To maintain productivity in today＇s healthcare market, it is important for institutions to have a delivery system that allows for transformation and adaptation. This paper introduces the Medical Home Transformation Model which demonstrates the effectiveness of employing a team of embedded coaches within a five-step capacity building program in conjunction with learning collaboratives, the adaptation medium. This model demonstrates the effectiveness of concurrently employing the coaching lifecycle and a five-step capacity building program. Moreover, the model details the formation of multidisciplinary teams at each clinical site, meaningful data reporting at site and physician levels and best practice sharing at collaboratives and seminars. A discussion then demonstrates the success of MMG （Montefiore Medical Group） as it applies the model to its ambulatory care network of 22 health centers in effort to achieve PCMH （Patient Centered Medical Home） recognition, improve clinical outcomes, and document financial benefits. While PCMH is currently the sought after delivery system framework, this paper will also demonstrate the adaptability of the model to any prescribed framework which continues advancement toward the ＂Triple Aim＂. Such a model has enabled MMG to surpass national standards of clinical care within a conf＇wmed vulnerable population and has placed MMG at the forefront for primary care delivery system reform.

Effect of different pre-sowing treatments on seed germination percentage and growth performance of Acacia auriculiformis

Seed morphology,germination and seedlings growth of Acacia auriculiformis were *** experiment was conducted in the nursery of Forestry and Wood Technology Discipline,Khulna University,*** seeds of the species were collected from healthy trees of road side plantation from different areas of Khulna District,Bangladesh and treated with five pre-sowing treatments（control,immersion in cold water,immersion in hot water,scarification with sand paper and immersion in concentrated H2SO4）.The average length,breadth and thickness were found to be（0.58±0.017） cm,（0.44±0.007） cm and（0.20±0.089） cm,*** was conducted in polybags with a mixture of top soil,coconut husk,coarse sand,and fine sand in a ratio of 3：4：1：*** reveal that pre-sowing treatments influences the germination rates of seeds that significantly increase the percentage germination compared with those in control（43%） and cold water treatment（52%）.The highest germination success rate was found 83% in hot water treatment followed by 78% in scarification with sand paper,and 75% with immersion in *** started from 7 to 12 days and completed between 28 and 35 days period in all *** showed the significant difference（p0.05） among the treatments in seed germination,but no significant difference among treatment with regard to starting day,closing day and total germination *** case of height and diameter growth,seedlings originated from the seeds with hot water treatment shows significantly higher in wet season（from May to July）.Hot water treatment can be recommended on seed germination of the species in rural Bangladesh.

Pennisetum purpureum is one of the most invasive perennial grasses of the Poaceae family,which are abundant in south-east Asia including Brunei *** pyrolysis process at a slow heating rate proved to be highly promising for biochar *** production and characterization of different Pennisetum purpureum biochars have been investigated at the pyrolysis temperatures of 400℃,500℃and 600℃with a heating and nitrogen flow rate of 5℃/min and 0.5 L/min,*** observed higher heating values were 22.18 MJ/kg,23.02 MJ/kg,23.75 MJ/kg,and the alkaline pH were 9.10,9.86,10.17 for the biochar at 400℃,500℃,600℃temperatures,*** water holding capacity was one hundred percent for all biochars and continued to increase for higher pyrolysis *** images show that the porosity of the biochars has been enhanced with increased temperatures due to the rearrangement of crystallinity and *** the other hand,the yields of biochar have been decreased from 35.13%to 23.02%for the increase of pyrolysis temperature from 400℃to 600℃.Energy dispersive X-ray analysis shows that the O/C atomic ratios were 0.15,0.08 and 0.06 for the biochar of 400,500 and 600℃which validates the improvement in heating ***-IR analysis revealed that the available functional groups in the biochars were C-O,C=C,and *** analysis(TGA)under pyrolysis condi-tion showed residue of 46.56%,51.13%and 55.67%from the biochar at 400,500,and 600℃,*** derivative thermogravimetry(DTG)graph indicates that the degradation rate is higher for 400℃biochar than the 600℃biochar.

The Miller Fisher variant is an uncommon but well known syndrome being described as a triad of areflexia, ataxia and complex ophthalmoplegia. It is characterized by antibodies against myelin that affects peripheral nerves, extraocular muscles and Schwann cells. Anti-ganglioside antibodies have been recognized in disease pathogenesis and decreasing antibody production is the mainstay of treatment. The course is usually benign with improvement after immunomodulation. This case report describes the approach to a patient suspected of having a demyelinating disorder. It delineates the subsets of immune mediated neuropathies in evaluating the diagnosis and emphasizes the need for early therapeutic intervention in achieving a good clinical outcome.

