Contralateral reversed distal femoral locking plate for fixation of subtrochanteric femoral fractures

Purpose:Subtrochanteric fractures of the femur are being managed successfully with various intramedullary and extramedulary implants with reasonable success. However, these implants require precise placement under image intensifier guidance, which exposes the surgeon to substantial amount of radiation. It also restricts the management of these fractures at peripheral centers where facility of image intensifiers is not available. Keeping this in mind we designed this study to identify if contralateral reversed distal femoral locking plate can be used successfully without the use of image intensifier.Methods:Twenty-four consecutive patients (18 men and 6 women) with a mean age of 28 years (range 19-47 years) suffering subtrochanteric fractures of the femur underwent open reduction and internal fixation with reversed contralateral distal femoral locking plate. The outcome was assessed at the mean follow-up period of 3.2 years (range 2-4.6 years) using the Harris hip score.Results:Twenty-one fractures united with the primary procedure, with a mean time of consolidation being 11 weeks (range, 9-16 weeks). One patient developed superficial suture line infection, which resolved with oral antibiotics. Another patient had a fall 3 weeks after surgery and broke the plate. Repeat surgery with reversed distal femoral locking compression plate was performed along with bone grafting and the fracture united. Two cases had nonunion, which went in for union after bone grafting. The mean Harris hip score at the time of final follow-up was 90.63 (range 82-97).Conclusion:The reversed contralateral distal femoral plate is a biomechanically sound implant, which when used for fixation of the subtrochanteric fractures with minimal soft tissue stripping shows results comparable to those achieved by using other extramedullary implants as well as intramedullary devices. The added advantage of this implant is its usability in the absence of an image intensifier.

Fuzzy entropy measures are valuable tools in decision-making when dealing with uncertain or imprecise *** exist many entropy measures for Pythagorean Fuzzy Sets(PFS)in the literature that fail to deal with the problem of providing reasonable or consistent results to the *** deal with the shortcomings of the existing measures,this paper proposes a robust fuzzy entropy measure for PFS to facilitate decision-making under *** usefulness of the measure is illustrated through an illustration of decision-making in a supplier selection problem and compared with existing fuzzy entropy *** Technique for Order Performance by Similarity to Ideal Solution(TOPSIS)approach is also explored to solve the decision-making *** results demonstrate that the proposed measure can effectively capture the degree of uncertainty in the decision-making process,leading to more accurate decision outcomes by providing a reliable and robust ranking of alternatives.

Traffic intersections are incredibly dangerous for drivers and pedestrians. Statistics from both Canada and the U.S. show a high number of fatalities and serious injuries related to crashes at intersections. In Canada, during 2019, the National Collision Database shows that 28% of traffic fatalities and 42% of serious injuries occurred at intersections. Likewise, the U.S. National Highway Traffic Administration (NHTSA) found that about 40% of the estimated 5,811,000 accidents in the U.S. during the year studied were intersection-related crashes. In fact, a major survey by the car insurance industry found that nearly 85% of drivers could not identify the correct action to take when approaching a yellow traffic light at an intersection. One major reason for these accidents is the “yellow light dilemma,” the ambiguous situation where a driver should stop or proceed forward when unexpectedly faced with a yellow light. This situation is even further exacerbated by the tendency of aggressive drivers to inappropriately speed up on the yellow just to get through the traffic light. A survey of Canadian drivers conducted by the Traffic Injury Research Foundation found that 9% of drivers admitted to speeding up to get through a traffic light. Another reason for these accidents is the increased danger of making a left-hand turn on yellow. According to the National Highway Traffic Safety Association (NHTSA), left turns occur in approximately 22.2% of collisions—as opposed to just 1.2% for right turns. Moreover, a study by CNN found left turns are three times as likely to kill pedestrians than right turns. The reason left turns are so much more likely to cause an accident is because they take a driver against traffic and in the path of oncoming cars. Additionally, most of these left turns occur at the driver’s discretion—as opposed to the distressingly brief left-hand arrow at busy intersections. Location prediction in social media， a growing research field， employs machine learning to identify users’ locations from their online activities. This technology， useful in targeted advertising and urban planning， relies on natural language processing to analyze social media content and understand the temporal dynamics and structures of social networks. A key application is predicting a Twitter user’s location from their tweets， which can be challenging due to the short and unstructured nature of tweet text. To address this challenge， the research introduces a novel machine learning model called the location-aware attention LSTM (LAA-LSTM). This hybrid model combines a Long Short-Term Memory (LSTM) network with an attention mechanism. The LSTM is trained on a dataset of tweets， and the attention network focuses on extracting features related to latitude and longitude， which are crucial for pinpointing the location of a user’s tweet. The result analysis shows approx. 10% improvement in accuracy over other existing machine learning approaches.

