bACKGROUND Over-the-scope clip-assisted endoscopic full thickness resection(eFTR) of subepithelial tumors is a novel and promising endoscopic technique. Recently,there have been prospective studies investigating its use for colonic masses, but data regarding its use and efficacy in the duodenum are limited to a few *** SUMMARY A 65-year-old African American female presents for evaluation of persistent gastroesophageal reflux disease not responsive to medical treatment. A 1 cm nodule was incidentally found in the duodenum and biopsies revealed a low grade well differentiated neuroendocrine tumor. The nodule was removed using over-the-scope clip-assisted eFTR and pathology revealed clear margins. We review the available literature with a discussion on the efficacy and safety of clipassisted eFTR s of subepithelial lesions in the *** Clip assisted eFTR appears to be a safe and efficacious treatment approach to duodenal subepithelial lesions. Further prospective studies are needed to investigate the long-term utility and safety of clip-assisted eFTR in the management of subepithelial duodenal lesions.

bACKGROUND A variety of immune-modulating drugs are becoming increasingly used for various cancers. Despite increasing indications and improved efficacy, they are often associated with a wide variety of immune mediated adverse events including colitis that may be refractory to conventional therapy. Although these drugs are being more commonly used by Hematologists and Oncologists, there are still many gastroenterologists who are not familiar with the incidence and natural history of gastrointestinal immune-mediated side effects, as well as the role of infliximab in the management of this *** SUMMARY We report a case of a 63-year-old male with a history of metastatic renal cell carcinoma who presented to our hospital with severe diarrhea. The patient had received his third combination infusion of the anti-CTLA-4 monoclonal antibody Ipilimumab and the immune checkpoint inhibitor Nivolumab and developed severe watery non-bloody diarrhea the same day. He presented to the hospital where he was found to be severely dehydrated and in acute renal failure. An extensive workup was negative for infectious etiologies and he was initiated on high dose intravenous steroids. However, he continued to worsen. A colonoscopy was performed and revealed no endoscopic evidence of inflammation. Random biopsies for histology were obtained which showed mild colitis, and were negative for Cytomegalovirus and Herpes Simplex Virus. He was diagnosed with severe steroid-refractory colitis induced by Ipilimumab and Nivolumab and was initiated on Infliximab. He responded promptly to it and his diarrhea resolved the next day with progressive resolution of his renal impairment. On follow up his gastrointestinal side symptoms did not *** Given the increasing use of immune therapy in a variety of cancers, it is important for gastroenterologists to be familiar with their gastrointestinal side effects and comfortable with their management, including prescribing infliximab.

Infectious diseases caused by arboviruses are a public health concern in ***,studies on data prevalence and threats posed by arboviruses are *** study investigated the seroprevalence of arboviruses in a healthy population in Pakistan,including severe fever with thrombocytopenia syndrome virus(SFTSV),Crimean-Congo hemorrhagic fever virus(CCHFV),Tamdy virus(TAMV),and Karshi virus(KSIV)based on a newly established luciferase immunoprecipitation system(LIPS)assays,and Zika virus(ZIKV)by enzyme-linked immunosorbent assays(ELISA).Neutralizing activities against these arboviruses were further examined from the antibody positive *** results showed that the seroprevalence of SFTSV,CCHFV,TAMV,KSIV,and ZIKV was 17.37%,7.58%,4.41%,1.10%,and 6.48%,respectively,and neutralizing to SFTSV(1.79%),CCHFV(2.62%),and ZIKV(0.69%)were identified,as well as to the SFTSV-related Guertu virus(GTV,0.83%).Risk factors associated with the incidence of exposure and levels of antibody response were ***,co-exposure to different arboviruses was demonstrated,as thirty-seven individuals were having antibodies against multiple viruses and thirteen showed neutralizing ***,individuals aged40 years,and outdoor workers had a high risk of exposure to *** these results reveal the substantial risks of infection with arboviruses in Pakistan,and indicate the threat from co-exposure to multiple *** findings raise the need for further epidemiologic investigation in expanded regions and populations and the necessity to improve health surveillance in Pakistan.

