Coronary Artery Vasospasm with Acute myocardial Infarction in a male Patient： Case Report

We present a 48-year-old male patient with a history of classic precordialgia, ST-segment elevation in inferior leads （II, III and aVF）, with t：oponin m：d CK-mB （creatine kinase-mB） elevated on the day of admission that received conventional anti-ischemic treatment. Hemodynamically stable, symptom-free, is referred to cardiac catheterization. In the selective catheterization of the left coronary artery, a moderate lesion was observed in the middle third of the anterior descending coronary artery; in the right coronary artery, a subocclusive proximal lesion with TImI （thrombolysis in myocardial infarction） II. We performed a single projection m：d opted for angioplasty. Before, it was decided to re-evaluate the left coronary artery, m：d it was observed in the contrast test that anterior descending artery was occluded. Intra-coronary nitroglycerin and new contrast injection were performed, which showed totally open descending artery with TImI III, without obstructive lesions as initially suggested, and improvement of pain. It was a severe coronary vasospasm. When a new contrast injection was performed in the right coronary, with a therapeutic catheter, the disappeaxance of the subocclusive lesion and the presence of a normal coronary flow were observed, although there was a moderate plaque in its proximal third, which motivated the maintenance of angioplasty with stent placement in a proxhnal third of right coronary artery. During the passage of the intracoronary guidewire it was once again evident that diffuse coronary artery vasoreactivity was present; the procedure was successfully completed.

AIm： To determine the prevalence of celiac disease in a group of volunteer blood donors at a blood bank in the city of Curitiba, Brazil through detection of the serum marker immunoglobulin A （IgA） antitransglutaminase antibody. mETHODS： Blood samples collected from 2086 healthy subjects at the Paraná State Center for Hematology and Hemotherapy in Curitiba were submitted to ELISA testing for the IgA antitransglutaminase antibody. Positive samples received IgA antiendomysium antibody test through indirect immunofluorescence using human umbilical cord as substrate. Subsequently, patients who were positive on both tests underwent small bowel （distal duodenum） biopsy. RESULTS： Six subjects, four males and two females, tested positive for the two serum markers. Five of the six were submitted to intestinal biopsy （one declined the procedure）. Biopsy results revealed changes in the distal duodenum mucosa （three classified as marsh mb lesions and two as marsh Ⅱ lesions）. most donors diagnosed having celiac disease presented multiple symptoms （gastrointestinal tract complaints）. One donor reported having a family history of celiac disease （in a niece）. CONCLUSION： Among apparently healthy blood donors,the prevalence of biopsy-confirmed celiac disease was approximately 1：417, similar to that seen in European countries.

Introduction: Transarterial chemoembolization (TACE) reduces tumor growth and increases survival in patients with hepatocellular carcinoma (HCC). Drug-eluting beads (DEB) deliver slow-release chemotherapy and reduce systemic toxicity during TACE. This study correlated initial tumor response according to modified RECIST (mRECIST) criteria and 1-year survival in patients with HCC treated with TACE-DEB, and identified predictors of tumor response. methods: Fifty-two patients with HCC received TACE-DEB loaded with doxorubicin 75 mg during a 6-month period. Tumor response was evaluated 1 month after the procedure according to mRECIST criteria. Results: most patients were cirrhotic and etiology of liver disease was hepatitis C in 26/52 (50%). Similar numbers of patients had Barcelona Clinic Liver Cancer (BCLC) A and BCLC B disease. most patients had one nodule (66%). Complete response (CR) was achieved in 12/52 (23%), partial response in 19/52 (37%), stable disease in 4/52 (8%) and progressive disease in 17/52 (32%). Largest HCC ≤58 mm and BCLC stage A were associated with CR. The 1-year survival was 74%, with survival rates of 95% and 56% in the BCLC A and B groups, respectively. Variables reflecting tumor extension were associated with better survival. CR according to mRECIST criteria was a predictor of better 1-year survival (100% vs. 64%, P mRECIST criteria predict improved 1-year survival in patients with HCC treated with TACE-DEB. Further studies are needed to evaluate other predictors of survival and to determine if tumor response predicts long-term survival.

