目的:在真实世界的前瞻性队列研究中观察心内科门诊“极高危ASCVD”患者在治疗起始阶段采用联合降脂策略的疗效与安全性。方法:连续入组2021年1月至6月在湘雅医院心内科高脂血症专病门诊就诊的“极高危ASCVD”患者,根据其实际降脂策略分为三组:阿托伐他汀单药组;阿托伐他汀联合依折麦布组;阿托伐他汀联合依洛尤单抗组。主要观察终点为治疗一个月后低密度脂蛋白胆固醇(LDL-C)、脂蛋白a[Lp(a)]、非-HDL-C变化,次要终点为总胆固醇(TC)、甘油三酯(TG)、LDL-C、超敏C反应蛋白(hs-CRP)变化及安全性指标。结果:无论哪种联合方式,在“极高危ASCVD”患者降脂起始阶段联合降脂治疗的疗效均显著优于阿托伐他汀单药组:LDL-C、Log[Lp(a)]、非-HDL-C、TC的降幅更大,差异均有统计学意义(均P0.05)。以“LDL-C<1.4 mmol/L或<1.8 mmol/L”作为降脂达标的标准时,两组联合降脂治疗的LDL-C达标率均高于阿托伐他汀单药组,差异均有统计学意义(均P<0.05);阿托伐他汀联合依洛尤单抗组的LDL-C达标率高于阿托伐他汀联合依折麦布组,差异均有统计学意义(均P<0.05)。单独或联合“LDL下降超过50%”的降幅为达标标准,三组均无人能达标。治疗后三组间的肝脏转氨酶水平变化差异均无统计学意义(均P<0.05);三组间心肌酶同工酶(CK-MB)均下降,但差异无统计学意义(P<0.05)。与阿托伐他汀单药组相比,两联合降脂组治疗组的血糖均下降,差异具有统计学意义(P<0.05);阿托伐他汀联合依折麦布组的血糖比阿托伐他汀联合依洛尤单抗组的降幅更大,差异有统计学意义(P<0.05)。结论:起始联合降脂治疗1个月对心内科门诊“极高危ASCVD”患者的降脂效果及LDL-C的达标率优于阿托伐他汀单药组。以LDL-C<1.4 mmol/L或<1.8 mmol/L为降脂达标目标时,阿托伐他汀联合依洛尤单抗组治疗1个月后LDL-C达标率高于联合依折麦布组;以“LDL下降超过50%”的降幅作为降脂达标标准时,1个月内很难达标。门诊“极高危ASCVD”患者采用起始联合的降脂治疗1个月无不良反应。起始联合降脂策略可用于心内科门诊“极高危ASCVD”患者中LDL-C数值需早期达标的人群,阿托伐他汀联合依洛尤单抗可用于需在1个月内LDL-C<1.4 mmol/L或<1.8 mmol/L的人群。

Multi-strain probiotics provide the most dependable approaches to improve health,immune response,and disease resistance in farmed *** the present study,we examined the effects of multi-species probiotics on the survival,growth,immune response,and disease resistance of rohu(Labeo rohita)*** hatched larvae from the day of first feeding(average weight of 0.003 g)were reared with multi-species probiotics having a combination of Bacillus subtilis(109 colony forming units(cfu)/mL)and Lactobacillus spp.(Lactobacillus plantarum,Lactobacillus buchneri-1011 cfu/mL)in water containing doses of 0(control-C),0.5 mL/L(treatment 1-T1),and 1.0 mL/L(treatment 2-T2)in triplicates for 90 *** the experiment,a challenge test was performed to assess the fish's resistance to pathogenic Aeromonas *** higher survival was recorded in larvae of treated groups(87%in T2 and 79%in T1)compared to the control(62%).Significantly higher growth performance(weight gain and specific growth rate—SGR)was shown by the probiotic-treated larval groups compared to the *** supplementation resulted in significantly higher counts of total viable colony(TVC)and lactic acid bacteria(LAB)in the *** immunological parameters(mucosal fold fattening,goblet cell abundance,expansion of lamina propria and enterocytes)of the gut were significantly better in probiotic-treated *** liver of treated fish showed irregular shape nuclei turning into regular shape and reducing spaces between the hepatic ***-treated fish had the highest post-challenge survival rate(90%)against *** *** erythrocytes of challenged fish treated with probiotics had significantly lower frequencies of various nuclear and cellular *** findings suggest that multi-species probiotic supplements could improve the survival,growth,health status,and immune response of rohu in the early stages of its development.

