Gene Expression Analysis of Extracellular Matrix and Cytokines after Uterine Artery Embolization

Arterial embolization of myomas (AEM) is an established option for the conservative treatment of uterine leio-myomas;it treats all present uterine nodules at once, is less invasive than other procedures and effective in controlling symptoms, and does not require long term hospitalizations. Nevertheless, the potential impact on endometrial morphological and functional outcomes after the procedure is still controversial based on reports of endometritis or eventual transient ischemia. This study evaluated endometrial reorganization in uterine leiomyoma patients, before and after AEM, through gene expression analyses of extracellular matrix and cytokines genes in theendometrial tissue. Eight patients with leiomyomas were evaluated before AEM and 6 months after. The examinations included transvaginal pelvic ultrasonography, dosing of the follicle-stimulating hormone, and endometrial biopsy during the second phase of the menstrual cycle. RNA was extracted from endometrial samples, cDNA was synthesized, and applied on PCR arrayTM plates to evaluate the expression of extracellular matrix (ECM) genes and cytokines and their receptors’ genes (CYT). The ECM overexpressed genes were MMP (1, 3, 10, 11, and 14), CTGF1, ICAM1, TBHS1, ITGA2, ITGA3, ITGB3, COL7A1, COL12A, SPP1, and TNC;ADAMTS8 was underexpressed. The CYT overexpressed genes were SPP1, BCL6, CXCL12, IL-8, and CEBPB;CXCL13 and CCL21 were underexpressed. The ECM results showed overexpression of proteases that are responsible for dysfunctions in the ECM, and of genes responsible for adhesion and membrane components. The CYT results showed overexpression of chemokines responsible for endometrial repair, and underexpression of cytokines involved in inflammatory processes in the endometrial tissue. AEM treatment did not negatively affect the endometrial function at 6 months after embolization. The concept of quality of life is multiple concerns as a cultural, social and environmental individuality. The aim of this study was to verify the levels of quality of life in elderly practitioners of physical activity comparing a active group with a control group The sample consisted of 159 physically independent and apparently healthy individuals of either sex, with mean age of 66.61 ± 4.73 years. Active group obtained mean 14.32 ± 0.763 in whoqol’s scores for levels of quality of life, the Control group (CG) obtained mean 13.31 ± 0.94 and p-value 0.05. The ANOVA with repeated samples showed higher values for active group when compared with control group after intervention, the significant level was p < 0.05. It must be recognized that not every aspect of human life is reduced to the practice of physical activity;however, it is an important instrument that generates well-being in this age group.

Relationship between maturity levels and neuromuscular capacity among youth soccer players and individuals not practicing soccer

The aim of this study was to compare maturational stage and neuromuscular skills among soccer players and non-athletes, as well as to investigate the relationship between maturation and neuromuscular performance. Twenty five adolescent males (14.3 ± 0.45 years) participated in the study and were divided into two groups: soccer players (SP – n = 13, 14.1 ± 0.3 years, 58.9 ± 6.90 kg, 1.72 ± 0.04 m, 19.9 ± 1.7 kg·m2, 13.3% ± 4.3% fat) and non-athletes (NA – n = 12, 14.5 ± 0.5 years, 57.3 ± 6.9 kg, 1,67 ± 0.06 m, 20.6 ± 3.9 kg·m2, 14.0% ± 5.7% fat). The square test and 20 m speed test were used to assess agility and speed, respectively. The Tanner self-assessment of pubic hair and genitalia development test was used to estimate maturational development. The Shapiro Wilk test was used to verify the normality of samples. For any data not normally distributed, the non-parametric Mann Whitney test, as well as Kendall’s Tau correlation test, were used. The p-values determined for agility (p = 0.017) and speed (p = 0.054) indicated that agility was the only variable significantly different between SP and NA. The SP and NA groups showed no difference in the levels of maturation (p = 0.41), and maturational status was not significantly correlated with agility (r = 0.013) or speed (r = ?0.003). Conclusion: Individuals who practiced football had better results for the agility test than non-athletes, even with no difference between the degree of maturation and speed. There is a low correlation between level of maturity and agility or speed.

AIM: To report a systematic review,establishing the available data to an unpublished 2a strength of evidence,better handling clinical ***: A systematic review was performed using MEDLINE,EMBASE,Cochrane,LILACS,Scopus and CINAHL databases. Information of the selected studies was extracted on characteristics of trial participants,inclusion and exclusion criteria,interventions(mainly,mucosal resection and submucosal dissection vs surgical approach) and outcomes(adverse events,different survival rates,mortality,recurrence and complete resection rates). To ascertain the validity of eligible studies,the risk of bias was measured using the Newcastle-Ottawa Quality Assessment Scale. The analysis of the absolute risk of the outcomes was performed using the software Rev Man,by computingrisk differences(RD) of dichotomous variables. Data on RD and 95%CIs for each outcome were calculated using the Mantel-Haenszel test and inconsistency was qualified and reported in χ2 and the Higgins method(I2). Sensitivity analysis was performed when heterogeneity was higher than 50%,a subsequent assay was done and other findings were ***: Eleven retrospective cohort studies were selected. The included records involved 2654 patients with early gastric cancer that filled the absolute or expanded indications for endoscopic resection. Threeyear survival data were available for six studies(n = 1197). There were no risk differences(RD) after endoscopic and surgical treatment(RD = 0.01,95%CI:-0.02-0.05,P = 0.51). Five-year survival data(n = 2310) showed no difference between the two groups(RD = 0.01,95%CI:-0.01-0.03,P = 0.46). Recurrence data were analized in five studies(1331 patients) and there was no difference between the approaches(RD = 0.01,95%CI:-0.00-0.02,P = 0.09). Endoscopic stenting for inoperable malignant biliary obstruction: A systematic review and meta-analysis

