We investigate the dynamics of convergent shock compression in solid cylindrical targets irradiated by an ultrafast relativistic laser *** particle-in-cell simulations and coupled hydrodynamic simulations reveal that the compression process is initiated by both magnetic pressure and surface ablation associated with a strong transient surface return current with density of the order of 10^(17) A/m^(2) and lifetime of 100 *** results show that the dominant compression mechanism is governed by the plasma β,i.e.,the ratio of thermal pressure to magnetic *** targets with small radius and low atomic number Z,the magnetic pressure is the dominant shock compression *** to a scaling law,as the target radius and Z increase,the surface ablation pressure becomes the main mechanism generating convergent ***,an indirect experimental indication of shocked hydrogen compression is provided by optical shadowgraphy measurements of the evolution of the plasma expansion *** results presented here provide a novel basis for the generation of extremely high pressures exceeding Gbar(100 TPa)to enable the investigation of high-pressure physics using femtosecond J-level laser pulses,offering an alternative to nanosecond kJ-laser pulse-driven and pulsed power Z-pinch compression methods.

BACKGROUND With improved survival in gastric cancer patients,health-related quality of life has become an important clinical endpoint alongside primary oncological *** To investigate health-related quality of life after various surgical procedures for gastric cancer *** The validated Slovenian version of the European Organization for Research and Treatment of Cancer Quality of Life Core Questionnaire(QLQ-C30)and its gastric cancer-specific module(QLQ STO-22)was sent for self-completion to patients that underwent curative resection for gastric adenocarcinoma between January 2014 and December 2018 at our *** total,116 patients *** were compared between patients after subtotal distal vs total gastrectomy and patients after subtotal distal gastrectomy with Billroth ii vs Roux-en-Y *** interestingly,the extent of resection did not influence daily functioning;however,more dysphagia and eating restrictions were reported in patients after total gastrectomy when compared to patients after subtotal distal ***,patients with Billroth ii reconstruction after subtotal distal resection experienced worse physical and role functioning and reported more pain,fatigue and reflux compared to Roux-en-Y *** Based on our results,Roux-en-Y reconstruction after subtotal distal gastrectomy should be preferred over Billroth ii *** data obtained from this study will help surgeons when preoperatively informing their patients about expected functional outcomes after gastrectomy and enable them to ensure proper supportive care of their patients in the postoperative period.

BACKGROUND Despite negligible absorption of elemental mercury after acute ingestion,retention in the appendix with subsequent local and systemic complications is possible. We present a case of elemental mercury sequestration in the appendix,managed by laparoscopic *** SUMMARY A 57-year-old Caucasian female was found unconscious following application of long-lasting insulin detemir and ingestion of elemental mercury in a suicidal attempt. Diagnostic investigations revealed several radiopaque collections in the gastrointestinal(Gi) tract and elevated mercury levels in the blood. Much of the ingested elemental mercury was eliminated from the Gi tract with stools stimulated by several enemas. However, a significant amount of mercury remained sequestrated in the appendix despite all conservative ***, following deliberations by an interdisciplinary team of specialists,laparoscopic appendectomy was performed 29 d after the mercury ingestion. The surgery itself and postoperative course were *** Since conservative measures are often unsuccessful in the management of mercury retention in the appendix, surgery remains a compelling option to prevent possible associated complications.

BACKGROUND Helicobacter pylori(***)eradication treatment for primary gastric mucosaassociated lymphoid tissue(MALT)lymphoma has already been ***,t(11;18)(q21;q21)/APi2-MALT1 translocation-positive lesions are a type of primary gastric MALT lymphoma in which a response to eradication treatment is difficult to *** addition,trisomy 18 may be associated with diffuse large B-cell lymphoma(DLBCL)transformation of gastric MALT *** SUMMARY A 66-year-old man was diagnosed with MALT lymphoma in the ascending colon by colonoscopy and *** years later,esophagogastroduodenoscopy revealed chronic atrophic gastritis that was positive for ***,and eradication treatment was *** years and nine months later(at the age of 70),a new ulcerative lesion suggestive of MALT lymphoma appeared in the gastric body,and six months later,a similar lesion was also found in the *** year later(4 years and 3 months after *** eradication),at the age of 72,the lesion in the gastric body had become deeper and had propagated.A biopsy revealed a pathological diagnosis of *** MALT lymphoma lesions in the ascending colon and DLBCL lesions in the stomach were positive for the t(11;18)(q21;q21)/APi2-MALT1 translocation,and trisomy 18q21 was also *** 6 courses of R-CHOP(rituximab,cyclophosphamide,doxorubicin,vincristine and prednisone)chemotherapy,all of the above lesions disappeared[complete remission(CR)],and CR has been maintained for more than 3 *** addition,both the colonic and gastric lesions were proven to have the same *** Because the patient had a MALT1 translocation with trisomy 18q21,it was thought that this gastric MALT lymphoma developed independently of *** infection and progressed.

A significant research focus is placed on identifying animal species and areas at future risk to human-induced alterations of the environment and long-term changes in climatic ***,the extent to which exposure to extreme climatic events and intense human pressure can increase the risk of harmful impacts on species remains poorly *** on terrestrial mammals in China,one of the world’s megadiverse countries,we investigated patterns of contemporary exposure to floods and human pressures and determined their taxonomic representation and distribution across three major area-based conservation schemes,namely,national nature reserves(NNRs),priority areas for biodiversity conservation(PABCs),and key biodiversity areas(KBAs).Among the 440 species assessed with moderate or high exposure to floods,327(∼75%)also qualified as moderate or high in exposure to intense human *** species mainly belong to the orders Chiroptera,Eulipotyphla,and ***,there were 305,311,and 311 species with moderate or high exposure to flood and intense human pressure represented across NNRs,PABCs,and KBAs,*** findings support the prioritization of KBAs for expansion of site-based protection efforts such as NNRs in China,considering threats to species from exposure to adverse effects from both extreme climate and human pressure.