AIMTo determine if the lymphocyte-to-monocyte ratio (LMR) could be helpful in predicting survival in patients with pancreatic adenocarcinoma. METHODSWe retrospectively reviewed the medical records of all patients diagnosed with pancreatic adenocarcinoma in the VA North Texas Healthcare System from January 2005 to December 2010. The LMR was calculated from peripheral blood cell counts obtained at the time of diagnosis of pancreatic cancer by dividing the absolute lymphocyte count by the absolute monocyte count. A Univariable Cox regression analysis was performed using these data, and hazard ratios (HR) and 95%CI were calculated. The median LMR (2.05) was used to dichotomize patients into high-LMR and low-LMR groups and the log rank test was used to compare survival between the two groups. RESULTSWe identified 97 patients with pancreatic adenocarcinoma (all men, 66% white, 30% African-American). The mean age and weight at diagnosis were 66.0 ± 0.9 (SEM) years and 80.4 ± 1.7 kg respectively. Mean absolute lymphocyte and monocyte values were 1.50 ± 0.07 K/μL and 0.74 ± 0.03 K/μL respectively. Mean, median and range of LMR was 2.36, 2.05 and 0.4-12 respectively. In the univariable Cox regression analysis, we found that an increased LMR was a significant indicator of improved overall survival in patients with pancreatic adenocarcinoma (HR = 0.83; 95%CI: 0.70-0.98; P = 0.027). Kaplan-Meier analysis revealed an overall median survival of 128 d (95%CI: 80-162 d). The median survival of patients in the high-LMR (> 2.05) group was significantly greater than the low-LMR group (≤ 2.05) (194 d vs 93 d; P = 0.03), validating a significant survival advantage in patients with a high LMR. CONCLUSIONThe LMR at diagnosis is a significant predictor for survival and can provide useful prognostic information in the management of patients with pancreatic adenocarcinoma.

The mobility edge （ME） model with single Gaussian density of states （DOS） is simplified based on the recent exper- imental results about the Einstein relationship. The free holes are treated as being non-degenerate, and the trapped holes are dealt with as being degenerate. This enables the integral for the trapped holes to be easily realized in a program. The J-V curves are obtained through solving drift-diffusion equations. When this model is applied to four organic diodes, an obvious deviation between theoretical curves and experimental data is observed. In order to solve this problem, a new DOS with exponential tall is proposed. The results show that the consistence between J-V curves and experimental data based on a new DOS is far better than that based on the Gaussian DOS. The variation of extracted mobility with temperature can be well described by the Arrhenius relationship.

In this paper, we consider the following nonlinear elliptic problem ： △2u =｜u｜n-4^-u＋u｜u｜q-1u,in Ω,△u=u=0 on ЭΩ,where Ω is a bounded and smooth domain in R^n,n∈{5,6,7},u is a parameter and q∈]4/（n-4）,（12-n）/（n-4）].We study the solutions which concentrate around two points of Ω. We prove that the concentration speeOs are the same order and the distances of the concentration points from each other and from the boundary are bounded. For Ω=（Ωa）a a smooth ringshaped open set, we establish the existence of positive solutions which concentrate at two points of Ω. Finally, we show that for u〉0, large enough, the problem has at least many positive solutions as the Ljusternik-Schnirelman category of Ω.

We study a class of nonlinear parabolic equations of the type：δb（u）/δt-div（a（x,t,u）△u）＋y（u）｜△u｜^2=f,where the right hand side belongs to L^1（Q）, b is a strictly increasing C^1-function and -div（a（x, t, u）△u） is a Leray-Lions operator. The function g is just assumed to be continuous on R and to satisfy a sign condition. Without any additional growth assumption on u, we prove the existence of a renormalized solution.

Water scarcity is the most significant barrier to agricultural development in arid and semi-arid *** irrigation is an effective solution for managing agricultural water in these *** use of additives such as vermicompost(VC)to improve soil characteristics and increase yield is a popular *** this,there is still a lack of understanding of the interaction between irrigation water and VC on various *** study aimed to investigate the interaction effect of irrigation water and VC on greenhouse cucumber yield,yield components,quality,and irrigation water use efficiency(IWUE).The trials were done in a split-plot design in three replicates in a semi-arid region of southeastern Iran in 2018 and *** levels of VC in the experiments,i.e.,10(V_(1)),15(V_(2)),and 20 t/hm^(2)(V_(3)),and three levels of irrigation water,i.e.,50%(I_(1)),75%(I_(2)),and 100%(I_(3))of crop water requirement were *** results showed that the amount of irrigation water,VC,and their interaction significantly affected cucumber yield,yield components,quality,and IWUE in both *** the amount of irrigation water and VC application rates reduced the weight,diameter,length,and cucumber *** maximum yield(175 t/hm^(2))was recorded in full irrigation using 20 t/hm^(2)of VC,while the minimum yield(98 t/hm^(2))was found in I_(1)V_(1)*** maximum and minimum values of IWUE were recorded for I_(1)V_(3)and I_(3)V_(1)treatments as 36.07 and 19.93 kg/(m^(3)•hm^(2)),***,reducing irrigation amount decreased chlorophyll a and b,but increased vitamin ***,the maximum carbohydrate and protein contents were obtained in mild water-stressed conditions(I_(2)).Although adding VC positively influenced the value of quality traits,no significant difference was observed between V_(2)and V_(3)*** on the results,adding VC under full irrigation conditions leads to enhanced yield and ***,in the case of applying deficit ir
