BACKGROUND Hepatitis C virus(HCV)can lead to chronic liver damage resulting in cirrhosis and hepatocellular *** clearance of HCV has been documented after an acute infection in 20%-45%of ***,spontaneously resolved chronic hepatitis C following liver transplant(LT)is rare and has been documented only in a few case *** phenomenon of spontaneous clearance of chronic hepatitis C occurs together with other meaningful events,which are typically associated with significant changes in the host *** SUMMARY We report three cases of spontaneous resolution of chronic hepatitis C following liver *** patients either failed or had no HCV treatment prior to transplant,but had spontaneous resolution of HCV post-LT as documented by undetectable polymerase chain reaction(PCR).Diagnosis of HCV was based on viremia through PCR or liver *** three patients currently undergo surveillance and have no recurrence of *** Examining each patient’s clinical course,we learned about many viral,host and cellular-factors that may have enhanced the host’s immunity leading to spontaneous clearance of *** HCV treatment has excellent cure rates,understanding this mechanism may provide clinicians with insights regarding timing and duration of treatment.

AIM： To investigate the effect of a new oral preparation, highly concentrated in fish cartilage, in a group of inflammatory bowel diseases （IBD） patients with chronic iron deficient anemia. METHODS： In an open label pilot study, we supplemented a group of 25 patients （11 with Crohn＇s disease and 14 with ulcerative colitis） in stable clinical conditions and chronic anemia with a food supplement which does not contain iron but contains a standardized fraction of fish cartilage glycosaminoglycans and a mixture of antioxidants （Captafer Medestea, Turin, Italy）. Patients received 500 mg, twice a day during meals, for at least 4 mo. Patients were suggested to maintain their alimentary habit. At time 0 and after 2 and 4 too, emocrome, sideremia and ferritin were examined. Paired data were analyzed with Student＇s t test. RESULTS： Three patients relapsed during the study （2 in the 3^rd too, 1 in the 4^th too）, two patients were lost to follow up and two patients dropped out （1 for orticaria, 1 for gastric burning）. Of the remaining 18 patients, levels of serum iron started to rapidly increase within the 2^nd mo of treatment, P 〈 0.05）, whereas serum ferritin and hemoglobin needed a longer period to significantly improve their serum levels （too 4） P 〈 0.05. The product was safe, easy to administer and well tolerated by patients. CONCLUSION： These data suggest a potential new treatment for IBD patients with iron deficiency chronic anemia and warrant further larger controlled studies.

Background:We utilized transcystic clearance and intra-operative papillotomy through a rendezvous technique for the treatment of *** goal of this study was to evaluate the reliability of pre-operative parameters to address the most suitable surgical ***:A total of 180 patients affected by calculi of the gallbladder and bile duct underwent the single-stage *** to several pre-operative parameters,141 patients had to supposedly undergo transcystic clearance of the bile duct,while 39 patients had to be treated with the rendezvous *** patients were treated with the sequential procedure:first,we tried the transcystic procedure and,if there was a failure,we used a rendezvous *** prospectively analysed each group based on a series of variables such as sex,age,operative time,success rate of proposed treatment,conversion rate,post-operative complications and hospital ***:Transcystic clearance was successful in 134 out of 141 patients(95.0%),while 2 patients needed to undergo a laparo-endoscopy procedure(failure).Thirty-five out of 39 patients(89.7%)obtained common bile-duct(CBD)clearance through the rendezvous technique,while 1 patient obtained clean-up through the simple transcystic procedure(failure).Five out of 141 patients with transcystic clearance and 3 out of 39 patients with the rendezvous technique underwent laparotomy CBD clearance with conversion rates of 3.5%and 7.7%,***-operative complications showed similar percentages for both ***,the surgical time turned out to be longer for the rendezvous ***:The one-stage procedure for the treatment of cholecysto-choledocolithiasis was possible in 94%of the cases utilizing a surgical technique selected according to the patient’s case *** pre-operative parameters,such as jaundice,CBD diameters and stone diameters,have certified their reliability as good predictors of the most suitable

Ampulla of Vater is a peculiar anatomical structure, characterized by the crossroad of three distinct epithelia: Intestinal, ductal pancreatic and biliary. Adenocarcinomas arising in this area represent an opportunity to understand the comparative biology of all periampullary malignancies. These neoplasms can exhibit intestinal, pancreaticobiliary or mixed features, whereas the subclassification based on morphology and immunohistochemical features failed in demonstrating a robust prognostic reliability. In the last few years, the molecular landscape of this tumor entity has been uncovered, identifying alterations that may serve as prognostic and predictive biomarkers. In this review, the histological and genetic characteristics of ampullary carcinomas are discussed, taking into account the main clinical and therapeutic implications related to this tumor type as well.

