Use of Topical Complementary and Alternative Medicines (CAM) in Diabetic Septic Foot Disease in Makkah Region, Western of Saudi Arabia, an Exploratory Study

Diabetes is a chronic condition that is significantly impacted daily by environment. At least one in five Saudis, suffer from diabetes. “Diabetes is a pandemic of unprecedented magnitude spiraling out of control” (Boulton). Saudi Arabia ranks the second highest in the Middle East, and is the seventh in the world for the prevalence of diabetes (WHO). Diabetes mellitus (DM) has been found to be related to high mortality, morbidity accompanied by poor general health and lower quality of life. Current Saudi diabetes patients’ behaviors, practices, and beliefs regarding foot health problems such as ulcers, are not well investigated. Diabetic patients frequently utilize natural remedies and Herbs for self-medication as a part of complementary and alternative Medicine. Aim of study: To explore the widespread use of the alternative remedies in the local communities of the Western Saudi Arabia, the drives behind using and to focus on the potential adverse effects following their usage. Method: A descriptive, cross-sectional study of 386 diabetic patients with feet problems in Makkah Region, West of Saudi Arabia during the period April - June 2023. Results: Out of the 531 participants, only 386 participants reported diabetic foot disorder (DFD). Most of the participant were above 65 years of age group (24.61 %). Male (52.08%) more than female (47.92%) in the research group. All of the participants had diabetic foot disorders at least once. The prevalence of complementary and alternative medicines (CAM) use among them was 67.88%, as solitary or in combination with hospital medicines. Honey headed the list followed by myrrh and black seeds. Acquaintances advice (59.54%) was the most reason for using the alternative medicine. 60.30% of Diabetic foot patient complained of adverse effects during CAM remedies use. The Effect of Gas Smelling among Fuel Stations Employees in Qassim Region

Benzene is a carcinogenic compound used in industrial manufacturing. Benzene exposure is associated with a multiplicity of clinical conditions ranging from hematologic diseases to chronic lung disorders. Respiratory system is affected more than the other system because the benzene is highly volatile and inhaled deeply into respiratory airway reaching the alveoli where gas exchange takes place. This study aims to show the effect of benzene on respiratory system of fuel station employees and its role in causing respiratory problems. The design of the study is an experimental study to evaluate the effect of benzene inhalation on the lung tissue by detecting cytological change among Fuel stations employees in Qassim region. The sputum sample was obtained from male stations employees in Qassim region and stained with Papanicolaou stain. The fuel station employees were suffering from dyspnea, nasal irritation and sore throat. In total 15 sputum samples collected are subdivided into three groups: group number one is those not exposed to benzene;the second group is exposed for 2 - 4 years. However, group number three is exposed for more than 4 years. Long-term exposure to benzene leads to cytological change and reduces immunity that is indicated by the present of neutrophil cells (pus) and other inflammatory features.

Micro RNAs and clinical implications in hepatocellular carcinoma

AIM To assess the role of some circulating miRNAs(miR-23a, miR-203, miR338, miR-34, and miR-16) as tumor markers for diagnosis of hepatocellular carcinoma(HCC).METHODS One hundred and seventy-one subjects were enrolled, 57 patients with HCC, 57 patients with liver cirrhosis(LC) and 57 healthy subjects as control group. Severity of liver disease was assessed by Child Pugh score. Tumor staging was done using Okuda staging system. Quantification of Micro RNA(miR-23a, miR-203, miR338, miR-34, and miR-16) was performed.RESULTS All studied miRNA showed significant difference between HCC and cirrhotic patients in comparison tohealthy control. miR-23a showed statistically significant difference between HCC and cirrhotic patients being higher in HCC group than cirrhotic. miR-23a is significantly higher in HCC patients with focal lesion size equal or more than 5cm, patients with multiple focal lesions and Okuda stage Ⅲ. At cutoff value ≥ 210, miR-23a showed accuracy 79.3% to diagnose HCC patients with sensitivity 89.47% and specificity about 64.91%. At cut off level ≥ 200 ng/mL, serum alpha fetoprotein had 73.68% sensitivity, 52.63% specificity, 43.75% PPV, 80% NPV for diagnosis of HCC.CONCLUSION MicroR NA 23a can be used as a screening test for early detection of HCC. Also, it is related to larger size of tumour, late Okuda staging and multiple hepatic focal lesions, so it might be a prognostic biomarker.

