Selecting lymph node-positive patients for adjuvant therapy after radical prostatectomy and extended pelvic lymphadenectomy:An outcome analysis of 100 node-positive patients managed without adjuvant therapy

Objective:The aim of the study is to evaluate the effect of deferred androgen deprivation therapy on biochemical recurrence(BCR)and other survival parameters in node-positive prostate cancer patients after robot-assisted radical prostatectomy with bilateral extended pelvic lymph node dissection(RARP+EPLND).Materials and methods:Of the 453 consecutive RARP procedures performed from 2011 to 2018,100 patients with no prior use of androgen deprivation therapy were found to be lymph node(LN)positive and were observed,with initiation of salvage treatment at the time of BCR *** were divided into 1 or 2 LNs(67)-and more than 2 LNs(33)-positive groups to assess survival ***:At a median follow-up of 21 months(1-70 months),the LN group(p<0.000),preoperative prostate-specific antigen(PSA,p=0.013),tumor volume(TV,p=0.031),and LND(p=0.004)were significantly associated with *** multivariate analysis,only the LN group(p=0.035)and PSA level(p=0.026)were statistically *** estimated BCR-free survival rates in the 1/2 LN group were 37.6%(27%-52.2%),26.5%(16.8%-41.7%),and 19.9%(9.6%-41.0%)at 1,3,and 5 years,respectively,with a hazard of developing BCR of 0.462(0.225-0.948)compared with the more than 2 LN-positive *** 5-year overall survival,cancer-specific,metastasis-free,and local recurrence-free survival rates were 88.4%(73.1%-100%),89.5%(74%-100%),65.1%(46.0%-92.1%),and 94.8%(87.2%-100.0%),respectively,for which none of the factors were *** on cutoff values for PSA,TV,and LND of 30 ng/mL,30%,and 10%,respectively,the 1/2 LN group was substratified,wherein the median BCR-free survival for the low-and intermediate-risk groups was 40 and 12 months,***:Nearly one fourth and one fifth of 1/2 node-positive patients were BCR-free at 3 and 5 years after RARP+*** substratification using PSA,TV,and LN density may help in providing individualized care regarding the initiation of adjuvant therapy.

Lower back pain is one of the most common medical problems in the world and it is experienced by a huge percentage of people *** to its ability to produce a detailed view of the soft tissues,including the spinal cord,nerves,intervertebral discs,and vertebrae,Magnetic Resonance Imaging is thought to be the most effective method for imaging the *** semantic segmentation of vertebrae plays a major role in the diagnostic process of lumbar *** is difficult to semantically partition the vertebrae in Magnetic Resonance Images from the surrounding variety of tissues,including muscles,ligaments,and intervertebral discs.U-Net is a powerful deep-learning architecture to handle the challenges of medical image analysis tasks and achieves high segmentation *** work proposes a modified U-Net architecture namely MU-Net,consisting of the Meijering convolutional layer that incorporates the Meijering filter to perform the semantic segmentation of lumbar vertebrae L1 to L5 and sacral vertebra ***-colour mask images were generated and used as ground truth for training the *** work has been carried out on 1312 images expanded from T1-weighted mid-sagittal MRI images of 515 patients in the Lumbar Spine MRI Dataset publicly available from Mendeley *** proposed MU-Net model for the semantic segmentation of the lumbar vertebrae gives better performance with 98.79%of pixel accuracy(PA),98.66%of dice similarity coefficient(DSC),97.36%of Jaccard coefficient,and 92.55%mean Intersection over Union(mean IoU)metrics using the mentioned dataset.

Signet ring colorectal carcinoma:Do we need to improve the treatment algorithm?

