Leishmaniasis, a neglected disease caused by Leishmania protozoans, primarily affects people in tropical and subtropical areas. Chemotherapy based on the use of pentavalent antimonials, amphotericin B, paromomycin, miltefosine and liposomal amphotericin B is currently the only effective treatment. However, adverse effects, long-term treatment and the emergence of parasite resistance have led to the search for alternative treatments. Natural products used in traditional medicine provide an unlimited source of molecules for the identification of new drugs, and the Amazon region has abundant biodiversity that includes several species of plants and animals, providing a rich source of new products and com- pounds. Although the literature describes numerous promising compounds and extracts for combating Leishmania protozoans, the results of such research have not been embraced by the pharmaceutical industry for the development of new drugs. Therefore, this review focused on the antileishmanial activity of extracts, isolated compounds and essential oils commonly used by the local population in the Brazilian Amazonian region to treat several illnesses and described in the literature as promising compounds for combating leishmaniasis.

Ethical issues in primary (health) care (PC) are common and are confronted daily. In the scarce literature that was available, Zoboli and Silva developed an instrument (IPE-APS) for perceiving ethical conflicts. Two empirical ex post facto studies to gather psychometric evidence for the instrument were outlined. In the first study, 88 nurses and physicians from Brazil’s Family Health (Saúde da Família—SF) responded to the IPE-APS, and the results confirmed its validity and accuracy. In total, 14 items were grouped in Factor I (relationship/responsibility), which was described as having human rights and deontological nature (Cronbach’s alpha = 0.92), and 11 items were grouped in Factor II, which was described as having a labor structure nature (Cronbach’s alpha = 0.90). In the second study, 207 SF professionals in Goiania, Goiás responded to the IPE-APS. The results showed that Factor I had a high factorial congruence index (0.98), which revealed high similarity and showed that the instrument displayed practical validity to identify and assess ethical problem perception in PC.

N-11-azaartemisinins potentially active against Plasmodium falciparum are designed by combining molecular electrostatic potential (MEP), ligand-receptor interaction, and models built with supervised machine learning methods (PCA, HCA, KNN, SIMCA, and SDA). The optimization of molecular structures was performed using the B3LYP/6-31G* approach. MEP maps and ligand-receptor interactions were used to investigate key structural features required for biological activities and likely interactions between N-11-azaartemisinins and heme, respectively. The supervised machine learning methods allowed the separation of the investigated compounds into two classes: cha and cla, with the properties εLUMO+1 (one level above lowest unoccupied molecular orbital energy), d(C6-C5) (distance between C6 and C5 atoms in ligands), and TSA (total surface area) responsible for the classification. The insights extracted from the investigation developed and the chemical intuition enabled the design of sixteen new N-11-azaartemisinins (prediction set), moreover, models built with supervised machine learning methods were applied to this prediction set. The result of this application showed twelve new promising N-11-azaartemisinins for synthesis and biological evaluation.

Background： Brazil is among the nations with the greatest rates of annual cocaine usage. Artemisinins tested against W-2 strains of malaria falciparum are investigated with molecular electrostatic potential (MEP), in an attempt to identify key features of the compounds that are necessary for their activities, as well as to investigate likely interactions with the receptor in a biological process and to use that information to propose new molecules. In order to discover the best geometry involving the ligand-receptor complexes (heme) studied and help in the proposition of the new derivatives, molecular simulations of interactions between the most negative charged region around the peroxide and heme locates (the ones around the Fe2+ ion) were carried out. In addition, PCA (principal components analysis), HCA (hierarchical cluster analysis), SDA (stepwise discriminant analysis), and KNN (K-nearest neighbor) multivariate models were employed to investigate which descriptors are responsible for the classification between the higher and lower antimalarial activity of the compounds, and also this information was used to propose new potentially active molecules. The information accumulated in studies of MEP, molecular docking, and multivariate analysis supported the proposal of new structures with potential antimalarial activities. The multivariate models constructed were applied to the new structures and indicated numbers 19 and 20 as the most prominent for syntheses and biological assays.

The determinants of Anthropometric deficits in Children under Five Years Old in Tete Province Mozambique Using Hierarchy Approach

Introduction: The malnutrition in children under 5 years is a serious public health problem in the developing countries such as Mozambique. The stunting can affect cognitive and physical development and compromise the school performance. Method: A cross sectional study was carried out which used the secondary data from a survey conducted by National Health Institute of Mozambique, in districts of Angonia, Tsangano, Magoe and Changara in Tete Province, Mozambique. The polynomial regression model and the hierarchical approach were used to examine the association between social economic factors and malnutrition in children measured by stunting (low height for age) and underweight (low weight for age). Results: The prevalence of moderate and severe stunting was 39.8%, and prevalence of moderate and severe underweight was 19.6%. In the basic level, the households that had 1 durable goods possession were associated to 1.46 times greater than the mild underweight, OR = 1.46 (C.I. 95% 1.02 - 2.11). In the intermediate level, lack of latrine increased 2.01 times greater than the moderate and severe underweight in children, OR = 2.01 (C.I. 95% 1.09 - 3.70);In the immediate level, the child deworming had 1.91 times greater than the moderate and severe stunting, OR = 1.91 (C.I.95% 1.04 - 3.52) and children who did not breastfeed had 2.42 times greater than the moderate and severe stunting, OR = 2.42 (C.I. 1.49 - 3.43). Conclusion: In the countries where the weak health system and the inadequate children food are frequent, the prevalence of undernutrition in children remains very high.
