Prospective evaluation of health status, quality of life and clinical outcomes following implantable defibrillator generator exchange

BACKGrOUND Little is known about health status and quality of life(QoL)after implantable cardioverter-defibrillator(ICD)generator exchange(GE).METHODS We prospectively followed patients undergoing first-time ICD *** assessments of health status were performed by administering the 36-Item Short Form Survey(SF-36).rESULTS Mean age was 67.5±14.3 years,left ventricle ejection fraction(LVEF)was 36.5%±15.0%and over 40%of the cohort had improved LVEF to>35%at the time of ***-36 scores were significantly worse in physical/general health domains compared to domains of emotional/social well-being(Pr each comparison).Physical health scores were significantly worse among those with medical comorbidities including diabetes,chronic obstructive pulmonary disease and atrial *** follow-up was 1.6±0.5 years after *** SF-36 scores remained stable across all domains during *** at 3 years post-GE was estimated at 80%.Five patients died during follow-up and most deaths were adjudicated as non-arrhythmic in *** patients experienced appropriate ICD shocks after GE,three of whom had LVEF which remains impaired LVEF(i.e.,rgoing ICD GE have significantly worse physical health compared to emotional/social well-being,which is associated with the presence of medical *** terms of clinical outcomes,the incidence of appropriate shocks after GE among those with improvement in LVEF is very low,and most deaths post-procedure appear to be non-arrhythmic in *** data represent an attempt to more fully characterize the spectrum of QoL and clinical outcomes after GE.

Clinical and anatomic predictors of need for repeat atrial fibrillation ablation

AIM To identify predictors of need for repeat procedures after initial atrial fibrillation(AF) ablation. METHODS We identified a cohort undergoing first time AF ablation at our institution from January 2004 to February 2014 who had cardiac magnetic resonance(CMr) imaging performed prior to ablation. Clinical variables and anatomic characteristics(determined from CMr) were assessed as predictors of need for repeat ablation. The decision regarding need for and timing of repeat ablation was at the discretion of the treating physician. rESULTS From a cohort of 331 patients, 142 patients(43%) underwent repeat ablation at a mean of 13.6 ± 18.4 mo afterthe index procedure. Both male gender(81% vs 71%, P = 0.05) and lower ejection fraction(57.4% ± 10.3% vs 59.8% ± 9.4%, P = 0.04) were associated with need for repeat ablation. On pre-ablation CMr, mean pulmonary vein(PV) diameters were significantly larger in all four PVs among patients requiring repeat procedures. In multivariate analysis, increased right superior PV diameter significantly predicted need for repeat ablation(odds ratio 1.08 per millimeter increase in diameter, 95%CI: 1.00-1.16, P = 0.05). There were also trends toward significance for increased left and right inferior PV sizes among those requiring repeat *** Increased PV size predicts the need for repeat AF ablation, with each millimeter increase in PV diameter associated with an approximately 5%-10% increased risk of requiring repeat procedures.

Effect of slake durability on time dependent swelling behavior of red-bed siltstone in Sichuan Basin

The swelling behavior of red-bed rocks is a significant factor in the abnormal uplift of subgrades for high-speed railways constructed on the red stratum in the Sichuan Basin,*** prime objective of this paper is to investigate the impact of mineralogical composition,moisture content,and overburden load on the time-dependent unconfined and oedometric swelling behavior of red-bed siltstone in the context of differences in the slake *** samples were prepared for the swelling test,with eleven used for the unconfined swelling and slake index tests and nine for the oedometric swelling *** temporal dependency of swelling is characterized by the viscosity coefficient of water absorption in a proposed swelling *** indicate that the swelling deformation of red-bed rocks is due to a combination of hydration swelling within the rock matrix and crack expansion caused by air *** the unconfined swelling test,the final axial swelling strain of red-bed rocks decreases linearly with increasing slake index,while the viscosity coefficient increases exponentially with the slake *** the oedometric swelling test,red-bed rocks with lower slake durability show greater sensitivity to lateral constraint and overburden load compared to those with higher slake durability.

