Background.Incomplete right bund.e branch block （ICRBBB） is commonly associated.with atrial septal d.fect （ASd., but lacks sufficient d.agnostic test characteristics. An abnormal T wave is also often observed.in ASd. with horizontal or inverted.d.splacement of the proximal T wave limb in the right precord.al lead., termed.＂d.fective T wave＂（d.W）. Method. We examined.the d.agnostic test characteristics of combining ICRBBB with d.W as a new ind.x to d.agnose ASd. A total of 132 consecutive patients with ASd.and.132 cases of age/gend.r-matched.controls without ASd.were enrolled. Results Sensitivities of d.W, ICRBBB, and.both were 87.1%-87.9%. Specificities were 97.0%, 96.2%, and.100%, respectively. Positive pred.ctive values were 1.3%, 1.1%, and.100.0% respectively, while negative pred.ctive values were 99.9% for each. Conclusion Combining ICRBBB with d.W in electrocard.ogram （ECG） as a new ind.x significantly increased.the specificity and.positive pred.ctive values while maintaining a high sensitivity in d.agnosing ASd.

Histones package d.A in all eukaryotes and.play key roles in regulating gene expression. Approximately 150 base pairs of d.A wraps around.an octamer of core histones to form the nucleosome, the basic unit of chromatin. Linker histones compact chromatin further by bind.ng to and.neutralizing the charge of the d.A between nucleosomes. It is well established.that chromatin packing is regulated.by a complex pattern of posttranslational mod.fications （PTMs） to core histones, but linker histone function is less well und.rstood. In this review, we d.scribe the current und.rstand. ing of the many roles that linker histones play in cellular processes, includ.ng gene regulation, cell d.vision, and.d.vel- opment, while putting the linker histone in the context of other nuclear proteins. Although intriguing roles for plant linker histones are beginning to emerge, much of our current und.rstand.ng comes from work in animal systems. Many unanswered.questions remain and.ad.itional work is required.to fully elucid.te the complex processes med.ated.by linker histones in plants.

AIM To d.termine the read.ission rate, its reasons, pred.ctors, and.cost of 30-d.read.ission in patients with cirrhosis and.*** A retrospective analysis of the nationwid. read.ission d.tabase(NRd. was performed.d.ring the calend.r year 2013. All ad.lts cirrhotics with a d.agnosis of ascites,spontaneous bacterial peritonitis, or hepatic encephalopathy were id.ntified.by ICd.9 cod.s. Multivariate analysis was performed.to assess pred.ctors of 30-d.read.ission and.cost of *** Of the 59597 patients includ.d.in this stud., 18319(31%) were read.itted.within 30 d. Majority(58%) of read.issions were for liver related.reasons. Paracentesis was performed.in 29832(50%) patients on ind.x ad.ission. Ind.pend.nt pred.ctors of 30-d.read.ission includ.d.age d.caid.OR: 1.21; CI: 1.04-1.41) and.Med.care coverage(OR: 1.13; CI: 1.02-1.26), > 3 Elixhauser comorbid.ty(OR: 1.13; CI: 1.05-1.22), nonalcoholic cirrhosis(OR: 1.16; CI: 1.10-1.23), paracentesis on ind.x ad.ission(OR: 1.28; CI: 1.21-1.36) and.having hepatocellular carcinoma(OR: 1.21; CI: 1.05; 1.39). Cost of ind.x ad.ission was similar in patients read.itted.and.not read.itted.P-value: 0.34); however cost of care was significantly more on 30 d.read.ission($30959 ± 762) as compared.to ind.x ad.ission($12403 ± 378), P-value: d.ission within 30-d. Younger patients, with public insurance, nonalcoholic cirrhosis and.increased.comorbid.ty who und.rwent paracentesis are at increased.risk of read.ission. Risk factors for unplanned.read.ission should.be targeted.given these patients have higher healthcare utilization.

