Monocyte chemoattractant protein-1, transforming growth factor-β1, nerve growth factor, resistin and hyaluronic acid as serum markers：comparison between recurrent acute and chronic pancreatitis

BACKGROUND： Diagnostic parameters that can predict the presence of chronic pancreatitis（CP） in patients with recurrent pain due to pancreatitis would help to direct appropriate therapy. This study aimed to compare the serum levels of monocyte chemoattractant protein-1（MCP-1）, transforming growth factor-β1（TGF-β1）, nerve growth factor（NGF）, resistin and hyaluronic acid（HA） in patients with recurrent acute pancreatitis（RAP） and CP to assess their ability to differentiate the two ***： Levels of serum markers assessed by enzymelinked immunosorbent assay（ELISA） were prospectively compared in consecutive patients with RAP, CP and in controls and stepwise discriminant analysis was performed to identify the markers differentiating RAP from ***： One hundred and thirteen consecutive patients（RAP=32, CP=81） and 78 healthy controls were prospectively enrolled. The mean（SD） age of the patients was 32.0（14.0）years; 89（78.8%） were male. All markers were significantly higher in CP patients than in the controls（P〈0.001）; MCP-1NGF and HA were significantly higher in RAP patients than in the controls（P〈0.001）. Stepwise discriminant analysis showed significant difference（P=0.002） between RAP and CP for resistin with an accuracy of 61.9%, discriminant scores of ≤-0.479 and ≥0.189 indicating RAP and CP, respectively. The other markers had no differential value between RAP and ***： Serum resistin is a promising marker to differentiate between RAP and CP and needs validation in future studies, especially in those with early CP.

Comparing acid steatocrit and faecal elastase estimations for use in M-ANNHEIM staging for pancreatitis

AIM To compare two tests for exocrine pancreatic function(EPF) for use in M-ANNHEIM staging for pancreatitis. METHODS One hundred and ninety four consecutive patients with acute pancreatitis(AP; n = 13), recurrent acute pancreatitis(RAP; n = 65) and chronic pancreatitis(CP; n = 116) were enrolled. EPF was assessed by faecal elastase-1(FE-1) estimation and stool fat excretion by the acid steatocrit method. Patients were classified as per M-ANNHEIM stages separately based on the results of the two tests for comparison. Independent Student's t-test, χ~2 test, Kruskal-Wallis test, Mann-Whitney U test and Mc Nemar's test were used as appropriate. RESULTS Sixty-one(52.5%) patients with CP had steatorrhoea when assessed by the acid steatocrit method; 79 (68.1%) with CP had exocrine insufficiency by the FE-1 test(χ~2 test, P < 0.001). The results of acid steatocrit and FE-1 showed a significant negative correlation(Spearman's rho =-0.376, P < 0.001). A statistically significant difference was seen between the M-ANNHEIM stages as classified separately by acid steatocrit and the FE-1. Thirteen(6.7%), 87(44.8%), 89(45.8%) and 5(2.5%) patients were placed in M-ANNHEIM stages 0,?Ⅰ, Ⅱ, and Ⅲ respectively, with the use of acid steatocrit as against 13(6.7%), 85(43.8%), 75(38.6%), and 21(10.8%) respectively by FE-1 in stages 0,?Ⅰ, Ⅱ, and Ⅲ thereby altering the stage in 28(14.4%) patients(P < 0.001, Mc Nemar's test). CONCLUSION FE-1 estimation performed better than the acid steatocrit test for use in the staging of pancreatitis by the M-ANNHEIM classification since it diagnosed a higher proportion of patients with exocrine insufficiency.

Continuing episodes of pain in recurrent acute pancreatitis: Prospective follow up on a standardised protocol with drugs and pancreatic endotherapy

The poly（methyl methacrylate）（PMMA） coatings onto surface of iron particles were electrochemically prepared and the effect on both surface structure and internal structure of the resulted material after compaction was carried *** electrochemical polymerization treatment was performed in a fluidized bed electrolyzer using sulphuric acid solution containing potassium persulphate and methyl methacrylate（MMA）.The surface topography and the microstructure of the samples were observed by scanning electron microscopy（SEM） and optical microscopy（OM）.It was found that the PMMA layer coated onto iron particles results in improvement of their compressibility compared with uncoated powders,and classical lubricants are not necessary for compacting particles coated.