Gelatin-coated,3D sponge-like scaffolds based on 45S5 bioactive glass were produced using the foam replication *** strength tests of gelatin-coated samples compared to uncoated scaffolds showed significant strengthening and toughening effects of the gelatin coating with compressive strength values in the range of cortical ***,the crosslinked gelatin network(using either caffeic acid or N-(3-Dimethylaminopropyl)-N′-ethylcarbodiimide hydrochloride(EDC)/N-hxdroxysuccinimide(NHS)as crosslinking agent)was shown to be a suitable candidate for the sustained release of the bioactive molecule *** bioactivity of the produced scaffolds,characterization by FTIR and SEM indicated the formation of hydroxyapatite(HA)in all samples after immersion in simulated body fluid(SBF)for 14 days,highlighting the favorable combination of mechanical robustness,bioactivity and drug delivery capability of this new type of scaffolds.

A large proportion of patients with idiopathic spermatogenic failure(SPGF;oligozoospermia or nonobstructive azoospermia[NOA])do not receive a diagnosis despite an extensive diagnostic *** evidence has shown that the etiology remains undefined in up to 75%of these patients.A number of genes involved in germ-cell proliferation,spermatocyte meiotic divisions,and spermatid development have been called into play in the pathogenesis of idiopathic oligozoospermia or ***,this evidence mainly comes from case ***,this study was undertaken to identify the molecular causes of *** accomplish this,15 genes(USP9Y,NR5A1,KLHL10,ZMYND15,PLK4,TEX15,TEX11,MEIOB,SOHLH1,HSF2,SYCP3,TAF4B,NANOS1,SYCE1,and RHOXF2)involved in idiopathic SPGF were simultaneously analyzed in a cohort of 25 patients with idiopathic oligozoospermia or NOA,accurately selected after a thorough diagnostic *** next-generation sequencing(NGS)analysis,we identified the presence of rare variants in the NR5A1 and TEX11 genes with a pathogenic role in 3/25(12.0%)*** other different variants were identified,and among them,13 have never been reported *** out of 17 variants were likely pathogenic and deserve functional or segregation *** genes most frequently mutated were MEIOB,followed by USP9Y,KLHL10,NR5A1,and *** alterations were found in the SYCP3,TAF4B,NANOS1,SYCE1,or RH0XF2 *** conclusion,NGS technology,by screening a specific custom-made panel of genes,could help increase the diagnostic rate in patients with idiopathic oligozoospermia or NOA.

The aim of this study was to examine the tissue expression and Iocalisation of the somatostatin receptors （SSTRs） in hormone-refractory （HR） prostate cancer （PCa）. Five SSTRs were evaluated immunohistochemically in 20 radical prostatectomies （RPs） with Gleason score （GS） 3＋3=6 PCa, in 20 RPs with GS 4＋4=8 and 4＋5=9 PCa, and 20 transurethral resection of the prostate specimens with HR PCa. The mean values in the cytoplasm （all five SSTRs were expressed）, membrane （only SSTR3 and SSTR4 were expressed） and nuclei （only SSTR4 and SSTR5 were expressed） of the glands in HR PCa were 20-70% lower than in the other two groups, the differences being statistically significant. All five SSTRs were expressed in the smooth muscle and endothelial cells of HR PCa, the mean values being lower than in the other two groups. In conclusion, this study expands our knowledge on the expression and Iocalisation of five SSTRs in the various tissue components in the HR PCa compared with hormone-sensitive PCa.

The thermodynamic properties of charged particles,such as the energy density,pressure,entropy density,particle density,and squared speed of sound at the kinetic freeze-out in the Au+Au collisions from the relativistic heavy ion collider (RHIC)beam energy scan program √S_(NN) and in the Cu+Cu collisions at √S_(NN),200 GeV are studied using the thermodynamically consistent Tsallis *** energy density,pressure,and particle density decrease monotonically with the collision energy for the same collision centrality;These properties also decrease monotonically from the central to peripheral collisions at the same collision *** the scaled energy densityε∕T^(4) and scaled entropy density s∕T^(3) demonstrate the opposite trend with the collision energy for the same collision *** is a correlation betweenε∕T^(4) and s∕T^(3) at the same *** addition,the squared speed of sound was calculated to determine that all the collision energies share nearly the same value at different collision centralities.

The evolution of the universe from an initial dramatic event, the Big-Bang, is firmly established. Hubble’s law [1] (HL) connects the velocity of galactic objects and their relative distance: v(r) = Hr, where H is the Hubble constant. In this work we suggest that HL is not valid at large distances because of total energy conservation. We propose an expansion of the velocity in terms of their relative distance and produce a better fit to the available experimental data. Using a simple “dust” universe model, we can easily calculate under which conditions an (unstable) equilibrium state can be reached and we estimate the values of the matter present in the universe as well as the “dark energy”. Within the same formalism we can derive the “deceleration parameter”. We do not need to invoke any “dark energy”, its role being played by the kinetic correction. The resulting picture is that the universe might reach an unstable equilibrium state whose fate will be decided by fluctuations: either collapse or expand forever.
