BACKGROUND Ureteropelvic junction obstruction(UPJO)is a common congenital urinary tract disorder in *** can be diagnosed as early as in utero due to the presence of hydronephrosis or later in life due to symptomatic *** To evaluate the discrepancy between dynamic contrast-enhanced magnetic resonance urography(dMRU)and scintigraphy 99m-technetium mercaptoacetyltriglycine(MAG-3)for the functional evaluation of *** Between 2016 and 2020,126 patients with UPJO underwent surgery at Robert Debré*** these,83 received a prenatal diagnosis,and 43 were diagnosed during *** of the 126 patients underwent surgery based on the clinical situation and postnatal ultrasound findings without undergoing functional imaging *** renal function was evaluated preoperatively using scintigraphy MAG-3(n=28),dMRU(n=53),or both(n=40).In this study,we included patients who underwent surgery for UPJO and scintigraphy MAG-3+dMRU but excluded those who underwent only scintigraphy MAG-3 or *** patients were divided into groups A(10%discrepancy).We examined the discrepancy in split renal function between the two modalities and investigated the possible risk *** The split renal function between the two kidneys was compared in 40 patients(28 boys and 12 girls)using scintigraphy MAG-3 and *** renal function,as determined using both modalities,showed a difference of10%in 9 *** of the relative renal function using dMRU revealed an excellent correlation coefficient with renal scintigraphy MAG-3 for both *** Our findings demonstrated that dMRU is equivalent to scintigraphy MAG-3 for evaluating split renal function in patients with UPJO.

Checkmate to liver biopsy in chronic hepatitis C?

Liver biopsy (LB) has traditionally been considered the gold standard for pretreatment evaluation of liver fibrosis in patients with chronic hepatitis C (CHC). However, LB is an invasive procedure with several shortcomings (intra-and interobserver variability of histopathological interpretation, sampling errors, high cost) and the risk of rare but potentially life-threatening complications. In addition, LB is poorly accepted by patients and it is not suitable for repeated evaluation. Further-more, the prevalence of CHC makes LB unrealistic to be performed in all patients with this disease who are candidates for antiviral therapy. The above-mentioned drawbacks of LB have led to the development of non-invasive methods for the assessment of liver fibrosis. Several noninvasive methods, ranging from serum marker assays to advanced imaging techniques, have proved to be excellent tools for the evaluation of liver fibrosis in patients with CHC, whereas the value of LB as a gold standard for staging fibrosis prior to antiviral therapy has become questionable for clinicians. Despite significant resistance from those in favor of LB, noninvasive methods for pretreatment assessment of liver fibrosis in patients with CHC have become part of routine clinical practice. With protease inhibitors-based triple therapy already available and substantial improvement in sustained virological response, the time has come to move forward to noninvasiveness, with no risks for the patient and, thus, no need for LB in the assessment of liver fibrosis in the decision making for antiviral therapy in CHC.

Knights in Action： Lectin Receptor-Like Kinases in Plant Development and Stress Responses

The Receptor-Like Kinase （RLK） is a vast protein family with over 600 genes in Arabidopsis and 1100 in rice. The Lectin RLK （LecRLK） family is believed to play crucial roles in saccharide signaling as well as stress perception. All the LecRLKs possess three domains： an N-terminal lectin domain, an intermediate transmembrane domain, and a C-terminal kinase domain. On the basis of lectin domain variability, LecRLKs have been subgrouped into three subclasses： L-, G-, and C-type LecRLKs. While the previous studies on LecRLKs were dedicated to classification, comparative structural analysis and expression analysis by promoter-based studies, most of the recent studies on LecRLKs have laid special emphasis on the potential of this gene family in regulating biotic/abiotic stress and developmental pathways in plants, thus mak- ing the prospects of studying the LecRLK-mediated regulatory mechanism exceptionally promising. In this review, we have described in detail the LecRLK gene family with respect to a historical, evolutionary, and structural point of view. Furthermore, we have laid emphasis on the LecRLKs roles in development, stress conditions, and hormonal response. We have also discussed the exciting research prospects offered by the current knowledge on the LecRLK gene family. The multitude of the LecRLK gene family members and their functional diversity mark these genes as both interesting and worthy candidates for further analysis, especially in the field of crop improvement.

