Neuromuscular scoliosis is a challenging problem to treat in a heterogeneous patient population. When thedecision is made for surgery the surgeon must select a technique employed to correct the curve and achieve the goals of surgery, namely a straight spine over a level pelvis. Pre-operatively the surgeon must ask if pelvic fixation is worth the extra complications and infection risk it introduces to an already compromised host. Since the advent of posterior spinal fusion the technology used for instrumentation has changed drastically. However, many of the common problems seen with the unit rod decades ago we are still dealing with today with pedicle screw technology. Screw cut out, pseudoarthrosis, non-union, prominent hardware, wound complications, and infection are all possible complications when extending a spinal fusion construct to the pelvis in a neuromuscular scoliosis patient. Additionally, placing pelvic fixation in a neuromuscular patient results in extra blood loss, greater surgical time, more extensive dissection with creation of a deep dead space, and an incision that extends close to the rectum in patients who are commonly incontinent. balancing the risk of placing pelvic fixation when the benefit, some may argue, is limited in non-ambulating patients is difficult when the literature is so mottled. Despite frequent advancements in technology issues with neuromuscular scoliosis remain the same and in the next 10 years we must do what we can to make safe neuromuscular spine surgery a reality.

Skin thermal damage or skin burns are the most commonly encountered type of trauma in civilian and military communities. besides, advances in laser, microwave and similar technologies have led to recent developments of thermal treatments for disease and damage involving skin tissue, where the objective is to induce thermal damage precisely within targeted tissue structures but without affecting the surrounding, healthy tissue. Further, extended pain sensation induced by thermal damage has also brought great problem for burn patients. Thus, it is of great importance to quantify the thermal damage in skin tissue. In this paper, the available models and experimental methods for quantification of thermal damage in skin tissue are discussed.

This study investigates the recent near-surface temperature trends over the Antarctic *** make use of available surface observations,ECMWF’s ERA5 and its predecessor ERA-Interim,as well as numerical simulations,allowing us to contrast different data *** use hindcast simulations performed with Polar-WRF over the Antarctic Peninsula on a nested domain configuration at 45 km(PWRF-45)and 15 km(PWRF-15)spatial resolutions for the period 1991?*** addition,we include hindcast simulations of KNMI-RACMO21P obtained from the CORDEX-Antarctica domain(~50 km)for further *** show that there is a marked windward warming trend except during *** windward warming trend is particularly notable in the autumn season and likely to be associated with the recent deepening of the Amundsen/bellingshausen Sea low and warm advection towards the Antarctic *** the other hand,an overall summer cooling is characterized by the strengthening of the Weddell Sea low as well as an anticyclonic trend over the Amundsen Sea accompanied by northward *** persistent cooling trend observed at the Larsen Ice Shelf station is not captured by ERA-Interim,whereas hindcast simulations indicate that there is a clear pattern of windward warming and leeward ***,larger temporal correlations and lower differences exhibited by PWRF-15 illustrate the existence of the added value in the higher spatial resolution simulation.

Dip-pen *** （DPN） is a useful method for directly printing materials on surfaces with sub-50nm resolution. because it, involves the physical transport of materials from a scanning probe tip to a surface and the subsequent chemical interaction of that material with the surface, there are many factors to consider when attempting to understand DPN. In this review, we overview the physical and chemical processes that are known to play a role in DPN, Through a detailed review of the literature, we classify inks into three general categories based on their transport properties, and highlight the myriad ways that. DPN can be used to perform chemistry at the tip of a scanning probe.

