The Influence of Alluvial Mining on the Morphology and Ecological Health of the Jhimruk River in Pyuthan, Nepal

Historically, alluvial rivers have been a desirable source of sediment for various industrial purposes. However, the demand for sand is rapidly increasing, leading to environmental concerns associated with its extraction. The extraction of sediment from riverbeds has both visible and invisible effects. This research aims to investigate the dynamics of the Jhimruk River’s morphology and its impacts by evaluating the river’s quality and studying its changing morphology. To assess morphological changes, satellite images from different years were compared using Geographic Information System (GIS). Physical assessments were conducted by calculating a Habitat score based on various parameters. Water quality assessments involved measuring factors such as pH, temperature, nitrate levels, and alkalinity. A water quality map of the river was generated using color coding to indicate different conditions. The Habitat score demonstrated variations in the data collected from all sites, with factors such as instream cover, bottom substrate stability, riparian vegetation, and aesthetic of the river stretch playing crucial roles in influencing the total score. The Water Quality Index value for the disturbed site indicated moderate pollution, falling under class II. Aquatic macroinvertebrates displayed natural responses to increasing levels of stressors across their life stages, as observed through the analysis of the GRS-BIOS/ASPT index. With the exception of the disturbed site, all other sites were classified under water quality class II, while the disturbed site belonged to class III, indicating a moderate to critically polluted state for most of the Jhimruk River. Furthermore, an increase of 180.87 meters in the river width at the disturbed site provided evidence of morphological changes occurring over the specified period. Mining activities were identified as a significant contributor to the alteration in river morphology.

Colorectal cancer(CRC) is the third most common cancer diagnosed worldwide in human beings. Surgery, chemotherapy, radiotherapy and targeted therapiesare the conventional four approaches which are currently used for the treatment of CRC. The site specific delivery of chemotherapeutics to their site of action would increase effectiveness with reducing side effects. Targeted oral drug delivery systems based on polysaccharides are being investigated to target and deliver chemotherapeutic and chemopreventive agents directly to colon and rectum. Site-specific drug delivery to colon increases its concentration at the target site, and thus requires a lower dose and hence abridged side effects. Some novel therapies are also briefly discussed in article such as receptor(epidermal growth factor receptor, folate receptor, wheat germ agglutinin, VEGF receptor, hyaluronic acid receptor) based targeting therapy; colon targeted proapoptotic anticancer drug delivery system, gene therapy. Even though good treatment options are available for CRC, the ultimate therapeutic approach is to avert the incidence of CRC. It was also found that CRCs could be prevented by diet and nutrition such as calcium, vitamin D, curcumin, quercetin and fish oil supplements. Immunotherapy and vaccination are used nowadays which are showing better results against CRC.

AIM:To study the in vivo anti-inflammatory activity of Tabernaemontana divaricata leaf extract on male albino ***:Aqueous decoction and methanol leaf extracts were tested for their ability to reduce croton oil-induced edema in the mouse ear after topical *** methanol leaf extract dose-dependently inhibited the croton oil-induced ear edema in mice(ID50<500μg·cm 2).A bioassay-guided liquid–liquid fractionation of this methanol extract gave four active fractions:water insoluble(F1),hexane(F2),ethyl acetate(F3)and water(F4).RESULTS:The hexane fraction showed a very high activity(42.1%inhibition at0.7μg·cm 2)as compared to the *** other fractions were less active(F1:56.1%at 506.2μg·cm-2;F3:57.3%at 289.3μg·cm 2;and F4:31.9%for 203.8μg·cm 2)while indomethacin gave 48.8%of inhibition at 90μg·cm *** activity of F1 and F3 may be at least in part explained by the presence of anti-inflammatory flavonoids,while the activity was not correlated to the tannin *** compounds were detected in the most active F2 ***:The results give a rational support to the traditional use of *** in tropical India as anti-inflammatory agent.

Most of the MHP （micro hydro power） plants use ELC （electronic load controller） for speed control. Various types of ELC have been developed so far. A dummy ballast load is connected across each phase of generator terminals and ELC controls the power consumed by the ballast load to result in constant speed operation. The ELC developed so far uses thyristor switches in each phase to control ballast load power. The ELC senses the system frequency and comparing it with reference frequency, it generates a common value of firing angle for all three thyristor pairs of each phase. The performance of such ELC is not perfect for unbalanced consumers load connected in each phase, which overloads the generator. This paper presents an advanced type of ELC which senses frequency as well as consumer＇s load current of each phase and fires the thyristor pairs with different value of firing angles for different phases. This solves the problem of overloading of the generator with unbalanced consumer＇s load. Simutink model is developed to perform transient analysis of the proposed scheme and the prototype of hardware is also fabricated. The simulation results and experimental results are presented.

Purpose： Total hip replacement （THR） is one of the most successful and cost-effective surgical procedures and remains the treatment of choice for long-term pain relief and restoration of function for patients with diseased or damaged hips. Acetabular fractures managed either conservatively or operatively by fixation tend to present later with secondary joint changes that require THR. In this study we evaluated the functional outcome and quality of life achieved by such patients. Methods： Our study was carried out as a retrospective trial by recruiting patients who underwent THR from June 2006 to May 2012. A total of 32 patients were included with a mean age of 46.08 years ranging from （25-65） years. We evaluated the quality of life in the patients using scoring techniques of Short Musculoskeletal Functional Assessment （SMFA） and the 12-1tem Short Form Health Survey （SF-12）. Functional outcome was assessed using Harris Hip Score （HHS）. Results： The mean HHS of the patients was 84.3 with a range from 56 to 100. The SMFA averaged 13.3. The SF-12 score averaged 49.1. The correlation of the HHS with SF-12 was positive （p - 0.001 ） while with SMFA there was a negative correlation （p - 0.001 ）. Conclusion： From this study it is inferred that the functional outcome of THR and quality of life in pa- tients who had acetabular fractures and were initially managed by open reduction and internal fixation is good.

AIM To assess outcomes of kidney transplantation including patient and allograft outcomes in recipients with hepatitis B virus(HBV) infection, and the trends of patient's outcomes *** A literature search was conducted using MEDLINE, EMBASE and Cochrane Database from inception through October 2017. Studies that reported odds ratios(OR) of mortality or renal allograft failure after kidney transplantation in patients with HBV [defined as hepatitis B surface antigen(HBs Ag) positive] were included. The comparison group consisted of HBs Agnegative kidney transplant recipients. Effect estimates from the individual study were extracted and combined using random-effect, generic inverse variance method of Der Simonian and Laird. The protocol for this metaanalysis is registered with PROSPERO(International Prospective Register of Systematic Reviews; no. CRD42017080657).RESULTS Ten observational studies with a total of 87623 kidney transplant patients were enrolled. Compared to HBs Ag-negative recipients, HBs Ag-positive status was significantly associated with increased risk of mortality after kidney transplantation(pooled OR = 2.48; 95%CI: 1.61-3.83). Meta-regression showed significant negative correlations between mortality risk after kidney transplantation in HBs Ag-positive recipients and year of study(slopes =-0.062, P = 0.001). HBs Agpositive status was also associated with increased risk of renal allograft failure with pooled OR of 1.46(95%CI: 1.08-1.96). There was also a significant negative correlation between year of study and risk of allograft failure(slopes =-0.018, P = 0.002). These associations existed in overall analysis as well as in limited cohort of hepatitis C virus-negative patients.
