Improvements in quality of life in septuagenarians versus octogenarians undergoing trans-catheter aortic valve replacement

Background Very elderly patients represent a distinct patient group in clinical setting in terms of a decision for trans-catheter aortic valve replacement （TAVR） when one considers the potential improvement in the quality of life （QoL） on one hand and the benefit to risk ratio on the other. This study aimed to compare functional and QoL outcomes of TAVR between octogenarians and septuagenarians. Methods This prospective cohort study included 136 elderly patients （70 to 89 years of age）, who tmderwent Wansfemoral TAVR due to degenerative aortic stenosis. Patients were allocated into one of the following age groups： septuagenarians （n = 67） and octogenarians （n = 69）. Preoperative and early postoperative clinical parameteT：s were recorded. In addition, QoL of the patients was evaluated using SF-36 questionnaire preoperatively and six month postoperatively. Results groups were similar in terms of early postoperative mortality and morbidity parameters. The mean New York Heart Association （NYHA） class improved after TAVR in both groups. In addition, all SF-36 norm-based scale and SF-36 summary scale scores improved significantly in both groups during the postoperative period. Postoperatively, physical functioning, general health and physical component summary scores were significantly better in the septuagenarian group （P = 0.02, 0.01, 0.03, respectively）. ConcLusion Although the improvement in the QoL in terms of physical health was more marked in septuagenarians than in octogenarians, substantial benefits on the QoL and particularly on mental health seem to justify re-consideration of TAVR indications in the very elderly.

In this present paper, the result of C6-C12 α-olefins oligomerizaton in the presence of recyclable chloroaluminate type ionic-liquid catalysts and their combination with Ti-containing complexes is presented. By different methods of analysis the composition and structure of oligoalkylnaphtenic products obtained in the presence of ionic-liquid catalysts and Ti-containing modifiers have been studied.

Effect of BMI on outcomes of surgical treatment for tibial plateau fractures： A comparative retrospective case series study

Purpose： Tibia plateau fracture （TPF） treatment aims at achieving a stable, aligned, mobile, painless knee and preventing post-traumatic osteoarthritis. To achieve this goal, surgeons consider criteria such as patients＇ characteristics, severity, risk of complications, fracture displacementJdepression, degree of soft tissue injury. However, body mass index （BMI） is not considered as a risk factor in literature. Our study was conducted to find out any possible correlation between BMI and functional scores or radiological score separately. Methods： Retrospective analysis of case series between 2011 and 2014 was done on the database of a tertiary hospital in Istanbul. There were 67 TPF patients （54 males, 13 females） in the study. Relationship between BM1 and functional knee scores or radiological score was compared statistically. Closed fractures with both high-energy and low-energy injury were included in the study. Patients with open fracture, multi-trauma presence, meniscus and/or ligamentous injury, increased co-morbidity, inadequate records （25 cases in all） were excluded. Surgery type, Schatzker classification, injury side, trauma energy, and gender were considered as possible risk factors. Binary regression analysis was done for possible factors affecting functional knee scores and radiologic score. Results： Model summary calculations were done as Nagelkerke R2 test for Knee Society score, Lysholm knee score, and Ahlback and Rydberg radiologic scores, which were 0.648, 0.831, and 0.327 respec- tively. Homer-Lemeshow test values were 0.976, 0.998, and 0.362, respectively. There is negative correlation between BMI and both knee function scores. There is no correlation between BMI and radiologic score. Conclusion： An increase in BMI has a negative effect on functional knee scores after surgical treatment of TPFs. Therefore, BMI should be considered as a risk factor for surgical treatment of TPFs.

An Empirical Research of Ethical Banking in Turkey

Ethical banks can be defined as financial institutions providing both social and economic profitability for the society and environment. Placement of assets and avoiding social exclusion are significant factors that differentiate ethical banks and the rest of financial intermediaries. Ethical banking is a new concept in Turkey. This paper analyzes the situation of ethical banking by examining the significant differences in lending behaviors of 49 Turkish banks in terms of their functions and ownership structures. We conducted non-parametric tests of Kruskal Wallis, Jonckheere-Terpstra, Mann-Whitney, Kolmogorov-Smirnov and Wald-Wolfowitz. We found that in terms of ownership structure, state-owned banks are more ethical than private banks when the placement of assets and avoiding social exclusion factors are considered. In terms of function, participation banks locate their funds to social value generating sectors more ethically than deposit and development banks. From the avoiding exclusion aspect, deposit banks are more ethical than other functional banking groups （i.e., participation and development banks）.

Isolated dislocation of the carpal scaphoid is a rare injury. Such dislocation is extremely rare. In this article the authors report a twenty-eight-year-old man with isolated dislocation of the scaphoid with associating radial styloid fracture. We manage the patient with open reduction and Kirschner wire fixation. One year later there is a good functional and radiographic result.

Achilles tendon ruptures and malleol fractures are commonly seen injuries in orthopaedic and traumatology practice, but what is rare is their concomitant. In this report, we aimed to present a rare case of a patient who has isolated medial malleolar fracture and achilles tendon rupture in ipsilateral ankle after an ankle sprain due to a fall from stairs and mechanism of injury.

Enhanced Fenton,photo-Fenton and peroxidase-like activity and stability over Fe_3O_4/g-C_3N_4 nanocomposites

We prepared the Fe3O4/g‐C3N4nanoparticles(NPs)through a simple electrostatic self‐assembly method with a3:97weight ratio to investigate their Fenton,photo‐Fenton and oxidative functionalities besides photocatalytic *** observed an improvement of the Fenton and photo‐Fenton activities of the Fe3O4/g‐*** improvement was attributed to efficient charge transfer between Fe3O4and g‐C3N4at the heterojunctions,inhibition of electron‐hole recombination,a high surface area,and stabilization of Fe3O4against leaching by the hydrophobic g‐*** obtained NPs showed a higher degradation potential for rhodamine B(RhB)dye than those of Fe3O4and g‐*** compared to photocatalysis,the efficiency of RhB degradation in the Fenton and photo‐Fenton reactions was increased by20%and90%,***,the horseradish peroxidase(HRP)activity of the prepared nanomaterials was studied with3,3,5,5‐tetramethylbenzidinedihydrochloride(TMB)as a *** oxidation was also *** indicate that Fe3O4/g‐C3N4nanocomposites offers more efficient degradation of RhB dye in a photo‐Fenton system compared with regular photocatalytic degradation,which requires a long *** study also confirmed that Fe3O4/g‐C3N4nanocomposites can be used as a potential material for mimicking HRP owing to its high affinity for *** findings suggest good potential for applications in biosensing and as a catalyst in oxidation reactions.

Association of inflammation with atrial fibrillation in hyperthyroidism

The aim of this studywas to evaluate the relationship between inflammation and development of atrial fibrillation (AF) in patients with *** A total of 65 patients with newly diagnosed hyperthyroidism, 35 of whom were in sinus rhythm and 30 of whom in AF. Thirty five age- and gender-matched patients in a control group were included in the study. Factors associated with the development of AF were evaluated by multivariate regression analysis. Results Factors associated with AF in multivariate analysis included high sensitivity C reactive protein (HsCRP) [odds ratio (OR): 11.19; 95% confidence interval (95% CI): 1.80–69.53; P = 0.003],free T4 (OR: 8.76; 95% CI: 2.09–36.7; P = 0.003), and left atrial diameter (OR: 1.25; 95% CI: 1.06–1.47; P = 0.008). Conclusions The results of the present study suggest that HsCRP, an indicator of inflammation, free T4 and left atrial diameter are associated with the development AF in patients with hyperthyroidism.
