Neuroendocrine carcinoma of the ampulla of Vater:a clinicopathologic evaluation

BACKGROUND:Only 105 cases of neuroendocrine tumor(NET)of the ampulla of Vater have been described, mostly as single case *** incidence of NET is *** changes in incidence may result from changes in *** study was to determine the relative incidence and clinicopathological characteristics of high- grade neuroendocrine carcinoma(small cell carcinoma and large cell carcinoma)of the ampulla of Vater at a single institution. mETHODS:Sections from paraffin blocks of tumors of the ampulla of Vater taken from 45 patients who underwent Whipple’s procedure and 6 patients who underwent palliative bypass between September 2003 and January 2007 were subjected to immunohistochemical *** clinical and pathological data from 5 patients diagnosed with NET of the ampulla of Vater were analyzed. RESULTS:The patients were 3 men and 2 women,ranging in age from 39 to 47 years(mean 44 years).Operative procedures included Whipple’s procedure in 4 patients and palliative bypass in 1 *** examination revealed large-cell neuroendocrine carcinoma in 2 patients,small cell carcinoma in 2,and carcinoid in *** patients with high-grade neuroendocrine carcinoma who had undergone Whipple’s procedure died at postoperatively 7,11,and 13 *** patient who had undergone palliative triple bypass died 3 months after ***:The relative incidence of high-grade neuroendocrine carcinomas of the ampulla of Vater is higher than that generally *** tumors behave aggressively and have a dismal prognosis despite aggressive treatment.

Long-term outcome of gastric access loop in hepaticojejunostomy

BACKGROUND:?The incidence of hepaticojejunostomy stricture is 4%-10% in experienced centres. many access loops have been designed and used to facilitate endoscopic intervention for this complication of hepaticojejunostomy. In this study, we investigated the effectiveness and safety of gastric access loop. mETHODS:A retrospective analysis was conducted on 13 patients who had undergone hepaticojejunostomy with gastric access loop between June 1999 and September 2003. Eleven patients were followed up for a mean period of 51 months (range 20-81 months). Two patients were lost to follow up. RESULTS:?On follow-up, 8 patients had patent jejuno- gastrostomy (end to side anastomosis between Roux loop of jejunum and stomach) and hepaticojejunostomy. Three patients developed stricture of jejunogastrostomy at 41 months, 63 months and 81 months of follow-up. Among these 3 patients, one also had hepaticojejunostomy stricture. In the patient with hepaticojejunostomy stricture, dilatation of jejunogastrostomy stricture was attempted but failed. None of the patients had any evidence of bile gastritis/cholangitis. There was no procedure related morbidity/mortality. CONCLUSIONS:Further studies involving large numbers of patients are required before wide application of gastric access loop in hepaticojejunostomy though it is a safer option than percutaneous transhepatic manipulations or revision surgery.

The Approximate Analytical Solution of Non-Linear Equation for Simultaneous Internal mass and Heat Diffusion Effects

For the first time a mathematical modelling of porous catalyst particles subject to both internal mass concentration gradients as well as temperature gradients, in endothermic or exothermic reactions has been reported. This model contains a non-linear mass balance equation which is related to rate expression. This paper presents an approximate analytical method (modified Adomian decomposition method) to solve the non-linear differential equations for chemical kinetics with diffusion effects. A simple and closed form of expressions pertaining to substrate concentration and utilization factor is presented for all value of diffusion parameters. These analytical results are compared with numerical results and found to be in good agreement.

Indirect Tensile Characterization of Graphite Platelet Reinforced Vinyl Ester Nanocomposites at High-Strain Rate

An indirect tensile testing method is proposed for characterizing low strength graphite platelet reinforced vinyl ester nanocomposites at high-strain rate. In this technique, the traditional Brazilian disk (diametrical compression) test method for brittle materials is utilized along with conventional split-Hopkinson pressure bars (SHPB) for evaluating cylindrical disk specimens. The cylindrical disk specimen is held snugly in between two concave end fixtures attached to the incident and transmission bars. To eliminate the complexities of conventional strain gage application, a non-contact Laser Occluding Expansion Gage (LOEG) has been adapted for measuring the diametrical transverse expansion of the specimen under high-strain rate diametrical compressive loading. Failure diagnosis using high-speed digital photography validates the viability of utilizing this indirect test method for characterizing the tensile properties of xGnP (exfoliated graphite nanoplatelets) reinforced and additional CTBN (Carboxyl Terminated Butadiene Nitrile) toughened vinyl ester based nanocomposites. Also, quasi-static indirect tensile response agrees with previous investigations conducted using the traditional dog-bone specimen in direct tensile tests. Investigation of both quasi-static and dynamic indirect tensile test responses shows the strain rate effect on the tensile strength and energy absorbing capacity of the candidate materials. The contribution of reinforcement to the tensile properties of the candidate materials is presented.

