Risk of Surgical Site Infection in Patients Undergoing Total Knee Arthroplasty

The aim of this study was to determine the participation of risk factors in the infection in patients who underwent total knee arthroplasty. This is a prospective longitudinal study, descriptive and analytical, that was carried out between January 2013 and December 2015. Eighty-one procedures were performed in 78 individuals, 16 males and 62 females. Three subjects underwent the procedure on both knees. The following predictive factors of infection were identified: Age, gender, comorbidities, evaluation of functional physical status using the American Society of Anesthesiologists Physical Status Classification System (ASAPS), length of hospital stay, use of prophylactic antibiotics and duration of surgery. The data was tabulated in a database using a Microsoft Excel spreadsheet. For statistical analysis, the chi-square test, chi-square test for independence and Fischer’s exact test were applied, all at a significance level of 5% (0.05). Surgical site infection was more prevalent in males (23.5%), age over 70 years (25.0%), for left gonarthrosis (17.9%), in the presence of comorbidities (17.9%), when hospital stay was 6 or more days (27.8%), in total arthroplasties of left knee (17,9%), in surgical times of up to 2 hours (20%), in the use of prophylactic antibiotics (16.7%) and in level II classification in the ASAPS (18.8%). The independence test was not significant in any of the evaluated factors (p-value greater than 0.05). The most prevalent bacteria identified in cases of infection was Staphylococcus aureus (33.2%). The risk factors studied were not statistically significant.

Healthcare access and the patterns of maternal health care utilization among poor and non-poor women living in urban areas in Portugal

Introduction: Studies on attitudes and practices are increasingly used but not specifically related to the motivations for the use of reproductive health care among women of fertile age, living in urban areas and in different social contexts. Objectives: The aim of this study was to estimate the associations between the variables of social status (degree of poverty in the studied groups) and the variables of fecundity (representations, tensions, practices and control of fertility) and, in addition, to compare access to health care in the different studied groups, assessing the association between use of maternal health care and poverty in urban areas. Design: A case-control study was conducted in the Municipality of Lisbon, Portugal, with a total sample of 1513 women of fertile age: 499 cases of women considered very poor were selected from the database of beneficiaries of RSI (Social Welfare Payment for Inclusion);1014 controls (two controls for each selected case), divided as 507 poor women selected from the other beneficiaries of Santa Casa da Misericórdia in Lisbon and 507 non-poor women selected from four Health Centers from the Municipality of Lisbon, Portugal. A total of 1054 women answered the questionnaire: 304 cases (response rate of 61%) and 750 (response rate of 74%) controls. The statistical analysis involved descriptive analysis and multinomial logistic regression. Results: The analysis confirms the association between poverty and patterns and representations of fecun dity regarding pregnancy planning. The results of this study thus show the existence of different distributions on several variables and the gradients of poverty. Regarding access to health care, the major impact of poverty on women is limiting access to pharmaceuticals. The incapacity to afford the cost of health care appears as a central aspect of access to health care. Brazil is responsible for 27% of the world production of soybeans and 7% of maize. Mato Grosso and Para states in Brazil are among the largest producer. The viability to the cultivation of maize (Zea mays) and soybeans (Glycine max), for future climate scenarios (2070-2100, GHG) is evaluated based on crop modeling (DSSAT) forced by observational data and regional climate simulations (HadRM3). The results demonstrated that a substantial reduction in the yield in particular for maize may be expected for the end of the 21st century. Distinct results are found for soybeans. By applying the A2 climate changes scenario, soybean yield rises by up top 60% assuming optimum soil treatment and no water stress. However, by analyzing the inter-annual variability of crop yields for both maize and soybean, could be demonstrated larger year-to-year fluctuations under greenhouse warming conditions as compared to current conditions, leading to very low productivity by the end of the 21st century. Therefore, these Brazilian states do not appear to be economically suitable for a future cultivation of maize and soybeans. Improved adaptation measures and soil management may however partially alleviate the negative climate change effect.

Syzygium aromaticum L.(Clove)Essential Oil as a Reducing Agent for the Green Synthesis of Silver Nanoparticles

The present investigation details a green synthesis of silver nanoparticles (AgNP) using the essential oil of Syzygium aromaticum L. (clove) as reducing agent, which is a matrix with a high content of eugenol, an important compound for the reducing action of silver nitrate. The synthesis of AgNP was performed at different pH conditions (pH 7, 8, 9 and 10), and was monitored by UV-Vis Spectroscopy, Dynamic Light Scattering (DLS) and Transmission Electron Microscopy. The synthesized nanoparticles presented characteristic Surface Plasmon Resonance bands with maximum absorbance between 405 and 460 nm. The DLS analysis revealed particle sizes from 31 to 72 nm and zeta potential between -30.1 and -50.8 mV indicating good stability against the agglomeration of the particles in solution. The micrographs obtained by TEM showed different particle shapes and a predominance of spherical-shaped nanoparticles, and average size ranging from 27 to 94 nm. The clove-based silver nanoparticles were efficient in controlling the growth of Escherichia coli and Staphylococcus aureus bacteria, and the minimum inhibitory concentration ranged from 60 to 100 μL/mL. This study highlights the feasibility of clove essential oil as an alternative for the synthesis of silver nanoparticles by a simple, inexpensive and eco-friendly method.

