BACKGROUND A 70-year-old man with hepatitis C virus-related recurrent hepatocellular carcinoma was admitted for further diagnosis of a 1 cm iso-hyperechoic nodule in segment(S)5.CASE SUMMARY Gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid-enhanced magnetic resonance imaging(EOB-MRI)revealed the nodule in S5 with a defect at the hepatobiliary phase,hyperintensity on diffusion weighted imaging(DWI)and hypointensity on apparent diffusion coefficient(ADC)map.Contrast-enhanced computed tomography revealed hypervascularity at the early phase,and delayed contrast-enhancement was observed at the late phase.Contrast-enhanced ultrasound(US)revealed incomplete defect at the late vascular phase.Inflammatory liver tumor,lymphoproliferative disease,intrahepatic cholangiocarcinoma(small duct type)and bile duct adenoma were suspected through the imaging studies.US guided biopsy,however,showed a noncaseating hepatic sarcoid-like epithelioid granuloma(HSEG),and histopathological analysis disclosed spindle shaped epithelioid cells harboring Langhans-type multinucleated giant cells.One month after admission,EOB-MRI signaled the disappearance of the defect at the hepatobiliary phase,of hyperintensity on DWI,of hypointensity on ADC map,and no stain at the early phase.CONCLUSION That the patient had received BNT162b2 messenger RNA(mRNA)coronavirus disease 2019 vaccination 3 mo before the occurrence of HSEG,and that its disappearance was confirmed 4 mo after mRNA vaccination suggested that the drug-induced sarcoidosis-like reaction(DISR)might be induced by the mRNA vaccination.Fortunately,rechallenge of drug-induced DISR with the third mRNA vaccination was not confirmed.

Clinical utility of treatment method conversion during single-session endoscopic ultrasound-guided biliary drainage

BACKGROUND Although several techniques for endoscopic ultrasound-guided biliary drainage(EUS-BD)are available at present,an optimal treatment algorithm of EUS-BD has not yet been established.AIM To evaluate the clinical utility of treatment method conversion during single endoscopic sessions for difficult cases in initially planned EUS-BD.METHODS This was a single-center retrospective analysis using a prospectively accumulated database.Patients with biliary obstruction undergoing EUS-BD between May 2008 and April 2016 were included.The primary outcome was to evaluate the improvement in EUS-BD success rates by converting the treatment methods during a single endoscopic session.Secondary outcomes were clarification of the factors leading to the conversion from the initial EUS-BD and the assessment of efficacy and safety of the conversion as judged by technical success,clinical success,and adverse events(AEs).RESULTS A total of 208 patients underwent EUS-BD during the study period.For 18.8%(39/208)of the patients,the treatment methods were converted to another EUSBD technique from the initial plan.Biliary obstruction was caused by pancreatobiliary malignancies,other malignant lesions,biliary stones,and other benign lesions in 22,11,4,and 2 patients,respectively.The reasons for the difficulty with the initial EUS-BD were classified into the following 3 procedures:Target puncture(n=13),guidewire manipulation(n=18),and puncture tract dilation(n=8).Technical success was achieved in 97.4%(38/39)of the cases and clinical success was achieved in 89.5%of patients(34/38).AEs occurred in 10.3%of patients,including bile leakage(n=2),bleeding(n=1),and cholecystitis(n=1).The puncture target and drainage technique were altered in subsequent EUSBD procedures in 25 and 14 patients,respectively.The final technical success rate with 95%CI for all 208 cases was 97.1%(95%CI:93.8%-98.9%),while that of the initially planned EUS-BD was 78.8%(95%CI:72.6%-84.2%).CONCLUSION Among multi-step procedures in EUS-BD,guidewire manipulation appeare

Simultaneous detection of the shuttling motion of liquid metal droplets in channels under alternating pressure and capacitive sensor signals

Implementing a signal-switching mechanism for the selective use of integrated sensors and actuators plays a crucial role in streamlining the functionality of miniaturized devices.Here,a liquid metal droplet(LMD)-based signal-switching mechanism is introduced to achieve such functionality.Pressure modulation with a 100-μm spatial resolution enabled precise control of the position of the LMDs within a channel.After integrating the channel with asymmetrically structured electrodes,the effect of the shuttle-like movement of LMD on the temporal changes in the overall capacitance was investigated.Consequently,analysis of the capacitive peaks revealed the directional movement of the LMDs,enabling estimation of the position of the LMDs without direct observation.In addition,we achieved successful signal extraction from the capacitive sensor that was linked to the activated electrodes,thereby enabling selective data retrieval.The proposed signal-switching mechanism method achieved a detection accuracy of~0.1 pF.The sensor’s ability to simultaneously detect the LMD position and generate a signal underscores its significant potential for multiplexing in multisensing systems,particularly in concealed environments,such as in vivo settings.

