A national longitudinal cohort study of factors contributing to UK medical students’mental ill-health symptoms

Background The mental health of current medical students is predictive of their mental health as future *** prevalence of anxiety,depression and burnout is high among medical students,but less is known about the occurrence of other mental ill-health symptoms,such as eating or personality disorders,and factors contributing to mental ***(1)To explore the prevalence of various mental ill-health symptoms in medical students and(2)to investigate what medical school factors and students’attitudes contribute to these mental ill-health *** Between November 2020 and may 2021,medical students from nine geographically spread medical schools in the UK participated by completing online questionnaires at two points in time,approximately 3 months *** Of the 792 participants who filled in the questionnaire at baseline,over half experienced medium to high somatic symptoms(50.8%;402)and drank alcohol at hazardous levels(62.4%;494).Adjusted longitudinal data analysis of 407 students who completed the follow-up questionnaire demonstrated that less supportive educational climates that were more competitive and less centralised around the students,lower feelings of belongingness,greater stigma towards mental ill-health and lower intentions to seek help for mental ill-health,all contributed to students’mental ill-health *** medical students experience a high prevalence of various mental ill-health *** study suggests that medical school factors and students’attitudes towards mental ill-health are significantly associated with students’mental health.

In this work three fuel consumption and exhaust emission models,ADVISOR,VT-mICRO and the European Environmental Agency Emission factors,have been used to obtain fuel consumption(FC)and exhaust *** models have been used at micro-scale,using the two signal treatment methods *** manuscript presents:1)a methodology to collect data in real urban driving cycles,2)an estimation of FC and tailpipe emissions using some available models in literature,and 3)a novel analysis of the results based on delivered wheel *** results include Fuel Consumption(FC),CO_(2),NO_(x) and Pm_(10) emissions,which are derived from the three *** the first part of the paper we present a new procedure for incomplete drive cycle data treatment,which is necessary for real drive cycle acquisition in high density *** the models are used to obtain second by second FC and exhaust ***,a new methodology named Cycle Analysis by Ordered Power(CAbOP)is presented and used to compare the *** method consists in the re-ordering of time dependant data,considering the wheel mechanical power domain instead of the standard time *** new strategy allows the 5 situations in drive cycles to be clearly visualized:hard breaking zone,slowdowns,idle or stop zone,sustained speed zone and acceleration *** complete methodology is applied in two real drive cycles surveyed in Barcelona(Spain)and the results are compared with a standardized WLTC urban cycle.

Osteoporosis is a systemic bone disease that affects more than 200 million people worldwide and is caused by the disruption of the equilibrium between osteoclastic bone resorption and osteoblastic bone formation. Sphingosine-1-phosphate(S1 P) is a natural,bioactive sphingolipid that has been shown to play a major role in cardiovascular and immunological pathologies by regulating biological and cellular processes, including migration, differentiation, proliferation and survival. Recent studies also suggest a central role for S1 P in bone diseases, including osteoporosis;however, the effects of S1 P, particularly in bone metabolism, remain to be further elucidated. In this review, we summarize the available literature on the role of S1 P in bone metabolism with a focus on osteoporosis. On the cellular level, S1 P acts as an osteoclast-osteoblast coupling factor to promote osteoblast proliferation and bone formation. moreover, the recruitment of osteoclast precursors to resorption sites is regulated by the interplay of S1 P gradients and S1 P receptor expression. From a clinical perspective, increasing evidence suggests that systemically elevated S1 P blood levels may serve as an independent risk factor for osteoporosis-related fractures. Taken together, S1 P signaling is a potential therapeutic target and may serve as a novel biomarker in patients with systemic bone disease.

SUmmARY Vaccine-induced immune thrombotic thrombocytopenia(VITT)and cerebral venous sinus thrombosis(CVST)have been recently described as rare complications following vaccination against SARS-CoV-2 with vector *** report a case of a young woman who presented with VITT and cerebral CVST 7 days following vaccination with ChAdOx1 nCov-19(AstraZeneca).While the initial mRI was considered void of pathological findings,mRI 3 days later revealed extensive CVST of the transversal and sigmoidal sinus with intracerebral *** tests including a platelet-factor-4-induced platelet activation assay confirmed the diagnosis of *** with intravenous immunoglobulins and argatroban resulted in a normalisation of platelet counts and remission of CVST.

