Background Haloperidol is the most frequently prescribed antipsycbotic for delirium symptoms. The risk of QTc prolongation often raises concerns, although the effect of haloperidol on QTc interval has not yet been investigated in a randomised placebo-controlled fixed-dose study. Methods A subanalysis of a randomised double-blind placebo-controlled study was conducted to evaluate the effect of prophylactic haloperidol 1 mg or placebo 1 mg orally twice-daily （maximum of 14 doses） on QTc interval in patients aged 70 years and over. Bedside, 12-lead ECGs were recorded before, during and after the one-week intervention period. Automatic QTc measurements were ob- tained in addition to manual measurements of QT and RR intervals, blinded for treatment status. Manual measurements were corrected （QTc） using Bazett （QTc-B）, Framingham （QTc-Fa）, Fridericia （QTc-Fi） and Hodges （QTc-H） methods. Mixed model analyses were used to test for differences in longitudinal course of QTc between patients receiving haloperidol and placebo. Results ECG recordings of 72 patients （haloperidol n = 38） were analysed, 45.8% male. Median （range） haloperidol serum concentration on day 4 was 0.71 （0.32-1.82） μg/L （n = 23）. Longitudinal course of mean QTc did not significantly differ between treatment arms for any of the automatic or manually derived QTc values. Conclusions Low dose oral haloperidol did not result in QTc prolongation in older acutely hospitalised patients. Results may not be generalizable to patients with existing ECG abnormalities such as atrial fibrillation.

Background:Transarterial radioembolization(TARE)has recently been recognized as a bridging/downstaging therapy to surgery for early hepatocellular carcinomas(HCCs)with high rates of complete pathological necrosis(CPN)on liver *** patients with portal vein tumoral thrombus(PVTT),multifocal or large tumors,TARE has mainly a palliative role and surgery remains controversial in this poor-prognosis *** dosimetry recently proved to outperform standard dosimetry used in prior negative Y90 randomized-controlled ***:In this retrospective study,we evaluated safety,radiological and pathological response and outcomes in HCC patients with PVTT,multifocal or large tumors,who underwent surgery after downstaging using TARE with Y90-loaded glass microspheres with personalized ***:Between December 2015 and October 2021,18 unresectable patients(14/18 with PVTT)had surgery(16 resections,2 liver transplantations)6.2 months(range,2-14.6 months)after a single Y90 *** 90-day mortality was *** modified response criteria in solid tumors(mRECIST)response were noted in all but one *** and extensive(50-99%)necrosis was observed in 36%and 45%of tumors,*** post-treatment tumor-absorbed dose significantly differed depending on the extent of pathological necrosis(P=0.045).Median overall survival and progression-free survival(PFS)were respectively of 61.8 months[95%CI:31.4 months-not reached(NR)]and 49.3 months(95%CI:14 months-NR).PFS was longer in patients with complete imaging response[median NR(none recurred or died)vs.21.5 months(95%CI:10.1 months-NR),P<0.001]and in those with complete pathological response[median NR vs.22.5 months(95%CI:10.1 months-NR),P<0.001].Conclusions:Y90 TARE using personalized dosimetry can provide high rates of imaging and pathological response in patients with PVTT,large or multifocal *** surgery is safe and leads to outcomes far exceeding expectations in a

An imported dog was confirmed to be positive with canine brucellosis in Sweden in 2010. The whole genome of Brucella canis SVA10 was subjected to phage analysis (WGS-PA) and was assigned to the Asian B. canis cluster. Further analysis indicated that the genome of B. canis SVA10 is smaller compared to genomes of the same species. A 35,781 bp genomic island (GI) was found to be absent in strain SVA10 which was detected by read mapping the paired reads to the genome of B. canis ATCC 23,365T. The lacking genes of genomic island GIFeGSH are mainly coding for iron uptake enzymes and parts of the glutathione pathway. A screening of all available whole genome sequences of Brucella strains confirmed that GIFeGSH is also missing in four more strains of B. canis but present in several strains of B. abortus, B. melitensis, B. suis, B. ovis, B. microti, B. pinnipedialis, and B. ceti. Parts of the GI were present, but scattered in two other B. canis strains. The aim of this study was to find differences in the genomes of Brucella which might explain former described differences in virulence. The analysis was extended to all available Brucella genomes after the detection of a genomic island in strain SVA10.

