Objective: Analyze the maternal mortality in the two departments of CHU Point “G” in Bamako, because of high maternal mortality rate in our country. Material and Methods: This was an analytical cross-sectional study on maternal deaths from February 19, 2005 to November 19, 2019 for patients admitted in both departments and who died during the pregnancy-puerperal period at CHU Point “G”. All the patients who died outside this pregnancy-puerperal period were not retained. The data were entered and analyzed using SPSS 12.0 software. The statistical test used was that of Chi2, the statistical significance threshold was fixed at 5%. Results: During our study, we recorded 389 maternal deaths out of 16,033 admissions in 15 years and 18,060 live births during the same period making a maternal mortality ratio of 2153.931 and a frequency of 2.426. At the end of our study, we noted that the frequency of maternal deaths was higher in 2014: 12.9% (50/389). The maternal death predominantly affected women aged of 20 - 24 with a frequency of 22.4% (87/389). The multiparity (166/389 making 42.7%), illiteracy (341/389 making 87.7%), the poor evacuation conditions (non-medicalized transport): 263/389 making 67.6%;the evacuation without any evacuation sheet: 259/389 making 66.6%), poor CPN (Prenatal consultation) quality (undone CPN: 191/389 making 49.1%) and the poor monitoring of delivery works (no use of partograph in 343/389 making 88.2%) were the factors favoring maternal deaths. The main causes of maternal deaths were direct in 231/389 making 59.4% with hemorrhage in first line: 21.1% (82/389), infection (61/389 cases making 15.68%), dystocia: 50 cases making 12.85% and high blood pressure and complications (38/389 making 9.76%);indirect in 158/389 cases making 40.6% (Figures 1-3). Determination of the Electrical Parameters of a Solar Cell in Steady State

Photovoltaic solar energy can be obtained by using several types of technologies, including silicon solar cells. The characterization of its solar cells makes it possible to know them better. This article presents, on the one hand, the work that has been carried out on these cells. On the other hand, a theoretical study of the cell under illumination using Lambert’s W function. On the basis of the electrical parameters provided by the manufacturer, the parameters such as the series and shunt resistances and the electrical quantities such as the photocurrent and the photovoltage, are determined and studied according to the ideality factor of the diode. From the results obtained the shunt resistance increases when the ideality factor increases, the series resistance decreases very weakly.

Surgical Management of Recent Traumatic Epiphyseal Fractures-Detachments of the Distal Femur at Gabriel TouréUniversity Hospital

Traumatic epiphyseal detachment fractures of the distal end of the femur are a rare lesion whose severity is linked to disabling sequelae such as limb length inequality or most often axial deviation. In this report, surgical treatment aims to reconstruct the anatomy of the distal femur, to avoid secondary displacements and to allow early mobilization of the knee. Surgery carried out mainly by the technique of plugging in or screwing, opened or better still closed, can be a source of complications including migration of pins, infections, knee stiffness, and growth disorders. The aim of this work was to describe their epidemiological, anatomo-clinical, therapeutic and evolutionary aspects in the Department of Orthopedics-Traumatology at the CHU Gabriel TOURE. It was a retrospective study over 30 months from July 2019 to December 2021. In fact, it concerned 42 patients with traumatic epiphyseal detachment of the distal femur occurring within 21 days or less, on a healthy knee with cartilage fertile treated surgically and followed in the department. However, the diagnosis of traumatic epiphyseal detachment of the distal femur was retained thanks to the clinical examination and supplemented by radiographic images of the knee from the front and from the side. The treatment was surgical. The functional results were evaluated according to the functional criteria of the Eastern Orthopedic Traumatology Society (SOTEST). Forty-two patients included 32 boys and 10 girls of average age of 12 years with extremes of 8 years and 16 years. The lesions were classified according to the Salter Harris classification. We noted 24% type I (n = 10), type II 71% (n = 30), type III 2% (n = 1), type IV 2% (n = 1). Union was achieved in all patients within an average of 6 weeks with extremes of 4 and 12 weeks. Infectious Complications after Cesarean Delivery: Trends of Incidences, Risk Factors, and Prognosis in a Third Health Level Center in Africa. Case of Gabriel TouréTeaching Hospital

