Effect of awareness,quarantine and vaccination as control strategies on COVID-19 with Co-morbidity and Re-infection

In this paper,a deterministic compartmental model is presented to assess the impact of vaccination and non-pharmaceutical interventions(social distance,awareness,face mask,and quarantine)on the transmission dynamics of COVID-19 with co-morbidity and reinfection.An expression for the basic reproduction number is then derived for this model.Theoretical analysis shows that the model exhibits backward bifurcation phenomenon when the basic reproduction number is less than unity.But for the case of no reinfection,the model has a globally asymptotically stable disease-free equilibrium(DFE)when the basic reproduction number is less than unity.Furthermore,it is shown that in the case of no re-infection,a unique endemic equilibrium point(EEP)of the model exists which is globally asymptotically stable whenever the reproduction number is greater than unity.From the global sensitivity and uncertainty analysis,we have identified mask coverage,mask efficacy,vaccine coverage,vaccine efficacy,and contact rate as the most influential parameters influencing the spread of COVID-19.Numerical simulation results show that the use of effective vaccines with proper implementation of non-pharmaceutical interventions could lead to the elimination of COVID-19 from the community.Numerical simulations also suggest that the control strategy that ensures a continuous and effective mass vaccination program is the most cost-effective control strategy.The study also shows that in the presence of any co-morbidity and with the occurrence of re-infection,the disease burden may increase.

Dynamics of novel COVID-19 in the presence of Co-morbidity

A novel coronavirus(COVID-19)has emerged as a global serious public health issue from December 2019.People having a weak immune system are more susceptible to coronavirus infection.It is a double challenge for people of any age with certain underlying medical conditions including cardiovascular disease,diabetes,high blood pressure and cancer etc.Co-morbidity increases the probability of COVID-19 complication.In this paper a deterministic compartmental model is formulated to understand the transmission dynamics of COVID-19.Rigorous mathematical analysis of the model shows that it exhibits backward bifurcation phenomenon when the basic reproduction number is less than unity.For the case of no re-infection it is shown that having the reproduction number less than one is necessary and sufficient for the effective control of COVID-19,that is,the disease free equilibrium is globally asymptotically stable when the reproduction threshold is less than unity.Furthermore,in the absence of reinfection,a unique endemic equilibrium of the model exists which is globally asymptotically stable whenever the reproduction number is greater than unity.Numerical simulations of the model,using data relevant to COVID-19 transmission dynamics,show that the use of efficacious face masks publicly could lead to the elimination of COVID-19 up to a satisfactory level.The study also shows that in the presence of co-morbidity,the disease increases significantly.

Constructing a biofunctionalized 3D-printed gelatin/sodium alginate/chitosan tri-polymer complex scaffold with improvised biological andmechanical properties for bone-tissue engineering

Sodium alginate(SA)/chitosan(CH)polyelectrolyte scaffold is a suitable substrate for tissue-engineering application.The present study deals with further improvement in the tensile strength and biological properties of this type of scaffold to make it a potential template for bone-tissue regeneration.We experimented with adding 0%–15%(volume fraction)gelatin(GE),a protein-based biopolymer known to promote cell adhesion,proliferation,and differentiation.The resulting tri-polymer complex was used as bioink to fabricate SA/CH/GEmatrices by three-dimensional(3D)printing.Morphological studies using scanning electron microscopy revealed the microfibrous porous architecture of all the structures,which had a pore size range of 383–419μm.X-ray diffraction and Fourier-transform infrared spectroscopy analyses revealed the amorphous nature of the scaffold and the strong electrostatic interactions among the functional groups of the polymers,thereby forming polyelectrolyte complexes which were found to improve mechanical properties and structural stability.The scaffolds exhibited a desirable degradation rate,controlled swelling,and hydrophilic characteristics which are favorable for bone-tissue engineering.The tensile strength improved from(386±15)to(693±15)kPa due to the increased stiffness of SA/CH scaffolds upon addition of gelatin.The enhanced protein adsorption and in vitro bioactivity(forming an apatite layer)confirmed the ability of the SA/CH/GE scaffold to offer higher cellular adhesion and a bone-like environment to cells during the process of tissue regeneration.In vitro biological evaluation including the MTT assay,confocal microscopy analysis,and alizarin red S assay showed a significant increase in cell attachment,cell viability,and cell proliferation,which further improved biomineralization over the scaffold surface.In addition,SA/CH containing 15%gelatin designated as SA/CH/GE15 showed superior performance to the other fabricated 3D structures,demonstrating its potential for use in bone-tissue engineering.

