Lower extremity amputations and long-term outcomes in diabetic foot ulcers:A systematic review

BACKGROUNDDiabetes mellitus causes a large majority of non-traumatic major and minoramputations globally. Patients with diabetes are clinically complex with amultifactorial association between diabetic foot ulcers (DFU) and subsequentlower extremity amputations (LEA). Few studies show the long-term outcomeswithin the cohort of DFU-associated *** highlight the long-term outcomes of LEA as a result of ***/MEDLINE and Google Scholar were searched for key terms, “diabetes”,“foot ulcers”, “amputations” and “outcomes”. Outcomes such as mortality, reamputation,re-ulceration and functional impact were recorded. Peer-reviewedstudies with adult patients who had DFU, subsequent amputation and follow upof at least 1 year were included. Non-English language articles or studiesinvolving children were *** total of 22 publications with a total of 2334 patients were selected against theinclusion criteria for review. The weighted mean of re-amputation was 20.14%,29.63% and 45.72% at 1, 3 and 5 years respectively. The weighted mean of mortality at 1, 3 and 5 years were 13.62%, 30.25% and 50.55% respectively withsignificantly higher rates associated with major amputation, re-amputation andischemic *** LEA, level of the LEA and patient comorbidities were significant riskfactors contributing to re-ulceration, re-amputation, mortality and depreciatedfunctional status.

Predicting the mechanical properties of additively manufactured parts is often a tedious process,requiring the integration of multiple stand-alone and expensive ***,as properties are highly location-dependent due to repeated heating and cooling cycles,the properties prediction models must be run for multiple locations before the part-level performance can be analyzed for certification,compounding the computational *** work has proposed a rapid prediction framework that replaces the physics-based mechanistic models with Gaussian process metamodels,a type of machine learning model for statistical inference with limited *** metamodels can predict the varying properties within an entire part in a fraction of the time while providing uncertainty *** framework was demonstrated with the prediction of the tensile yield strength of Ferrium?PH48S maraging stainless steel fabricated by additive *** agreement was found between the metamodels and the mechanistic models,and the computation was dramatically decreased from hours of physics-based simulations to less than a second with *** method can be extended to predict various materials properties in different alloy systems whose processstructure-property-performance interrelationships are linked by mechanistic *** is powerful for rapidly identifying the spatial properties of a part with compositional and processing parameter variations,and can support part certification by providing a fast interface between materials models and part-level thermal and performance simulations.

Revisions to the Tumor,Node,Metastasis staging of lung cancer(8th edition):Rationale,radiologic findings and clinical implications

The Tumor, Node, Metastasis(TNM) staging system,adopted by the Union for International Cancer Control(UICC) and the American Joint Committee on Cancer(AJCC), has been recently revised, with the new 8^(th) edition of the staging manual being published in January 2017. This edition has few but important evidencebased changes to the TNM staging system used for lung cancer. Radiologists should be aware of the updated classification system to accurately provide staging information to oncologists and oncosurgeons. In this article, we discuss the rationale, illustrate the changes with relevance to Radiology, and review the clinical implications of the 8^(th) edition of the UICC/AJCC TNM staging system with regards to lung cancer.

该文对印度公路和道路运输部所规定的3种级配沥青混合料进行了四分点疲劳试验,并评价了其疲劳寿命。试验采用应变控制模式,加载为频率10Hz的正弦加载,温度为20℃。所有混合料的空隙率都控制在(4.0±0.2)%,同时对沥青混合料进行了马歇尔残留稳定度和间接拉伸强度的测试以评价其性能。在此基础上,提出了一个评价沥青混合料疲劳寿命的新现象学模型。试验研究表明:弹性改性沥青疲劳寿命最长,并且具有最低的应变敏感性。塑性改性沥青混合料太脆导致在高应变水平下表现较差,SMA的疲劳寿命比密级配沥青混合料的疲劳寿命高近5倍。新的模型不仅与试验结果高度吻合,而且在统计上也比传统模型更加可靠。

Objective Sulfanilamide,sulfadiazine,and dapsone were the first sulfonamides to be used to treat malaria by disrupting the folate biosynthesis process,which is essential for parasite ***,we aimed to synthesize novel N-(2-arylaminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide derivatives through a rational drug design *** All compounds were synthesized by the conventional method,and the products were characterized by spectral analysis(1 H NMR and mass spectrometry).The progression of the reaction was monitored using thin-layer chromatography(TLC).All the derivatives were analyzed for their effective binding mode in the allosteric site of the plasmodium cysteine protease *** and antifungal activities were determined using the broth dilution *** S6(N-(2-thiazol-4 yl)-acetyl-aminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide and S9(N-(1 H-benzo[d]imidazol-2-yl)aminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide formed five hydrogen bonds;S8(N-(2-1 H-imidazol-2 yl)aminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide and S10(N-(1 H-benzo[d]imidazol-5-yl)aminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide formed four hydrogen bonds with the allosteric site of the *** the docking scores and formation of hydrogen bonds with the target enzyme,the novel derivatives were processed for wet lab *** the newly synthesized derivatives were subjected to in vitro antimalarial,antifungal,and antibacterial *** the derivatives exhibited sufficient sensitivity to the Plasmodium falciparum strain compared to the ***,compounds S9 and S10 showed the most potent dual antimicrobial and antimalarial *** also exhibited powerful molecular interactions in molecular docking *** Based on the above results,it was concluded that N-(2-arylaminophenyl)-2,3-diphenylquinoxaline-6-sulfonamide derivatives have excellent biological potential to act as antimalarial,antifungal,
