Tight focusing with very small f-numbers is necessary to achieve the highest at-focus ***,tight focusing imposes strong demands on precise target positioning in-focus to achieve the highest on-target irradiance We describe several near-infrared,visible,ultraviolet and soft and hard X-ray diagnostics employed in a~10^(22)W/cm^(2)laser±plasma *** used nearly 10 J total energy femtosecond laser pulses focused into an approximately1.3-μm focal spot on 5±20μm thick stainless-steel *** discuss the applicability of these diagnostics to determine the best in-focus target position with approximately 5μm accuracy(i.e.,around half of the short Rayleigh length)and show that several diagnostics(in particular,3ωreflection and on-axis hard X-rays)can ensure this *** demonstrated target positioning within several micrometers from the focus,ensuring over 80%of the ideal peak laser intensity *** approach is relatively fast(it requires 10±20 laser shots)and does not rely on the coincidence of low-power and high-power focal planes.

背景关于亚临床甲状腺功能减退治疗效果的证据十分有限,特别是对于老年患者的治疗效果。目的探讨全科医生在不同国家和针对不同类型的患者对老年亚临床甲状腺功能减退治疗的不同方式。设计和环境在荷兰、德国、英格兰、爱尔兰、瑞士和新西兰进行基于案例的全科医生调查。方法评估全科医生对于8例表现为亚临床甲状腺功能减退患者的治疗策略(治疗是/否,起始剂量甲状腺素)。患者的特征如年龄(70岁与85岁),活力状态(充满活力与脆弱)和促甲状腺激素(TSH)浓度(6 m U/L与15 m U/L)均有差异。结果共有526名全科医生参与此项研究(瑞士262名、荷兰129名、德国61名、新西兰31名、英格兰22名、爱尔兰21名,总有效率为19%)。不同国家的治疗策略会有所不同。相比较于来自德国(平均治疗比例为73%)、爱尔兰(平均治疗比例为62%)和瑞士(平均治疗比例为52%)的全科医生,来自荷兰(平均治疗比例为34%)、英格兰(平均治疗比例为40%)和新西兰(平均治疗比例为39%)的全科医生更加不倾向于实施治疗(P=0.05)。总体来讲,全科医生更倾向于对70岁的女性患者实施治疗,而不是85岁的女性患者〔比值比=0.74,95%CI(0.63,0.87)〕。TSH为15 m U/L的患者比TSH为6m U/L的患者更易于得到治疗〔比值比=9.49,95%CI(5.81,15.5)〕。结论全科医生对于不同国家和不同类型的老年亚临床甲状腺功能减退患者的治疗策略有很大差异,而这种差异说明有必要建立新一代基于初级卫生医疗随机临床试验的国际指南。

Sepsis and the blood-brain barrier(BBB):Sepsis is a life threatening systemic inflammatory condition involving multi-organ *** World Health Organization estimates that about 30 million people are affected by sepsis every year,and at least 6 million people die from sepsis each *** of approximately 24 million sepsis survivors,it is estimated that 70%of these individuals will experience some form of long-term neurological impairment(Iwashyna et al.,2010).Thus,when combined with prevention,diagnosis,and therapeutic management strategies,a functional understanding of the mechanisms that promote sepsis-associated neurological impairment is necessary to address the clinical challenges and economic burdens faced when treating sepsis.

This research was carried out on nine provenances from the north Western Ghats to quantify proximate principles,mineral matter,total phenols and other phytochemicals in melia dubia drupe *** biometric,biomass attributes,proximate principles,mineral matter and total phenols diff ered signifi cantly among the *** average drupe length and width was 26.5 and 22.3 mm,*** drupe and pulp fresh weight,pulp dry weight and stone weight were 783.1,552.1,165.1,and 179.8 g/100 drupes,***,dry matter and moisture per cent were 70.5%,29.9%and 70.0%,*** crude fi bre,protein,ether extract,total ash and nitrogen-free extract were 8.3%,7.6%,5.1%,6.3%,and 72.6%,*** Ca,mg,P and K levels were 0.5%,0.2%,0.2%,and 2.0%,*** phenols were 0.4%.Cluster analysis showed that the study area may be divided into three distinct provenances,each having diff erent drupe traits and proximate composition,including mineral ***-chromatography mass-spectrometry analysis reveale. 27 phytochemicals,many of which possess benefi cial biological *** implications of this provenance variation study should be useful for future improvement in terms of better drupe pulp nutrition for livestock *** study suggests that *** pulp could be categorized as a good feed source with low fi bre,energy,fat and mineral *** proximate parameters,mineral matter content and total phenols are within prescribed limits.