Clinical outcomes of triceps reflecting anconeus pedicle and olecranon osteotomy approach for distal humerus intercondylar fractures

BACKGROUND The preferred treatment for distal humeral intercondylar fractures is open reduction and internal fixation.While there is consensus about the posterior approach,several posterior approaches have been developed.It is debatable as to which approach is best.AIM To compare triceps reflecting anconeus pedicle(TRAP)and olecranon osteotomy approaches for internal fixation of distal humeral intercondylar fracture.METHODS In total,40 cases of Arbeitsgemeinschaft für Osteosynthesefragen/Association of the Study of Internal Fixation type C,closed,and Gustilo type I intercondylar humeral fractures were included.Patients ranged in age from 18 years to 70 years.The patients were randomized into two groups:TRAP group and olecranon osteotomy group,with 20 cases in each.All were followed up at 6 wk,3 months,6 months,and 12 months.Functional outcomes were measured in terms of flexion-extension arc,Disabilities of Arm,Shoulder and Hand score,and Mayo Elbow Performance Score.RESULTS The mean age was 43.2 years in the TRAP group and 37.5 years in the olecranon osteotomy group.The mean operative time and mean duration of hospital stay in the TRAP group were significantly higher than in the olecranon osteotomy group(119.5 vs 111.5 min and 9.85 vs 5.45 d,respectively).The mean arc of flexion-extension,Disabilities of Arm,Shoulder and Hand score,and Mayo Elbow Performance Score were comparable without any significant difference in the groups at the 12-month follow-up(107.0 vs 106.2,18.3 vs 15.7,and 84.2 vs 86.2,respectively).Ulnar paresthesia and superficial infections were comparable in both groups(2 cases vs 3 cases and 3 cases vs 2 cases,respectively).Hardware prominence was significantly higher in the olecranon osteotomy group,mostly due to tension band wiring.CONCLUSION Both approaches were equivalent,but there is a need for further study including higher numbers of subjects and longer study duration to prove the benefits of one approach over the other.

Transcranial direct current stimulation as early augmentation in adolescent obsessive compulsive disorder:A pilot proof-of-concept randomized control trial

BACKGROUND Transcranial direct current stimulation(tDCS)is proven to be safe in treating various neurological conditions in children and adolescents.It is also an effective method in the treatment of OCD in adults.AIM To assess the safety and efficacy of tDCS as an add-on therapy in drug-naive adolescents with OCD.METHODS We studied drug-naïve adolescents with OCD,using a Children’s Yale-Brown obsessive-compulsive scale(CY-BOCS)scale to assess their condition.Both active and sham groups were given fluoxetine,and we applied cathode and anode over the supplementary motor area and deltoid for 20 min in 10 sessions.Reassessment occurred at 2,6,and 12 wk using CY-BOCS.RESULTS Eighteen adolescents completed the study(10-active,8-sham group).CY-BOCS scores from baseline to 12 wk reduced significantly in both groups but change at baseline to 2 wk was significant in the active group only.The mean change at 2 wk was more in the active group(11.8±7.77 vs 5.25±2.22,P=0.056).Adverse effects between the groups were comparable.CONCLUSION tDCS is safe and well tolerated for the treatment of OCD in adolescents.However,there is a need for further studies with a larger sample population to confirm the effectiveness of tDCS as early augmentation in OCD in this population.

Aerodynamics of a Tandem-Bladed Axial Compressor Rotor under Circumferential Distortion at Different Rotational Speeds

For most aircraft engines,inflow distortion is inevitable.Inflow distortion is known to degrade the aerodynamic performance and stable operating limits of a compressor.Tandem rotor configuration is an arrangement that effectively controls the growth of the boundary layer over the suction surface of the blade.Therefore,a higher total pressure rise can be achieved through this unconventional design approach involving the splitting of the blade into forward and aft sections.It is expected that the effect of inlet flow distortion would be more severe for a tandem-rotor design due to the greater flow turning inherent in such designs.However,this aspect needs to be thoroughly examined.The present study discusses the effect of circumferential distortion on the tandem-rotor at different rotational speeds.Full-annulus RANS simulations using ANSYS CFX are used in the present study.The performance of the rotor at a particular flow coefficient and different rotational speeds is compared.The total pressure and efficiency are observed to drop at lower mass flow rates under the influence of circumferential distortion.The loss region in each blade passage is mainly associated with the blade wake,tip leakage vortex,secondary flow,and boundary layer.However,their contribution varies from passage to passage,particularly in the distorted sector.At the lower span,the wake width is found to be higher than that at a higher span.Due to the redistribution of the mass flow,the circumferential extent reduces at a higher span.In the undistorted sector,the strength of the tip leakage vortex is significantly higher at the design rotational speed than at lower speeds.The distortion near the tip region promotes an early vortex breakdown even at the design operating condition.This adversely affects the total pressure,efficiency,and stall margin.Under clean flow conditions,this phenomenon is only observed near the stall point.At the design operating condition,the breakdown of the forward rotor tip leakage vortex is detected in four blade passages