Objective: To delineate reliable morphological characteristics for identifying and separating female Phlebotomus caucasicus and Phlebotomus mongolensis which exist sympatrically in the main foci of zoonotic cutaneous leishmaniasis in ***: Sand flies were collected using sticky trap papers from active colonies of rodent burrows installed from 16 catching sites. Morphometric measurements were analyzed of 87 Phlebotomus caucasicus and 156 Phlebotomus mongolensis. Univariate and multivariate analysis were carried out to determine significant morphometric variables for discrimination of the two species. Finally, seven morphological characteristics of 65 female Phlebotomus caucasicus and 124 female Phlebotomus mongolensis were ***: Univariate and multivariate analyses of 10 morphometric variables via Discriminant Function Analysis(DFA) and Principal Component Analysis(PCA) showed that five morphometric variables had an accuracy of 100% for discriminating female Phlebotomus caucasicus and Phlebotomus mongolensis. Moreover, PCA revealed that the five morphometric variables with the highest loadings separated these two species. Morphological studies on antennal flagellum(and its associated structures) and mouth-parts of female specimens demonstrated significant differences in several ***: The results show that morphological and morphometrical features can be used to discriminate two female isomorphic species, Phlebotomus caucasicus and Phlebotomus mongolensis accurately.

Kota Kinabalu Wetland is importantly habited of mangrove, diverse aquatic flora and fauna as well as feeding stop for migratory birds. This wetland is inundated with the tidal flow, as connected with a small river and nearby coastal areas, thus. A study was carried out to determine the spatial distribution of dissolved inorganic nutrients and phytoplankton diversity at Kota Kinabalu (KK) Wetlands. Five stations, in which river mouth of Likas Bay, river channel (two stations) and inundated area (two stations) in KK Wetland were selected for this study. In-situ parameters of water, water for nutrients and phytoplankton samples were collected from May 2019 until October 2019. The highest concentration of nitrate (0.115 mg/L) was recorded at inundated area of wetland (S5) while the lowest nitrate concentration (0.0047 mg/L) was found at river (S3) flowing towards wetland. The concentrations of ammonia (0.2004 to 2.311 mg/L) were recorded relatively higher at every station compared to other dissolved inorganic nutrients (DIN). The concentration of phosphate was determined in the ranges of 0.0089 - 0.0513 mg/L. Nitrate, ammonia and phosphate showed no significant difference (P = 0.737) in terms of DIN concentration at all five sampling stations during the study period. Lithological and Structural Control of Hattian Bala Rock Avalanche Triggered by the Kashmir Earthquake 2005, Sub-Himalayas, Northern Pakistan

The Kashmir earthquake 2005 （magnitude MW 7.6） triggered thousands of mass move-ments in northern Pakistan. These mass movements were mainly rock falls, debris falls, rockslides and rock avalanches. The mass movements vary in size from a few hundred cubic meters up to about 100 million cubic meters estimated for the Hattian Bala rock avalanche, the biggest one associated with this earthquake. This mass movement, which moved in southeastern direction, created two natural dams on the valley bottom and blocked the water ways of the Karli and Tung tributaries of the Jhelum River. Topographic, lithologic and structural information were used to investigate the Hattian Bala rock ava-lanche. Geotechnical and structural maps were prepared to understand relationship between geology and structure of Hattian Bala rock avalanche. The geometry and failure mode of this rock avalanche are controlled by southeast plunging synclinal structures, lithology, a bedding parallel slip surface and a pre-existing old rockslide. The structural map shows that the mass movement failure was due to Danna and Dandbeh synclinal structures plunging southeast on the hanging wall block of the reacti-vated Muzaffarabad fault. The slip surface of the mass movement followed the bedding planes along mudstone, claystone and sandstone surfaces. The mass movement perfectly followed the pre-existing synclinal morphology of the Danna and Dandbeh synclines.

Comparison of Protein and Amino Acids in the Extracts of Two Edible Mushroom, <i>Pleurotus sajor-caju</i>and <i>Schizophyllum commune</i>

This study was undertaken to determine total protein (%) and profiles of amino acid and made comparison between the aqueous and organic solvent extracted mushroom. Extraction was made from two edible, Pleurotus sajor-caju (commercial) and Schizophyllum commune (wild) types of mushrooms. Four types of solvents were used for the extraction include 100% aqueous, 50% ethanol, 50% methanol and 50% acetone. True protein of mushroom extract was analyzed with colorimetric Lowry method and amino acids were determined by using high-performance liquid chromatograph (HPLC). The range of 1.06% to 3.43% and 1.30% to 2.17% total protein value were obtained in the extracts of P. sajor-caju and S. commune respectively, while the highest total protein of 3.43% was determined in aqueous extracted P. sajor-caju mushroom. The amount of total amino acids of S. commune and P. sajor-caju were in the range of 308.65 mg/g to 443.84 mg/g and 172.52 mg/g to 400.76 mg/g, respectively. The highest content of 443.84 mg/g total amino acids and 77.08 mg/g of essential amino acids were obtained in the aqueous extracted Schizophyllum commune.