AIM To d.termine the impact of transjugular intrahepatic porto-systemic shunt(TIPS) on post liver transplantation(LT) *** Utilizing the United.Network for Organ Sharing(UNOS) d.tabase, we compared.patients who und.rwent LT from 2002 to 2013 who had.und.rwent TIPS to those without TIPS for the management of ascites while on the LT waitlist. The impact of TIPS on 30-d.mortality, length of stay(LOS), and.need.for re-LT were stud.ed. For evaluation of mean d.fferences between baseline characteristics for patients with and.without TIPS, we used.unpaired.t-tests for continuous measures and.χ~2 tests for categorical measures. We estimated.the impact of TIPS on each of the outcome measures. Multivariate analyses were cond.cted.on the stud. population to explore the effect of TIPS on 30-d.mortality post-LT, need.for re-LT and.LOS. All covariates were includ.d.in logistic regression *** We includ.d.ad.lt patients(age ≥ 18 years) who und.rwent LT from May 2002 to September 2013. Only those und.rgoing TIPS after listing and.before liver transplant were includ.d.in the TIPS group. We exclud.d.patients with variceal bleed.ng within two weeks of listing for LT and.those listed.for acute liver failure or hepatocellular carcinoma. Of 114770 LT in the UNOS d.tabase, 32783(28.5%) met inclusion criteria. Of these 1366(4.2%) had.TIPS between the time of listing and.LT. We found.that TIPS increased.the d.ys on waitlist(408 ± 553 d. as compared.to those without TIPS(183 ± 330 d., P d.that TIPS had.no effect on 30-d.post LT mortality(OR = 1.26; 95%CI: 0.91-1.76) and.re-LT(OR = 0.61; 95%CI: 0.36-1.05). Pre-transplant hepatic encephalopathy ad.ed.3.46 d.95%CI: 2.37-4.55, P d.by 2.16 d.95%CI: 0.92-3.38, P = 0.001) by TIPS to LOS. CONCLUSION TIPS d.d.increase time on waitlist for LT. More importantly, TIPS was not associated.with 30-d.mortality and.re-LT, but it d.d.lengthen hospital LOS after transplantation.

Abstract: To d.termine the d.agnostic test characteristics of the S3 and.S4 for pred.ction of left ventricular d.sfunction using a computerized.heart sound.d.tection algorithm. d.sign, Setting, and.Participants: Prospective stud. of 90 ad.lt patients und.rgoing elective left-sid.d.heart catheterization at a single US teaching hospital between August 2003 and.June 2004. The mean age was 62(Sd. 13) years(range, 24- 90 years) and.61(68% ) were male. Within a 4-hour period. participants und.rwent computerized.heart sound.phonocard.ographic analysis, card.ac catheterization, transthoracic echocard.ography, and.blood.sampling for assessment of an S3/S4, left ventricular end.d.astolic pressure(LVEd.), left ventricular ejection fraction(LVEF), and.B-type natriuretic peptid.(BNP), respectively. Main Outcome Measures: d.agnostic test characteristics of the computerized.phonocard.ographic S3 and.S4 using markers of left ventricular function as criterion stand.rd.. Results: Mean(Sd. LVEd. was significantly elevated.18.4[6.9] mm Hg vs 12.1[7.3] mm Hg; Pd. LVEF was red.ced.49.4% [20.2% ] vs 63.6% [14.8% ]; Pd.med.an(interquartile range) BNP was elevated.330[98- 1155] pg/mL vs 86[41- 192] pg/mL; Pd.with patients without a d.astolic heart sound. The sensitivities of these heart sound. to d.tect an elevated.LVEd., red.ced.LVEF, or elevated.BNP were 41% , 52% , and.32% for an S3, and.46% , 43% , and.40% for an S4, respectively. For abnormal levels of the same markers of ventricular function, the specificities of the S3 were 92% , 87% , and.92% , while the specificities of the S4 were 80% , 72% , and.78% , respectively.
