We evaluated the prevalence of use of complementary and alternative medicine (CAM), as well as the transitional nature of its use, before and after radical prostatectomy in Japa-nese patients with localized prostate cancer. Methods: We enrolled 376 patients, who an-swered a self-administered questionnaire on CAM use, psychological health locus of control (HLC), and general-health-related quality of life (GHQL). Detailed information regarding CAM use according to the transtheoretical model, and the time at initiation and abandonment of CAM use were assessed. Medical information was also extracted from patient charts. Results: 45.7% of patients belonged to the “precontem-plation” stage, 29.8% to the “contemplation” stage, 1.9% to the “preparation” stage, 14.4% to the “action” stage, and 8.2% to the “relapse” stage. Although patient age and educational status had a significant impact on stage of CAM use, HLC and GHQL were not associated with them. The time-course of prevalence of CAM use during follow-up was divided into three phases: “initial,” “rapid-increase,” and “main-tenance”. Conclusions: Among patients under-going radical prostatectomy, non-users can be classified into several behavioral stages, while users do not use CAM constantly during fol-low-up.

电子封装业界正遭受着前所未有的来自手机和其他移动通讯终端设备挑战。在这一领域里,IC封装的关键是尺寸微型化,缩减成本和市场时机。这一挑战的背后隐含着手机技术发展的两大趋势:系统模块化和日益增长的复杂性及功能。越来越多的功能正在被组合到手机上即PDA、MP3、照相机、互联网等等。功能的增加需要靠模块化来实现,而模块化又促进了更多功能的组合。同时,模块化使得移动通讯终端设备得以微型化、降低成本和缩短设计周期。业界越来越多地感受到整体射频模块和通讯模块解决方案的必要性。这些整体模块把手机设计师从电路设计的细节中解脱出来,从而能专著于高层的手机应用和系统的设计。为了满足上诉移动通讯产品的苛刻要求,大量的新兴电子封装技术和封装产品应运而生。最引人注目的例子在于对系统模块穴SiP雪和三维穴3D雪封装的重点资金和技术投入。这两项先进封装技术有着各自不同的特征和应用范围。总体介绍先进封装技术在移动通讯中的应用,重点讨论电子封装材料和工艺所面临的挑战和最新发展趋势。对移动通讯带来的新一轮集成化及其所产生的潜在供应链问题也做了适当的讨论。

简要介绍地震地形变观测系统的组成及地形变观测技术的发展,指出不同观测技术的时空特点,光纤和长基线应变仪等新技术的优点以及不同地形变观测技术进行联合观测的可行性和方法优势。通过对日本岐阜县安装的1500 m臂长应变仪与GNSS地球观测网(GEONET)中的连续观测站获取的应变进行比对的实例分析,得出从连续观测站获取的水平应变去趋势后与应变仪得到的连续应变结果基本一致,地震会引起应变异常,应变仪对环境因素的影响更敏感。实际工作中应注意发展优势观测技术,进行观测技术和研究方法的融合、提高监测密度,增强时空分辨率,提升地震前兆特征识别能力,更好地为科学防震减灾服务。

This investigation was undertaken to enhance the solubility and consequent antibacterial activity of cefuroxime axetil (CA), a β-lactamase-stable broad spectrum second generation cephalosporin through solid dispersion (SD) technique. For this purpose, CA loaded SDs (CSDs) were prepared by solvent evaporation method using different concentrations of microcrystalline cellulose (MCC) as carrier. The CSDs were characterized by in-vitro dissolution study, thermal analysis (DSC), crystallinity (PXRD), interactions (FTIR) and morphology (SEM). Among the formulations, CSD-2 showed the highest dissolution rate which was 2.59-fold higher than pure CA with a drug-carrier (CA: MCC) ratio of 1:3. Enhanced dissolution rate was attributed to conversion of drug from crystalline to amorphous state during preparation of SDs, which was validated by DSC, PXRD, FTIR and SEM analyses. Antibacterial activity of CSD-2 against Staphylococcus aureus (ATCC 25923) and Escherichia coli (ATCC 25922) showed 1.94- and 6.75-fold higher relative zone of inhibition (RZOI), respectively than pure CA. CSD-2 has been found to be the most effective optimized formulation in terms of both enhanced dissolution rate and antibacterial activity. Thus, it can be an effective alternative to conventional dosage forms of CA. The diagnosis of climacteric disorders in gynecology is performed on the basis of symptoms rather than hormonal levels. Generally, the follicle-stimulating hormone (FSH) level increases when the 17β-estradiol (E2) level decreases in menopause. However, the problem of these hormones should be determined by strict timing of sampling, and there are individual specificities of decreases of hormone levels. We considered that anti-Mullerian hormone (AMH) can be measured at any time of the menstrual cycle and that it shows ovary functional decline earlier than FSH/E2, and we examined whether AMH would possibly become a good index for climacteric disorders. The subjects were 163 healthy females and 21 patients with climacteric disorders between 20 and 59 years old. We examined AMH, FSH and E2 at the same time. It is understood that in healthy females, AMH decreases with age and a decline in ovary function occurs at a relatively early age. Patients visiting clinics for climacteric disorders often have normal-range serum FSH/E2 levels, and it is clear that these values could not serve as indices of menopause at these inspections. Upon measurement of AMH in patients with climacteric disorders, most showed less than normal range (< 14 pmol/L), suggesting a decline in ovarian function. In addition, AMH was low in females with climacteric disorders compared with those without them. In conclusion, AMH was suggested as an objective index for climacteric disorders and possibly as a new diagnostic marker.

