Background:Pulmonary sarcomatoid carcinoma(PSC)is a rare and aggressive subtype of non-small cell lung cancer(NSCLC),characterized by the presence of epithelial and sarcoma-like components.The molecular and immune landscape of PSC has not been well defined.Methods:Multiomics profiling of 21 pairs of PSCs with matched normal lung tissues was performed through targeted high-depth DNA panel,whole-exome,and RNA sequencing.We describe molecular and immune features that define subgroups of PSC with disparate genomic and immunogenic features as well as distinct clinical outcomes.Results:In total,27 canonical cancer gene mutations were identified,with TP53 the most frequently mutated gene,followed by KRAS.Interestingly,most TP53 and KRAS mutations were earlier genomic events mapped to the trunks of the tumors,suggesting branching evolution in most PSC tumors.We identified two distinct molecular subtypes of PSC,driven primarily by immune infiltration and signaling.The Immune High(IM-H)subtype was associated with superior survival,highlighting the impact of immune infiltration on the biological and clinical features of localized PSCs.Conclusions:We provided detailed insight into the mutational landscape of PSC and identified two molecular subtypes associated with prognosis.IM-H tumors were associated with favorable recurrence-free survival and overall survival,highlighting the importance of tumor immune infiltration in the biological and clinical features of PSCs.

Bacteriological Profile of Infections Encountered in a Pneumology Service in a Moderate-Income Country [Pneumology Department of CHU Cocody (Côte d’Ivoire)]

Introduction: Infections represent a real public health problem aggravating the morbidity and mortality of hospitalized patients. Methods: This was a retrospective study with descriptive purposes over a period of 05 years, in the Pneumology Department of the University Hospital of Cocody. Results: The average age in our population was 42 years. We observed a male predo- minance of 64.5%, with a sex ratio of 1.8. Medical history was dominated by HIV infection (23.5%), followed by tuberculosis (15.6%). Concerning lifestyle, smoking was found in 38% of cases. Symptoms progressed chronically in 80% of cases. An infectious syndrome was found in 75% of cases. Microbial culture was positive in 42% of cases. Pseudomonas aeruginosa and Klebsiella pneumoniae were encountered in 26.4% of cases each, followed by Echerichia coli in 10.4% of cases. Klebsiella pneumoniae in the first three years topped the list, but gradually. Pseudomonas aeruginosa maintained its leadership over the last three years. Pseudomonas aeruginosa isolates expressed a resistance rate of 9.8% to ceftazidime and 8.1% to imipenem;to aztreonam (36%), ticarcillin (33.3%) and levofloxacin. These strains were susceptible to fosfomycin (100%), mero- penem (96.6%) and amikacin (96%). For isolated strains of Entero- bacteria- ceae, resistance was observed about ticarcillin (83.3%) and amoxicillin clavula- nic acid (71.2%). Streptococcaceae showed resistance to tetracycline (69.2%) and erythromycin (50%). Over the years there has been an increase in re- sistance to amoxicillin-clavulanic acid ceftriaxone. The death rate was 14%. Conclusion: The bacterial profile of infections is dominated by germs respon- sible for nosocomial infection with significant mortality.

glutamate is the main exc i tatory neurotransmitter in the brain and binds to two major classes of receptors,theα-amino-3-hydroxy-5-methyl-4-isoxazole propionic acid(AMPA)and the N-methyl-D-aspartate(NMDA)receptors.Unlike AMPA receptors that are immediately activated by glutamate release,NMDA receptors are blocked by magnesium and can only be activated by glutamate after membrane depolarization.Thus,NMDA receptors are only activated after repeated AMPA receptor activation by glutamate.NMDA receptors are,for the most part,calcium-permeable channels.Calcium influx through NMDA receptors modulates synaptic transmission in neurons based on prior history of excitation,and provides a means of scaling the strength of synapses required for Hebbian plasticity.

This is a multicenter benchmarking study on liver transplantation(LT)for perihilar cholangiocarcinoma(PHC)after neoadjuvant chemoradiotherapy(1).The study comprises of 134 patients from 2014 to 2018 among whom the majority had underlying primary sclerosing cholangitis(PSC).Dropout rate was available from 4 out of 17 centers and was 28%,primarily for oncological reasons(tumor progression and lymph node invasion).All patients followed neo-adjuvant chemoradiotherapy according to the Mayo(88%)or Mayo-like protocol.Liver grafts originated from brain-dead donors in 61.3%of cases and from living donors in 29.7%of cases.

