A 73-year-old female presented to the emergency department with chief complaint of dizziness after sustaining a fall one month prior to dizziness *** careful examination of eye movement patterns during positional testing was attempted at varying stages of her inpatient admission,her complex nystagmus patterns as a result of traumatic benign paroxysmal vertigo were difficult to *** particular,the nystagmus pattern from this case suggests the BPPV was variable and affecting either 1)left posterior semicircular canal(pScc)exclusively 2)left pScc and right anterior semicircular canal,3)left and right pScc *** case illustrates the importance of two critical details;positional testing should include observing nystagmus with fixation removed and an experienced clinician should be involved as early as possible.

The bivalved arthropod Branchiocaris？ yunnanensis Hou, 1987 is redescribed on the basis of new fossil material from the early cambrian chengjiang biota, South china. In total, 81 specimens have been examined. The carapace comprises two identical valves, each valve are sub-circular in outline, ranging from 24 mm to 58 mm in length, and from 15 mm to 46 mm in height. The dorsal margin is straight and bears two short cardinal spines. The valve surface is either smooth or ornamented with polygonal reticular structures, which may result from different preservation conditions. Two morphotypes have been recognized based on the presence or absence of a subdorsal swelling of the valve, which is convex dorsolaterally and extends beyond the dorsal margin when it appears. Statistic and ontogenetic analyses show both morphotypes grow isometrically and have the same growth trajectory. Therefore, the two morphotypes are interpreted as dimorphism within the same species. Our results are helpful for investigating the taxonomy and ontogeny of arthropod fossils and suggest that dimorphism might be fairly common in these early arthropods.

Winter synoptic conditions that produce snowfall with bitterly cold temperatures create both social and economic hazards in the capital city of Albany, NY. Sometimes these systems are forecasted in error to produce rain or mixed precipitation. It is beneficial for meteorologists to better understand the commonly used 5400 and 1300 GPM line to better forecast rain versus snow events. Other studies have looked into the use of the 5400 GPM (540 dm) line but none have assessed the validity of this boundary with respect to weather type characterization at Albany. This study aims to determine the reliability of the widely referenced guides for depicting the rain-snow line, and improve forecast aids for the vertical atmosphere during winter precipitation events. The mean daily 500, 850, 925 and 1000 mb heights and weather type frequency of the Spatial Synoptic classification between November and March of 1980 - 2012 are analyzed. Results indicate that the standard vertical boundaries are inaccurate indicators of a rain versus snow event in Albany. More reasonable rain-snow cut offs for the 1000 - 500 and 1000 - 850 mb thicknesses are 5222 and 1262 GPM. For the 1000 - 925 mb level, 606 GPM is a helpful aid of identifying the rain-snow boundary. Further scrutinizing by weather type indicates that the rain-snow boundary also varies depending on what air mass/weather type is present on a given day. For instance, when the most prominent weather type is observed over Albany (Dry Polar), at the 1000 - 850 mb and 1000 - 500 mb layers, a boundary of 1242 GPM and 5152 GPM is found to be most representative. Results indicate only for the rarest of winter weather types observed over Albany, Moist Tropical, are the standard cut offs useful. Determining the reliability of this precipitation indicator at a specific station, like Albany, could enable meteorologists in other regions of the country to draw parallels between weather type, precipitation, and thickness in their forecast zones.

BAcKGROUND Non-responsive celiac disease(NRcD) is defined as the persistence of symptoms in individuals with celiac disease(ceD) despite being on a gluten-free diet(GFD). There is scant literature about NRcD in the pediatric *** To determine the incidence, clinical characteristics and underlying causes of NRcD in *** Retrospective cohort study performed at Boston children’s Hospital(BcH). children ceD by positive serology and duodenal biopsies compatible with Marsh Ⅲ histology between 2008 and 2012 were identified in the BcH’s celiac Disease Program database. Medical records were longitudinally reviewed from the time of diagnosis through September 2015. NRcD was defined as persistent symptoms at 6 mo after the initiation of a GFD and causes of NRcD as well as symptom evolution were detailed. The children without symptoms at 6 mo(responders) were compared with the NRcD group. Additionally, presenting signs and symptoms at the time of diagnosis of ceD among the responders and NRcD patients were collected and compared to identify any potential predictors for NRcD at 6 mo of GFD *** Six hundred and sixteen children were included. Ninety-one(15%) met criteria for NRcD. Most were female(77%). Abdominal pain [odds ratio(OR) 1.8 95% confidence interval(cI) 1.1-2.9], constipation(OR 3.1 95%cI 1.9-4.9) and absence of abdominal distension(OR for abdominal distension 0.4 95%cI 0.1-0.98) at diagnosis were associated with NRcD. NRcD was attributed to a wide variety of diagnoses with gluten exposure(30%) and constipation(20%) being the most common causes. Other causes for NRcD included lactose intolerance(9%), gastroesophageal reflux(8%), functional abdominal pain(7%), irritable bowel syndrome(3%), depression/anxiety(3%), eosinophilic esophagitis(2%), food allergy(1%), eating disorder(1%), gastric ulcer with Helicobacter pylori(1%), lymphocytic colitis(1%), aerophagia(1%) and undetermined(13%). 64% of children with NRcD

Natural killer(NK)cells are attractive effector cells of the innate immune system against human immunodeficiency virus(HIV)and ***,NK cell therapies are limited by the fact that target cells evade NK cells,for example,in latent reservoirs(in HIV)or through upregulation of inhibitory signals(in cancer).To address this limitation,we describe a biodegradable nanoparticlebased“priming”approach to enhance the cytotoxic efficacy of peripheral blood mononuclear cell-derived NK *** present poly(lactic-co-glycolic acid)(PLGA)nanodepots(NDs)that co-encapsulate prostratin,a latency-reversing agent,and anti-cD25(acD25),a cell surface binding antibody,to enhance primary NK cell function against HIV and *** utilize a nanoemulsion synthesis scheme to encapsulate both prostratin and acD25 within the PLGA NDs(termed Pro-acD25-NDs).Physicochemical characterization studies of the NDs demonstrated that our synthesis scheme resulted in stable and monodisperse *** NDs successfully released both active prostratin and anti-cD25,and with controllable release *** Pro-acD25-NDs were administered in an in vitro model of latent HIV and acute T cell leukemia using J-Lat 10.6 cells,the NDs were observed to prime J-Lat cells resulting in significantly increased NK cell-mediated cytotoxicity compared to free prostratin plus anti-cD25,and other *** findings demonstrate the feasibility of using our Pro-acD25-NDs to prime target cells for enhancing the cytotoxicity of NK cells as antiviral or antitumor agents.