Objective: Dual bronchodilation with long-acting muscarinic antagonist and long-acting β2-agonist combinations are available worldwide in COPD patients. However, the choice of agents remains under debate. We hypothesized that switching devices between dry powder and soft mist inhalers without a wash-out period to mimic clinical practice would improve clinical symptoms and lung function. The aim of this study was to examine the effects of switching between once-daily glycopyrronium/indacaterol (GLY/IND) or umeclidinium/vilanterol (UMEC/VI), dry powder inhalers, and tiotropium/olodaterol (TIO/OLO), a soft mist inhaler, in COPD patients. Methods: This was a prospective, open-label, 8-week, observational study with follow-up. Subjects included 57 COPD patients, who attended outpatient clinics at Shizuoka General Hospital for routine check-ups between February and December 2015, receiving GLY/IND (50/110 μg) or UMEC/VI (62.5/25 μg). After an 8-week run-in period, medications were switched to TIO/OLO (5/5 μg). Study outcomes included patient’s global rating (PGR), modified MRC (mMRC), COPD assessment test (CAT), and spirometric and forced oscillatory parameters after 8 weeks. PGR used in this study was a 7-point scale ranging from 1 to 7, with 4 in the middle. Patients who consented to switch from TIO/OLO to GLY/IND or UMEC/VI were followed-up thereafter. Results: In total, 53 patients completed the study (mean age, 75 years;48 males and 5 females;GOLD 1/2/3/4 = 19/27/6/1;mMRC 0/1/2/3/4 = 14/22/12/4/1;UMEC/VI 26, GLY/IND 27). PGR, mMRC, and CAT improved in 20 (38%), 9 (17%), and 15 patients (28%), respectively. Respiratory system resistance at 5 Hz (R5), 20 Hz (R20), and the difference between R5 and R20 (R5 - R20) significantly improved. In a follow-up of 16 patients after switching from TIO/OLO to UMEC/VI (9) or GLY/IND (7), PGR, mMRC, and CAT improved in 5 (31%), 3 (12%), and 4 patients (25%), respectively, and R20 significantly improved (p = 0.011). Background: Some patients present clinical features of both asthma and chronic obstructive pulmonary disease (COPD), which has led to the recent proposal of asthma-COPD overlap (ACO) as a diagnosis. Fractional exhaled nitric oxide (FeNO) is a candidate biomarker to diagnose ACO. We assessed the effect of an add-on treatment with budesonide/formoterol (BUD/FM) combination in patients with ACO, which was diagnosed by FeNO. Methods: This was a prospective, single-arm, open-label, before and after comparison study. Subjects included 83 patients with COPD who attended outpatient clinics for routine checkups at Shizuoka General Hospital between June and November 2016. All patients fulfilled the GOLD definition of COPD and were receiving long-acting muscarinic antagonist (LAMA) or LAMA/long-acting β2 agonist (LABA) combinations. After an 8-week run-in period, BUD/FM was added to the patients with FeNO levels of ≥35 ppb, defined as having ACO. For patients receiving LAMA/LABA, BUD/FM was added after the discontinuation of LABA. The modified British Medical Research Council (mMRC) score, COPD assessment test (CAT) score, spirometric indices, forced oscillation parameters, and FeNO were assessed before and after 8 weeks of BUD/ FM add-on treatment. Results: Twenty-four patients (28.9%) had FeNO levels ≥ 35 ppb, and 17 patients completed the study (mean age: 73 years and GOLD I/II/III/IV, 5/10/1/1). The mean CAT scores significantly improved (9.2 to 5.4, p = 0.015) and 10 patients (58.8%) showed ≥2 points improvement, a minimal clinically important difference. The mean FeNO levels significantly decreased from 63.0 to 34.3 ppb (p Conclusions: FeNO-guided treatment with BUD/FM improves symptoms in patients with ACO.

We report on the design and characterization of the plasma mirror system installed on the J-KAREN-P laser at the Kansai Photon Science Institute,National Institutes for Quantum Science and *** reflectivity of the single plasma mirror system exceeded 80%.In addition,the temporal contrast was improved by two orders of magnitude at 1 ps before the main ***,the laser near-field spatial distribution after the plasma mirror was kept constant at plasma mirror fluence of less than 100 kJ/cm^(2).We also present the results of investigating the difference and the fluctuation in energy,pulse width and pointing stability with and without the plasma mirror system.

We simultaneously observed both the fast proton generation and terahertz(THz) radiation in the laser pulse interaction with a thin-foil target. The maximum proton energy of ～2.3 Me V and an intense THz radiation were observed at the pulse duration of～30 fs. We also measured the proton beam and UV harmonics from a thin-foil target by changing the laser pulse duration. In the case of the ～500 fs, peaks of UV harmonics up to fourth order appeared. This unique combination of the multiple beams will provide useful applications such as pump-probe experiments.