The existence of squeezing ground conditions can lead to significant challenges in designing an adequate support system for *** empirical,observational and analytical methods have been suggested over the years to design support systems in squeezing ground conditions,but all of them have some *** this study,a novel experimental setup having physical model for simulating the tunnel boring machine(TBM)excavation and support installation process in squeezing clay-rich rocks is *** observations are made to understand better the interaction between the support and the squeezing *** physical model included a large true-triaxial cell,a miniature TBM,laboratoryprepared synthetic test specimen with properties similar to natural mudstone,and an instrumented cylindrical aluminum support *** were conducted at realistic in situ stress levels to study the time-dependent three-dimensional tunnel support *** tunnel was excavated using the miniature TBM in the cubical rock specimen loaded in the true-triaxial cell,after which the support was *** confining stress was then increased in stages to values greater than the rock’s unconfined compressive strength.A model for the time-dependent longitudinal displacement profile(LDP)for the supported tunnel was proposed using the tunnel convergence measurements at different times and stress *** LDP formulation was then compared with the unsupported model to calculate the squeezing amount carried by the *** increase in thrust in the support was backcalculated from an analytical solution with the assumption of linear elastic *** on the test results and case studies,a recommendation to optimize the support requirement for tunnels in squeezing ground is proposed.

Ulcerative colitis is a colonic disease characterized by the disruption of the mucosal epithelial layer and inflammation. For the treatment of this disease, various chemotherapeutic agents are ***, the toxicities associated with chemotherapeutics greatly hamper treatment. Polysaccharide from natural resources is emerging as a potentially therapeutic substance with comparative minimum adverse effects. In this article, we are discussing polysaccharide from diverse sources(plants, edible mushrooms,and algae) which are being used in the treatment of ulcerative colitis. These polysaccharides exert their therapeutic action on ulcerative colitis through several mechanisms, including suppression of inflammatory cascades NF-κB, MAPK, IL-6/JAK2/STAT3,preventing the release of certain inflammatory mediators, modulating the intestinal microbiome, maintaining the integrity of intestinal barriers, and regulating the certain inflammatory markers. The present review compiles the role of different polysaccharides being used successfully in the management/treatment of ulcerative *** emphasis was given to explaining the biomolecular pathway.

The excitation function of the 58Ni(n,p)58Co reaction was measured using the well-established neutron activation technique andγ-ray *** in the energy range of 1.7 to 2.7 MeV were generated using the 7Li(p,n)*** neutron flux was measured using the standard 115In(n,n’)115mIn monitor *** results of the neutron spectrum averaged cross-section of 58Ni(n,p)58Co reactions were compared with existing cross-section data available in the EXFOR data library as well as with various evaluated data libraries such as ENDF/B-VIII.0,JEFF-3.3,JENDL-4.0,and *** calculations were performed using the nuclear reaction code *** nuclear level density(NLD)models were tested,and their results were compared with the present *** NLDs were also obtained through the spectral distribution method(SDM).The cross-section results,along with the absolute errors,were obtained by investigating the uncertainty propagation and using the covariance *** forγ-ray true coincidence summing,low-energy background neutrons,andγ-ray self attenuation were *** experimental cross-section obtained in the present study is consistent with previously published experimental data,evaluated libraries,and theoretical calculations carried out using the TALYS code.
