct effects in vertebrate dominated arctic communities, especially where key herbivore populations are strongly affected by factors outside of the Arctic [Current Zoology 60 （2）： 189-202, 2014].

BACKGROUND Enteric-coated medications are supposed to pass intact through the gastric environment and to release the drug content into the small intestine or the *** dissolution of the enteric coating,they may appear hyperdense on computed tomography(CT).Unfortunately,few reports have been published on this topic so *** this case report,the hyperdense appearance on contrastenhanced CT of an enteric-coated mesalamine tablet was initially misinterpreted as a jejunal gastrointestinal stromal tumor(GIST).CASE SUMMARY An asymptomatic 81-year-old male patient,who had undergone laparoscopic right nephrectomy four years earlier for stage 1 renal carcinoma,was diagnosed with a jejunal GIST at the 4-year follow-up thoraco-abdominal CT *** was referred to our hub hospital for gastroenterological evaluation,and subsequently underwent 18-fluorodeoxyglucose positron emission tomography,abdominal magnetic resonance imaging,and video capsule *** of these examinations detected any lesion of the small *** reviewing all the CT images in a multidisciplinary setting,the panel estimated that the hyperdense jejunal image was consistent with a tablet rather than a *** tablet was an 800 mg delayed-release enteric-coated oral mesalamine tablet(Asacol®),which had been prescribed for non-specific colitis,while not informing the hospital *** Delayed-release oral mesalamine(Asacol®),like other enteric-coated medications,can appear as a hyperdense image on a CT scan,mimicking a small intestinal ***,adetailed knowledge of the patients’medications and a multidisciplinary review of the images areessential.

Removing harvest residues from hardwood stands affects tree growth, wood density and stem wood nutrient concentration in European beech (Fagus sylvatica) and oak (Quercus spp.)

Background:Higher exportation of harvest residues from forests due to increased demand for woody biomass,has reportedly diminished soil mineral resources and may lead to degraded tree nutrition as well as ***,as nutrients become less available in the soil,the remobilization of nutrients in biomass tissues(plant internal cycling)helps sustain tree *** study aims to quantify the impact of Removing Harvest Residues and Litter(RHRL)during five years on tree growth,wood density,and stem wood nutrient concentrations in young beech and oak forest ***:Our study found that,RHRL significantly decreased tree growth ring width by 14%,and wood density by 3%,in beech trees,in near bark *** also significantly reduced nutrient concentration in near bark and near pith areas of both studied ***,Na and S were found lower by 44%,76%,and 56%,respectively,in near bark area of beech *** near bark area of oak trees,K,Ca,Mg,Na,S,and Fe were lower by 20%,25%,41%,48%,41%,and 16%,respectively.K and Mg concentrations decreased more strongly in near pith area compared to near bark area suggesting internal translocation of these two ***:In beech trees,wood density proved to be an important factor while quantifying the effect of removing harvest residuals on tree growth and *** nutrient loss intensified the remobilization of nutrients con-tained in older tree rings(close to the pith)towards newly formed rings(close to bark).In our study,in beech trees,K was found to be the most recycled major *** results demonstrate the potential of such analysis for providing valuable insight into the effect of RHRL in premature stands on the physiological adaptive strategies of trees and an indication of soil fertility status.