Background: As the half-life of intact parathyroid hormone (iPTH) is very low, it reflects parathyroid insufficiency within minutes to hours after total thyroidectomy. Therefore, iPTH level assessment in the postoperative period can be used to predict the development of hypocalcaemia. The optimal time point to measure serum iPTH is important for the accurate prediction of hypocalcaemia. Aim: This paper aims to evaluate the ability of iPTH as an early predictive marker of hypocalcaemia and determine which time iPTH is more able to predict postoperative hypocalcaemia. Method: This prospective observational study was conducted in the Department of Otolaryngology-Head & Neck Surgery, BSMMU, Dhaka, from July 2020 to December 2021, with 67 patients who underwent total thyroidectomy. iPTH levels were measured on the day before the operation and at 1 hour, 4 hours, and 24 hours after the operation. *** levels were measured on the day before the operation and 1st postoperative day. All the data were compiled and sorted properly and were analyzed statistically. Results: Postoperative hypocalcaemia developed in 18 cases, with an incidence of 26.9%. Pearson correlation showed a significant correlation between postoperative iPTH at 1 hr, 4 h, and 24 hr with 1st postoperative calcium value. The Receiver operating characteristic (ROC) curve was processed for the postoperative iPTH at 1 hr, 4 h, and 24 hr. The sensitivity, specificity, cut-off value, and mean AUC found 93.9%, 94.4%, ≤14.0, 0.988;95.9%, 94.4%, ≤09.5, 0.993 and 91.8%, 94.4%, ≤11.0, 0.993 respectively. Conclusion: iPTH can be used as an early predictor of post-thy-roidectomy hypocalcaemia. 4 hr iPTH showed more sensitivity and specificity for a cut-off value near the laboratory reference range.

The capacity of a massive MIMO cellular network depends on user and antenna selection algorithms,and also on the acquisition of perfect Channel State Information(CSD).Low computational cost algorithms for user and an-tenna selection significantly may enhance the system capacity,as it would consume a smaller bandwidth out of the total bandwidth for downlink *** objective of this paper is to maximize the system sum-rate capacity with efficient user and antenna selection algorithms and linear *** consider in this paper,a slowly fading Rayleigh channel with perfect acquisition of CSI to explore the system sum-rate capacity of *** MIMO *** user selection,we apply three algorithms,namely Semi orthogonal user selection(SUS),Descending Order of SNR-based User Scheduling(DOSUS),and Random User Selection(RUS)*** all the user selection algorithms,the selection of Base Station(BS)antenna is based on the maximum Signal-to-Noise Ratio(SNR)to the selected *** users are characterized by having both Small Scale Fading(SSF)due to slowly fading Rayleigh channel and *** Fading(ISF)due to distances from the base ***,we use linear precoding techniques,such as Zero Forcing(ZF),Minimum Mean Square Error(MMSE),.and Maximum Ratio Transmission(MRT)to reduce interferences,thereby improving average system sum-rate *** using SUS,DOSUS,and RUS user selection algorithms with ZF,MMSE,and MRT precoding techniques are *** also analyzed and compared the computational complexity of all the three user selection *** computational complexities of the three algorithms that we achieved in this paper are 0(1)for RUS and DOSUS,and 0(M^2N)for suS which are less than the other conventional user selection methods.

