Over-ground walking in Parkinson’s disease: A pilot study utilizing a portable metabolic analyzer

Alterations in gait biomechanics are common during early stages of Parkinson’s disease (PD), potentially elevating energy requirements of walking and leading to impaired economy of gait. Although gait economy is traditionally assessed during treadmill walking with simultaneous ox-ygen consumption (VO2) monitoring, treadmill gait mechanics, particularly in PD, may be different from over-ground walking mechanics, possibly providing a distorted picture of true gait economy. Currently, no studies have directly examined the energy cost of over-ground walking in PD patients. The purpose of this study was to test the feasibility of measuring energy expenditure during over-ground walking in mild to moderate PD using portable gas exchange monitoring technology. Additionally, we sought to determine whether energy expenditure, as assessed through VO2 measures, related to disease severity for PD. Seventeen PD patients underwent separate 6-minute walk (6mW) tests both with and without the COSmED K4b2 portable oxygen monitoring system. Gait economy was calculated as measured VO2 during 6mW divided by the predicted VO2 for non-PD age-matched subjects, according to a standard estimation equation utilizing ground speed. Distance covered during the 6mW with the portable system (420 ± 12 meters) was highly correlated (r = 0.96, p 2 peak for normal floor walking, and show impaired gait economy relative to prediction equations. Interestingly, the degree of elevated energy expenditure during gait did not relate to disease severity.

The use of pelletized poultry litter (PPL) as a substitute for inorganic fertilizers is increasingly being en-couraged in states like Delaware which have a considerable surplus of poultry litter. However, we know very little about the impacts of PPL on runoff water quality and whether it is an environmentally-sound and sus-tainable alternative to inorganic fertilizer. To address these questions we compared the exports of nutrients (NH4-N, NO3-N and PO4-P) and trace elements (As, Cu, and Zn) in surface runoff from agricultural plots receiving PPL, raw poultry litter (RPL), urea and no-fertilizer (control) treatments. The study was conducted on agricultural land located in middletown, Delaware with corn as the cover crop. The experimental plots were 5 m wide and 12 m long with reduced tillage and no-tillage management practices. Sampling was con-ducted for six natural rainfall events from April through August 2008. Nutrient (NH4-N, NO3-N and PO4-P) exports from plots receiving PPL were less than those with urea or raw litter applications. While exports of trace elements from the PPL treatment exceeded those from urea, they were much lower than the corre-sponding exports from the RPL treatments. mass exports of nutrients and trace elements were correlated with event size (rainfall amount) but were not correlated with timing of event (days since litter application). Results from this study suggest that the use of PPL in combination with no-tillage may provide an environ-mentally safe alternative to synthetic fertilizers.

Background—Wenxin Keli is a Chinese herb extract reported to be of benefit in the treatment of cardiac arrhythmias,cardiac inflammation and heart *** and Results—We evaluated the electrophysiologic effects of Wenxin Keli in isolated canine arterially-perfused right atrial preparations with a rim of right ventricular tissue(n = ll).Transmembrane action potentials and a pseudo-electrocardiogram were simultaneously ***(ACh,1 μm) was used to induce atrial fibrillation(AF) and to test the anti-AF potential of Wenxin Keli(5 g/L).Wenxin Keli produced preferential abbreviation of action potential duration(APD90) in atria,but caused atrial-selective prolongation of effective refractory period,due to development of post-repolarization *** maximum rate of rise of the action potential upstroke(Vmax)was preferentially reduced in *** threshold of excitation increased in both atria and ventricles,but much more in *** duration of the "P wave"(index of atrial conduction time)was prolonged to a much greater extent than the duration of the "QRS complex"(index of ventricular conduction time).Wenxin Keli significantly reduced INa and shifted steady-state inactivation to more negative potentials in HEK293 cells stably expressing SCN5 *** Keli prevented induction of persistent AF in 100%atria(6/6)and,in another experimental series was found to terminate persistent ACh-mediated AF in 100%of atria(3/3).Conclusions—Wenxin Keli produces atrial-selective depression of INa-dependent parameters in canine isolated coronary perfused preparations via a unique mechanism and is effective in suppressing AF and preventing its induction,with minimal effects on ventricular electrophysiology.

依恋的研究是西方儿童社会性和个性发展领域的主导理论之一。依恋是个体对特定他人持久而强烈的情感联结,依恋类型一旦形成就会对个体的认知、情感、行为产生影响。而自主性神经系统(autonomic nervous system,ANS)是表达压力反应的主要系统,包括交感神经系统和副交感神经系统,多迷走神经理论为理解副交感神经调节如何对个体适应性和不适应性功能做出贡献提供了框架。因此,为了探究不同依恋风格个体压力情境下的情绪反应性差异及生理指标的变化,本研究首先采用问卷法(ECR)对136名被试进行调查,其中有效被试123人,男生52名,女生61名,m=19,SD=1.83。经过换算得出安全性依恋个体72人,不安全依恋个体51人(其中,依恋焦虑型个体30人,依恋回避型个体21人)。其次,创设实验室压力情境,将基线RSA(呼吸性窦性心律不齐)水平作为其迷走神经张力的指标,将压力任务下迷走张力的降低作为迷走抑制的指标,利用修订的正性负性情绪量表(PANAS)作为情绪评定的材料,通过实验的方法,考察生理调节是否调节依恋风格与情绪反应性之间的关系。结果显示:(1)安全型依恋个体的基线RSA水平、迷走抑制水平要显著高于不安全依恋个体;(2)压力情境下,不安全依恋个体的消极情绪体验显著多于安全型依恋个体;(3)压力任务后,不安全依恋个体尤其是依恋焦虑型个体在情绪恢复量上显著低于安全型依恋个体;(4)个体的迷走抑制水平在依恋风格与情绪调节能力之间起调节作用,当个体表现出较低水平的迷走抑制时,对不安全依恋个体尤其是依恋焦虑个体而言,可预测较低的情绪调节能力,而安全型依恋与个体情绪调节的迷走抑制没有相关。因此,迷走抑制是依恋风格与情绪调节的一个重要的调节变量,且安全型依恋个体比不安全依恋个体有更强的适应性。

Background: Significant resource constraints and critical care training gaps are responsible for the limited development of intensive care units (ICUs) in resource limited settings. We describe the implementation of an ICU in Haiti and report the successes and difficulties encountered throughout the process. We present a consecutive case series investigating an anesthesiologist, emergency, and critical care physician implemented endotracheal intubation and mechanical ventilation protocol in an austere environment with the assistance of telemedicine. methods: A consecutive case series of fifteen patients admitted to an ICU at St. Luc Hospital located in Portau-Prince, Haiti, between the months of February 2012 to April 2014 is reported. Causes of respiratory failure and the clinical course are presented. Patients were followed to either death or discharge. Results: Fifteen patients (eight women and seven men) were included in the study with an average age of 37.7 years. The mean duration of ventilation was three days. Of the fifteen patients intubated, five patients (33.3%) survived and were discharged from the ICU. Of the five surviving patients, two were intubated for status epilepticus, one for status asthmaticus and one for hyperosmolar coma associated with intracerebral hemorrhage. Of the patients dying on the ventilator, four patients died from pneumonia, two from renal failure, and one from tetanus. The remaining three died from strokes and cardiac arrests. Conclusions: mortality of mechanically ventilated patients in a resource-limited country is significant. Focused training in core critical care skills aimed at increasing the endotracheal intubation and ventilatory management capacity of local medical staff should be a priority in order to continue to develop ICUs in these austere environments.
