Waste heat recovery is one of the possible solutions to improve the efficiency of internal combustion *** of wasting the exhaust stream of an energy conversion system into the environment,its residual energy content can be usefully recovered,for example in Organic Rankine Cycles(ORC).This technology has been largely consolidated in stationary power plants but not yet for mobile applications,such as road transport,due to the limitations in the layout and to the constraints on the size and weight of the ORC *** ORC system installed on the exhaust line of a bus powered by a natural gas spark ignition engine has been *** thermal power available at engine exhaust has been evaluated by measuring gas temperature and mass flow rate during real driving *** waste thermal power has been considered as heat input for the ORC plant simulation.A detailed heat exchanger model has been developed because it is a crucial component for the ORC *** exergy analysis of the ORC was performed comparing different working fluids:R601,R1233zd(E)and two zeotropic blends of the two organic pure *** model allowed the evaluation of the ORC produced energy over the driving cycle and the potential benefit on the engine efficiency.

Expression of matrix metalloproteinase-11 is increased under conditions of insulin resistance

AIM To investigate matrix metalloproteinase-11(MMP-11) expression in adipose tissue dysfunction, using in vitro and in vivo models of insulin *** Culture of mouse 3T3-L1 preadipocytes were induced to differentiation into mature 3T3-L1 adipocytes. Cellular insulin resistance was induced by treating differentiated cultured adipocytes with hypoxia and/or tumor necrosis factor(TNF)-α, and transcriptional changes were analyzed in each condition thereafter. For the in vivo studies, MMP-11 expression levels were measured in white adipose tissue(WAT) from C57BL/6J mice that underwent low fat diet or high-fat feeding in order to induce obesity and obesity-related insulin resistance. Statistical analysis was carried out with GraphP ad Prism *** MMP-11 m RNA expression levels were significantly higher in insulin resistant 3T3-L1 adipocytes compared to control cells(1.46±0.49vs0.83±0.21, respectively;Pdicate that dysregulation of MMP-11 expression is an early process in the adipose tissue dysfunction, which leads to obesity and obesity-related insulin resistance.

Rhabdomyolysis after midazolam administration in a cirrhotic patient treated with atorvastatin

The administration of statins in patients with liver disease is not an absolute contraindication. Hepatotoxicity is a rare and often dose-related event and in the literature there are only a few described cases of fatal rhab-domyolysis in patients with chronic liver disease after statin administration. During treatment with 3-hydroxy-3-methylglutaryl-coenzyme A reductase inhibitors,the factors responsible for myopathy may either be related to the patient,or due to interactions with other medications that are metabolic substrates of the same isozymes and therefore able to increase blood statin concentration. The most important side effects consist of increased transaminase levels,abdominal pain or muscle weakness,increased serum levels of creatine kinase and rhabdomyolysis. In this article we report a case of fatal rhabdomyolysis with acute renal failure after gastric endoscopy,where midazolam was used as a sedation agent in a patient with chronic liver disease treated with a high dose of atorvastatin. Therefore,we suggest paying particular attention to the potential risks of associating atorvastatin and midazolam in patients with chronic liver disease who need to undergo gastric endoscopy.

Gaseous and Particulate Exhaust Emissions of Hybrid and Conventional Cars over Legislative and Real Driving Cycles

Road transport exhaust emissions represent the main sources of atmospheric pollution in urban areas, due to the growing number of circulating vehicles and travelled distances. In order to reduce this pollution source, stricter emission standards are periodically set by governments through- out the world. Consequently, the concentrations of gaseous pollutants and particulate mass to be measured during type-approval tests of new vehicles are becoming progressively lower;moreover from 2011, diesel cars have to comply with particle number limit. In order to assess emission levels of different technology vehicles and investigate the use of a particulate number measurement technique at the exhaust of very low-emitting vehicles, an experimental activity was carried out on three in-use vehicles: a diesel car equipped with a particulate trap (DPF), a hybrid gasoline-elec- tric car and a bi-fuel passenger car fuelled with compressed natural gas (CNG). Cold and hot gaseous and particulate emission factors and fuel consumption were measured during the execution of real and regulatory driving cycles on a chassis dynamometer. Particulate was characterized in terms of mass only for the diesel car and of particle number for all vehicles. The emissions measured over the NEDC show that all three vehicles comply with their standard limits, except CO for CNG passenger car and NOx for diesel car. Cold start influences CO and HC emissions and fuel consumption for all the tested vehicles and in particular for the hybrid car. The real driving cycle is the most critical pattern for the emissions of almost all pollutants. During constant speed tests, the emissions of particles of hybrid car are an order of magnitude lower than those of the CNG car.

