Transient receptor potential(TRP)channels are strongly associated with colon cancer development and *** study leveraged a multivariate Cox regression model on publicly available datasets to construct a TRP channels-associated gene signature,with further validation of signature in real world samples from our hospital treated patient ***-Meier(K-M)survival analysis and receiver operating characteristic(ROC)curves were employed to evaluate this gene signature’s predictive accuracy and robustness in both training and testing cohorts,***,the study utilized the CIBERSORT algorithm and single-sample gene set enrichment analysis to explore the signature’s immune infiltration landscape and underlying functional *** support vector machine algorithm was applied to evaluate the signature’s potential in predicting chemotherapy *** findings unveiled a novel three TRP channels-related gene signature(MCOLN1,TRPM5,and TRPV4)in colon adenocarcinoma(COAD).The ROC and K-M survival curves in the training dataset(AUC=0.761;p=1.58e-05)and testing dataset(AUC=0.699;p=0.004)showed the signature’s robust predictive capability for the overall survival of COAD *** of the immune infiltration landscape associated with the signature revealed higher immune infiltration,especially an increased presence of M2 macrophages,in high-risk group patients compared to their low-risk ***-risk score patients also exhibited potential responsiveness to immune checkpoint inhibitor therapy,evident through increased CD86 and PD-1 expression ***,the TRPM5 gene within the signature was highly expressed in the chemoresistance group(p=0.00095)and associated with poor prognosis(p=0.036)in COAD patients,highlighting its role as a hub gene of ***,this signature emerged as an independent prognosis factor for COAD patients(p=6.48e-06)and expression of model gene are validated by public data a

Novel insights into the historical biogeography of the streak-breasted scimitar babbler complex （Aves： Timaliidae： Pomatorhinus ruficollis complex）

The Streak-breasted Scimitar Babblers of the Pomatorhinus ruficollis species complex are found in most of the forested habitats across southern and eastern Asia. The diversification history of this group is obscured by high plumage variation across populations and conflicting genetic signal across loci. We combined genetic and geographic data from several recent studies to investigate how these species diversified across China using both phylogenetic and ecological niche modeling analyses. These two lines of evidence are consistent in showing that two well-sampled species, P. reconditus and P. nigrostellatus, in central and southern China respectively, likely experienced a history of isolation and expansion as suitable habitat contracted during the last interglacial and expanded in patchy extent during the glacial maximum. The genetic analysis showed that populations of P. nigrostellatus on Hainan Island are very similar to the ones in nearby mainland southern China. We recovered two well-supported clades within P. reconditus that were not geographically structured with both containing individuals from the same localities across central China. This phylogenetic result corresponded to the ecological niche models that showed expansion from refugia since the last interglacial. This study illustrates the value of using an integrative approach and detailed geographic sampling to help understand the recent diversification of birds in China [Current Zoology 61 （5）： 910-921, 2015].

Hypokalemic myopathy in a patient with gluten-sensitive enteropathy and dermatitis herpetiformis Duhring: A case report

The case of a 22-year-old patient with symptomatic hypokalemia caused by rhabdomyolysis is presented as a rarely reported complication of gluten-sensitive enteropathy (GSE) and dermatitis herpetiformis Duhring. The patient's myopathy ceased on potassium supplementation and her other complaints resolved while on gluten-free *** was otherwise uneventful with a rapid decline in serum CPK level. At the time of her last follow-up a few months later, she was free of symptoms and CPK remained stable. Patients with GSE may present with hypokalemia in association with diarrhea and emesis, and if potassium loss is rapid, rhabdomyolysis may occur.

Application of the modified vaccination technique for the prevention and cure of chronic ailments

Over the years vaccination has proven to be the most successful health protection program for large populations,to prevent them from acquiring serious infectious and contagious diseases caused by exogenous antigens(ags) such as bacteria and *** is generally achieved by an active immunization program,though passive immunization has also been employed, especially in the past,to combat diseases caused by certain bacterial infections(***, diphtheria,etc.). Most recently,encouraging research data suggests that therapeutic approaches employing vaccination techniques can also be used to correct or deal with mishaps induced by or involving endogenous ***,most attempts at employing conventional vaccination techniques to do so have proven less than *** the case of cancer,one of the reasons for this is that the presentation of cancer related ags in presently available immunization frameworks is unable to evoke a powerful,specific cancer killing ***,drug treatments have been required in order to achieve additional beneficial effects. Recently,the Barabas group has developed a new vaccination technique(the third vaccination method,after active and passive immunization) called Modified Vaccination Technique(MVT). In experiments the MVT has been able to prevent-and with equal effectiveness,terminatemishaps induced by or involving endogenous ags,*** an experimental autoimmune kidney disease called slowly progressive Heymann nephritis(SPHN). The MVT is safe,and is able to initiate a specific immune response in the injected host(provided the injected components are in pure form).The MVT promises to provide the next generation of vaccines for the prevention,treatment,and termination of chronic disorders in humans,such as autoimmune diseases,cancer,and chronic infections.

