Autom.ted Telephone Calls in the Follow-Up of Self-Care in Outpatients with Type 2 Diabetes: A Feasibility Study

Aim. and objectives: To test an autom.ted telephone service as a follow-up and support strategy for self-care in a population of adult m.xican patients with type 2 diabetes. m.thods: The design was a two-group com.arative design, with pre and post-intervention m.asurem.nts. Patients were assigned into intervention (n = 31), or com.arison (n = 33) groups. Over 12 weeks, each participant in the intervention group received autom.ted phone calls with instructions to facilitate accom.lishm.nt of the treatm.nt, and self-care activities. Two focus groups helped to appreciate patients’ follow-up experience. Random.zed grouping was perform.d by selecting num.ers from.an urn;the final sam.le com.rised 31 (48%) patients in the intervention group, and 33 (52%) in the com.arison group;these procedures were perform.d by the case nurse m.nagem.nt. The study was approved by the Ethics Com.ittee of the Autonom.us University of Nuevo Leon and the University of m.chigan;all of the participants signed inform.d consent. Results: Of the 372 program.ed phone calls m.de to the participants, 234 were com.leted, representing a 62.9% response rate. Phone calls m.y be associated to a 0.82% decrease in the HbA1c values and to lower depression scores. HbA1c m.asurem.nts decreased 0.82% (m.= 7.41 - 6.69;t = 25;4.11;p = ?0.001) from.the baseline values over the 12-week study in the intervention group and 0.49% (m.= 7.24 - 6.75;t = 26;2.11;p = 0.044) in the com.arison group. Variables related to m.dication intake did not exhibit differences between the baseline and second m.asurem.nts, except with respect to the scale of depression.

We investigate nanorod assem.lies of two 64-substituted pentacenes, nam.ly （2,3-X2-9,10-Y2）-substituted pentacenes with X -- Y = OCH3 （m.P） and with X = F, Y-- OCH3 （m.PF）, grown on Au（111） single crystals. By using a m.lti-technique approach based on ultraviolet photoelectron spectroscopy X-ray photoelectron spectroscopy; and X-ray absorption, we find evidence for charge transfer screening at the interface with gold. Furtherm.re, the m.P and m.PF nanorods show a rough surface m.rphology, which was investigated with atom.c force m.croscopy. We use m.lecular sim.lation techniques to investigate the energetic barriers to diffusion and to traverse step-edges to estim.te their influence on the nanorod roughness. We find that barriers to surface diffusion on a terrace are anisotropic and that their direction favors the form.tion of nanorods in these m.terials.

Background The m.ntal health of current m.dical students is predictive of their m.ntal health as future *** prevalence of anxiety,depression and burnout is high am.ng m.dical students,but less is known about the occurrence of other m.ntal ill-health sym.tom.,such as eating or personality disorders,and factors contributing to m.ntal ***(1)To explore the prevalence of various m.ntal ill-health sym.tom. in m.dical students and(2)to investigate what m.dical school factors and students’attitudes contribute to these m.ntal ill-health *** Between Novem.er 2020 and m.y 2021,m.dical students from.nine geographically spread m.dical schools in the UK participated by com.leting online questionnaires at two points in tim.,approxim.tely 3 m.nths *** Of the 792 participants who filled in the questionnaire at baseline,over half experienced m.dium.to high som.tic sym.tom.(50.8%;402)and drank alcohol at hazardous levels(62.4%;494).Adjusted longitudinal data analysis of 407 students who com.leted the follow-up questionnaire dem.nstrated that less supportive educational clim.tes that were m.re com.etitive and less centralised around the students,lower feelings of belongingness,greater stigm. towards m.ntal ill-health and lower intentions to seek help for m.ntal ill-health,all contributed to students’m.ntal ill-health *** m.dical students experience a high prevalence of various m.ntal ill-health *** study suggests that m.dical school factors and students’attitudes towards m.ntal ill-health are significantly associated with students’m.ntal health.