Modern crop varieties display a degree of mismatch between their current distributions and the suitability of the local climate for their *** address this issue,we present Oryza CLIMtools(https://***/CLIMtools/oryza_v1.0/),thefirst resource for pan-genome prediction of climate-associated genetic variants in a crop *** CLIMtools consists of interactive web-based databases that enable the user to(1)explore the local environments of traditional rice varieties(landraces)in South-East Asia and(2)investigate the environment by genome associations for 658 Indica and 283 Japonica rice landrace accessions collected from georeferenced local environments and included in the 3K rice Ge-nomes *** demonstrate the value of these resources by identifying an interplay betweenflowering time and temperature in the local environment that is facilitated by adaptive natural variation in OsHD2 and disrupted by a natural variant in *** quantitative trait locus analysis has suggested the impor-tance of heterotrimeric G proteins in the control of agronomic ***,we analyzed the climate associations of natural variants in the different heterotrimeric G protein *** identified a coordi-nated role of G proteins in adaptation to the prevailing potential evapotranspiration gradient and revealed their regulation of key agronomic traits,including plant height and seed and panicle *** conclude by highlighting the prospect of targeting heterotrimeric G proteins to produce climate-resilient crops.

Wireless technology is transforming the future of transportation through the development of the Internet of Vehicles(IoV).However,intricate security challenges are intertwinedwith technological progress:Vehicular ad hoc Networks(VANETs),a core component of IoV,face security issues,particularly the Black Hole Attack(BHA).This malicious attack disrupts the seamless flow of data and threatens the network’s overall reliability;also,BHA strategically disrupts communication pathways by dropping data packets from legitimate nodes *** the importance of this challenge,we have introduced a new solution called ad hoc On-Demand Distance Vector-reputation-based mechanism Local Outlier Factor(AODV-rL).The significance of AODVrL lies in its unique approach:it verifies and confirms the trustworthiness of network components,providing robust protection against *** additional safety layer is established by implementing the Local Outlier Factor(LOF),which detects and addresses abnormal network *** testing of our solution has revealed its remarkable ability to enhance communication in ***,Our experimental results achieve message delivery ratios of up to 94.25%andminimal packet loss ratios of just 0.297%.Based on our experimental results,the proposedmechanismsignificantly improves VANET communication reliability and *** results promise a more secure and dependable future for IoV,capable of transforming transportation safety and efficiency.

When designing high-voltage elements,knowing if the corona effect will be present during their life cycle is ***,designers consider several prediction criteria based on physical features related to breakdown discharge principles to predict the corona *** introduced practical set-up consists of a concave cone electrode with a hemispheric tip above a plate to evaluate selected corona prediction *** hemisphere has a fixed diameter of 7 mm,and the electrode separation ranges from 2.5 to 39 *** about the corona mode inception under different voltage sources was extracted using an intensified charge-coupled device camera and a partial discharge *** prediction criteria were connected to a specific corona mode depending on the main discharge structure behind its *** average deviation between these criteria and experimental results was around eight *** assumptions in criteria are also discussed in light of the experimental results.

Facial emotion recognition(FEr)has become a focal point of research due to its widespread applications,ranging from human-computer interaction to affective *** traditional FEr techniques have relied on handcrafted features and classification models trained on image or video datasets,recent strides in artificial intelligence and deep learning(DL)have ushered in more sophisticated *** research aims to develop a FEr system using a Faster region Convolutional Neural Network(FrCNN)and design a specialized FrCNN architecture tailored for facial emotion recognition,leveraging its ability to capture spatial hierarchies within localized regions of facial *** proposed work enhances the accuracy and efficiency of facial emotion *** proposed work comprises twomajor key components:Inception V3-based feature extraction and FrCNN-based emotion *** experimentation on Kaggle datasets validates the effectiveness of the proposed strategy,showcasing the FrCNN approach’s resilience and accuracy in identifying and categorizing facial *** model’s overall performance metrics are compelling,with an accuracy of 98.4%,precision of 97.2%,and recall of 96.31%.This work introduces a perceptive deep learning-based FEr method,contributing to the evolving landscape of emotion recognition *** high accuracy and resilience demonstrated by the FrCNN approach underscore its potential for real-world *** research advances the field of FEr and presents a compelling case for the practicality and efficacy of deep learning models in automating the understanding of facial emotions.