AIMTo elaborate about this peculiar variant from a tertiary cancer center from India. METHODSIt’s a retrospective study (2011-2014) of all patients diagnosed with signet ring colo-rectal cancer (SRCC). Various clinico-pathological variables were studied. RESULTSOne hundred and seventy consecutive patients with SRCC were diagnosed (11.4% of all colorectal cancers). Median Age of the cohort was 41 years. Most common location was recto-sigmoid area (54.7%). Majority patients presented in stage III and IV (91.2%). Most of the stage IV patients had isolated peritoneal metastases (86.5%). Colonic tumors had higher incidence of peritoneal metastases (91.8% vs 83.3%) as well as isolated peritoneal recurrences (37.5% vs 16.7%) than rectal primaries. Thirty-seven point five percent of patients recurred after curative surgery. Amongst them 63.63% patients had isolated peritoneal recurrences. Circumferential resection margin (CRM) was involved in 17.9% patients. Median relapse free survival (RFS) and overall survival (OS) of the cohort were 14.9 and 18.13 mo respectively. CRM involvement, colonic primary were associated with poorer RFS and OS. CONCLUSIONSRCC has predilection for peritoneal dissemination. More aggressive and/or extended chemotherapy schedules as well as prophylactic hyperthermic intra-peritoneal chemotherapy at the time of primary surgery may be attempted in these patients.

Open Air Interface(OAI)alliance recently introduced a new disaggregated Open Radio Access Networks(O-RAN)framework for next generation telecommunications and *** disaggregated architecture is open,automated,software defined,virtual,and supports the latest advanced technologies like Artificial Intelligence(AI)Machine Learning(AI/ML).This novel intelligent architecture enables programmers to design and customize automated applications according to the business needs and to improve quality of service in fifth generation(5G)and Beyond 5G(B5G).Its disaggregated and multivendor nature gives the opportunity to new startups and small vendors to participate and provide cheap hardware software solutions to keep the market *** paper presents the disaggregated and programmable O-RAN architecture focused on automation,AI/ML services,and applications with Flexible Radio access network Intelligent Controller(FRIC).We schematically demonstrate the reinforcement learning,external applications(xApps),and automation steps to implement this disaggregated O-RAN *** idea of this research paper is to implement an AI/ML enabled automation system for software defined disaggregated O-RAN,which monitors,manages,and performs AI/ML-related services,including the model deployment,optimization,inference,and training.

A Transition from Wood Fuel to LPG and Its Impact on Energy Conservation and Health in the Central Himalayas, India

The aim of the study was to evaluate the impacts of the transition from wood fuel to Liquefied Petroleum Gas （LPG） from energy use and health perspectives along an altitudinal gradient （viz., lower altitude; middle altitude; and higher altitude） of the Central Himalayas. Empirical field study and questionnaire based survey was conducted for obtaining the data. A total of 2o households from each altitude were selected for obtaining reliable information on the actual quantity of fuelwood consumed. Of the 2o households, five households each based on the family size i.e., small families （〈4 members）, medium （5-8 members） and large （〉9 members） from all the altitudinal regions were selected. This was followed by an administration of a questionnaire on the quantity of fuelwood consumed. After the completion of the questionnaire survey, the data was validated using a weighted survey for the randomly selected households for obtaining precise information on the actual quantity of fuelwood consumed. Energy analysis is done with respect to the time spent on fuelwood collection and energy value of burning of per kg of fuelwood. Study indicates that declining biomass requirement from forests contributes significantly towards energy conservation, also has positive impact on human health. Per capita annual energy expenditure on collection of fuelwood is 752 MJ which is higher than any other activity in villages of Central Himalaya. The LPG substitution has contributed to energy saving which is equivalent to 2976-3,742 MJ per capita per year in middle and lower altitudes respectively. In the higher altitude the energy saving is calculated to be about 257 MJ per capita per year. Replacing fuelwood with LPG has made positive impact on society in terms of improving the health while reducing diseases that are caused due to indoor air pollution.

