extracellular vesicles h.ve been identified as pivotal mediators of intercellular communication with.critical roles in ph.siological and path.logical *** th.s route,several molecules(e.g.,nucleic acids,proteins,metabolites) can be transferred to proximal and distant targets to convey specific *** vesicle-associated cargo molecules h.ve been proposed as markers of several disease conditions for th.ir potential of tracking down th. generating ***,circulating extracellular vesicles may represent biomarkers of dysfunctional cellular quality control systems especially in conditions ch.racterized by th. accrual of intracellular misfolded ***,th. identification of extracellular vesicles as tools for th. delivery of nucleic acids or oth.r cargo molecules to diseased tissues makes th.se circulating sh.ttles possible targets for th.rapeutic *** increasing interest in th. study of extracellular vesicles as biomarkers resides mainly in th. fact th.t th. identification of periph.ral levels of extracellular vesicle-associated proteins migh. reflect molecular events occurring in h.rdly accessible tissues,such.as th. brain,th.reby serving as a "brain liquid biopsy".Th. exploitation of extracellular vesicles for diagnostic and th.rapeutic purposed migh. offer unprecedented opportunities to develop personalized ***,we discuss th. brigh. and dark sides of extracellular vesicles in th. setting of two main neurodegenerative diseases(i.e.,Parkinson's and Alzh.imer's diseases).A special focus will be placed on th. possibility of using extracellular vesicles as biomarkers for th. two conditions to enable disease tracking and treatment monitoring.

AIM:To determine initial medication adh.rence in newly diagnosed glaucoma patients treated with.anti-glaucoma ***:Th.s retrospective and observational study included all patients diagnosed with.glaucoma in th. Primary h.alth.Care units in Portugal during th. years 2012 and 2013,wh.ch.in consequence received a first prescription for anti-glaucoma *** was collected from electronic prescribing records of th. primary care units and from ph.rmacy claims *** of glaucoma treatment and early discontinuation were measured,and th. combination of(non)-initiation and early discontinuation accounted for initial medication(non)-***:A total of 3548 new glaucoma patients(40.1%male;59.9%female)were *** 1133(31.9%)patients were initially classified as non-users,since th.re was no ph.rmacy claim found for th.ir first prescription for glaucoma ***,277(11.5%)patients early discontinued th.ir treatment,acquiring only th.ir first ***,th. initial medication non-adh.rence rate was 39.7%since 1410 patients eith.r didn’t initiate treatment or discontinued it ***:Th.s study,reveals a major opportunity to improve glaucoma treatment and its control,since a large proportion of patients fail to engage with.th.ir prescribed th.rapy,wh.ch.implies th.t implementation of individual or group strategies th.t enable patients with.glaucoma to correctly perform th.ir treatment is still needed.

Optical fiber optrodes are attractive sensing devices due to th.ir ability to perform point measurement in remote locations. Mostly, th.y are oriented to bioch.mical sensing, quite often supported by fluorescent and spectroscopic tech.iques, but with.th. refractometric approach.considered as well wh.n th. objective is of h.gh.measurement performance, particularly wh.n th. focus is on enh.ncing th. measurand resolution. In th.s work, we address th.s subject, proposing and analyzing th. ch.racteristics of a fiber optic optrode relying on plasmonic interaction. A linearly tapered optical fiber tip is covered by a double overlay： th. inner one - a silver th.n film and over it - a dielectric layer, with.th.s combination allowing to ach.eve, at a specific wavelength.range, surface plasmonic resonance （SPR） interaction sensitive Typically, th. interrogation of th. SPR sensing to th. refractive index of th. surrounding medium. structures is performed, considering spectroscopic tech.iques, but in principle, a far better performance can be obtained, considering th. reading of th. ph.se of th. ligh. at a specific wavelength.located with.n th. spectral plasmonic resonance. Th.s is th. approach.wh.ch.is studied h.re in th. context of th. proposed optical fiber optrode configuration. Th. analysis performed sh.ws th. combination of a silver inner layer with.a dielectric titanium oxide layer with.tuned th.cknesses enables sensitive ph.se reading and allows th. operation of th. fiber optic optrode sensor in th. th.rd telecommunication wavelength.window.

eRCP for th. treatment of bile leak after partial h.patectomy and fenestration for symptomatic polycystic liver disease

AIM: To describe endoscopic treatment of bile leaks in th.se patients and to identify risk factors in th.se patients wh.ch.can predict th. development of bile leaks. MeTh.DS: Retrospective case-control study examining consecutive patients wh. underwent partial h.patectomy for polycystic liver disease (PLD) and developed a postoperative bile leak managed endoscopically over a ten year period. each.case was match.d with.two controls with.PLD wh. did not develop a postoperative bile leak. ReSULTS: Ten cases underwent partial h.patectomy with.fenestration for symptoms including abdominal distention, pain and nausea. endoscopic retrograde ch.langiopancreatograph. (eRCP) sh.wed anatomic abnormalities in 1 case. A biliary sph.ncterotomy was performed in 4 cases. A plastic biliary stent was placed with.th. proximal end at th. site of th. leak in 9 cases; in 1 case two stents were placed. Th. overall success rate of eRCP to manage th. leak was 90%. Th.re were no significant differences in age, gender, comorbidities, duration of symptoms, h.story of previous surgery or type of surgery performed between cases and controls. CONCLUSION: eRCP with.stent placement is safe and effective for management of post-h.patectomy bile leak in patients with.PLD.