Current evidence on the management of hepatitis B in pregnancy

Hepatitis B virus(HBV) infection is one of the main public health problems across the globe,since almost one third of the world population presents serological markers of contact with the virus. A profound impact on the epidemiology has been exerted by universal vaccination programmes in many countries,nevertheless the infection is still widespread also in its active form. In the areas of high endemicity(prevalence of hepatitis B surface antigen positivity > 7%),mother-to-child transmission represents the main modality of infection spread. That makes the correct management of HBV in pregnancy a matter of utmost importance. Furthermore,the infection in pregnancy needs to be carefully assessed and handled not only with respect to the risk of vertical transmission but also with respect to gravid women health. Each therapeutic or preventive choice deserves to be weighed upon attentively. On many aspects evidence is scarce or controversial. This review will highlight the latest insights into the paramount steps in managing HBV in pregnancy,with particular attention to recommendations from recent guidelines and data from up-do-date research syntheses.

Migraine attack restores the response of vascular smooth muscle cells to nitric oxide but not to norepinephrine

AIM:To clarify whether the vasoconstrictory response is impaired and to study vascular function in patients with migraine during the headache ***:We studied vascular reactivity in the resistance arteries by using the forearm perfusion technique associated with *** measuredforearm blood flow by strain-gauge plethysmography during intra-brachial infusion of acetylcholine,sodium nitroprusside or norepinephrine in 11 controls and 13patients with migraine,11 of them(M) in the interval between the migraine attacks and 4 during a headache attack(MH).Written informed consent was obtained from patients and healthy controls,and the study was approved by the Ethics Committee of the University Federico Ⅱ.RESULTS:Compared to healthy control subjects,in patients with migraine studied during the interictal period,the vasodilating effect of acetylcholine,that acts through the stimulation of endothelial cells and the release of nitric oxide,was markedly reduced,but became normal during the headache attack(Pdirectly relaxes vascular smooth muscle cells(VSMCs),was depressed in patients with migraine studied during the interictal period,but normal during the headache attack(Pdied during or free of the headache attack forearm blood flow did not change compared to the baseline value(-3%±13% and-10.4%±15%,P>0.05).CONCLUSION:In migrainers,the impaired relaxation of VSMCs is restored during the headache *** vasoconstrictory response is impaired and remains unchanged during the migraine attack.

Aripiprazole augmentation in treating comorbid bipolar disorder and obsessive-compulsive disorder

Obsessive-compulsive disorder(OCD) is one of the most difficult additional diagnoses to manage in patients with bipolar disorder(BD) since the gold standard treatment for one disease(antidepressants for OCD) can worsen the other. This case report describes the efficacy of aripiprazole augmentation as maintenance therapy in a young patient with comorbid BD-OCD. Our patient presented complete remission of affective and obsessivecompulsive symptoms with remarkable improvement in social and occupational functioning for 24 *** drug reactions were not severe enough to result in drug discontinuation. In consideration of the important nosological, clinical and therapeutic implications, future research efforts may lead to more grounded guidelines,which are greatly needed in patients with comorbid BDOCD.

Management of chronic heart failure:Role of home echocardiography in monitoring care programs

AIM:To identify a possible role of home echocardiography for monitoring chronic heart failure(CHF)***:We prospectively investigated 118 patients hospitalized during the last year for CHF who could not easily reach the pertaining district Healthcare *** patients were followed up with 2 home management programs:one including clinical and electrocardiographic evaluations and also periodic home echocardiographic examinations(group A),the other including clinical and electrocardiographic evaluations only(group B).RESULTS:At the end of the 18-mo follow-up no significant differences were observed between the 2 groups as regards the primary endpoint:rehospitalization occurred in 4 patients of the group A and in 6 patients of the group B;major cardiovascular events occurred in 2 and in 3 patients,*** significant differences were observed with respect to the secondary endpoints:one vascular event appeared in both the groups,3 cardiovascular deaths occurred in group A and 2 in group *** significant differences were observed between the 2 groups as regards the composite endpoint of death plus ***:Home echocardiography for monitoring of CHF patients does not improve the cardiovascular *** our CHF patients,a low incidence of vascular events was observed.

报道了实验室规模利用未经改性的虾壳做为吸附材料去除鞣制废水中的三价铬。实验发现对铬的去除率超过90%,证明该材料有能力去除三价铬。其对铬的吸附过程可以用Brauner-Emmett-Telller吸附等温线很好地描述,表明虾壳材料同时存在强、弱两种吸附位点。动力学实验表明吸附过程在最初的2 h内迅速完成,之后形成一个平台,可以用拟二级方程很好地模拟。论文还分析比较了虾壳与其他经常用于脱铬的化学品的效果差异,结合扫描电子显微镜形貌分析结果,表明虾壳的处理效率与碳酸氢钠和氢氧化钠的处理效率相当,但是去除机理不同,铬在虾壳多糖基质上的吸附占主导,少部分是铬盐的沉淀。用回收的三价铬直接鞣制的皮革性能差,而将吸附产生的铬泥调节p H值后作为鞣剂,能够获得令人满意的鞣制效果。