Analytical Expression for the Concentration of Substrate and Product in Immobilized Enzyme System in Biofuel/Biosensor

In this paper, an approximate analytical method to solve the non-linear differential equations in an immobilized enzyme film is presented. Analytical expressions for concentrations of substrate and product have been derived for all values of dimensionless parameter. Dimensionless numbers that can be used to study the effects of enzyme loading, enzymatic gel thickness, and oxidation/ reduction kinetics at the electrode in biosensor/biofuel cell performance were identified. Using the dimensionless numbers identified in this paper, and the plots representing the effects of these dimensionless numbers on concentrations and current in biosensor/biofuel cell are discussed. Analytical results are compared with simulation results and satisfactory agreement is noted.

Fault Identification and Islanding in DC Grid Connected PV System

Nowadays, the DC distribution system has been suggested, as a replacement for the AC power distribution system with electric propulsion. This idea signifies a fresh approach of issuing energy for low-voltage installations. It can be used for any electrical application up to 20 mW and works at a nominal voltage of 1000 V DC. The DC distribution system is just an extension of the multiple DC links that previously available in all propulsion and thruster drives, which typically comprise more than 80% of the electrical power consumption on electric propulsion vessels. A fault detection and islanding scheme for DC grid connected PV system is presented in this paper. Unlike traditional ac distribution systems, protection has been challenging for dc systems. The goals of this paper are to classify and detect the fault in the PV system as well as DC grid and to isolate the faulted section so that the system keeps operating without disabling the entire system. The results show the measured values of power at PV panel and DC grid side under different fault condition, which indicates the type of fault that occurs in the system.

The statistical downscaling models which are used as a bridging model to connect the global climate model output and the local weather variables have uncertainty associated with *** uncertainty present in the model as well as in the results should be quantified for reliable climate change impact *** sources of uncertainty include natural variability, uncertainty in the climate model(s), downscaling model, model inadequacy and in the predicted *** analysis and quantification in the models is a promising approach for climate change impact *** this paper, a new approach called BUASCSDSEC(Bayesian uncertainty analysis for stochastic classification and statistical downscaling with stochastic dependent error coupling) is *** is a robust Bayesian uncertainty analysis methodology and tools for combined classification(to predict the occurrence of rainfall) and statistical *** is based on coupling dependent modelling error which is viewed as a function modelled as a stochastic process with classification and statistical downscaling models in a way that the dependency among modelling errors will impact the result of the classification and statistical downscaling model calibration and uncertainty analysis for future *** Process is considered in the error *** data are used and the uncertainty and prediction results are obtained for the validation period(1995-2000).It is observed that the CDFs of the daily predicted samples are consistent with the observed CDF of *** uncertainty is smaller for the extreme rainfall and the uncertainty for smaller amount of rainfall is more compared to that for the extreme *** the results obtained, ongoing research for improvement is briefly presented.

Dynamic Flexural modulus and Low-Velocity Impact Response of Supercomposite<sup>Tm</sup>Laminates with Vertical Z-Axis milled Carbon Fiber Reinforcement

In work reported here, the dynamic properties and low-velocity impact response of woven carbon/epoxy laminates incorporating a novel 3D interlaminar reinforcement concept with dense layers of Z-axis oriented milled carbon fiber SupercompositeTM prepregs, are presented. Impulse-frequency response vibration technique is used for non-destructive evaluation of the dynamic flexural modulus (stiffness) and loss factor (intrinsic damping) of woven carbon/epoxy control and SupercompositeTM laminates. Low-velocity punch-shear tests were performed on control and SupercompositeTM laminates according to ASTM D3763 Standard using a drop-weight impact test system. Control panels had all layers of 3K plain woven carbon/epoxy prepregs, with a dense interlaminar reinforcement of milled carbon fibers in Z- direction used in designing the SupercompositeTM laminate—both having same areal density. Impulse-frequency response vibration experiments show that with a 50% replacement of woven carbon fabric in control panel with milled carbon fibers in Z direction dynamic flexural modulus reduced 25% - 30% (loss in stiffness) and damping increased by about the same 25% - mily:Verdana;">
