Irreducible traumatic posterior hip dislocation with entrapment and a buttonhole effect

The authors reported the case of a 27- year-old man who sustained an irreducible postero-lateral traumatic dislocation of the hip with capsular and labral entrapment. Initial X-rays showed only a small acetabular fragment. After two attempts to reduce the hip with muscle paralysis under general anaesthesia failed, the patient was treated by immediate open reduction through a posterolateral approach. Surgical exploration of the hip revealed a small osteochondral fragment attached to a large piece of labrum and capsule, clogging the acetabulum. The femoralhead crossed over the torn capsule with a buttonhole effect. These elements were relieved, the bone fragment was fixed with a 2 mm screw and the capsule was repaired. At the 10- year follow-up, the functional outcome was excellent with a Harris score of 100 points and no signs of necrosis or osteoarthritis. The authors propose a literature review of this uncommon lesion.

Effects of orthodontic experience on the reliability of the GOSLON Yardstick

Objective: The aim of this study was to investigate the importance of the clinical experience on the reliability of the GOSLON Yardstick. Design: Comparative-measures study. Material: 3D images of the study models of children with complete unilateral cleft lip and palate (n: 119) at an average age of 9 years were included to the study. All subjects had their cleft lip and palate repaired in many hospitals by different surgeons, but no alveolar graft or any orthodontic treatment was performed. Method: Digital views of the models were used by Orthomodel software program. 3D images of the study models were scored by two groups of raters (Group I: 10 experienced orthodontists, Group II: 10 post-graduate orthodontic stu-dents) after careful perusal of published literature concerning GOSLON scoring and examining dental models samples. Main outcome measures: Statistics of intra- and inter-rater agreement. Results: Intra-rater agreement measurements of the two groups were high and high inter-rater agreement was also found between the two groups. Conclusion: Based on the results of this study clinical orthodontic experience does not affect significantly the reliability of the GOSLON Yardstick when all the raters trained before scoring.

Of the goat population in Turkey, 98% of the goat population consists of hair goats. They are the prime breeds of goats and they can be bred in the Mediterranean, Aegean and Southeast regions of Turkey. Hair goats are the main income for their breeders. The goats are typically raised in scarce conditions, thus giving rise to greater yield of meat. In spite of the fact that the studies are limited to hair goats bred by the public, in 2005, Republic of Turkey Ministry of Food, Agriculture and Livestock （TAGEM） initiated a countywide reclamation project aiming to identify the regional differences and characteristics of the hair goats. This paper focuses on the yearly quantity of hair goats＇ produce, particularly observing the proportions of goat milk, meat and other animal produce, and to highlight the importance of the hair goats. Recommendations are made from these findings.

Introduction: Intra-articular distal radial fractures in young patients occur in high energy trauma which can lead to a combination of bone and ligaments lesions. An articular step superior to 2 mm is pejorative, for that an adequate radiologic analysis is necessary accessing to the best surgical treatment. The purpose of our study was to investigate this type of fracture and expose radiologic analysis (radiograph and CT-scan) of bone and ligamentous associated lesions of this specific twisted lunate facet fracture for recognizing it and accessing to the best surgical treatment. Patients and Methods: Seven patients with severely displaced type 4 articular fractures of the distal radius whose wrists had been exposed to violent compression are reported in our series, they are required open treatment for the reduction and fixation of disrupted articular surfaces by using an antero-medial approach which allows direct access to the fragment of the lunate facet and easier visualization of the distal radio-ulnar joint. Results: All the patients were clinically examined with antero-posterior and lateral X ray, completed by a CT scan, all the fractures were type IV according to Melone classification. In all of our seven cases there was a lesion of the distal radioulnar joint (DRUJ) identified in the CT scan. The CT scan allowed us to individualize well the void of the lunate fossa and calculate his exact degree of rotation, which is very important for planning therapeutic management. Discussion: In all of our cases the CT scan was to expose all of the osseous and ligamentous injuries;in the sagittal reconstruction we were able to objectify and calculate the exact degree of the rotation of the lunate fossa fragment;in the frontal reconstruction the void of the lunate fossa is visible and in the axial reconstruction we can determine the incongruence of the distal radio-ulnar joint (DRUJ). 本研究的目的是通过临床和组织学评价牙周手术后探针进入牙周组织的程度。研究对象为4只比格犬，共制备38个三壁骨袋，并保持3个月。随后，对其中26个骨袋进行牙周手术（手术组），其余12个骨袋作对照。分别在术后第4、8、12、16周处死动物，处死前即刻用银根管探针代替牙周探针置于龈沟内并固定于牙面。将组织块制成切片后进行组织形态学评价，其指标如下：探针尖端位置与结合上皮根端的关系、新结合上皮的长度、结合上皮根方结缔组织附着的平均长度。结果表明，术后4周时手术组和对照组的探针尖端分别位于结合上皮最根端的根方一1．37±1．73mm和-0．20±0．15mm；术后16周时，两组的探针尖端分别位于结合上皮最根端的冠方0．58±0．3lmm和0．40±0．20mm。术后4周和8周时手术组新结合上皮的长度均小于对照组，并具有统计学显著性（P〈0．05）。术后4周时实验组和对照组新结合上皮的长度分别为0．73±0．60mm和1．19±0．02mm；8周时分别为1．77±0．52mm和2．15±0．00mm。新生结缔组织的长度在两组之间没有显著性差异。本研究表明，至少在术后2个月内不应进行牙周探诊；如果在此之前探诊，探诊力可能会损伤软组织一牙齿之间的联系。

背景:某一转诊医院的结核病门诊部。目的:评估影响复发及中断过治疗的肺结核病人复治疗效的各种危险因素。设计:评估1997.1-1999.6期间本中心治疗、督导的57例复治结核病人的疗效,并对影响疗效的因素进行了调查。结果:57例病人中,37例(64.9％)被分类为复发病例,20例(35.1％)为中断过治疗者。治疗成功率为71.9％(治愈率68.4％完成治疗率3.5％),失败率为22.8％。至少耐利福平(RMP)一种药者26例(45.6％),耐多药率(MDR)为18.5％,对所检测药物全部敏感者及至少耐一种药物者的治疗成功率各为100％及50％。耐RMP者治疗成功者为68.8％,而MDR者则仅为20％。在复治方案治疗第二月末及三月末痰培养仍阳性者其失败率各为80％及100％。结论:本分析证明对抗结核药物的耐药性是影响疗效最重要的因素。耐BMP或MDR的复治病人的治疗成功率低。治疗二月末及三月末痰菌阴转对治疗成功似乎也是一个重要的参数。