Chronic infection with viral hepatitis affects half a billion individuals worldwide and can lead to cirrhosis,cancer,and liver *** cancer is the third leading cause of cancer-associated mortality,of which hepatocellular carcinoma(HCC)represents 90%of all primary liver *** tumors like HCC are complex and have heterogeneous tumor genomic profiles contributing to complexity in diagnosis and *** infection with hepatitis b virus(HbV),hepatitis delta virus(HDV),and hepatitis C virus(HCV)are the greatest etiological risk factors for *** to the significant role of chronic viral infection in HCC development,it is important to investigate direct(viral associated)and indirect(immune-associated)mechanisms involved in the pathogenesis of *** mechanisms used by HbV,HCV,and HDV that drive hepatocarcinogenesis include persistent liver inflammation with an impaired antiviral immune response,immune and viral protein-mediated oxidative stress,and deregulation of cellular signaling pathways by viral *** integration to promote genome instability is a feature of HbV infection,and metabolic reprogramming leading to steatosis is driven by HCV *** current review aims to provide a brief overview of HbV,HCV and HDV molecular biology,and highlight specific viral-associated oncogenic mechanisms and common molecular pathways deregulated in HCC,and current as well as emerging treatments for HCC.

bACKGROUND Presence of liver metastatic disease in pancreatic ductal adenocarcinoma(PDAC),either synchronous or metachronous after pancreatic resection,is a terminal diagnosis that warrants management with palliative intent as per all international practice ***,there is an increasing interest on any potential value of surgical treatment of isolated oligometastatic disease in selected *** To present the published evidence on surgical management of PDAC liver metastases,synchronous and metachronous,and compare the outcomes of these treatments to the current standard of *** A systematic review was performed in line with the Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines to compare the outcomes of both synchronous and metachronous liver metastases resection to standard *** 356 studies were identified,31 studies underwent full-text review and of these 10 were suitable for *** synchronous resection of liver metastases was compared to standard care,most studies did not demonstrate a survival benefit with the exception of one study that utilised neoadjuvant ***,resection of metachronous disease appeared to confer a survival advantage when compared to treatment with chemotherapy *** A survival benefit may exist in resection of selected cases of metachronous liver oligometastatic PDAC disease,after disease biology has been tested with time and systemic *** survival benefit is less clear in synchronous cases;however an approach with neoadjuvant treatment and consideration of resection in some selected cases may confer some *** studies should focus on pathways for selection of cases that may benefit from an aggressive approach.

Venous resection at pancreaticoduodenectomy(PD)has established itself as a viable treatment for borderline resectable cancer,with equivalent survival to those patients undergoing PD for cancer without venous resection[1].Thus surgery for these patients is clearly desirable,when possible,compared to palliative ***,there is concern and debate about the safety of venous resection at PD[2].There is a need to improve patient pathways,outcome and experience[3].To this end,in patients who present with jaundice,it is clear that surgery without preoperative biliary drainage(PbD)reduces complications[4].In 2015,our team introduced a‘fast track’program to routinely perform PD in the presence of jaundice[5].This change of practice has become routine,with most patients now undergoing surgery without *** is,however,unclear whether portal vein(PV)resection+PD is safe in the setting of *** aim of this study,therefore,was to compare outcomes and complications between patients with or without jaundice undergoing PD with associated resection of the superior mesenteric vein(SMV)and/or PV.

African Americans have exceptionally high rates of hypertension and hypertension related complications. It is commonly reported that the blood pressure lowering efficacy of renin angiotensin system(RAS) inhibitors is attenuated in African Americans due to a greater likelihood of having a low renin profile. Therefore these agents are often not recommended as initial therapy in African Americans with hypertension. However, the high prevalence of comorbid conditions, such as diabetes, cardiovascular and chronic kidney disease makes treatment with RAS inhibitors more compelling. Despite lower circulating renin levels and a less significant fall in blood pressure in response to RAS inhibitors in African Americans, numerous clinical trials support the efficacy of RAS inhibitors to improve clinical outcomes in this population, especially in those with hypertension and risk factors for cardiovascular and related diseases. Here, we discuss the rationale of RAS blockade as part of a comprehensive approach to attenuate the high rates of premature morbidity and mortality associated with hypertension among African Americans.