Improving wood volume predictions in dry tropical forest in the semi-arid Brazil

The volumetric variability of dry tropical forests in Brazil and the scarcity of studies on the subject show the need for the development of techniques that make it possible to obtain adequate and accurate wood volume *** this study,we analyzed a database of thinning trees from a forest management plan in the Contendas de SincoráNational Forest,southwestern Bahia State,*** data set included a total of 300 trees with a trunk diameter ranging from 5 to 52 ***,validation and statistical selection of four volumetric models were *** to the difference in height values for the same diameter and the low correlation between both variables,we do not suggest models which only use the diameter at breast height(DBH)variable as a predictor because they accommodate the largest estimation *** comparing the best single entry model(Hohenald-Krenn)with the Spurr model(best fit model),it is noted that the exclusion of height as a predictor causes the values of 136.44 and 0.93 for Akaike information criterion(AIC)and adjusted determination coefficient(R2 adj),which are poorer than the second best model(Schumacher-Hall).Regarding the minimum sample size,errors in estimation(root mean square error(RMSE)and bias)of the best model decrease as the sample size increases,especially when a larger number of trees with DBH≥15.0 cm are randomly *** sampling by diameter class produces smaller volume prediction errors than random sampling,especially when considering all *** summary,the Spurr and Schumacher-Hall models perform *** models suggest that the total variance explained in the estimates is not less than 95%,producing reliable forecasts of the total volume with *** estimates indicate that the bias around the average is not greater than 7%.Our results support the decision to use regression methods to build models and estimate their parameters,seeking stratification strategies in diameter classes for the sample trees.

Evaluation of Pulmonary Function in Adults with Grade Ⅲ Obesity

Pulmonary function studies in obese individuals have alerted the pathophysiological changes due to weight gain. However, these changes are not fully explained yet. Objective: To evaluate and analyze the pulmonary function of obese adults in the city of Manaus (AM, Brazil) and to correlate the parameters of spirometry with weight and BMI. Method: A descriptive cross-sectional study, in which pulmonary function of 22 adults with grade III obesity was evaluated pre and post bronchodilator administration, performed at the Adriano Jorge Hospital Foundation—FHAJ. Results: Of the 22 patients evaluated, 6 were male and 16 were female;mean age was 37.1 ± 7.4 years, with a weight of 134.2 ± 28.6 kg and a BMI of 51.1 ± 9.4 kg/m2, corresponding to grade III obesity. Spirometry showed a reduction in values obtained from Forced Vital Capacity (FVC), Forced Expiratory Volume in the first second (FEV1) and Peak Expiratory Flow (PEF) in relation to those predicted, with 23% of restrictive disorders. There were negative correlations between BMI and PEF, weight and FVC, weight and PEF. The P value for correlation of weight and FEV1 is not statistically significant. Conclusion: Restrictive and obstructive disorders are prevalent in the population with grade III obesity. Correlations between pulmonary function and BMI/weight were negative for this specific population.

The objectives of this study were to assess the safety and efficacy of intralesionally administered diclofenac in the treatment of cutaneous neurofibromas in patients with NF1. This was a proof-of-concept, prospective, safety and efficacy study of the effect of intralesionally administered diclofenac 25 mg/ml given once a week to 3 target cutaneous neurofibromas for 4 consecutive weeks. Overall, there was no significant change in neurofibroma size. During the study, some treated lesions developed signs of necrosis and fell off after a few weeks, but none of the control neurofibromas fell off. There were no significant changes in patient’s vital signs. A few adverse events occurred, mostly at the injection sites. During the study, some neurofibromas developed necrosis after the diclofenac injections and eventually detached from the patient. Overall, diclofenac was well tolerated, suggesting minimal systemic exposure, which required confirmation and further studies, including bioavailability analysis.

Antioxidant,Antibacterial and Antitumor Activity of Ethanolic Extract of the Psidium guajava Leaves

Psidium guajava L. has extensive use in folk medicine. The aim of this study was to quantify the levels of phenolic, flavonoids, antioxidant activity, leathality assay and antibacterial and antitumoral activities of the extract of P. guajava. In the dry guava extract there were high levels of phenolics (766.08 ± 14.52 mg/g), flavonoids (118.90 ± 5.47 mg/g) and antioxidant activity (87.65%). The LD50 was 185.15 μg/ml. The MIC value was 250 μg/ml for Streptococcus mutans, Streptococcus mitis and Streptococcus oralis. IC50 of the extract tested in the HeLa, RKO and Wi cell lines was 15.6 ± 0.8 μg/ml, 21.2 ± 1.1 μg/ml and 68.9 ± 1.5 μg/ml, respectively. The results of all analyses allow us to conclude that the dry extract of guava leaves has promising activity to be applied topically in the oral cavity or in the development of antitumor formulation or even be used as a functional food.