A Cocktail of Natural Compounds Holds Promise for New Immunotherapeutic Potential in Head and Neck Cancer

Objective: To obtain detailed understanding on the gene regulation of natural compounds in altering prognosis of head and neck squamous cell carcinomas(HNSC). Methods: Gene expression data of HNSC samples and peripheral blood mononuclear cells(PBMCs) of HNSC patients were collected from Gene Expression Omnibus(GEO). Differential gene expression analysis of GEO datasets were achieved by the GEO2R tool. Common differentially expressed gerres(DEGs) were screened by comparing DEGs of HNSC with those of PBMCs. The combination was further analyzed for regulating pathways and biological processes that were affected. Results: Totally 110 DEGs were retrieved and identified to be involved in biological processes related to tumor regulation. Then 102 natural compounds were screened for a combination such that the expressions of all 110 commonly DEGs were altered. A combination of salidroside, ginsenoside Rd, oridonin, britanin, and scutellarein was chosen. A multifaceted, multi-dimensional tumor regression was showed by altering autophagy, apoptosis, inhibiting cell proliferation, angiogenesis, metastasis and inflammatory cytokines production. Conclusions: This study has helped develop a unique combination of natural compounds that will markedly reduce the propensity of development of drug resistance in tumors and immune evasion by tumors. The result is crucial to developing a combinatorial natural therapeutic cocktail with accentuated immunotherapeutic potential.

Machine vision for drill string slip status detection

Slip status of drill string is system generated binary value computed by comparison of sensor generated real time hook load value with a minimum threshold value of hook load stored in measurement *** research article describes a novel method of slip status detection by machine vision technology which helps overcome the constraints of slip status detection with legacy measurement *** also helps improve the real time drilling data quality and optimize and automate drilling operations.A method to detect drill string slip status with high-resolution digital camera installed on mast near rig floor is described along with backend vision processing and communication modules,which generate binary values of slip *** binary values are transferred in real time to drilling measurement system of rig to compute other drilling parameters like bit depth,hole depth and stand *** method includes deploying active optical sensors at the rig floor,obtaining 1-D,2-D,or 3D image data,and processing it to obtain the status of drill *** measurement of slip status by machine vision helps reduce non-productive time(NPT)by reliable real time surveillance of drilling operations.

Study of plasma parameters of coaxial plasma source using triple Langmuir probe and Faraday cup diagnostics

Coaxial plasma guns are a type of plasma source that produces plasma which propagates radially and axially controlled by the shape of the ground electrode, which has attracted much interest in several applications. In this work, a 120° opening angle of CPG nozzle is used as a plasma gun configuration that operates at the energy of 150 J. The ionization of polyethylene insulator between the electrodes of the gun produces a cloud of hydrogen and carbon *** triple Langmuir probe and Faraday cup are used to measure plasma density and plasma temperature. These methods are used to measure the on-axis and off-axis plasma divergence of the coaxial plasma gun. The peak values of ion densities measured at a distance of 25 mm on-axis from the plasma gun are(1.6±0.5)×10^(19)m^(-3)and(2.8±0.6)×10^(19)m^(-3)for hydrogen and carbon plasma respectively and the peak temperature is 3.02±0.5 eV. The mean propagation velocity of plasma is calculated using the transit times of plasma at different distances from the plasma gun and is found to be 4.54±0.25 cm/μs and 1.81±0.18 cm/μs for hydrogen and carbon plasma respectively. The Debye radius is obtained from the measured experimental data that satisfies the thin sheath approximation. The shot-to-shot stability of plasma parameters facilitates the use of plasma guns in laboratory experiments. These types of plasma sources can be used in many applications like plasma opening switches, plasma devices, and as plasma sources.

First Total Synthesis and Biological Potential of a Heptacyclopeptide of Plant Origin

Synthesis of a natural glycine-rich heptacyclopeptide mahafacyclin A （7） was accomplished by solution-phase technique of peptide synthesis via coupling of tetrapeptide unit Boc-L-Thr-L-Ile-L-Leu-Gly-OH with tripeptide unit L-Val-L-Phe-Gly-OMe followed by cyclization of linear heptapeptide fragment. Structure of the newly synthesized cyclopolypeptide was confirmed by means of chemical, spectroscopic analyses and subjected to antibacterial, antifungal and anthelmintic activity studies. Bioactivity results showed potent antifungal and anthelmintic activities of the synthesized peptide against dermatophytes T. mentagrophytes, M. audouinii and earthworm species M. kon-kanensis, P. corethruses and E. eugeniea.