Response of Rice Cultivars to Elevated Air Temperature and Soil Amendments: Implications towards Climate Change Adaptations and Mitigating Global Warming Potentials

Global mean surface air temperature is expected to increase 1.1˚C - 6.4˚C by the end of 21st century which may affect rice productivity and methane emissions in the future climate. This experiment was conducted to investigate the response of rice cultivars to elevated air temperature (+1.5˚C higher than ambient) and soil amendments in regards to rice yield, yield scaled methane emissions and global warming potentials. The experimental findings revealed that replacement of inorganic fertilizers (20% - 40% of recommended NPKS) with Vermicompost, Azolla biofertilizer, enriched sugarcane pressmud, rice husk biochar and silicate fertilization increased rice yield 13.0% - 23.0%, and 11.0% - 19.0% during wet aman and dry boro season, respectively. However, seasonal cumulative CH4 fluxes were decreased by 9.0% - 25.0% and 5.0% - 19.0% during rainfed wet aman and irrigated dry boro rice cultivation, respectively with selected soil amendments. The maximum reduction in seasonal cumulative CH4 flux (19.0% - 25.0%) was recorded with silicate fertilization and azolla biofertilizer amendments (9.0% - 13.0%), whereas maximum grain yield increment 10.0 % - 14.0% was found with Vermicompost and Sugarcane pressmud amendments compared to chemical fertilization (100% NPKS) treated soils at ambient air temperature. However, rice grain yield decreased drastically 43.0% - 50.0% at elevated air temperature (3˚C higher than ambient air temperature), eventhough accelerated the total cumulative CH4 flux as well as GWPs in all treatments. Maximum seasonal mean GWPs were calculated at 391.0 kg CO2 eq·ha−1 in rice husk biochar followed by sugarcane pressmud (mean GWP 387.0 kg CO2 eq·ha−1), while least GWPs were calculated at 285 - 305 kg CO2 eq·ha−1 with silicate fertilizer and Azolla biofertilizer amendments. Frequency of Uveitis among Juvenile Idiopathic Arthritis Patients in a Tertiary Care Hospital of Bangladesh: A Retrospective Study

Background: Juvenile idiopathic arthritis (JIA) is the most frequently encountered pediatric rheumatologic disorder with an unknown etiology. At present there is no published data regarding the frequency of uveitis in patients with JIA in Bangladesh. This study aimed to observe the frequency of JIA-associated uveitis (JIAU) and distribution of uveitis among different sub-categories of JIA at the Pediatric Rheumatology division, both outdoor and indoor patients, Department of Pediatrics, Bangabandhu sheikh Mujib Medical University (BSMMU) in Bangladesh. Methods: This was a retrospective study of 1784 JIA patients at the Pediatric Rheumatology division, BSMMU from July 2010 to March 2023. Results: Among the 1784 enrolled JIA patients, we observed that 0.73% of cases had uveitis. Here, 61.5% of JIAU cases were male. Most of the cases (92.3%) had bilateral uveitis and only 7.7% cases had unilateral uveitis. Among JIAU patients, the majority were Oligo JIA (53.8%), followed by ERA 30.8%, Poly JIA RF(−) 7.7% and Systemic JIA 7.7% cases respectively. This study also revealed that 15.4% of JIAU patients had ANA positivity and 23% had HLA B-27 positivity. Here we also found ocular complications associated with uveitis such as band keratopathy (23.1%), posterior synechiae (15.4%) and cataract (15.4%). Conclusions: In this study, we observed only 0.73% of patients of JIA had developed uveitis which is lower than the frequency observed in other European studies. This study also showed various ocular complications amongst JIA-associated uveitis patients which signifies the importance of adherence to periodic ophthalmological follow-up to prevent these ocular complications.

We reviewed the literature for different diagnostic approaches for dry eye disease(DED) including the most recent advances, contradictions and promising diagnostic tools and technique. We performed a broad literature search for articles discussing different methods for diagnosis of DED including assessment of tear osmolarity, tear film stability, ocular biomarkers and others. Articles indexed in PubMed and google scholar were included. With the growing cosmetic industry, environmental pollution, and booming of digital screens, DED is becoming more prevalent. Its multifactorial etiology renders the diagnosis challenging and invites the emergence of new diagnostic tools and tests. Diagnostic tools can be classified, based on the parameter they measure, into tear film osmolarity, functional visual acuity, tear volume, tear turnover, tear film stability, tear film composition, ocular biomarkers and others. Although numerous methods exist, the most accurate diagnosis can be reached through combining the results of more than one test. Many reported tests have shown potential as diagnostic/screening tools, however, require more research to prove their diagnostic power, alone or in combination. Future research should focus on identifying and measuring parameters that are the most specific to DED diagnosis.