The complexity of the healthcare organization with emphasis on efficiency and productivity has led to an increase in value conflicts as well as moral and ethical conflicts. The aim of this study was to explore nurse managers’ perceptions of ethical conflicts when caring for older patients who wish to die. A qualitative method was employed and data were collected by means of a focus group interview with eight nurse managers. The results revealed one theme: A struggle due to responsibility for decision making and four sub-themes:?Understanding the meaning of free will, wondering about how serious the death wish is, providing more worthy elder care and lack of competent healthcare professionals to provide safe care. Nurse managers’ have no structure for bringing ethical conflicts to the attention of their leaders or the next level of the organization. Consequently, they appear to need supervision, support and clear, flexible lines of communication with leadership.

随着20世纪以来服务经济的较快发展,服务经济在世界经济中占据了突出的地位,服务业显然已经成为吸纳就业的主要形式。现实中,大部分的服务型企业是采取顾客导向的策略,即追求良好的服务行为和服务质量来提高顾客的满意度进而提高组织绩效。为保障服务型企业的正常运转并获得较好的组织绩效,服务型企业的领导者需要具备优秀的领导力以积极影响下属员工的服务行为,实现较高的服务质量。本研究关注到服务型领导是最真诚的领导方式,它以下属员工利益为首,旨在满足下属员工的需求以充分发挥下属员工的潜力。服务型领导帮助、鼓励、服务下属员工,为员工创造良好的工作氛围,增加了员工的工作满意感,因而能够促进员工在工作中表现良好的服务行为。除了领导需要具备良好的领导力,团队成员拥有能够理解彼此的情商对其服务行为以及团队的绩效也具有积极的影响。情商是指个体识别和理解情绪的能力,服务人员拥有情商以便耐心、准确地完成公司所期望的服务行为,因此关于情商对服务行为的研究也是具有意义的。综上,本研究旨在探讨服务型领导和情商对员工服务行为的影响。实际上,服务型企业要求员工在工作中对顾客持续表达积极的情绪,因而员工的服务行为包含其对情绪的表达与控制,即情绪劳动（表层扮演和深层扮演）是员工重要的服务行为。目前大多数学者主要探究服务型领导对下属员工的工作绩效、服务质量、组织公民行为、亲社会行为等工作结果的影响,而关于服务型领导对下属员工情绪劳动的影响的研究较少。此外,虽然已有中国学者发现员工的情绪智力可以通过内外动机影响员工的情绪劳动,但还未有学者探究情商在服务型领导与情绪劳动之间存在调节作用。本文将进一步验证服务型领导和情商对员工情绪劳动的正向影响,同时创新性的探究员工情商在服务型领导与员工情绪劳动（表层扮演和深层扮演）之间的调节作用。基于服务行业的研究背景,本研究以情绪劳动理论为基础,构建了服务型领导和员工情商影响员工情绪劳动的研究模型。以法国服务业的雇员为研究对象,本研究采用理论研究与实证研究相结合的方式,探究服务型领导和情商对员工服务行为的影响。主要研究内容集中在三个方面:第一,探究服务型领导对两种情绪劳动策略（表层扮演和深层扮演）的正向影响。第二,探究情商对两种情绪劳动策略（表层扮演和深层扮演）的正向影响。第三,探究情商在服务型领导与员工两种情绪劳动（表层扮演和深层扮演）关系之间的调节作用。服务型领导是员工情绪劳动两种策略（表层扮演和深层扮演）的前因。一方面,表层扮演行为是服务型员工常用的一种情绪策略,即员工通过改变外在的情绪表达来达成组织所要求的情绪表现,但并不会改变自己内在的真实感受。如果员工的上级（或主管）是服务型领导者,即其上级（或主管）将下属员工的利益放置于首位,遵循道德,倾听下属需求并尽可能的满足,帮助下属解决问题,助力下属发挥潜能,实现成长,员工受到该上级领导的鼓励后,为回报上级领导,非常有可能展现出领导所期望的对工作有利的情绪,即使这种情绪并不是其真心想表达的,他仍可以假装表现出来。因此,本研究做出假设1:服务型领导与表层扮演呈正相关。另一方面,深层扮演行为是指个体感知到自己的情绪体与组织所要求的情绪规则不一致时,通过积极的思考努力,激起或压抑某种情绪,从改变自己的内部情绪体验。服务型领导为员工创造了一种公平、值得信任的工作环境,在值得信赖的服务型领导的引导下,员工能够尽可能的改变自己的内在情绪使其与组织所需情绪相匹配。尽管有时候员工可能因服务顾客而疲惫不堪,但是服务型领导与下属员工之间紧密的联系,很容易将员工内心的消极情绪转化为积极情绪。因此,本研究做出假设2:服务型领导与深层扮演呈正相关。情商也是员工情绪劳动两种策略（表层扮演和深层扮演）的前因。情商是指通过识别和理解情绪来调节情绪以实现组织的目标的能力,包括自我意识、自我调节、对他人情绪的意识和社会意识,旨在保持良好的人际关系。情商高的员工能理解自己和他人的感受,并根据自己在危急情况下所获得的经验来调节自己的情绪表现。因此,情商高的员工更能识别和理解情绪,更善于利用和调节情绪以满足公司和服务工作的需要,即使所表现的情绪并不是自己内心的感受。因此,本研究做出假设3:情商与表层扮演呈正相关。情商是一种社会智力,其能够区分对组织无用的情绪并使用以积极的情绪信息来指导自己的思想和行动,其范围包括对情绪言语和非言语的评价和表达,对自己和他人的情绪的调节,以及对情绪内容的利用。因此,它包含了对自我和他人内在情感的调节。从这个意义上说,情商与深层扮演有关。因此,我做出假设4:情商与深层扮演呈正相关。此外,本研究还认为情商在服务型领导与表层扮演的关系之间存在调节作用,并做出假设5:情商对服务型领导与表层扮演的正向关系具有显著的调节作用以及假设6:情商对服务型领导与深层扮演的正向关系具有显著的调节作用。本研究采用问卷调查法进行

