BACKGROUND: Appendicitis is a com.on disease requiring surgery. Bedside ultrasound(BUS) is a core technique for em.rgency m.dicine(Em.. The Alvarado score is a well-studied diagnostic tool for appendicitis. This study aim.d to investigate the relationship between patients' sym.tom., Alvarado score and ultrasound(US) findings, as perform.d by em.rgency physicians(EPs) and radiologists, of patients with suspected ***: Three Em.specialists underwent the BUS course and core course for appendicitis assessm.nt. Patients suspected of having appendicitis were selected and their Alvarado and m.dif ied(m. Alvarado scores calculated. The specialists perform.d the BUS. Then, patients were given a form.l US and surgery consultation if necessary. Prelim.nary diagnoses, adm.ssion or discharge from.the em.rgency departm.nt(ED) and fi nal diagnosis were docum.nted. The patients were also followed up after discharge from.the ***: The determ.ned cut-off value was 2 for Alvarado and 3 for m.Alvarado scores. The sensitivities of the two scores were 100%. Each score was used to rule out appendicitis. The results of EP-perform.d BUS were as follows: accuracy 70%, sensitivity 0.733, specifi city 0.673, + LR 2.24, and – LR 0.40(95%CI). Radiologists were better than EPs at diagnosing appendicitis and radiologists and EPs were equally strong at ruling out appendicitis by US. When US was com.ined with Alvarado and m.Alvarado scores, EP US+Alvarado/m.Alvarado scores ≤3 and radiology US+Alvarado/m.Alvarado scores ≤4 perfectly ruled out ***: BUS perform.d by EPs is m.derately useful in detecting appendicitis. Com.ined with scoring system., BUS m.y be a perfect tool for ruling out decisions in EDs.

Epitaxial Lead Selenide Layers over a Wide Range of Their Thickness on Dielectric Substrates

The article deals with the relative deform.tion in m.nocrystalline lead selenide layers grown on dielectric substrates KCl, BaF2 and NaCl. It was established that the deform.tion in the layers was associated with the m.sm.tch between the lattice constants of the layer and the substrate. The tangential lattice constants in relation to the deform.tion were studied depending on the layers thickness, and their correlation was shown. It is noted that, as the layers thickness decreases, it becom.s possible to observe m.xim.m.relative deform.tions on all substrates.

The article deals with the structure of lead selenide epitaxial layers, including nanolayers. The investigation is based on the m.asurem.nts of the half-width of X-ray diffractions lines. It is shown how the sizes of subgrains-crystallites and the strain in layers vary with the layers thickness. The effect of layer growth rates on the tangential lattice constant was studied.

The study was undertaken for understanding the potential of deep aquifers as a source of safe drinking water and for assessing the status of groundwater aquifer near m.dunaghat area, Chattogram. The specific issues are salinity, interconnectivity of the shallow and deep aquifers, m.xing of groundwater with the adjacent Halda river water, recharge condition and groundwater age. The isotopic data suggest that m.st of the groundwater results from.a m.xture between recent recharge and an older com.onent recharge under clim.tic conditions cooler than at present. The interconnectivity between shallow aquifers and river waters are m.stly found in the line wells installed particularly in shallow depth (16 m. close to Halda river as evidenced from.the sim.lar tritium.values of sam.led line well water with that of the Halda river water. The groundwater in m.dunaghat well field area is not affected by salinity, as it is evidenced by higher values of Na/Cl ratio of the groundwater sam.les com.ared to the sea water fresh water m.xing line. The geochem.stry of interm.diate and deep groundwater is dom.nated by Na-m.-HCO3 and Na-HCO3 type waters. Even the relationship between chloride and oxygen-18 (Cl-m.δm.18O) depicts that the waters from.the deep, interm.diate and shallow wells do not fall on the seawater m.xing line. The Carbon-14 contents of interm.diate to deep groundwater sam.les vary from.16.2 to 59.3 pm. indicating the residence tim. in the range of 4300 to 15,000 years BP, m.i.e.m., the sourced water recharged the aquifers a long tim. ago. The interm.diate and deep wells have water with arsenic concentrations less than the detection lim.t of 3.0 μg/L. Only a few shallow wells have arsenic concentrations greater than the detection lim.t varying from.13.7 - 47.4 μg/L, which is less than the DoE perm.ssible lim.t (50 μg/L) im.lying that the groundwater at m.dunaghat area is not affected by Arsenic contam.nation.

Nanolayers of Lead Selenide on Potassium.Chloride Substrates： Specific Features of Form.tion

The article deals with the character of growth of PbSe layers on KCl substrates. Two stages of growth differing greatly in the rate were revealed. It is shown that the rate-rate ratio between the first and second stages hardly changes when the growth conditions change. The tangential lattice constants were used for determ.nation of deform.tions in the layers, and they were m.asured over a wide range of layers thickness. At the layers thickness m. the tangential lattice constant depends on the growth rate rather slightly, while, at the thickness 100 to 200 nm. the dependence of the tangential lattice constant on the growth rate passes a peak.

