Esophageal adenocarcinoma is the most rapidly increas- ing cancer in western ***-grade dysplasia (HGD)arising from Barrett’s esophagus(BE)is the most important risk factor for its development,and when it is present the reported incidence is up to 10% per *** in the setting of BE develops through a well known histological sequence,from non-dysplastic Barrett’s to low grade dysplasia and then HGD and *** surveillance programs have been established to detect the presence of neo- plasia at a potentially curative *** developed endoscopic treatments have dramatically changed the therapeutic approach of *** neoplasia is confined to the mucosal layer the risk for developing lymph node metastasis is negligible and can be successfully eradi- cated by an endoscopic approach,offering a curative in- tention treatment with minimal *** therapies include resection techniques,also known as tissue-acquiring modalities,and ablation therapies or non-tissue acquiring *** aim of endoscopic treatment is to eradicate the whole Barrett’s segment,since the risk of developing synchronous and metachro- nous lesions due to the persistence of molecular aberra- tions in the residual epithelium is well established.

We show that in the study of certain convolution operators, functions can be replaced by measures without changing the size of the constants appearing in weak type （1, 1） inequalities. As an application, we prove that the best constants for the centered Hardy-Littlewood maximal operator associated with parallelotopes do not decrease with the dimension.

Selection of patients with hepatocellular carcinoma for livertransplantation:Past and future

The aim of liver transplantation(LT) for hepatocellular carcinoma(HCC) is to ensure a rate of disease-free survival similar to that of patients transplanted due to benign disease. Therefore, we are forced to adopt strict criteria when selecting candidates for LT and prioritizing patients on the waiting list(WL), to have clarified indications for bridging therapy for groups at risk for progression or recurrence, and to establish certain limits for downstaging therapies. Although the Milan criteria(MC) remain the standard and most employed criteria for indication of HCC patients for LT by far, in the coming years, criteria will be consolidated that take into account not only data regarding the size/volume and number of tumors but also their biology. This criteria will mainly include the alpha fetoprotein(AFP) values and, in view of their wide variability, any of the published logarithmic models for the selection of candidates for LT. Bridging therapy is necessary for HCC patients on the WL who meet the MC and have the possibility of experiencing a delay for LT greater than 6 mo or any of the known risk factors for recurrence. It is difficult to define single AFP values that would indicate bridging therapy(200, 300 or 400 ng/m L); therefore, it is preferable to rely on the criteria of a French AFP model score > 2. Other single indications for bridging therapy include a tumor diameter greater than 3 cm, more than one tumor, and having an AFP slope greater than 15 ng/m L per month or > 50 ng/m L for three months during strict monitoring while on the WL. When considering the inclusion of patients on the WL who do not meet the MC, it is mandatory to determine their eligibility for downstaging therapy prior to inclusion. The upper limit for this therapy could be one lesion up to 8 cm, 2-3 lesions with a total tumor diameter up to 8 cm, or a total tumor volume of 115 cm^3. Abdominal aortic aneurysm (AAA) is a common degenerative vascular disease with a prevalence up to 8% in males over 60 years of age.1 This is a complex disorder characterized by permanent and focal dilation of abdominal aorta that exceeds at least 50% of the normal diameter. In these patients, the aortic diameter progressively expands boosted by smoking and aging as major risk factors. Unfortunately, aneurysm growth increases the risk of aortic rupture, a life-threatening emergency that carries a mortality rate of 80%. Despite the increasing effort of the scientific community to identify therapeutic strategies for AAA, there are currently no pharmacological tools that ameliorate aneurysm expansion, while the underlying mechanisms involved in this disease are not completely understood.

Background: There have been few studies of the long- term prognosis of young adults with ischemic stroke. The present study aimed to evaluate the long- term clinical outcome in a large series of young adults with ischemic stroke admitted to a tertiary medical center over the last 27 years, and to identify possible predictors for mortality, stroke recurrence and poor functional recovery. Methods: We retrospectively reviewed 272 young adults (15- 45 years) with a first- ever ischemic stroke admitted to the Neurology Department of University Hospital “ 12 de Octubre" between 1974 and 2001. Follow- up assessments were performed by review of medical records and telephone interviews. Results Nine patients (3 % ) died as the result of their initial stroke and follow- up information about the status of 23 (8 % ) patients was not available. The remaining 240 patients (89 % ) were followed. Two hundred and ten of them (88 % ) were alive with a mean follow- up of 12.3 years and 30 (12 % ) died during follow- up. The average annual mortality rate was 1.4 % , being notably higher during the first (4.9 % ) than in the subsequent years (0.9 % ) after the initial stroke. Ninety per cent of the followed patients were independent and 53 % returned to work, although adjustments were necessary for 23 % of them. The annual stroke recurrence rate during the first year was 3.6 % dropping to 1.7 % in subsequent years. Age over 35 years, male gender, the presence of cardiovascular risk factors and large- artery atherosclerosis in the carotid territory were predictors of negative long- term outcome after the initial stroke. Conclusions The long- term prognosis for the ischemic stroke in the young is better than in the elderly, but the risk of mortality in young adults with ischemic stroke is much higher than in the general population of the same age.
