Aim: To evaluate the effect of oral p.locarp.ne treatment on conjunctival ep.thelium of p.tients with Sjgren’s syndrome (SS). Methods: 15 p.imary SS p.tients were included in this p.osp.ctive, single masked, comp.rative study. p.tients underwent oral p.locarp.ne treatment for 2 months and were studied before (TO) and after 1 month (T1), 2 months (T2), and 15 days after treatment susp.nsion (T3). Systemic and ocular symp.oms, tear film break up.time (BUT), corneal fluorescein vital staining, Schirmer I test, tear basal secretion test, and conjunctival imp.inting were p.rformed. Student’s t test and Mann-Whitney U test were used for statistics. Results: The conjunctival imp.inting showed an increase of goblet cells number at T1 (1.6 (1.2) v 0.6 (0.7) at T0, p.0.025) imp.oving at T2 (5.1 (1.7); p. 0.001 v T0 and T1). At T3 the number of goblet cells significantly decreased (1.9 (1.1); p. 0.001 v T2). An imp.ovement of dry mouth started at T1 and returned towards baseline values at T3. For ocular symp.oms, burning and foreign body sensation were imp.oved at T1 while ocular dryness imp.oved at T2. BUT showed a statistically significant imp.ovement at T2. Conclusions: Oral p.locarp.ne induced an increase in goblet cells number and an amelioration of conjunctival ep.thelium not dep.ndent on tear secretion.

Aim and p.rp.se: To evaluate the effects of two NSAIDs on corneal sensitivity and ocular surface in Sj¨ogren’s syndrome (SS) p.tients. Methods: In all, 20 SS p.tients with ep.thelial corneal defects, were randomly divided into two group.: group.1 (10 females, age 35-63 years), treated with 0.1%indomethacin, one drop.three times a day; group.2 (nine females, one male, age 38-65 years) treated with 0.1%diclofenac, at the same regimen. No systemic NSAIDs were allowed. Use of tear substitute was allowed. Corneal sensitivity, corneal staining, BUT, and ocular discomfort, were evaluated before and after 15, 30 days of treatment and 7 days after NSAID discontinuation. For statistical analysis, the Student’s t-test and Mann-WhitneyU test were used. Results: Both group. showed at day 30 a statistically significant reduction of corneal sensitivity (p.p.showed a statistically significant lower sensitivity if comp.red to the indomethacin-treated group.(p.p.2, 7 days after the discontinuation of the therap. (p.0.02). The ocular discomfort score was statistically significantly reduced in both group. starting from day 15 (p.p.oms of ocular discomfort in SS p.tients. However, they should be usedwith caution and under close monitoring, and the treatment should be p.omp.ly discontinued if corneal ep.thelial defects develop.or worsen during treatment.

Ultrasound detection of abdominal lymp.adenomegaly in subjects with hep.titis C virus infection and p.rsistently normal transaminases:a p.edictive index of liver histology severity

AIM The indications for liver biop.y in anti HCV p.sitive p.tients with p.rsistently normal alanine aminotransferase levels are not clearly established. Recent studies have correlated the p.esence of abdominal lymp.oadenomegaly with disease severity in p.tients with chronic hep.titis C. Our study aimed to evaluate the frequency of abdominal lymp.oadenomegaly in an anti HCV p.sitive blood donor p.p.lation with p.rsistently normal alanine aminotransferase and the relationship.of abdominal lymp.oadenomegaly with the severity of liver changes. METHODS Eighty six anti HCV p.sitive blood donors (58 M, 28 F) with normal alanine aminotransferase were followed up.for a median of 31 months (range 12-50). To evaluate the frequency of abdominal lymp.oadenomegaly, all p.tients underwent ultrasound scan. The common p.rameters of liver function as well as serum HCV RNA levels were determined. Histological changes were evaluated both in a conventional manner and using the numerical scoring systems of Knodell and Desmet. RESULTS Of the 86 donors, 68 (79%) maintained p.rsistently normal alanine aminotransferase levels during follow up. and abdominal lymp.oadenomegaly was p.esent in 15 of them (22 0%). The remaining 18 donors (21%) showed rises in alanine aminotransferase above normal levels during the follow up.and seven of them (38%) had abdominal lymp.oadenomegaly ( p. =n.s.). In the subjects with normal alanine aminotransferase, there were no significant differences in the common p.rameters of liver function and the serum p.esence of HCV RNA between those with or without abdominal lymp.oadenomegaly. Normal liver was found in five p.tients without abdominal lymp.oadenomegaly, but never in p.tients with abdominal lymp.oadenomegaly. Analysis with the Mantel Haenszel test showed a trend toward more serious changes in p.tients with abdominal lymp.oadenomegaly (chi square MH=9 5, p. <0 003).
