Free deep inferior epigastric perforator flap after abdominal liposuction:reconsidering a contraindication

Autologous breast reconstruction with perforators has been previously avoided in tissues that have undergone *** present a case series and literature review of breast reconstruction with deep inferior epigastric perforator(DIEP)flaps after abdominal wall *** MEDLINE search was performed for all relevant articles describing breast reconstruction with DIEP flap technique following the abdominal wall *** search words used included“DIEP”,“DIEAP”,“deep inferior epigastric perforator”,“liposuction”and“free flap”.All published data on the topic from 1965 to December 2014 were *** were assessed for reports of clinical cases,complications,age,liposuction amount,time since liposuction and number of perforators for *** have also presented 2 patients who underwent a DIEP procedure with a previous history of *** cases of autologous breast reconstruction using a DIEP flap after liposuction were identified in the literature in addition to the presented *** preoperative and postoperative course was uneventful in all cases except one patient who had a mild cellulitis managed with antibiotics and a second patient with a drainable *** average age was 52 years±6.4 years old,one perforator was used in all cases except one where 2 were used,and the average amount of total liposuction was 1,084 *** major complications were *** liposuction is not an absolute contraindication for free-flap breast *** management should include evaluation of suitable perforators by duplex ultrasound or computed tomography *** case series are needed to better understand the safety of perforator flaps after liposuction.

THERMOSENSITIVE SUPERABSORBENTS BASED ON POLY(N,N-DIETHYLACRYL AMIDE-co-SODIUM ACRYLATE)

A series of poly（N,N-diethylacrylamide-co-sodium acrylate） with a degree of crosslinking of 1 mol% have been prepared as therrnosensitive superabsorbents for water. The critical swelling temperatures or the volume phase transition temperature （VPTT） and the water absorption capacity of the polymers can be modulated by varying the amount of sodium acrylate （0-60 tool%） in the copolymers. The water absorption and swelling properties of the different hydrogels have been studied as function of temperature. The crosslinked copolymers can absorb large amounts of water at ambient temperatures and dehydrate at higher temperatures with relative ease, making the absorbent materials thermally responsive and thus reusable. The water absorption capacity of the copolymers depends on the pH of the media as the acrylate monomer has a higher water absorption in its deprotonated state. Added urea in the media raises and sharpens the VPTT values of the copolymers containing sodium acrylate.

In-situ measurement of the surface temperature during holographic recording

Photo-induced processes in organic materials mostly occur on molecular levels. Excited molecules may split to form radicals, starting a polymerization process with diffusing monomers. The azo-dyes perform an optically induced cis-trans isomerization. During pattern formation like a holographic grating, the local temperature increase, especially in thin films, is up to date a subject of estimation from absorption and dissipation data. However, the exact knowledge of the surface temperature would help a lot in understanding the resulting refractive index and thickness patterns during holographic exposure. In this paper, in-situ pyrometer measurements are presented. As examples, different photosensitive materials, varying from a photopolymer to polycrystalline azo dyes, are used in order to outline the magnitude of this effect and demonstrate the feasibility of this technique.

Incident frailty and cognitive impairment by heart failure status in older patients with atrial fibrillation: the SAGE-AF study