Alcohol use disorder(AUD)is a worldwide problem for individuals of varying ages and backgrounds and is associated with the underlying cause of alcoholic liver disease,liver cirrhosis,liver cancer,or many other common diseases and health *** treatments such as cognitive behavioral therapy(CBT)have been demonstrated as an evidence-based treatment to aid individuals struggling with ***,these treatments have excessive costs and time demand with trained *** this paper,we examine the efficacy and long-term impacts of digitally delivered CBT and other online telehealth tools and apps for AUD *** show the effectiveness in the ability of digitally delivered CBT to decrease alcohol use in AUD *** additional use of online technologies and smartphone apps for post-CBT care demonstrates that such computer-aided apps could have long-term effects when it is continually employed,which opens the door for AUD patients who were not seeking treatment *** longitudinal examination research is needed to evaluate the lasting effects in liver health and other chronic diseases associated with digitally delivered alcohol reduction for AUD patients.

Successful use of adalimumab for treating fistulizing Crohn's disease with pyoderma gangrenosum:Two birds with one stone

Crohn＇s disease （CD） is a chronic relapsing and remitting autoinflammatory disorder of the gastrointestinal tract that has many intestinal and extraintestinal complications. The purpose of treatment is long-term remission, reduction of complications, and improvement of patients＇ quality of life. In many cases, this can be quite challenging and it is necessary to have a well thought out management strategy. We present the case of a 38-year-old woman with fistulizing CD that manifested as diffuse abdominal pain and bloody diarrhea accompanied by arthralgia. In addition, there were ulcerative lesions surrounded by cutaneous inflammation and erythema on her extremities, indicative of pyoderma gangrenosum. The patient was treated with high doses of parenteral methylprednisolone without any improvement and was started on adalimumab. A positive response to adalimumab therapy was observed： after 2 mo of therapy, the ulcerative skin lesion healed completely and the enterogastric fistula was closed affcer 5 mo adalimumab treatment. Adalimumab might be a suitable initial as well as maintenance therapy in patients with complicated CD.

Metabolic changes in the lower esophageal sphincter influencing the result of anti-reflux surgical interventions in chronic gastroesophageal reflux disease

AIM: With the availability of a minimally invasive approach, anti-reflux surgery has recently experienced a renaissance as a cost-effective alternative to life-long medical treatment in patients with gastroesophageal reflux disease (GERD). We are not aware of the fact whether reflux episodes causing complaints for a long time i.e., at least for one year are associated with metabolic changes in the lower esophageal sphincter, and if so, whether these may influence functional results achieved after anti-reflux surgery. METHODS: Between 1 January 2001 and 31 December 2002 we performed anti-reflux surgery on 79 patients. Muscle samples were taken from the lower esophageal sphincter (LES) in 33 patients during anti-reflux surgery. Inclusion criteria were: LES resting pressure below 10 mmHg and a marked, pH proven acid exposure to the esophagus of at least one year's duration, causing subjective complaints and requiring continuous proton pump inhibitor treatment. Control samples were obtained from muscle tissue in the gastroesophageal junction that had been removed from 17 patients undergoing gastric or esophageal resection. Metabolic and lysosomal enzyme activities and special protein concentrations 16 parameters in total were evaluated in tissue taken from control specimens and tissue taken from patients with GERD. The biochemical parameters of these intra-operative biopsies were used to correlate the results of anti-reflux operations (Visick Ⅰ and Ⅱ-Ⅲ). RESULTS: In the reflux-type muscle, we found a significant increase of the energy-enzyme activities e.g., creatine kinase, lactate dehydrogenase, β-hydroxybutyrate dehydrogenase, and aspartate aminotransaminase-. The concentration of the structural protein S-100 and the myofibrillar protein troponin Ⅰ were also significantly increased. Association of extraintestinal manifestations of inflammatory bowel disease in a province of western Hungary with disease phenotype:Results of a 25-year follow-up study

AIM:IBD is a systemic disease associated with a large number of extraintestinal manifestations (EIMs).Our aim was to determine the prevalence of EIMs in a large IBD cohort in Veszprem Province in a 25-year follow-up study. METHODS:Eight hundred and seventy-three IBD patients were enrolled (ulcerative colitis/UC/:619,m/f:317/302, mean age at presentation:38.3 years,average disease duration:11.2 years;Crohn's disease/CD/:254,m/f:125/129, mean age at presentation:32.5 years,average disease duration:9.2 years).Intestinal,extraintestinal signs and laboratory tests were monitored *** alteration suggesting an EIMs was investigated by a specialist. RESULTS:A total of 21.3% of patients with IBD had EIM (UC:15.0%,CD:36.6%).Age at presentation did not affect the likelihood of *** of EIMs was higher in women and in CD,ocular complications and primary sclerosing cholangitis (PSC) were more frequent in *** UC there was an increased tendency of EIM in patients with a more extensive *** complications were more frequent in CD (22.4% vsUC 10.2%,P<0.01).In UC positive family history increased the risk of joint complications (OR:3.63).In CD the frequency of type-1 peripheral arthritis was increased in patients with penetrating disease (P=0.028).PSC was present in 1.6% in UC and 0.8% in *** complications were present in 3.8% in UC and 10.2% in CD,the rate of ocular complications was around 3% in both *** complications were glomerulonephritis,autoimmune hemolytic anaemia and celiac disease. CONCLUSION:Prevalence of EIM in Hungarian IBD patients is in concordance with data from Western *** high number of EIM supports a role for complex follow-up in these patients.