Post cesarean infections are the main sources of fever in the postpartum. We have undertaken this study in an African health setting where conditions of working are different from those in developed countries. Objectives: The objectives of this survey were to appreciate incidences, risk factors and prognosis of post cesarean infections. Method: We conducted a randomized historical cohort study in the department of gynecology and obstetrics of Gabriel Touré teaching hospital from 2010 to 2015. Data have been analyzed using X2 or Fisher test according their application conditions, p value Results: From 2010 to 2015 we performed 15,963 deliveries within 5263 cesareans sections (32.97%). According to all the deliveries, the global frequency of infection fluctuates from 1.5% in 2010 to 2.1% in 2015. The main risks that influenced the occurrence of post cesarean infections were: the context of cesarean section (RR = 2.05;CI95% (1.35 - 3.11);p 0.01), the prolonged labor (RR = 1.38;CI95% (1.05 - 1.81);p 0.01), the length of cesarean (RR = 3.00;CI95% (1.89 - 4.90);p 0.01), and genital bleeding (RR = 1.50;CI95% (1.10 - 1.90);p 0.01). The complications reported were endometritis (43.55%), wound infection (18.11%), breast infection (32.05%), puerperal psychosis (9 cases). We recorded six cases of pelviperitonitis and three cases of sepsis. Four cases of maternal death due to septic shock have been recorded (1.43%). Conclusion: Post cesarean infections constitute a real problem of public health in developing countries. In our survey, the main factors of infections have been context of cesarean, prolonged labor and length of cesarean. Endometritis, breast infection and wound infection are the major complications after cesarean section. The respect of protocol of the management of patients should permit to prevent this deadly complication.

Background: Multi-scar uterus is a uterus with two or more scars due to surgery or trauma. Objective: The aim was to compare the maternal and fetal prognosis of emergency C-sections of bi-scar uteruses to those of uteruses with at least 3 scars at the Nianankoro Fomba Hospital in Segou. Materials and Methods: It was a descriptive and analytical cross-sectional study with prospective data collection over a 24-month period from March 20, 2018, to March 20, 2020. Results: In 2 years, we collected 103 emergency C-sections for multi-scar uterus out of 1198 C-sections with a frequency of 8.6%. The age group of 20 to 35 years was the most represented with 86.4%. The bi-scar uteruses were the most frequent with 77.7%. In 71.8% of cases, the C-section was performed during the latent phase of labor. The C-section was performed under loco-regional anesthesia in 89.3% of cases. Difficulties in hemostasis and bladder injury were the most frequent intraoperative accidents. Surgical site infection was the main postoperative complication. No maternal deaths were recorded. The perinatal prognosis was marked by 4.3% fresh stillbirths in bi-scar uterus against 3.7% in three or more scar uteruses and including 2 fresh stillbirths and 1 macerated. We did not record any early neonatal deaths after C-section. Conclusion: The frequency of emergency C-sections in patients with a multi-scar uterus remains very high in our hospital. A better awareness of the patients, their spouses and all the actors involved in the management of pregnancies and childbirth, can contribute to curb this trend.