We also extend the characterization of Cc∞omp(Rd)BMO(Rd)by Uchiyama(1978) to ■. Moreover, using this characterization, we prove the boundedness of singular and fractional integral operators on ■.The function space Lp,φ(Rd) treated in this paper covers the case that it is coincide with Lipαon one area, with BMO on another area and with the Morrey space on the other area, for example.

Acute pancreatitis is one of the most common diseases in gastrointestinal,liver and pancreatic diseases,with about 300,000 annual admissions in the United States(1).Acute pancreatitis is complicated by pancreatic necrosis in 20%,and the mortality rate is 20%to 30%once infection occurs in pancreatic necrosis(2).Severe acute pancreatitis(SAP)was purely a surgical disease in 19th century due to its poor prognosis(3),but the importance of multidisciplinary approach is increasingly recognized as shown in the recent publication of French guidelines(4)by the collaboration of multisociety of surgery,gastroenterology,digestive endoscopy,interventional radiology,intensive care and nutrition.

A Generalization of Hardy Spaces H^p by Using Atoms

Let X = （X, d,μ） The purpose of this paper is to be a space of homogeneous type in the sense of Coifman and Weiss. generalize the definition of Hardy space H^P（X） and prove that the generalized Hardy spaces have the same property as H^P（X）. Our definition includes a kind of Hardy- Orlicz spaces and a kind of Hardy spaces with variable exponent. The results are new even for the R^n case. Let （X, δ, μ） be the normalized space of （X, d, μ） in the sense of Macias and Segovia. We also study the relations of our function spaces for （X, d, μ） and （X, δ,μ）.

Comparison of imo and Humphrey field analyzer perimeters in glaucomatous eyes

AIM:To compare the imo perimeter,a new portable head-mounted perimeter unit that enables both eyes to be examined quickly and simultaneously,with the Humphrey field analyzer(HFA)perimeter to investigate correlations and their diagnostic ability in glaucomatous eyes.METHODS:The performance of the equipment in 128 glaucomatous eyes and 40 normal eyes were tested.We investigated the correlations of mean deviation,pattern standard deviation,visual field index,and the sensitivity.RESULTS:Measurements of mean deviation(r=0.886,P<0.001),pattern standard deviation(r=0.814,P<0.001),and visual field index(r=0.871,P<0.001)in both perimeters were strongly and positively correlated.The sensitivities in the imo perimeter were 80.5%for mean deviation,81.2%for pattern standard deviation,and 80.5%in visual field index;those in the HFA were 63.3%for mean deviation,74.5%for pattern standard deviation,and 80.5%for visual field index.Both perimeters demonstrated high diagnostic ability.CONCLUSION:The parameters by the imo and HFA in glaucomatous eyes show strong positive correlations with favorable sensitivity,specificity,and diagnostic ability.However,the difference between imo and HFA results increases with the increase in visual field disturbance.

Information Science Should Take a Lead in Future Biomedical Research

In this commentary, I explain my perspective on the relationship between arti cial intelligence (AI)/data science and biomedicine from a long-range retrospective view. The development of modern biomedicine has always been accelerated by the repeated emergence of new technologies. Since all life systems are basically governed by the information in their own DNA, information science has special importance for the study of biomedicine. Unlike in physics, no (or very few) leading laws have been found in biology. Thus, in biology, the “data-to-knowledge” approach is important. AI has historically been applied to bio- medicine, and the recent news that an AI-based approach achieved the best performance in an interna- tional competition of protein structure prediction may be regarded as another landmark in the eld. Similar approaches could contribute to solving problems in genome sequence interpretation, such as identifying cancer-driving mutations in the genome of patients. Recently, the explosive development of next-generation sequencing (NGS) has been producing massive data, and this trend will accelerate. NGS is not only used for “reading” DNA sequences, but also for obtaining various types of information at the single-cell level. These data can be regarded as grid data points in climate simulation. Both data science and AI will become essential for the integrative interpretation/simulation of these data, and will take a leading role in future precision medicine.

Autism spectrum disorder(ASD) is gathering concerns in socially developed countries. ASD is a neuropsychiatric disorder of genetic origin with high prevalence of 1%–2%. The patients with ASD characteristically show impaired social skills. Today, many genetic studies identify numerous susceptible genes and genetic loci associated with ASD. Although some genetic factors can lead to abnormal brain function linked to ASD phenotypes, the pathogenic mechanism of ASD is still unclear. Here, we discuss a new mouse model for ASD as an advanced tool to understand the mechanism of ASD.
