Should hot biopsy forceps be abandoned for polypectomy of diminutive colorectal polyps?

Standardized approach to polypectomy of diminutive colorectal polyps(DCPs) is lacking since cold biopsy forceps have been associated with high levels of recurrence, hot biopsy forceps are considered inadequate and risky and cold snaring is currently under investigation for its efficacy and safety. This has led to confusion and a gap in clinical practice. This article discusses the usefulness and contemporary practical applicability of hot biopsy forceps and provides wellintentioned criticism of the new European guidelines for the treatment of DCPs. Diminutive colorectal polyps are a source of frustration for the endoscopist since their small size is accompanied by a considerable risk of premalignant neoplasia and a small but nonnegligible risk of advanced neoplasia and even cancer. Since the proportion of diminutive colorectal polyps is substantial and exceeds that of larger polyps, their effective removal poses a considerable workload and a therapeutic challenge. During the last decade, the introduction of cold snaring to routine endoscopy practice has attempted to overcome the use of prior techniques, such as hot biopsy forceps. It is important to recognize that with the exception of endoscopic methods that are obviously unsafe and inadequate to serve their purpose, all other interventional endoscopic methods are operator-dependent in the sense that specific expertise and training are obligatory for the success of any therapeutic intervention. Since relevant publications on hot biopsy forceps are still in favor of its careful use, as it has not yet demonstrated inferiority compared with newer techniques, it would be prudent for any medical practitioner to evaluate the available tools and judge any new proposed technique based on the evidence before it is adopted.

Electro-Hydrodynamic(EHD) Thrust Analysis in Wire-Cylinder Electrode Arrangement

The thrust generation by electro-hydrodynamic （EHD） effect has been studied for a wire-cylinder arrangement under high DC voltage. Series of measurements have been conducted in order to determine the relationship between generated thrust and corona discharge current, as well as its dependence on geometrical characteristics of the electrodes, e.g. electrode gap, wire and cylinder radii. The experimental investigation has shown a linear relationship between the generated thrust and the discharge current, while parametric analysis showed that increased electrode gap and emitter radius reduces the thrust. On the other hand, large gaps favor the thrust per unit power ratio.

The emergence of orthopedic tissue engineering devices as viable clinical solutions demands that the field address significant clinical complications associated with the implantation of foreign materials in the body,especially *** tissue engineering has focused on the development of methods to regenerate and repair,until recently there has been a relative dearth of literature regarding the intersection of tissue engineering and *** particular,local delivery of antimicrobials has long been of clinical interest,but only recently has that been translated into the realm of tissue *** this perspective,we briefly review major modes of local delivery for infection prevention and treatment and discuss possible strategies for preventing implant-associated infections.

Inferior mesenteric arteriovenous fistula: Case report and world-literature review

Arteriovenous fistulas between the inferior mesenteric artery and vein are rare, with only 26 primary and secondary cases described in the literature. Secondary fistulas occur following operations of the left hemicolon and manifest as abdominal pain, abdominal mass, gastrointestinal bleeding, colonic ischemia and portal hypertension. Symptom intensities are flow-dependent, and can range from minimal symptoms to severe heart failure due to left to right shunt. Diagnosis is usually established by radiological or intraoperative examination. Treatment options include embolization and/or surgical resection. Therapeutic decisions should be adapted to the unique characteristics of the fistula on an individual basis. A new case of a primary arteriovenous fistula is described and discussed along with a complete review of the literature. The patient in this report presented with signs and symptoms of colonic ischemia without portal hypertension. The optimal treatment for this pa-tient required a combination of embolization and surgical operation. The characteristics of these rare inferior mesenteric arteriovenous fistulas are examined and some considerations concerning diagnostic and therapeutic strategies that should be followed are presented.

Data-driven comparison of federated learning and model personalization for electric load forecasting

Residential short-term electric load forecasting is essential in modern decentralized power *** forecasting methods mostly rely on neural networks and require access to private and sensitive electric load data for model *** neural network training aggregates all data on a centralized server to train one global ***,the aggregation of user data introduces security and data privacy *** contrast,this study investigates the modern neural network training methods of federated learning and model personalization as potential solutions to security and data privacy *** an extensive simulation approach,the investigated methods are compared to the conventional centralized method and a pre-trained baseline predictor to compare their respective *** study identifies that the underlying data structure of electric load data has a significant influence on the loss of a *** therefore conclude that a comparison of loss distributions will in fact be considered a comparison of data structures,rather than a comparison of the model *** an alternative method of comparison of loss values,this study develops the"differential comparison".The method allows for the isolated comparison of model loss differences by only comparing the losses of two models generated by the same data sample to build a distribution of *** differential comparison method was then used to identify model personalization as the best performing model training method for load forecasting among all analyzed methods,with a superior performance in 59.1%of all cases.

Non-alcoholic fatty liver disease(NAFLD)is an umbrella term,describing a range of chronic liver disorders associated with fat deposition in hepatocytes,defined as steatosis,and it is mostly emerged in the presence of obesity,hyperlipidemia and insulin *** has been classified as the most frequent liver condition worldwide with more than 10%of NAFLD patients progressing to the inflammatory and fibrotic disorder of non-alcoholic steatohepatitis(NASH),which in turn can lead to end stage liver disease including hepatocellular carcinoma(HCC);the most frequent malignant liver tumor(1).During HCC development the immune system interacts with the tumour microenvironment and it can stimulate its growth by maintaining a chronic inflammatory milieu and the production of factors that favor tumor growth,survival,and *** phenomena may also take place and have significant implications in the transition from NASH to HCC(2).Although,it is well established that chronic inflammation within the hepatic microenvironment is of great significance in HCC development,the exact mechanisms leading to the transition from NASH to HCC are yet to be elucidated.