AIM: To analyze through meta-analyses the benefits of two types of stents in the inoperable malignant biliary ***: A systematic review of randomized clinical trials(RCT) was conducted, with the last update on March 2015, using EMBASE, CINAHL(EBSCO), MEDLINE, LILACS/CENTRAL(BVS), SCOPUS, CAPES(Brazil), and gray literature. Information of the selected studies was extracted in sight of six outcomes: primarily regarding dysfunction, complication and reintervention rates; and secondarily costs, survival, and patency time. The data about characteristics of trial participants, inclusion and exclusion criteria and types of stents were also extracted. The bias was mainly assessed through the JADAD scale. This meta-analysis was registered in the PROSPERO database by the number CRD42014015078. The analysis of the absolute risk of the outcomes was performed using the software Rev Man, by computing risk differences(RD) of dichotomous variables and mean differences(MD) of continuous variables. Data on RD and MD for each primary outcome were calculated using the MantelHaenszel test and inconsistency was qualified and reported in χ2 and the Higgins method(I2). Sensitivity analysis was performed when heterogeneity was higher than 50%, a subsequent assay was done and other findings were compiled. Student's t-test was used for the comparison of weighted arithmetic means regarding secondary ***: Initial searching identified 3660 studies; 3539 were excluded through title, repetition, and/or abstract, while 121 studies were fully assessed and were excluded mainly because they did not compare self-expanding metal stents(SEMS) and plastic stents(PS), leading to thirteen RCT selected, with 13 articles and 1133 subjects meta-analyzed. The mean age was 69.5 years old, that were affected mostly by bile duct(proximal) and pancreatic tumors(distal). The interest in non-invasive brain stimulation techniques is increasing in recent years. Among these techniques, transcranial direct current stimulation(t DCS) has been the subject of great interest among researchers because of its easiness to use, low cost, benign profile of side effects and encouraging results of research in the field. This interest has generated several studies and randomized clinical trials, particularly in psychiatry. In this review, we provide a summary of the development of the technique and its mechanism of action as well as a review of the methodological aspects of randomized clinical trials in psychiatry, including studies in affective disorders, schizophrenia, obsessive compulsive disorder, child psychiatry and substance use disorder. Finally,we provide an overview of t DCS use in cognitive enhancement as well as a discussion regarding its clinical use and regulatory and ethical issues. Although many promising results regarding t DCS efficacy were described, the total number of studies is still low, highlighting the need of further studies aiming to replicate these findings in larger samples as to provide a definite picture regarding t DCS efficacy in psychiatry.

Are There Schizophrenia Genetic Markers and Mutations? A Systematic Review and Meta-Analyses

Background: Schizophrenia is a severe psychiatric disorder with a complex genetic factor determining its disease onset. Nevertheless, it is not clear in this mental disorder. Objective: To conduct a systematic review of articles regarding the genetic markers and mutations in schizophrenia. Methods: A systematic review of articles on genetic markers and mutations in schizophrenia, published from January 1, 2011, to September 7, 2015, on SCOPUS database was carried out. Search terms were “Genetic markers”, “Mutation”, and “Schizophrenia”. Results: Of the 527 retrieved studies, 31 met the eligibility criteria. Genetic polymorphism, Immune-associated genes, TCF4 and ZNF804A association with microRNA, Neuregulin gene, Chromosome 13q32 and 11p15.4, genes involved in glutamatergic via schizophrenia and brain structure, appeared to be associated with the origin of schizophrenia. Conclusion: Some studies show genes involved in several pathways leading to the disease pathogenesis such as that one related with the dopaminergic and immune system, or rare alleles. Some genes present no involvement in the etiology of this mental disorder. These findings clarify the genetic complexity of schizophrenia and affirm that together, the genes have an overall effect greater than the sum of the individual effect of each gene.

Primary Effusion Lymphoma in a HIV-1/2-Infected Patient

Background: Primary effusion lymphoma (PEL) is a lymphoid proliferation related to Kaposi sarcoma herpesvirus 8/human herpesvirus 8 (KSHV/HHV8) that affects mainly human immunodeficiency virus (HIV) infected individuals but can also occur in other immunodeficiency settings. It is characterized by lymphomatous effusions in different serous body cavities without the presence of a detectable tumor mass. The diagnosis is challenging and the clinical outcomes are poor. Aim: The aim of this paper is to report a rare case of PEL in a man who have sex with women (MSW) with HIV-1/2 infection, history of visceral Kaposi sarcoma (KS) and the development of a seronegative arthritis previous to the lymphoproliferative disease diagnosis. PEL presented with ascites, was treated with high-dose chemotherapy and autologous stem cell transplantation, with a good clinical outcome. Case Presentation: We describe a case of a 48-year-old HIV-1/2-infected patient from a high HHV8 seroprevalent country, hospitalized following a three-month history of increased abdominal volume and general constitutional symptoms. Laboratory data revealed normocytic normochromic anemia and a high level of lactate dehydrogenase. A diagnostic paracentesis was performed with cytology compatible with high-grade B-cell lymphoma. Peritoneal fluid cytology showed large lymphoid cells expressing leucocyte-common antigen CD45 without expression of the CD20 antigen (B-lymphocytes) and positivity for HHV8 by immunocytochemical staining, compatible with the diagnosis of PEL.