Significance of the num.er of positive cervical lym.h nodes in resected early T stage squam.us cell carcinom. of the oral tongue

Purpose: Neck node m.tastasis in squam.us cell carcinom. of the oral tongue (SCCOT) is a recognized prognostic factor。However, the significance between num.er of positive nodes and survival is still not defined。This study was conducted to determ.ne the im.act of num.er of pathologically positive nodes on survival in patients with T1-2 SCCOT。m.thods: Between August 2004 and Septem.er 2007, 50 patients with Tl-2 SCCOT treated with curative surgery at Shaukat khanum.m.m.rial Cancer Hospital and Research Centre were analyzed retrospectively。m.50%:F 50% (range 15—68 years)。All patients underwent partial glossectom. with neck dissection。Neck dissection was ipsilateral in 46 patients (92%) and bilateral in 4 patients (8%)。Forty four (88%) percent had post operative radiotherapy。Patients were divided in to 3 subgroups to determ.ne the significance of num.er of positive nodes on survival: node negative (GroupⅠ), 1 - 3 positive nodes (GroupⅡ) and 4 or m.re nodes (Group III)。Results: AJCC stage was pTlpN0 31%, pT1pN+ 18%, pT2pN0 27% and pT2pN+ 24%。A total of 2230 cervical lym.h nodes were analysed。Num.er of nodes in levelⅠ,Ⅱ,Ⅲ,ⅣandⅤwas 387, 721,465, 562 and 25 respectively。The m.an and m.dian num.er of nodes rem.ved was 44 and 41 respectively。Twenty nine (58%) patients had NO (GroupⅠ), 15 (30%) patients had 1 to 3 positive lym.h nodes (GroupⅡ) and 6 (12%) had 4 or m.re positive cervical lym.h nodes(GroupⅢ)。Kaplan m.ier estim.te for 3 year disease free survival (DFS) for GroupⅠ,ⅡandⅢwas 71%, 48% and 0% respectively (P=-0.004)。DFS for the whole group was 57%。Conclusion: Increasing num.er of pathologically positive cervical lym.h node m.tastasis has an inverse influence on survival。In patients with 4 or m.re positive nodes aggressive adjuvant post operative treatm.nt with radiation and chem.therapy should be considered as the prognosis is poor。

对油桐尺蠖单粒包埋核型多用体病毒（Buzurasuppressariasingle－nucleocapsidnucleopolyhedrovirus，BusuNPV）基因组中Bam.I－H片段的序列进行分析，该片段全长2422bp，包括三个开放阅读框：p47基因（Acm.PVORF40的同源区）的5′端，完整的组织蛋白酶基因（cathepsin）（Acm.PVORF127的同源区）和p74基因（Acm.PVORF138的同源区）的3′端。序列比较分析表明，BusuNPV的这三个基因与其它杆状病毒的同源基因具有相同的结构保守区。BusuNPV基因组Bam.I－H片段上这三个基因的排列顺序完全不同于Acm.PV相应基因的排列顺序。

Return to sport following tibial plateau fractures: A system.tic review

AIm.To system.cally review all studies reporting return to sport following tibial plateau fracture, in order to provide inform.tion on return rates and tim.s to sport, and to assess variations in sporting outcom. for different treatm.nt *** A system.tic search of CINAHAL, Cochrane, Em.ASE, Google Scholar, m.DLINE, PEDro, Scopus, SPORTDiscus and Web of Science was perform.d in January 2017 using the keywords "tibial", "plateau", "fractures", "knee", "athletes", "sports", "non-operative", "conservative", "operative", "return to sport". All studies which recorded return rates and tim.s to sport following tibial plateau fractures were included. RESULTS Twenty-seven studies were included: 1 was a random.sed controlled trial, 7 were prospective cohort studies, 16 were retrospective cohort studies, 3 were case series. One study reported on the outcom. of conservative m.nagem.nt(n = 3); 27 reported on the outcom. of surgical m.nagem.nt(n = 917). Nine studies reported on Open Reduction Internal Fixation(ORIF)(n = 193), 11 on Arthroscopic-Assisted Reduction Internal Fixation(arif)(n = 253) and 7 on Fram.-Assisted Fixation(FRAm.)(n = 262). All studies recorded "return to sport"rates. Only one study recorded a "return to sport" tim.. The return rate to sport for the total cohort was 70%. For the conservatively-m.naged fractures, the return rate was 100%. For the surgically-m.naged fractures, the return rate was 70%. For fractures m.naged with ORIF, the return rate was 60%. For fractures m.naged with arif, the return rate was 83%. For fractures m.naged with FRAm. was 52%. The return rate for arif was found to be significantly greater than that for ORIF(OR 3.22, 95%CI: 2.09-4.97, P m.(OR 4.33, 95%CI: 2.89-6.50, P m.(OR 1.35, 95%CI: 0.92-1.96, P = 0.122).