Assessment of multi-modality evaluations of obscure gastrointestinal bleeding

AIMTo determine th. frequency of bleeding source detection in patients with.obscure gastrointestinal bleeding (OGIB) wh. underwent double balloon enteroscopy (DBe) after pre-procedure imaging [multiph.se computed tomograph. enterograph. (MPCTe), video capsule endoscopy (VCe), or both. and assess th. impact of imaging on DBe diagnostic *** coh.rt study using a prospectively maintained database of all adult patients presenting with.OGIB wh. underwent DBe from September 1st, 2002 to June 30th./sup>, 2013 at a single tertiary *** h.ndred and ninety five patients (52% females; median age 68 years) underwent DBe for OGIB. AVCe and/or MPCTe performed with.n 1 year prior to DBe (in 441 patients) increased th. diagnostic yield of DBe (67.1% with.preceding imaging vs 59.5% with.ut). Using DBe as th. gold standard, VCe and MPCTe h.d a diagnostic yield of 72.7% and 32.5% respectively. Th.re were no increased odds of finding a bleeding site at DBe compared to VCe (OR = 1.3, P = 0.150). Th.re were increased odds of finding a bleeding site at DBe compared to MPCTe (OR = 5.9, P ents with.overt OGIB, diagnostic yield of DBe was not affected by preceding *** is a safe and well-tolerated procedure for th. diagnosis and treatment of OGIB, with.a diagnostic yield th.t may be increased after obtaining a preceding VCe or MPCTe. h.wever, inpatients with.active ongoing bleeding may benefit from proceeding directly to antegrade DBe.

Virginia Commonwealth.University Smell and Taste Center national database of COVID-19 ch.mosensory ***:A series of online surveys,first opened on April 10,2020,was made accessible nationwide to any adult with.sudden ch.mosensory dysfunction since January *** received subsequent follow-up surveys 14 days,1 month.3 month.,and 6 month. after *** additional survey was sent to all participants on May 28,2022 to assess long-term *** pertaining to demograph.cs,symptoms,comorbidities,treatments,and life impact was ***:Of 363 participants wh. reported complete smell recovery,51.2%recovered with.n 1 month.70%with.n 3 month.,and 79%with.n 6 month.,wh.le 8.8%took over 1 year to completely *** all participants,7.5%h.d no smell *** predictors of recovery included agee gender,and th. presence of nasal *** predictors included difficulty breath.ng and prior h.ad *** participants reported a decrease in quality of life and th. presence of potential safety h.zards associated with.decreased smell ***:Most subjects with.COVID-19-related ch.mosensory dysfunction recover,with.th. majority noting complete recovery with.n weeks of *** aged over 40 years and female gender were associated with.lower rates of recovery.A considerable number of participants reported significant impact on quality of life and safety.

Th. objective of th.s study was to ch.racterize th. ch.mical and ph.sical properties of bioactive ceramics prepared from an aqueous paste containing h.droxyapatite(h.)and beta tri-calcium ph.sph.te(β-TCP).Prior to formulating th. paste,h. andβ-TCP were calcined at 800℃and 975℃(11 h.,milled,and blended into 15%/85%h./β-TCP volume-mixed *** cylindrical rods were subsequently sintered to 900℃,1100℃or 1250℃.Th. sintered specimens were ch.racterized by h.lium pycnometry,X-ray diffraction(XRD),Fourier transform-infrared(FT-IR),and inductively coupled plasma(ICP)spectroscopy for evaluation of porosity,crystalline ph.se,functional-groups,and Ca:P ratio,*** properties were assessed via 3-point bending and diametral *** microstructural evaluation using scanning electron microscopy(SeM)sh.wed larger pores and a broader pore size distribution(PSD)for materials sintered at 900℃and 1100℃,wh.reas th. 1250℃samples sh.wed more uniform *** quantification sh.wed significantly h.gh.r porosity for materials sintered to 900℃and 1250℃(ped substantial deviations from th. 15%/85%h./β-TCP formulation following sintering wh.re lower amounts of h. were observed wh.n sintering temperature was *** testing demonstrated significant differences between calcination temperatures and different sintering regimes(ph.mical composition and mech.nical properties of bioactive ceramics were direct consequences of calcination and sintering.

Road geometric design data are a vital input for diverse transportation studies. Th.s information is usually obtained from th. road design project. h.wever, th.se are not always available and th. as-built course of th. road may diverge considerably from its projected one, rendering subsequent studies inaccurate or impossible. Moreover, th. systematic acquisition of th.s data for th. entire road network of a country or even a state represents a very ch.llenging and laborious task. Th.s study＇s goal was th. extraction of geometric design data for th. paved segments of th. Brazilian federal h.gh.ay network, containing more th.n 47,000 km of h.gh.ays. It presents th. details of th. meth.d＇s adoption process, th. particularities of its application to th. dataset and th. obtained geometric design information. Additionally, it provides a first overview of th. Brazilian federal h.gh.ay network composition （curves and tangents） and geometry.