Iron versus Iron and Vitamin B6 Supplementation in Treatment of Iron Deficiency Anemia during Second Trimester of Pregnancy: Quasi Experimental Trial

Background: Anemia is one of the most prevalent complications during pregnancy. It is commonly considered a risk factor for poor pregnancy outcomes and can result in complications that threaten the life of both mother and fetus, such as preterm birth, and low birth weight. There is clear evidence to support prompt treatment in all patients with iron deficiency anemia because it is known that treatment improves quality of life and physical condition as well as alleviates fatigue and cognitive deficits. Objective: The aim of the study was to evaluate the value of addition of vitamin B6 to iron in treatment of iron deficiency anemia in pregnant women during the second trimester. Patients and Methods: The study was done by giving anemia pregnant women iron therapy and vitamin B6 which represent group A and iron therapy alone which represents group B. For each pregnant woman, age, parity and gestational history were taken before treatment. All pregnant women took their allocated treatment regularly for three weeks after diagnosis of iron deficiency anemia with complete blood picture and followed up after three weeks. Results: Results of the study revealed that there was no statistically significant difference between the two groups of therapy according to the hemoglobin level before treatment (p-value = 0.734), statistically significant higher mean value in after treatment than before treatment (p-value = 0.048), there was a significant difference in the rate of change of hemoglobin (p-value = 0.011) and body mass index (p-value 0.001). Conclusion: Iron and vitamin B6 seems to increase hemoglobin level more than iron only. Thus, in pregnant women with iron deficiency anemia iron plus vitamin B6 may be considered as a more effective alternative treatment than iron only.

The Potential Sources of Transmitting of Hospital Acquired Infection by Routine Devices in Adult ICU in Alrass General Hospital

Hospital-acquired infections (HAI) reflect as a major global safety concern for both patients and health-care professionals. These infections could be in the form of cross-infection, endogenous infection and environmental Infection. Over 80% of these infections are related to devices’ utilization needed for patients’ life support. Methods show this is an observational and cross-sectional study, to identify the microorganism and determine the potential source of transmitting of hospital acquired infection by routine devices in adult ICU. The samples were collected using Amies transport media;three swabs were taken from the surfaces of indwelling urinary catheter, mechanical ventilation device and central venous catheter used from every twelve patients. The samples were cultured and analyzed by using microbiologic technique. Finally, all samples analyzed by MicroScan WalkAway 96 pulse. Results showing the most bacteria isolated are “Klebsiella pneumonia” (18.37%), “Acinetobacter baumannii” (11.48%), “Staphylococcus epidermidis” (4.59%), “Staphylococcus haemolyticus” (4.59%), “E. coli” (4.59%), “Serratia marcescens” (2.3%), “Pseudomonas luteola” (2.3%), “Kocurio kristinae” (2.3%) and “Photorhabdus luminscens” (2.3%). This study detects a high contamination of routine devices and resistant organisms. In the end it is recommended that effective infection control practices and effective strategies to control antibiotic-resistant bacteria should be applied.

BACKGROUND Hepatobiliary manifestations occur in ulcerative colitis(UC)patients.The effect of laparoscopic restorative proctocolectomy(LRP)with ileal pouch anal anastomosis(IPAA)on hepatobiliary manifestations is debated.AIM To evaluate hepatobiliary changes after two-stages elective laparoscopic restorative proctocolectomy for patients with UC.METHODS Between June 2013 and June 2018,167 patients with hepatobiliary symptoms underwent two-stage elective LRP for UC in a prospective observational study.Patients with UC and having at least one hepatobiliary manifestation who underwent LRP with IPAA were included in the study.The patients were followed up for four years to assess the outcomes of hepatobiliary manifestations.RESULTS The patients'mean age was 36±8 years,and males predominated(67.1%).The most common hepatobiliary diagnostic method was liver biopsy(85.6%),followed by Magnetic resonance cholangiopancreatography(63.5%),Antineutrophil cytoplasmic antibodies(62.5%),abdominal ultrasonography(35.9%),and Endoscopic retrograde cholangiopancreatography(6%).The most common hepatobiliary symptom was Primary sclerosing cholangitis(PSC)(62.3%),followed by fatty liver(16.8%)and gallbladder stone(10.2%).66.4%of patients showed a stable course after surgery.Progressive or regressive courses occurred in 16.8%of each.Mortality was 6%,and recurrence or progression of symptoms required surgery for 15%.Most PSC patients(87.5%)had a stable course,and only 12.5%became worse.Two-thirds(64.3%)of fatty liver patients showed a regressive course,while one-third(35.7%)showed a stable course.Survival rates were 98.8%,97%,95.8%,and 94%at 12 mo,24 mo,36 mo,and at the end of the follow-up.CONCLUSION In patients with UC who had LRP,there is a positive impact on hepatobiliary disease.It caused an improvement in PSC and fatty liver disease.The most prevalent unchanged course was PSC,while the most common improvement was fatty liver disease.