Hybrid High‑Order Methods for the Acoustic Wave Equation in the Time Domain

We devise hybrid high-order(HHO)methods for the acoustic wave equation in the time domain.We frst consider the second-order formulation in time.Using the Newmark scheme for the temporal discretization,we show that the resulting HHO-Newmark scheme is energy-conservative,and this scheme is also amenable to static condensation at each time step.We then consider the formulation of the acoustic wave equation as a frst-order system together with singly-diagonally implicit and explicit Runge-Kutta(SDIRK and ERK)schemes.HHO-SDIRK schemes are amenable to static condensation at each time step.For HHO-ERK schemes,the use of the mixed-order formulation,where the polynomial degree of the cell unknowns is one order higher than that of the face unknowns,is key to beneft from the explicit structure of the scheme.Numerical results on test cases with analytical solutions show that the methods can deliver optimal convergence rates for smooth solutions of order O(hk+1)in the H1-norm and of order O(h^(k+2))in the L^(2)-norm.Moreover,test cases on wave propagation in heterogeneous media indicate the benefts of using high-order methods.

The neuropsychiatric disease named obsessive-compulsive disorder is composed by obsessions and/or compulsions.Obsessive-compulsive disorder etiologies are undefined.However,numerous mechanisms in several localizations are implicated.Some studies showed that both glutamate,inflammatory factors and oxidative stress could have main functions in obsessive-compulsive disorder.glycogen synthase kinase-3β,the major negative controller of the WNT/β-catenin pathway is upregulated in obsessive-compulsive disorder.In obsessive-compulsive disorder,some studies presented the actions of the different circadian clock genes.WNT/β-catenin pathway and circadian clock genes appear to be intricate.Thus,this review focuses on the interaction between circadian clock genes and the WNT/β-catenin pathway in obsessive-compulsive disorder.

Experimental study on the wear evolution of different PVD coated tools under milling operations of LDX2101 duplex stainless steel

Duplex stainless steels are being used on applications that require,especially,high corrosion resistance and overall good mechanical properties,such as the naval and oil-gas exploration industry.The components employed in these industries are usually obtained by machining,however,these alloys have low machinability when compared to conventional stainless steels.In this work,a study of the wear developed when milling duplex stainless-steel,LDX 2101,is going to be presented and evaluated,employing four types of milling tools with different geometries and coatings,while studying the influence of feed rate and cutting length in the wear of these tools.Tools used have been provided with two and four flutes,as well as three different coatings,namely:TiAlN,TiAlSiN and AlCrN.The cutting behavior of these tools was analyzed;data relative to the cutting forces developed during the process were obtained;and roughness measurements of the machined surfaces were executed.The tools were then submitted to scanning electron microscope(SEM)analysis,enabling the identification of the wear mechanisms that tools were subjected to when machining this material,furthermore,the early stages of these mechanisms were also identified.All this work was done with the goal of relating the machining parameters and cutting force values obtained,identifying,and discussing the wear patterns that were observed in the coating and tools after the milling tests,providing further information on the machining of these alloys.

Comparative Study of Transvesical and Retropubic Prostatic Adenomectomies in the Urology-Andrology Department of Ignace Deen University Hospital Center

Objective: To compare the results of transvesical and retropubic prostatic adenomectomies at the Urology-Andrology Department of Conakry University Hospital. Methodology: This was a prospective multi-operator analytical study lasting 6 months. It included 70 patients operated on for benign prostatic hypertrophy (BPH), divided into 2 groups of 35 according to the retropubic or transvesical technique. The data was collected using a questionnaire and entered into the Epi info application. The analysis was initially descriptive and then univariate. To identify confounding factors, a multivariate analysis was performed. The application conditions of each test were checked before their application. A p-value Results: Upper adenomectomies represented 53.6% of surgeries performed for BPH. The average age was 73.2 years. The mean prostate volume was 117 ± 51.6 cc and the mean total PSA was 9.2 ± 11.4 ng/ml. The majority of patients had an ASA1 (60%) and ASA2 (37.1%) score. Spinal anesthesia, a double-current silicone probe with hinged hinge 22, and drainage of the prevesical space were carried out in all patients, i.e. 100%. The average overall blood loss was 229 ± 98.2 ml. It was lower in the group of patients operated via the retropubic route (187 ± 60.4 ml). The same was true for the duration of intervention (49.1 ± 7.17 vs 55.4 ± 7.9 min), the catheter wearing time (5.14 ± 0.97 vs 9.29 ± 2 .9 days) and the length of hospitalization (=6.26 ± 1.15 vs 12.1 ± 5.74 days). Conclusion: The retropubic technique has advantages over the transvesical one. These advantages are translated by a gain in the duration of the intervention, blood loss, catheter wearing time and length of hospitalization.
