Out-of-hospital cardiac arrest(OHCA)is one of the three leading causes of death in industrialized countries.[1,2]Some studies have described the impact of the first COVID-19 pandemic wave in terms of the number of cases and OHCA survival rates in various regions,[3-5]but few have addressed the relationship between the successive phases and how they affected OHCA.[6,7]The 14-d cumulative incidence peaked at more than 990 cases,with these rates remaining above 200 for an eight-month period between 15 March 2020 and 15 March 2021 in madrid,Spain.

Long-Term Follow-Up of 82 Cavernous Sinus Meningiomas Treated with Radiosurgery

The cases of 82 patients with cavernous sinus meningioma (CSM) treated with Stereotactic Radiosurgery (SRS) at our institution from 1992 to 2005 were retrospectively reviewed. The mean follow-up time was 8.38 years. Patients’?age ranged between 16 and 78 years (mean 51). There were 35 patients who had been operated before, and two of them had been treated with fractionated radiotherapy. Twenty-three from 35 patients were surgically intervened?(65.7%)?and?presented post-surgical morbidity. Only in 3 cases the surgery was considered complete. The patients were referred for SRS treatment due to having tumour remains or a tumour growth. The mean volume of the tumour was 17.96 (+/?13.67) cm3.?All the patients had been treated with a Linear Accelerator (LINAC) using a high precision positioning and radiation system (SRS 2000) University of Florida. The clinical progress of the patients was assessed using preand post-SRS radiological imaging, post-surgical and pre-post-SRS morbidity and mortality. Tumour volume decreased significantly with RS in 61 of 82 patients (74.4%). The tumour volume remained stable 12 patients (14.6%) and only in 9 patients (11%) was there tumour growth after SRS. Of these, 5 required surgical intervention, and 7 of the 82 underwent SRS or another fractionated stereotactic radiotherapy after the RS. Morbidity due to the SRS was only seen in 14 out of the 82 patients treated with SRS, five of them recovered completely. SRS is a high precise and effective treatment with low morbidity, becoming more and more the option of choices?in the treatment of cavernous sinus meningioma.

Postanesthetic Emergence Delirium in Children: Qualitative Differences between Sevoflurane vs Desflurane: A Randomized Clinical Trial

Background: Emergence Delirium (ED) is considered a usual complication in pediatric anesthesia. Aim: Analyze the quantitative and qualitative differences in ED in children receiving general anesthesia with sevoflurane or desflurane in day case surgery. Materials and Methods: Two hundred and two children, ASA I-II, who required outpatient elective day case surgery, were assigned to receive anesthesia with sevoflurane or desflurane. ED was assessed by a masked investigator using the Pediatric Anesthesia Emergence Delirium (PAED) scale at 5 and 15 minutes (min). Results: Mean time to wake up was shorter with desflurane compared with sevoflurane (6.0 versus 8.3 min, p = 0.0001). The overall incidence of ED was 21.3% and Postoperative Maladaptive Behavior Changes (POMBC) incidence was 22%;however, these were not related. Main factors found to be associated with ED were younger age, postoperative pain and preoperative anxiety. Although there were not statistically significant differences in ED incidence between sevoflurane (26.4%, 95% CI 17.3% - 35.4%) and desflurane anesthesia (16.3%, 95% CI 8.8% - 23.8%) (p = 0.18), scores of items 1 and 2 from PAED scale (eye contact and purposeful actions, items related to the patients’ connection with their surroundings) were significantly higher in sevoflurane than in desflurane group (p = 0.034 and p = 0.021 respectively). Conclusion: Recovery after anesthetic maintenance with desflurane is faster and as safe as sevoflurane, including postoperative behavioral disorders. Although desflurane did not statistically decrease ED incidence as compared with sevoflurane, patients who were agitated with desflurane were qualitatively less disoriented and disconnected from their surroundings.

Background: The main objective of this study is to describe the rate of the different serotypes of HPV in cervical cytologies and biopsies in three different periods: 2002-2006 (prior to the implementation of the vaccination programs in Spain), 2009-2011 (shortly after this implementation) and 2020 (almost 15 years after introduction of the vaccine) at a single hospital. Methods: This is an observational, descriptive, retrospective study based on the review of the results of the determination of the HPV serotype using the commercial kit (Genomica®;PharmaMar LTD) in cervical liquid-based cytologies and biopsies at a single large tertiary hospital, Hospital Clínico San Carlos, in madrid, Spain. We have collected the data from three different time periods: 2002-2006;2009-2011, and 2020 to try to understand the potential changes associated with the use of the vaccine. Results: In these time periods we have reviewed the data from 1420 women. In the three periods the most frequent serotype was HPV 16, followed by HPV 18 or a combination of both. The most frequent low risk serotype was HPV 6 followed by the combination of HPV 6 and 11. It has been verified in our study that the prevalence of the category “others”, constituted by the three risk groups, has undergone a progressive increase, beginning with an infection rate of 65.43% in 2002-2006 to finally rise up to 90.92% in the year 2020. Conclusions: Our study reveals an increase in the number of infections by the HPV serotypes that are not included in the tetravalent vaccine.

Mechanical CPR (cardiopulmonary resuscitation) devices help performing correct chest compressions in the event of a cardiorespiratory arrest. These devices are comfortable and useful, they keep chest compression following the recommendations as they do not depend on interpersonal variability, they do not get tired, their use is simple and one of the rescuers is released from this task, thus facilitating the assistance. Besides, their use in transport conditions makes it safer. However, when coming to results, these mechanical CPR devices have not clearly demonstrated such an advantage, neither in the field of cardiac arrest, nor in organ preservation in the case of donors after cardiac death. In donors after cardiac death they are widely used by most of the emergency services involved, but a number of injuries produced in lungs during the early years of their use have made it controversial. In this paper we make a review of the road traveled by mechanical CPR devices and of the main articles which mark the way.

Single sperm selection and DNA fragmentation analysis: The case of MSOME/IMSI

Sperm of poor quality may affect syngamy after fertilization, embryo development up to the blastocyst stage and reproductive outcome. Subsequently, sperm selection based on morphological characteristics and sperm DNA quality may help to partially avoid these problems. Today, highly efficient sperm selection based on morphological characteristics can be attained using the motile sperm organelle morphology (MSOME) examination, and the spermatozoa selected can be used for ICSI through a fertilization strategy known as intra-cytoplasmic morphologically selected sperm injection (IMSI). The aim of this investigation was to develop a simple methodology to assess sperm DNA fragmentation in single spermatozoa following MSOME/ IMSI, to test the hypothesis that morphologically normal spermatozoa, with an absence of vacuolization, is free of DNA damage. The results indicated that MSOME/IMSI-selected sperm, combined with the Sperm Chromatin Dispersion test (SCD;Oligo-Halosperm), can be reliably used to assess sperm DNA damage in selected single spermatozoa (75% average efficiency), thereby establishing a direct relationship between a good morphological pattern on the sperm and a good DNA quality. Furthermore, results showed spermatozoa presenting a normal morphology and no traces of vacuolization to be fully free of DNA damage. However, traces of vacuolization and more severe morphological alterations were accompanied by significant increases in the proportion of sperm containing a damaged DNA molecule. Interestingly, subtle morphological differences observed between normal and non-vacuolated and normal but vacuolated sperm exhibited significant differrences in the ability of the SCD-Oligo-Halosperm treated sperm to expand DNA fibers following protein depletion.
