Final results of the first phase of the PROTO-SPHERA experiment: obtainment of the full current stable screw pinch and first evidences of the jet + torus combined plasma configuration

In astrophysics, the boundary conditions for plasma phenomena are provided by nature and the astronomer faces the problem of understanding them from a variety of observations [Hester J J et al 1996 Astrophys. J. 456 225], on the other hand, in laboratory plasma experiments the electromagnetic boundary conditions become a major problem in the set-up of the machine that produces the plasma, an issue that has to be investigated step by step and to be modified and adapted with great patience, in particular in the case of an innovative plasma confinement experiment. The PROTO-SPHERA machine [Alladio F et al 2006 Nucl. Fusion 46 S613] is a magnetic confinement experiment, that emulates in the laboratory the jet + torus plasma configurations often observed in astrophysics: an inner magnetized jet of plasma centered on the(approximate) axis of symmetry and surrounded by a magnetized plasma torus orthogonal to this jet. The PROTO-SPHERA plasma is simply connected, i.e., no metal current conducting rod is linked to the plasma torus, while instead it is the inner magnetized plasma jet(in the following always called the plasma centerpost) that is linked to the torus. It is mandatory that no spurious plasma current path modifies the optimal shape of the plasma centerpost. moreover, as the plasma torus is produced and sustained, in absence of any applied inductive electric field, by the inner plasma centerpost through magnetic reconnections [Taylor J B and Turner m F 1989 *** 29 219], it is required as well that spurious current paths do not surround the torus on its outboard, in order not to lower the efficiency of the magnetic reconnections that maintain the plasma torus at the expense of the plasma centerpost. Boundary conditions have been corrected,up to the point that the first sustainment in steady state has been achieved for the combined plasma.

Objectives:This study aimed to test the association between of type and number of D'Amico high-risk criteria(DHRCs)with cancer-specific mortality(CSm)in high-risk prostate cancer patients treated with radical *** and methods:In the Surveillance,Epidemiology,and End Results database(2004–2016),we identified 31,281 radical prostatectomy patients with at least 1 DHRC,namely,prostate-specific antigen(PSA)>20 ng/mL(hrPSA),biopsy Gleason Grade Group(hrGGG)score of 4 and 5,or clinical tumor stage≥T3(hrcT).multivariable Cox regression models and competing risks regression models(adjusting for other cause mortality)tested the association between DHRCs and 5-year ***:Of 31,281 patients,14,394(67%)exclusively harbored hrGGG,3189(15%)harbored hrPSA,and 1781(8.2%)harbored *** 2132 patients(6.8%)harbored a combination of the 2 DHRCs,and 138(0.6%)had all 3 ***-year CSmrates ranged from 0.9%to 3.0%when any individual DHRC was present(hrcT,hrPSA,and hrGGG,in that order),1.6%to 5.9%when 2 DHRCs were present(hrPSA-hrcT,hrcT-hrGGG,and hrPSA-hrGGG,in that order),and 8.1%when all 3 DHRCs were *** regression models and competing risks regression confirmed the independent predictor status of DHRCs for 5-year CSm that was observed in univariable analyses,with hazard ratios from 1.00 to 2.83 for 1 DHRC,2.35 to 5.88 for combinations of 2 DHRCs,and 7.13 for all 3 ***:Within individual DHRCs,hrcT and hrPSA exhibited weaker effects than hrGGG ***,a dose-response effect was identified according to the number of ***,the type and number of DHRCs allow further risk stratification within the high-risk subgroup.

Objective: To evaluate transperineal laser ablation (TPLA) with Echolaser® (Echolaser® TPLA, Elesta S.p.A., Calenzano, Italy) as a treatment for benign prostatic hyperplasia (BPH) and prostate cancer (PCa) using the Delphi consensus ***: Italian and international experts on BPH and PCa participated in a collaborative consensus project. During two rounds, they expressed their opinions on Echolaser® TPLA for the treatment of BPH and PCa answering online questionnaires on indications, methodology, and potential complications of this technology. Level of agreement or disagreement to reach consensus was set at 75%. If the consensus was not achieved, questions were modified after each round. A final round was performed during an online meeting, in which results were discussed and ***: Thirty-two out of forty invited experts participated and consensus was reached on all topics. Agreement was achieved on recommending Echolaser® TPLA as a treatment of BPH in patients with ample range of prostate volume, from mL (80%) to >80 mL (80%), comorbidities (100%), antiplatelet or anticoagulant treatment (96%), indwelling catheter (77%), and strong will of preserving ejaculatory function (100%). majority of respondents agreed that Echolaser® TPLA is a potential option for the treatment of localized PCa (78%) and recommended it for low-risk PCa (90%). During the final round, experts concluded that it can be used for intermediate-risk PCa and it should be proposed as an effective alternative to radical prostatectomy for patients with strong will of avoiding urinary incontinence and sexual dysfunction. Almost all participants agreed that the transperineal approach of this organ-sparing technique is safer than transrectal and transurethral approaches typical of other techniques (97% of agreement among experts). New totally intracorporeal reconstructive approach after robotic total gastrectomy: Technical details and short-term outcomes

To show outcomes of our series of patients that underwent a total gastrectomy with a robotic approach and highlight the technical details of a proposed solution for the reconstruction *** of gastrectomies performed from may 2014 to October 2016, were extracted and analyzed. Basic characteristics of patients, surgical and clinical outcomes were reported. The technique for reconstruction (Parisi Technique) consists on a loop of bowel shifted up antecolic to directly perform the esophago-enteric anastomosis followed by a second loop, measured up to 40 cm starting from the esojejunostomy, fixed to the biliary limb to create an enteroenteric anastomosis. The continuity between the two anastomoses is interrupted just firing a linear stapler, so obtaining the Roux-en-Y by avoiding to interrupt the ***-five patients were considered in the present analysis. Estimated blood loss was 126.55 ± 73 mL, no conversions to open surgery occurred, R0 resections were obtained in all cases. Hospital stay was 5 (3-17) d, no anastomotic leakage occurred. Overall, a fast functional recovery was shown with a median of 3 (3-6) d in starting a solid *** surgery and the adoption of a tailored reconstruction technique have increased the feasibility and safety of a minimally invasive approach for total gastrectomy. The present series of patients shows its implementation in a western center with satisfying short-term outcomes.