Nephropathy secondary to BK virus, a member of the Papoviridae family of viruses, has been recognized for some time as an important cause of allograft dysfunction in renal transplant recipients. In recent times, BK nephropathy(BKN) of the native kidneys has being increasingly recognized as a cause of chronic kidney disease in patients with solid organ transplants, bone marrow transplants and in patients with other clinical entities associated with immunosuppression. In such patients renal dysfunction is often attributed to other factors including nephrotoxicity of medications used to prevent rejection of the transplanted organs. Renal biopsy is required for the diagnosis of BKN. Quantitation of the BK viral load in blood and urine are surrogate diagnostic methods. The treatment of BKN is based on reduction of the immunosuppressive medications. Several compounds have shown antiviral activity, but have not consistently shown to have beneficial effects in BKN. In addition to BKN, BK viral infection can cause severe urinary bladder cystitis, ureteritis and urinary tract obstruction as well as manifestations in other organ systems including the central nervous system, the respiratory system, the gastrointestinal system and the hematopoietic system. BK viral infection has also been implicated in tumorigenesis. The spectrum of clinical manifestations from BK infection and infection from other members of the Papoviridae family is widening. Prevention and treatment of BK infection and infections from other Papovaviruses are subjects of intense research.

A 24-year-old female patient with parathyroid carcinoma,the rarest endocrine malignancy,had two *** the first pregnancy,she had severe nausea and *** and hyperparathyroidism were diagnosed in the postpartum *** gravidarum masked a diagnosis of *** ultrasound and Tc-99 m sestamibi found an enlarged lower right parathyroid *** gland was surgically removed,and an initial pathology report described atypical *** afterward,she became pregnant *** the second pregnancy,her calcium level was frequently controlled but was always in the normal *** is explained by the specific physiology of pregnancy accompanied by hemodilution,hypoalbuminemia and maternal hypercalciuria(mediated by increased glomerular filtration).During lactation,calcium levels rose,and a newneck ultrasound showed a solitary mass in the area of prior surgery and an enlarged pretracheal lymph *** needle aspiration of the solitary mass and node showed parathyroid carcinoma *** tumor mass was resected en bloc with the contiguous tissues and surrounding lymph nodes(pathology report; parathyroid carcinoma with metastases).Over the next five years,four consecutive surgeries were performed to remove malignant parathyroid tissue,lymph nodes and local *** the surgical procedures,no hypocalcemia was *** serious hypercalcemia recurred; the calcium level was difficult to control with a combination of pamidronate,cinacalcet and loop *** elements of multiple endocrine neoplasia were present.

We investigate nanorod assemblies of two 64-substituted pentacenes, namely （2,3-X2-9,10-Y2）-substituted pentacenes with X -- Y = OCH3 （mOP） and with X = F, Y-- OCH3 （mOPF）, grown on Au（111） single crystals. By using a multi-technique approach based on ultraviolet photoelectron spectroscopy X-ray photoelectron spectroscopy; and X-ray absorption, we find evidence for charge transfer screening at the interface with gold. Furthermore, the mOP and mOPF nanorods show a rough surface morphology, which was investigated with atomic force microscopy. We use molecular simulation techniques to investigate the energetic barriers to diffusion and to traverse step-edges to estimate their influence on the nanorod roughness. We find that barriers to surface diffusion on a terrace are anisotropic and that their direction favors the formation of nanorods in these materials.

Previous literature showed mixed results about the impact of CEOs’financial literacy(CFL)on small and medium-sized enterprises’(SmEs)*** relationship can be motivated by relevant variables,which are missing in the previous literature and make a difference as *** this sense,based on the theoretical framework related to upper echelon theory and resource-based view,this study focuses on the mediating effect of risk-taking attitude and management control systems(mCS)*** data from 310 SmEs gathered using a qualitative research questionnaire are analyzed using structural equation modeling ***,estimations are carried out considering the partial least square *** show that mCS and managers’risk attitudes fully mediate the relationship between financial literacy(FL)and *** these two mediating variables,the implementation of mCS stands out because it also enables the mediating effect of CEOs’risk-taking in the CFL–technological innovation *** the results do not support the significant direct relationship between FL and risk attitude,they confirm an indirect effect through ***,based on the study findings,SmEs’directors and owners,business associations,and public authorities can improve SmEs’technological innovation by implementing training programs and policies to foster *** can also acknowledge the interdependency between organizational factors and individual characteristics to enhance SmEs’technological innovation.