Seed Potential of Ruminants Organic Manures in Sahelian Agro-Pastoral Systems

In degraded Sahelian agro-systems, livestock manures increase agricultural production and ensure natural regeneration through their selective seed potential. Yet, this seed potential that contributes to species dissemination is still poorly known. This study aimed to determine the seed potential of different livestock ruminant manures. To this end, cattle (Bos indicus), goat (Capra hircus) and sheep (Ovis aries) manures were collected in 45 distinct enclosures in the Sahelian zone of Burkina Faso following the three seasonal periods of the year. A total of 36 species in 13 families and 26 genera was identified in the coarse fraction of the three type of manure. The most abundant seeds in the manure are those of Fabaceae-mimosoideae, Fabaceae-caesalpinoideae, Rhamnaceae and Balanitaceae. The results showed that the contribution of goats to the total seed potential was 61% against 36% for sheep and 3% for cattle. The average number of seeds was 205 seeds/Kg of manure for goats, 125 seeds/Kg for sheep and 11 seeds/Kg for cattle. Depending on the collection period, the cold-dry season contributed 70% to the total annual seed potential against 22% of the hot-dry season and 8% for the rainy season. Following the species functional traits, goat spread more seeds of woody indehiscent pods (barochores species) containing one, 4 to 10 seeds with hard cores and integuments. Sheep spread more seeds of annual legumes having dehiscent pods (autochores) and more than 10 seeds. The most abundant seeds in the manure are those of agroforestry (Vachellia nilotica, Faidherbia albida, Piliostigma reticulatum), ruderal (Ipomoea eriocarpa) or invasive (Senna obtusifolia) species. The most frequently (RI Dactyloctenium aegyptium, Eleusine indica, Corchorus tridens, Sporobolus festivus and Ipomoea eriocarpa. Taxonomic and functional characteristics of the seed potential of ruminants manures shape the regeneration traits of agro-ecosystems through selective seed dissemination.

Introduction:?The factors of nephroblastomas’ relapse are a set of elements playing a role in the reappearance of cancer cells in the same place of the kidney or in other regions of the body after a 5-year remission.?Objectives:?To determine the frequency and the factors of nephroblastomas’ relapse in the pediatric oncology unit and pediatric surgery of the academic hospital Gabriel Touré. Materials and methods:?This was a retrospective study carried out from January 1, 2005 to December 31, 2019 in all children treated for nephroblastoma relapses in the pediatric oncology unit and in the pediatric surgery service.?Results:?In 15 years, we have managed 182 cases of nephroblastoma, of which 128 cases were declared in complete remission after 5 years and 12 cases presented a recurrence. The relapses factors were: capsular break, intraoperative tumor rupture, presence of an associated malformation, surgery without neoadjuvant chemotherapy and tumor surgical stage (p??0.05). Patients’ age at diagnosis, sex, number of neoadjuvant and adjuvant chemotherapy sessions and histological type were not decisive (p?> 0.05).?Conclusion:?Relapses are more and more frequent in cases of nephroblastoma. The correct use of the protocol would avoid these recurrences.

Effectiveness of Hermetic Storage Using PICS Bags and Plastic Jars for Post-Harvest Preservation of <i>Acacia macrostachya</i>Seeds

In Burkina Faso, the availability of Acacia macrostachya “Zamnè” seeds throughout the year is threatened by the attacks of pests. So, the effectiveness of airtight (hermetic) storage for the preservation of these seeds was evaluated using 20-litre plastic jars and 50 kg PICS bags as hermetic containers with 50 kg polypropylene bags as controls. Seeds of Acacia macrostachya (17.5 kg) were stored in each of these storage devices for six months under ambient conditions in the laboratory. The number of storage pest Bruchidius silaceus increased significantly in polypropylene bags from 235 to 715 individuals on average/500 g of seeds. But in PICS bags and plastic drums, the number of this pest did not vary significantly (191 and 239 individuals on average/500 g of seeds for plastic jars and PICS bags respectively). A Survey of Arsenic Level in Tube-Wells in Bam Province (Burkina Faso)

Groundwater is a main source of water supply for populations in Burkina Faso where there is a serious problem for drinking water access. However, water quality has not been always followed regularly. Recent studies showed that more than 50% of drillings in Yatenga province (north of Burkina Faso) have an arsenic concentration above the recommended WHO requirement. Preliminaries studies conducted in Bam (Center North of Burkina Faso) showed high arsenic level in certain localities. This work presents for the first time data on a large scale evaluation that had been performed in all the localities of Bam province in other to have a better assessment of contamination scale. A total of 707 drilling water samples have been selected randomly in 9 communes and analysed for the arsenic level using atomic absorption spectrometry with hydride generation. The results showed that 11% of drillings have an arsenic concentration above the recommended WHO requirements with Rouko commune having the highest rate of high arsenic level (22%).