The simulation of multiphase flows is an outstanding challenge,due to the inherent complexity of the underlying physical phenomena and to the fact that multiphase flows are very diverse in nature,and so are the laws governing their *** the last two decades,a new class of mesoscopic methods,based on minimal lattice formulation of Boltzmann kinetic equation,has gained significant interest as an efficient alternative to continuum methods based on the discretisation of the NS equations for non ideal *** this paper,three different multiphase models based on the lattice Boltzmann method(LBm)are discussed,in order to assess the capability of the method to deal with multiphase flows on a wide spectrum of operating conditions and multiphase *** particular,the range of application of each method is highlighted and its effectiveness is qualitatively assessed through comparison with numerical and experimental literature data.

The northern Chilean Patagonia region is a key feeding ground and a nursing habitat in the southern hemisphere for blue whales(Balaenoptera musculus).From 2014 to 2019,during 6 separate research cruises,the dive behavior of 28 individual blue whales was investigated using bio-logging tags(DTAGs),generating≈190 h of *** dove to significantly greater depths during the day compared to nighttime(day:32.6±18.7 m;night:6.2±2.7 m;Pmost time was spent close to the surface(86%±9.4%;Pmultaneously with whale diving *** whales appeared to perform dives relative to the vertical migration of prey during the *** association between diurnal prey migration and shallow nighttime dive behavior suggests that blue whales are at increased risk of ship collisions during periods of darkness since the estimated maximum ship draft of vessels operating in the region is also≈12 *** recent decades,northern Chilean Patagonia has seen a large increase in marine traffic due to a boom in salmon aquaculture and the passenger ship *** strike risks for large whales are likely underestimated in this *** reported in this study may be valuable for policy and mitigation decisions regarding conservation of the endangered blue whale.

Gastric autoimmune disorders in patients with chronic hepatitis C before,during and after interferon-alpha therapy

AIm:To explore the prevalence of autoimmune gastritis in chronic hepatitis C virus (HCV) patients and the influence of α-interferon (IFN) treatment on autoimmune gastritis. mETHODS:We performed a prospective study on 189 patients with positive anti-HCV and viral RNA enrolled in a 12-month IFN *** evaluated:a) the baseline prevalence of autoimmune gastritis,b) the impact of IFN treatment on development of biochemical signs of autoimmune gastritis (at 3,6 and 12 months),c) the evolution after IFN withdrawal (12 months) in terms of anti-gastric-parietal-cell antibodies (APCA),gastrin,anti-thyroid,and anti-non-organ- specific antibodies. RESULTS:APCA positivity and 3-fold gastrin levels were detected in 3 (1.6%) and 9 (5%) patients,respectively,at baseline,in 25 (13%) and 31 (16%) patients at the end of treatment (both Pmonths after withdrawal (P=0.002 and P=0.01 respectively,vs baseline;P=not significant vs end of treatment).The development of autoimmune gastritis was strictly associated with the presence of autoimmune thyroiditis (P=0.0001),no relationship was found with other markers of autoimmunity. CONCLUSION:In HCV patients,IFN frequently precipitates latent autoimmune gastritis,particularly in *** our 12-month protocol,the phenomenon generally *** APCA positivity and high gastrin levels are associated with the presence of antithyroid antibodies, development of autoimmune thyroiditis during IFN treatment may provide a surrogate preliminary indicator of possible autoimmune gastritis to limit the need for invasive examinations.

Objective:We performed a population-based analysis focusing on primary extranodal lymphoma of either testis,kidney,bladder or prostate(PGUL).methods:We identified all cases of localized testis,renal,bladder and prostate primary lymphomas(PL)versus primary testis,kidney,bladder and prostate cancers within the Surveillance,Epidemiology,and End Results database(1998e2015).Estimated annual proportion change methodology(EAPC),multivariable logistic regression models,cumulative incidence plots and multivariable competing risks regression models were ***:The rates of testis-PL,renal-PL,bladder-PL and prostate-PL were 3.04%,0.22%,0.18%and 0.01%,*** with PGUL were older and more frequently *** rates significantly decreased for renal-PL(EAPC:5.6%;pZ0.004)and prostate-PL(EAPC:3.6%;pZ0.03).In multivariable logistic regression models,older ager independently predicted testis-PL(odds ratio[OR]:16.4;pmale gender independently predicted bladder-PL(OR:5.5;pmulative incidence plots showed significantly higher 10-year cancer-specific mortality(CSm)rates for testis-PL,renal-PL and prostate-PL versus their primary genitourinary *** multivariable competing risks regression models,only testis-PL(hazard ratio[HR]:16.7;pm ***:PGUL rates are extremely low and on the decrease in kidney and prostate but stable in testis and *** to primary genitourinary tumors,PGUL are associated with worse CSm for testis-PL and renal-PL but not for bladder-PL and prostate-PL,even after adjustment for other-cause mortality.