Osteoporosis is a system.c bone disease that affects m.re than 200 m.llion people worldwide and is caused by the disruption of the equilibrium.between osteoclastic bone resorption and osteoblastic bone form.tion. Sphingosine-1-phosphate(S1 P) is a natural,bioactive sphingolipid that has been shown to play a m.jor role in cardiovascular and im.unological pathologies by regulating biological and cellular processes, including m.gration, differentiation, proliferation and survival. Recent studies also suggest a central role for S1 P in bone diseases, including osteoporosis;however, the effects of S1 P, particularly in bone m.tabolism. rem.in to be further elucidated. In this review, we sum.arize the available literature on the role of S1 P in bone m.tabolism.with a focus on osteoporosis. On the cellular level, S1 P acts as an osteoclast-osteoblast coupling factor to prom.te osteoblast proliferation and bone form.tion. m.reover, the recruitm.nt of osteoclast precursors to resorption sites is regulated by the interplay of S1 P gradients and S1 P receptor expression. From.a clinical perspective, increasing evidence suggests that system.cally elevated S1 P blood levels m.y serve as an independent risk factor for osteoporosis-related fractures. Taken together, S1 P signaling is a potential therapeutic target and m.y serve as a novel biom.rker in patients with system.c bone disease.

A 24-year-old fem.le patient with parathyroid carcinom.,the rarest endocrine m.lignancy,had two *** the first pregnancy,she had severe nausea and *** and hyperparathyroidism.were diagnosed in the postpartum.*** gravidarum.m.sked a diagnosis of *** ultrasound and Tc-99 m.sestam.bi found an enlarged lower right parathyroid *** gland was surgically rem.ved,and an initial pathology report described atypical *** afterward,she becam. pregnant *** the second pregnancy,her calcium.level was frequently controlled but was always in the norm.l *** is explained by the specific physiology of pregnancy accom.anied by hem.dilution,hypoalbum.nem.a and m.ternal hypercalciuria(m.diated by increased glom.rular filtration).During lactation,calcium.levels rose,and a newneck ultrasound showed a solitary m.ss in the area of prior surgery and an enlarged pretracheal lym.h *** needle aspiration of the solitary m.ss and node showed parathyroid carcinom. *** tum.r m.ss was resected en bloc with the contiguous tissues and surrounding lym.h nodes(pathology report; parathyroid carcinom. with m.tastases).Over the next five years,four consecutive surgeries were perform.d to rem.ve m.lignant parathyroid tissue,lym.h nodes and local *** the surgical procedures,no hypocalcem.a was *** serious hypercalcem.a recurred; the calcium.level was difficult to control with a com.ination of pam.dronate,cinacalcet and loop *** elem.nts of m.ltiple endocrine neoplasia were present.

目的探讨肿瘤坏死因子α（TNF-α）在激光诱导的小鼠脉络膜新生血管（CNV）形成中的作用。方法野生型C57BL／6J小鼠36只（72只眼），每只眼视网膜上做4个光凝斑诱导CNV生成。小鼠分别于光凝前3d或后3d腹腔内植入渗透泵，给予TNF-α抑制剂依那昔普（5μg/h，n=12）或英利西单抗（5μg/h，n=12），持续7d，对照组给予PBS（n=12）。光凝前3d的用药组分别于光凝后1和2周，光凝后3d的用药组于光凝后10d做荧光素眼底血管造影（FFA），之后立刻取左眼行视网膜组织学检查，右眼制作脉络膜平片，免疫组化方法标记内皮细胞，测定荧光素渗漏面积及脉络膜平片CNV相关荧光面积，比较CNV病变的大小及程度。用免疫印迹法分析比较小鼠光凝前后视网膜色素上皮层及脉络膜上TNF-α蛋白的表达。结果光凝后1周，免疫印迹法结果显示小鼠脉络膜及视网膜色素上皮层TNF-α表达增加。光凝前3d用药，光凝后1周，荧光素渗漏面积为（0．74±0．33）×10^4像素，2周时则为（0．63±0．20）×10^4像素，与对照组[（2．61±0．80）×10^4像素]比较均显著减少（t=3．69，3．56；P〈0．05）。光凝后1周对照组、依那昔普组和英利西单抗组CNV面积分别为（3．61±0．93）×10^5、（1．89±0．62）×10^5和（2．14±0．72）×10^5像素，与对照组比较，差异均有统计学意义（t=3．10，2．51；P〈0．05。但光凝后3d用药，则只有依那昔普能显著抑制CNV病变。组织病理学检查进一步证实，治疗组小鼠的CNV病变直径及厚度小于对照组。结论TNF-α参与激光诱导的CNV的形成。TNF-α抑制剂能抑制激光诱导的CNV的形成及渗漏。