Malaria treatment, compliance and cure in Gandhinagar, Gujarat

Early diagnosis and complete treatment are the most important components of the malaria control program, which have a direct bearing on prevention of mortality and reduction in the morbidity, therefore, follow-up of the patient treatment is an important aspect of monitoring disease in the community. Aims: To assess treatment compliance among confirmed malaria cases in Gandhinagar District and to determine the proportion of cure rate with complete treatment as per the national drug policy. Study Variables: Exploratory-rural and urban areas;outcome-compliance, cure rate. Analysis: Percentage, proportions. Results: Compliance among 192 subjects under the study conducted in 2008 i.e. confirmed that malaria cases were 88% even after the introduction of 14 day therapy for “Plasmodium vivax” cases, while 100% cure rate was observed on the follow-up till the 28th day among “P. vivax” and “P. falciparum” cases which helped in reducing malaria incidence in the rural area by 50% as compared to the preceding year of the study. Conclusion: Follow-up of the treatment given to malaria patients has enhanced compliance and cure rate, which in turn contributed towards interruption of transmission. Therefore, a better cure rate through the administration of effective antimalarial drugs as per the drug policy in combination with other containment measures is the right strategy to keep morbidity and mortality due to malaria under effective control.

Efficient separation between trivalent americium and lanthanides enabled by a phenanthroline-based polymeric organic framework

Neuropathic pain-like symptoms and pre-surgery radiographic severity contribute to patient satisfaction 4.8 years post-total joint replacement

AIM To investigate a comprehensive range of factors that contribute to long-term patient satisfaction post-total joint replacement(TJR) in people who had undergone knee or hip replacement for *** Participants(n = 1151) were recruited from Nottinghamshire post-total hip or knee replacement. Questionnaire assessment included medication use, the pain-DETECT questionnaire(PDQ) to assess neuropathic pain-like symptoms(NP) and TJR satisfaction measured on average 4.8 years post-TJR. Individual factors were tested for an association with post-TJR satisfaction, before incorporating all factors into a full model. Data reduction was carried out using LASSO and receiveroperator characteristic(ROC) curve analysis was used to quantify the contribution of variables to post-TJR *** After data reduction, the best fitting model for post-TJR satisfaction included various measures of pain, history of revision surgery, smoking, pre-surgical X-ray severity, WOMAC function scores and various comorbidities. ROC analysis of this model gave AUC = 0.83(95%CI: 0.80-0.85). PDQ scores were found to capture much of the variation in post-TJR satisfaction outcomes: AUC = 0.79(0.75-0.82). Pre-surgical radiographic severity was associated with higher post-TJR satisfaction: OR_(satisfied) = 2.06(95%CI: 1.15-3.69), P = *** These results highlight the importance of pre-surgical radiographic severity, post-TJR function, analgesic medication use and NP in terms of post-TJR satisfaction. The PDQ appears to be a useful tool in capturing factors that contribute to post-TJR satisfaction.

Progress Achieved in Gandhinagar District of Gujarat State for Elimination of Malaria, 2007 to 2015—A Case Study

Background: Government of India has lunched the frame work for eliminating malaria from the country by 2030 [1] [2]. But progressive States like Gujarat has to achieve the target by 2024. The first step in this direction is to bring down the Annual Parasitic Incidence less than 1.0. Under National Vector Borne Disease Control Programme (NVBDCP) various activities for Vector control coupled with complete treatment to confirm malaria cases within 24 hours were given more focus and implemented diligently and effectively. Aim: To evaluate the progress of the efforts being undertaken for malaria elimination in Gandhinagar district. Design: Impact of malaria control efforts in Gandhinagar district covering all areas were studied in detail. Setting: Malaria prone villages of Gandhinagar district where preventive actions were undertaken in an integrated manner during the period of 2005 to 2015. Exploratory: Data pertaining to rural and urban areas of the district covering all PHCs, UHCs and Towns were analyzed and interpreted. Outcome: Reduction in malaria incidence and scaling up of non chemical methods for control of malaria. Analysis: Percentage and proportions. Results: Area specific approach adopted in rural areas of Gandhinagar district by putting more emphasis on good surveillance, ensuring complete treatment to malaria cases within 24 hours and effective strategy for vector control mainly non chemical methods resulted in reducing Annual Parasitic Incidence (API) from 1.96 in 2005 to 0.17 in 2015 in Gandhinagar district. 226 villages (74.83%) out of 302 in the district are malaria free and only 9 villages are having API > 1.0. The district aims to achieve malaria elimination in the next five years. Conclusion: Gandhinagar district has made rapid stride towards malaria elimination in a cost effective manner by utilizing the available resources.
