Panorama, Reasons for Seeking Care and Evolution of Systemic Autoimmune Diseases in Benin Hospitals in 2021

Introduction: Systemic autoimmune diseases have been poorly studied in sub-Saharan Africa and their frequency is not well known. This study provided an overview of the main reasons for the use of care and their evolution in the main hospitals in Benin. Methods: This was a multi-centric descriptive cross-sectional study conducted in the internal medicine, rheumatology, dermatology and nephrology departments of nine (09) public and private hospital centers in Benin over a 57-month period, from January 1st, 2017 to September 30th, 2021. It involved patients followed for a systemic autoimmune disease. The data was collected with a digital survey sheet and then processed and analyzed with the R software (version 3.4). Results: Two hundred and three (203) patients were recorded, which represents a hospital frequency of 0.13%. The mean age was 44 years and the sex ratio (M/F) was 0.29. Connective tissue disease accounted for 95.07% of systemic autoimmune diseases which were dominated by rheumatoid arthritis (40.9%) and systemic lupus (37.4%). Ten cases of vasculitis have been reported and dominated by Behçet’s disease (40%). The main reasons for seeking care were asthenia, weight loss and fever. Arthralgia and skin lesions are the main guiding signs. Six deaths (3.1%) were recorded among connective tissue disease and 1 death (10%) among vasculitis. Conclusion: In spite of being rare, systemic autoimmune diseases are a reality in Benin. A general population study would provide a better understanding of clinical characteristics and identify prognostic factors.

Clinical Presentation and Evolution of Systemic Lupus at the CNHU-HKM of Cotonou (Benin)

Introduction: Systemic lupus erythematosus (SLE) is a non-organ-specific autoimmune disease with an unknown origin. The unchanged trend of premature mortality in systemic lupus erythematosus shows the critical unmet need for improved and optimized management of systemic lupus erythematosus. Objectives: To analyze the clinical features and the prognostic factors of death in SLE at the CNHU-HKM of Cotonou. Patients and Methods: This was a retrospective cohort study. It was conducted over the period from January 1st 2010 to December 31st 2021. The study population consisted of all patients followed for SLE in the internal medicine, rheumatology, nephrology and dermatology wards at the CNHU-HKM of Cotonou. Results: 88 cases were recorded in 12 years, i.e. an incidence of 7 cases per year. There were 80 women and 8 men with a mean age of 36.4 ± 13.1 years. The clinical picture was dominated by mucocutaneous (86.3%) and osteoarticular (71.5%) disorders. The biological abnormalities observed were anemia (78.8%), lymphopenia (43.1%) and thrombocytopenia (17.7%). 39 patients had renal damage (44.3%). The incidence of death was 17%. Factors associated with death were renal involvement, infectious complications, high initial SLEDAI score, flare, thrombocytopenia and lymphopenia. Conclusion: Mortality related to SLE remains high at the CNHU-HKM of Cotonou. Renal involvement, infectious complications, high initial SLEDAI score, flare, lymphopenia and thrombocytopenia were the factors associated with death. The presence of these factors should lead to an evaluation of the treatment.

95 g/kin is the allowed quantity of CO2 emission normalized to NEDC to be set in 2020. In addition, NEDC will be replaced by more severe driving cycles and will be united worldwide. To respond to those criteria, automotive industries are working on every possible field. Thermal management has been proved to be effective in reducing fuel consumption. Cold start is a primordial reason of ovcrconsumption, as the engine highest efficiency is at its optimal temperature. At cold start, the engine＇s oil is at its lowest temperature and thus its higher viscosity level. A high viscosity oil generates more friction, which is one of the most important heat losses in the engine. In th/s paper, hot oil storage is studied. Numerical simulations on GT-suite model were done. The model consists of a 4-cylinder turbocharged Diesel engine using a storage volume of 1 liter of hot oil. Ambient temperature vari- ation were taken into consideration as well as different driving cycles. Furthermore, different configurations of the thermal strategy （multifunction oil sump） were proposed and evaluated. Lubricant temperature and viscosity profiles are presented in the article as well as fuel consumption savings for different configurations, driving cycles and ambient temperatures.

Prevalence and Risk Factors of Infectious Spondylodiscitis in Benin’s Children

Aim: To determine the prevalence and risk factor of infectious spondylodiscitis in children in Benin. Patients and Method: A descriptive cross-sectional study over 5 years was conducted in rheumatology and paediatric departments of National Hospital University Hubert Koutoukou Maga of Cotonou among children suffered from osteoarticular disorders. We selected patients with infectious spondylodyscitis. The data collected were analyzed using SPSS 20.0 software. Results: Among 179 children treated for osteo-articular disorders, 29 (16.2%) had infectious spondylodiscitis. There were 11 girls and 18 boys (sex ratio = 1.55). The main symptoms were dominated by low back pain (72.4). Fever was present in 58.6% of patients. Laboratory parameters of inflammation were higher than normal in all of the patients. Paraparesia was present in one case. L2-L3 disc was most affected (45.2% of cases). Mycobacterium tuberculosis was incriminated in 75,8%. Common places germs were dominated by negative baccillus germs (62.5%). The treatment was medical with a minimum duration of 3 months. All patients were immobilized with a lumbostat over a period of 3 months. Sicle cells disease, HIV, no BCG vaccination were the risk factors identified respectively in 9.4%, 11.3% and 16%. Conclusion: Diagnosis of spondylodiscitis in children is difficult in our country Keeping in mind unspecific subjective complaints and clinical findings. Early diagnosis uses magnetic resonance imaging which cost high and remains beyond the reach of our patients. Therefore, effective antibiotic therapy should be started promptly as soon as the diagnosis is suspected to avoid serious complications.

Anti-inflammatory and antipyretic properties of Kang 601 heji,a traditional Chinese oral liquid dosage form

Objective: To evaluate the scientific basis for the use of Kang 601 heji(K-601) as an anti-in flammatory and antipyretic agent using appropriate animal ***: Carrageenan-induced rat paw and xylene-induced ear oedemas were models used to investigate anti-in flammatory actions of K-601. Lipopolysaccharide-induced pyrexia model was used to evaluate antipyretic activity in Wistar rats. The antiin flammatory and antipyretic mechanisms were evaluated by detecting prostaglandins E2, nitric oxide, interleukin-1b and tumour necrosis factora levels using appropriate reagents and ELISA ***: The results revealed that K-601 reduced the level of in flammations in both antiin flammatory models in a dose-dependent manner. The same was true for the antipyretic model. The possible mechanisms of actions were through the inhibition of prostaglandins E2, interleukin-1b, tumour necrosis factora and nitric ***: K-601 has proven anti-in flammatory and antipyretic actions. The findings provide a scientific basis for the use of K-601 as anti-in flammatory and antipyretic agent in traditional Chinese medicinal practice.