This text sets out the different arguments regarding surrogacy as a reproductive medicine tech-nique in Colombia. The procedure is described, as well as the legal matters that this issue generates. Gestational carriers appear as an alternative for couples who want to have a child but they are not able to conceive because of infertility issues and do not want to go through an adoption process. Therefore, they adhere to surrogacy as an alternative of assisted human reproduction;a procedure that in Colombia faces serious legal gaps and causes ethical dilemmas in the collective imagery. These situations suggest an open discussion through a dialectics between the scientific progress advances, the lack of legal regulation and the constants against ethical positions that the surrogacy procedure causes in Colombia and other parts of the world.

提出了一种将储集层饱和度测井数据二元简化形式作为油藏模型选择标准的改进历史拟合方法,使用半合成开源油藏模型作为基础模型对提出的方法进行评估,并与常规方法进行了比较。历史拟合阶段引入流体饱和度测井数据,有助于调整或选择能够更好地表征近井地带水驱前缘推进的模型,从而有效降低水驱油藏后续井间干扰评价的不确定性。研究表明,对含水饱和度测井数据进行二元分类时,马修斯相关系数作为分类指标的效果最佳。与常规方法相比,根据提出的方法可以选出一组更可靠的储集层(特别是强非均质性储集层)模型,选出的模型历史拟合效果更好,对近井地带含水饱和度、生产井及其不同层位含水率的拟合质量更好。