Eurycoma Longifolia as a potential adoptogen of male sexual health: a systematic review on clinical studies

Eurycoma longifolia(EL) has been well recognized as a booster of male sexual health. Over the past few decades, numerous in vivo animal studies and human clinical trials have been conducted across the globe to explore the promising role of EL in managing various male sexual disorders, which include erectile dysfunction, male infertility, low libido, and downregulated testosterone levels. The aim of the present review is to analyze and summarize the literature on human clinical trials which revealed the clinical significance and therapeutic feasibility of EL in improving male sexual health. This systematic review is focused on the following databases: Medline, Wiley Online Library, Bio Med Central, Hindawi, Web of Knowledge, Pub Med Central and Google Scholar, using search terms such as "Eurycoma longifolia", "EL", "Tongkat ali", "male sexual health", "sexual infertility", "erectile dysfunction", "male libido", and "testosterone levels". Notably, only human clinical studies published between 2000 and 2014 were selected and thoroughly reviewed for relevant citations. Out of 150 articles, 11 met the inclusion criteria. The majority of articles included were randomized placebo-controlled trials, multiple cohort studies, or pilot trials. All these studies demonstrated considerable effects of EL on male sexual health disorders. Among them, 7 studies revealed remarkable association between the use of EL and the efficacy in the treatment of male sexual disorders, and remaining 4 studies failed to demonstrate sufficient effects on male sexual health. In summary, there is convincing evidence for the prominence of EL in improving the male sexual health. The review also substantiates the use of current methodology in the development of novel and more rationale natural herbal medicines for the management of male sexual disorders.

Effect of Using Mouthwash Solution on Commensal Flora of the Oral Cavity among Female Students in Qassim University

Oral health is the pathway to one’s general health and wellness. Mouthwash is a product used as an adjunct to oral hygiene practices as an antiseptic and antiplaque and antimicrobial agent. The awareness of using mouthwash solution is so limited worldwide. And maybe there’s effect of mouthwash on commensal flora. The aim of this study is to measure the effect of using Mouthwash solution on commensal flora of the oral cavity. In this study, we determined three groups, the first group is considered as a basic group where Female students are not using any mouthwash, the second group used mouthwash twice daily for two weeks, group number three used mouthwash twice weekly. We take cytological sample scraping from oral cavity. All the cytological sections of all groups stained with Papanicolaou stain and examined the slides under the microscope. The study came out with the following results: The basic group that did not use any mouthwash remained unchanged, while the second group, who used mouthwash twice daily, showed a clear epithelial tissue for any bacteria and a clear background. Group number three, who used mouthwash twice a week, showed countable bacteria in epithelial tissue with a clear background. Finally, the study concluded: Cosmetic mouthwash solution has a bad effect on commensal flora if you over used it and leads to killing all your normal flora in oral cavity as much as therapeutic mouthwash will do.

Transient Stability Analysis during an Improved Coupling Procedure for an Induction Generator Based Wind Generation System to the Grid

For fixed speed wind turbines, the connection of its squirrel cage induction generator (SCIG) to the grid leads to inrush current which can reach an average of 2 p.u. up to 2.5 p.u. in higher wind speed even by using a soft starter. We propose in this paper a new soft starting of squirrel cage induction generator based wind turbine connected to the grid. Our strategy overcomes such transient instability problems and pinpoints rapidly synchronous speed regardless the wind speed acting on pitch angle. The proposed strategy ensures at least 50% reduction of inrush current and 18% gain of WTG starting time. A state model of the system is given including the wind turbine model and the SCIG model in the synchronous reference frame. Simulation results are analysed and compared to the classic coupling procedure.

Knowledge Attitude and Practice about Water-Pipe Smoking among Saudi Population

Water pipe smoking has been practiced extensively for about 400 years. Water pipe smoking is common in the Saudi Arabia. Water-pipe smoking exposes smokers to high levels of tobacco and toxins. An observational and cross-sectional survey carried among 1170 participants to determine the knowledge, attitude, and practice about water-pipe smoking among Saudi population. Data was collected and obtained by the online questionnaire that has been created in Google form and then published in different social media platforms over a period from September 2019 to November 2019. The data were entered in Excel and analyzed in EpiInfo7 statistical software to get descriptive statistics. Results: About 70% were females and 30% male, 43% of them are university students with 83% in the age group of the 18 - 25 high participants. Low knowledge of Carbon monoxide poisoning effects = 78%, While 54% of the participants showed low knowledge about infectious disease transmitted by water-pipe. The Knowledge about water pipe contents and carbon monoxide poisoning was low. Further efforts will be necessary to increase the level of awareness among participants in Saudi Arabia regarding water pipe contents and carbon monoxide poisoning.