Background Atrial fibrillation(AF) and heart failure(HF) frequently co-occur in older individuals. Among patients with AF, HF increases risks for stroke and death, but the associations between HF and incident cognition and physical impairment remain unknown. We aimed to examine the cross-sectional and prospective associations between HF, cognition, and frailty among older patients with AF. Methods The SAGE-AF(Systematic Assessment of Geriatric Elements in AF) study enrolled 1244 patients with AF(mean age 76 years, 48% women) from five practices in Massachusetts and Georgia. HF at baseline was identified from electronic health records using ICD-9/10 codes. At baseline and 1-year, frailty was assessed by Cardiovascular Health Survey score and cognition was assessed by the Montreal Cognitive Assessment. Results Patients with prevalent HF(n = 463, 37.2%) were older, less likely to be non-Hispanic white, had less education, and had greater cardiovascular comorbidity burden and higher CHA2 DS2 VASC and HAS-BLED scores than patients without HF(all P's < 0.01). In multivariable adjusted regression models, HF(present vs. absent) was associated with both prevalent frailty(adjusted odds ratio [a OR]: 2.38, 95% confidence interval [CI]: 1.64-3.46) and incident frailty at 1 year(a OR: 2.48, 95% CI: 1.37-4.51). HF was also independently associated with baseline cognitive impairment(a OR: 1.60, 95% CI: 1.22-2.11), but not with developing cognitive impairment at 1 year(a OR 1.04, 95%CI: 0.64-1.70). Conclusions Among ambulatory older patients with AF, the co-existence of HF identifies individuals with physical and cognitive impairments who are at higher short-term risk for becoming frail. Preventive strategies to this vulnerable subgroup merit consideration.

Hearing loss and cognitive decline among older adults with atrial fibrillation:the SAGE-AF study

An In Vivo Evaluation of the Mycobacterial Filtration Efficacy of Three Breathing Filters Used in Anesthesia

Background:The use of breathing filters (BFs) has been recommended to protect the anesthesia apparatus in proven or suspected cases of tuberculosis. Some investigators have also suggested the use of BF to alleviate the need to change anesthesia breathing circuits after each case. This study evaluated the filtration efficacy of three different BFs to prevent mycobacterial contamination of breathing circuits in a model that uses a test animal. Methods:Ten Pall BB25A (pleated hydrophobic) (Pall Canada Ltd., Mississauga, Ontario, Canada), six DAR Barrierbac S (felted electrostatic; Mallinckrodt DAR, Mirandola, Italy), and six Baxter Airlife (felted electrostatic; Baxter Canada, Mississauga, Ontario, Canada) BFs were studied. For each BF tested, 20 ml of a high concentration suspension of Mycobacterium chelonae (range, 2.0×10^7 to 9.0×10^7 colony-forming units/ml) was nebulized during 2h at the proximal end of the endotracheal tube of anesthetized pigs. At the end of the nebulization period, the BFs were sampled for culture. The titer reduction value (number of microorganisms challenging the BF divided by the number of microorganisms recovered downstream of the BF) and the removal efficiency (difference between the number of microorganisms challenging the BF and the number of microorganisms recovered downstream of the BF, divided by the number of microorganisms challenging the BF) were calculated. Results:The median titer reduction values were 5.6×10^5, 6.0×10^5, and 8.0×10^8 (P < 0.0005), and the median removal efficiencies were greater than 99.999%, greater than 99.999%, and 100% (P= not significant) for the DAR Barrierbac S, the Baxter Airlife, and the Pall BB25A, respectively. Conclusions:Among the three BFs studied, only the Pall BB25A completely prevented the passage of M. chelonae, thus protecting the anesthesia breathing circuit from mycobacterial contamination.

＜正＞Background and Aims: The treatment of prostate cancer （PCa） depends on stage. To improve treatment strategies, it would be helpful to have reliable molecular markers able to predict PCa progression. However, to date, such markers have not yet been included in therapeutic decisions. Recently, several studies from our laboratory described the prognostic potential of molecular markers that include ErbB3, Akt and Nuclear Factor kappaB （NF- κB） . ErbB3 is a member of the growth factor receptor family ErbB. Akt is a signaling component of the ErbB receptor family. Phos-phoinositol 3 - kinase （PI3K） through Akt regulates downstream signaling proteins such as NF - κB. Most of these molecules are often overexpressed and/or activated in cancer cells. Furthermore, they can be detected in the cytoplasm and/or nucleus of PCa cells. They are known to be involved in cell survival, cell growth, cell cycle progression and apoptotic response. The aim of our study was to determine whether these molecules could be used as markers to predict PCa progression.