BACKGROUND There is scant literature on hepatocellular carcinoma(HCC)in patients with Budd-Chiari syndrome(BCS).AIM To assess the magnitude,clinical characteristics,feasibility,and outcomes of treatment in BCS-HCC.METHODS A total of 904 BCS patients from New Delhi,India and 1140 from Mumbai,India were included.The prevalence and incidence of HCC were determined,and among patients with BCS-HCC,the viability and outcomes of interventional therapy were evaluated.RESULTS In the New Delhi cohort of 35 BCS-HCC patients,18 had HCC at index presentation(prevalence 1.99%),and 17 developed HCC over a follow-up of 4601 person-years,[incidence 0.36(0.22-0.57)per 100 person-years].BCS-HCC patients were older when compared to patients with BCS alone(P=0.001)and had a higher proportion of inferior vena cava block,cirrhosis,and long-segment vascular obstruction.The median alpha-fetoprotein level was higher in patients with BCS-HCC at first presentation than those who developed HCC at follow-up(13029 ng/mL vs 500 ng/mL,P=0.01).Of the 35 BCS-HCC,26(74.3%)underwent radiological interventions for BCS,and 22(62.8%)patients underwent treatment for HCC[transarterial chemoembolization in 18(81.8%),oral tyrosine kinase inhibitor in 3(13.6%),and transarterial radioembolization in 1(4.5%)].The median survival among patients who underwent interventions for HCC compared with those who did not was 3.5 years vs 3.1 mo(P=0.0001).In contrast to the New Delhi cohort,the Mumbai cohort of BCS-HCC patients were predominantly males,presented with a more advanced HCC[Barcelona Clinic Liver Cancer C and D],and 2 patients underwent liver transplantation.CONCLUSION HCC is not uncommon in patients with BCS.Radiological interventions and liver transplantation are feasible in select primary BCS-HCC patients and may improve outcomes.

A Study of Triangle Inequality Violations in Social Network Clustering

Clustering a social network is a process of grouping social actors into clusters where intra-cluster similarities among actors are higher than inter-cluster similarities. Clustering approaches, i.e. , k-medoids or hierarchical, use the distance function to measure the dissimilarities among actors. These distance functions need to fulfill various properties, including the triangle inequality (TI). However, in some cases, the triangle inequality might be violated, impacting the quality of the resulting clusters. With experiments, this paper explains how TI violates while performing traditional clustering techniques: k-medoids, hierarchical, DENGRAPH, and spectral clustering on social networks and how the violation of TI affects the quality of the resulting clusters.

Recent progress in carbonaceous materials based Z-scheme and Sscheme heterojunctions for photocatalytic clean energy generation

Carbonaceous materials including carbon nanotubes/nanofibers, graphene, graphene oxide, reduced graphene oxide,graphyne, graphdiyne, carbon quantum dots and fullerenes have gained considerable attention in the recent years for their unique properties such as high conductivity, excellent stability and biocompatibility. The integration of these carbonaceous materials into Zscheme and S-scheme heterojunctions has emerged as a transformative strategy to enhance the photocatalytic efficiency for energy conversion applications. This review delves into the fundamental principles of clean energy generation such as photocatalytic H2generation and CO2reduction, elucidating their respective mechanisms and advantages. Furthermore, various types of carbonaceous materials, their synthesis and construction of Z-scheme and S-scheme heterojunctions are discussed emphasizing their role in promoting charge separation, reducing recombination losses and extending the spectral response range. With the focus on solar fuel production,the recent advancements in carbonaceous based Z-scheme and S-scheme heterojunctions are discussed and summarized for photocatalytic H2generation and CO2reduction. Lastly, the current bottlenecks and challenges in the field of carbonaceous based photocatalysts are discussed with the valuable insights for future development in this particular field.