moringa oleifera Lam(moringaceae) is a plant with high nutritional and medicinal value. Native to India, it is now widely distributed throughout tropical and subtropical regions of the world. Its different parts are sources of proteins, vitamins and minerals and present different pharmacological and biotechnological potential. moreover, m. oleifera seeds are widely used in water and effluent treatment, for their coagulation, flocculation and sedimentation properties, their ability of improving water quality, by reducing organic matter and microbial load, with special applicability in intensive animal production systems, such as aquaculture. In addition, due to its high nutritional value and several medicinal properties, this tree may act as a nutritional and medical alternative for socially neglected populations. In this context, this review gathers information on m. oleifera, emphasizing its chemical constituents, nutritional, pharmacological and antimicrobial properties, applications in the treatment of water effluents, and ecological and social aspects.

This study aims to investigate the toxicity and possible effects and mechanism of action of the essential oil obtained from the Piper tuberculatum Jacq.‘s fruit(EOPT)on the central nervous system(CNS)of *** the Hippocratic test,it was observed ambulation reduction,sedation,piloerection and mortality of 25%at the dose of 1000 mg/kg and 50%at the dose of 5000 mg/*** results found demonstrate that the EOPT is endowed with a possible depressant effect,and is characterized by a likely sedative(open field test),anxiolytic-like(elevated plus maze test),anticonvulsant(pentylenetetrazole-induced seizure test)and hypnotic action(pentobarbital-induced sleeping time).These effects may possibly occur due to the interaction between the monoterpenes,present in its essential oil,and the GABAergic pathway confirmed by reversion of the effects by ***,these results demonstrate the therapeutic potential and the validation of the ethnopharmacological use of this species in the treatment of disorders affecting the central nervous system.

Denileukin diftitox (Ontak ), a recombinant fusion protein of diphtheria toxin and ligand, IL-2, binds to the IL-2 receptor, is internalized, and causes cell death. Denileukin diftitox was approved for the treatment of cutaneous T-cell lymphomas (CTCLs) with CD25+ expression. We prospectively stained lesional skin biopsy specimens from 113 mycosis fungoides and S′ ezary Syndrome patients for activation markers CD25 and CD30 to correlate expression with clinical tumor-node metastasis (TNm) stage, histologic grade, and response to denileukin diftitox. High expression was defined as positivity of ≥ 20% of lesional T-cells using immunohistochemistry (IHC). CD25 and CD30 expression was more common in lesions from advanced patients (P = 0.04 and 0.002, respectively). Advanced TNm(T3 or T4) was significantly associated with intermediate grade (P = 0.002) and large-cell transformation histology (P = 0.04). Of interest, clinical responses were observed in 78.5% of patients with high CD25 expression versus 20% with low to undetectable CD25 expression (P = 0.01) among 24 patients receiving standard 5-day infusions of denileukin diftitox at 18 μ g/kg/day. These data suggest that high CD25 expression by IHC is associated with advanced CTCL and with clinical response to denileukin diftitox therapy.

Freshwater biota are more comprehensive and direct indicators of biological impacts,and more meaningful to the public than water quality or physical habitat *** biotic data and the multiple biological indicators developed from them offer a much richer array of data for assessing the impacts of pollution controls than a limited set of physical or chemical *** recent decades,assemblage-based assessments by ecologists,environmental scientists,and water quality agencies have been employed globally for determining the condition of,and threats to,freshwater ecosystems.A key step in this advance has been the development of multimetric indices(mmIs)or indices of biotic integrity(IBIs)based on quantitative assessments of algae,macrophyte,macroinvertebrate,fish or riparian bird *** Europe,where biological assemblages are mandated for assessing freshwater ecosystem health,many indices are ***,the proliferation of mmIs globally has not always occurred through the application of rigorous study designs and monitoring protocols,nor have they always effectively incorporated functional metrics,stressor assessments,and statistical ***,in this review,we discuss eleven major concerns with the development and application(including logistical limitations)of multimetric indicators based on freshwater biota to encourage more rigorous and widely applicable(transferable)mmI use and ***,our concerns focus on reference conditions;sampling effort,methods,and season;trophic guild definition;metric comprehensiveness,options,screening and scoring;and mmI *** could also benefit from increased attention to ecological mechanisms and metric development,to further improve our understanding of anthropogenic impacts as well as rehabilitation effects on freshwater ecosystems *** closer attention to study designs,ecological mechanisms and metric development should further improve our understa