Gauge Gravitational Field in a Fractal Space-Time

Considering the fractal structure of space-time, the scale relativity theory in the topological dimension DT = 2 is built. In such a conjecture, the geodesics of this space-time imply the hydrodynamic model of the quantum mechanics. Subsequently, the gauge gravitational field on a fractal space-time is given. Then, the gauge group, the gauge-covariant derivative, the strength tensor of the gauge field, the gauge-invariant Lagrangean, the field equations of the gauge potentials and the gauge energy-momentum tensor are determined. Finally, using this model, a Reissner- Nordstrom type metric is obtained.

Update on adrenal insufficiency in patients with liver cirrhosis

The ClasSi coefficient for the evaluation of ranking quality in the presence of class similarities

Evaluation measures play an important role in the design of new approaches, and often quality is measured by assessing the relevance of the obtained result set. While many evaluation measures based on precision/recall are based on a binary relevance model, ranking correlation coefficients are better suited for multi-class problems. State-of-the-art rank- ing correlation coefficients like Kendall＇s T and Spearman＇s p do not allow the user to specify similarities between differ- ing object classes and thus treat the transposition of objects from similar classes the same way as that of objects from dissimilar classes. We propose ClasSi, a new ranking corre- lation coefficient which deals with class label rankings and employs a class distance function to model the similarities between the classes. We also introduce a graphical representation of ClasSi which describes how the correlation evolves throughout the ranking.

There has been an increase in attacks on mobile devices,such as smartphones and tablets,due to their growing *** malware is one of the most dangerous threats,causing both security breaches and financial *** malware is likely to continue to evolve and proliferate to carry out a variety of cybercrimes on mobile *** malware specifically targets Android operating system as it has grown in *** rapid proliferation of Android malware apps poses a significant security risk to users,making static and manual analysis of malicious files ***,efficient identification and classification of Androidmalicious files is *** ConvolutionalNeuralNetwork(CNN)basedmethods have been proposed in this regard;however,there is still room for performance *** this work,we propose a transfer learning and stacking approach to efficiently detect the Android malware files by utilizing two wellknown machine learning models,ResNet-50 and Support Vector Machine(SVM).The proposed model is trained on the DREBIN dataset by transforming malicious APK files into grayscale *** model yields higher performance measures than state-of-the-art works on the DREBIN dataset,where the reported measures are accuracy,recall,precision,and F1 measures of 97.8%,95.8%,95.7%,and 95.7%,respectively.

Production of succinic acid in basket and mobile bed bioreactors-Comparative analysis of substrate mass transfer aspects

The glucose mass transfer in the biosynthesis of succinic acid with immobilized Actinobacillus succinogenes cells has been comparatively analyzed for a bioreactor with mobile bed vs. a stationary basket bioreactor. The process has been considered to occur under substrate and product inhibitory effects. The results indicated that the bioreactor with mobile bed is more efficient for biocatalyst particles with a diameter over 3 mm, while the basket bioreactor is more efficient for smaller biocatalyst particles and basket bed thickness below 5 mm. The performances of both configurations of immobilized A. succinogenes cell beds were found to be superior to the column packed bed bioreactor.

Ulcerative colitis worsened after Clostridium difficile infection:Efficacy of infliximab

The incidence of Clostridium difficile (C. difficile) infection (CDI) is 1.8%-5.7% in admitted patients with ulcerative colitis (UC). CDI can worsen UC and increase the risk for colectomy or even death, thus necessitating therapy escalation, such as increasing the corticoid therapy or starting a biologic treatment. Several reported cases with infliximab therapy have provided favorable outcomes in UC patients with CDI, suggesting that infliximab treatment may be protective; however, the optimal infliximab treatment regimen for UC patients with CDI remains to be established. Here, we report a case of worsening UC in the presence of recurrent CDI. The patient had received prior ciprofloxacin and immunosuppressive therapy during a prolonged hospital stay. The deterioration in the patient’s condition likely resulted from the ability of C. difficile to promote relapsing of UC by activating the immune response. Ultimately, the patient was treated with a high dose of infliximab after a low trough level of infliximab at week 8 was identified, yielding better clinical results. Infliximab was found to be safe after repetitive episodes of CDI. The trough level of infliximab was therefore a useful indicator to guide therapy and correlated well with the patient’s outcome.
